A psoriasis és az oxidatív stressz | Psoriasis and oxidative stress
Absztrakt
A psoriasis hazánkban és világviszonylatban is a leggyakrabban előforduló
bőrbetegségek egyike. Jelentőségét hangsúlyozza a gyakran élethossziglan
fennálló plakkok okozta fájdalom, mozgáskorlátozottság és a pszichés terhelés
életminőségre gyakorolt hatása. Szövődményeit tekintve a nemegyszer maradandó
károsodásokat okozó arthritis psoriaticán túl, a psoriasisos betegek esetében
nagyobb cardiovascularis kockázattal kell számolni, gyakrabban alakulnak ki
gyulladásos bélbetegségek és bizonyos daganatok. Az exogén és endogén tényezők
okozta oxidatív stressz és a genetikai prediszpozíció hozzájárulhat a
keratinocyták hiperproliferációjához és abnormális differenciálódásához, ezáltal
a psoriasis kialakulásához, illetve fenntartásához. A nagyfokú oxidatív stressz
felelős lehet továbbá a psoriasis szövődményeinek megjelenéséért. Jelen
közleményben, rövid patofiziológiai áttekintés után, néhány jól ismert biomarker
(aszimmetrikus dimetilarginin, malonilaldehid, szuperoxid dizmutáz, kataláz)
segítségével a szerzők bemutatják az oxidatív stressz szerepét a psoriasis és

Abstract
Psoriasis is among the most common dermatological diseases worldwide. Its
significance is emphasized by adverse effects on quality of life, caused by
chronic pain, physical and psychical disability due to psoriatic plaques.
Besides the development of psoriatic arthritis, which often causes permanent
joint damage, former studies revealed an increased risk of inflammatory bowel
disease, cardiovascular disease and certain types of cancer. Genetic
predisposition and oxidative stress caused by exogenous and endogenous factors
can contribute to abnormal differentiation and hyperproliferation of
keratinocytes, accordingly the development and maintenance of psoriasis.
Moreover, excessive oxidative stress can be responsible for the onset of
psoriasis complications. After a brief pathophysiological summary the authors
discuss the role of oxidative stress in the development of psoriasis and its
complications through several well studied biomarkers (asymmetric
dimethylarginine, malondialdehyde, superoxide dismutase, catalase). Orv. Hetil.,

Aszimmetrikus dimetilarginin: a cardiovascularis betegségek prediktora?
Absztrakt
A szív-ér rendszeri megbetegedések világszerte a leggyakrabban előforduló
betegségek. Felelősek a globális halálozás egyharmadáért, vezető okai a
rokkantságnak. A prevenció különböző szintjeinek hatékony kockázatbecsléssel
együtt történő alkalmazása sokat javított a fent említett statisztikai adatokon.
A klasszikus kockázati tényezőkön alapuló kockázatbecslés az utóbbi időben
számos új markerrel egészült ki. Ilyen biomarker az aszimmetrikus
dimetilarginin, amely a nitrogén-monoxid-szintáz endogén kompetitív inhibitora.
Emelkedett szintjét leírták többek között elhízott, dohányos,
hypercholesterinaemiás, hypertoniás, diabeteses betegcsoportokban. Irodalmi
adatok szerint az aszimmetrikus dimetilarginin képes jelezni az atherosclerosis
megelőző állapotának tartott endotheldiszfunkciót. Számos nagy betegszámú
kutatás talált pozitív összefüggést a magasabb aszimmetrikus dimetilarginin
koncentráció és a coronariabetegségek kialakulása, illetve a már meglévő
coronariabetegség progressziója között. Egy 3000 fős beteganyagot vizsgáló
tanulmány szerint az aszimmetrikus dimetilarginin független kockázati faktorként
önmagában is képes előre jelezni a cardiovascularis okokból bekövetkező
halálozást a coronariabetegségben szenvedők körében. Jelen írásukban a szerzők
összefoglalják az aszimmetrikus dimetilarginin cardiovascularis betegségek
előrejelzésében betöltött szerepét, és hangsúlyozzák kiemelt fontosságát a

Simple and choice reaction times are prolonged following extracorporeal circulation : a potential method for the assessment of acute neurocognitive deficit
Cognitive deficit related to open heart surgery came into the
focus of
interest according to professional and social expectations. The
negative effects on quality of life and the large number of
involved
patients emphasize its investigation. The bed-side measurement
of
simple and choice reaction time (sRT and cRT) has the
objectivity of
cortical evoked potential analysis; however there is no need for
EEG
instrumentation and laboratory. This is a functional assessment
similarly to neuropsychological tests, but requires
significantly
shorter time, and is less demanding for the patient.
We found a statistically significant positive correlation of sRT
and
cRT elongation and perfusion time. At the same time there were
no
statistically significant changes of mean sRT and cRT values
before
(sRT: 208±54 s, cRT: 369±59 s) and after (sRT: 229±67 s, cRT:
392±105
s) the surgery probably due to the inhomogeneous patient
population.
The weak correlation (coefficients: 0.1418-0.8484) for sRT and
cRT
changes as a function of perfusion time confirm the presence of
other
factors of postoperative brain damage.
The investigated bed-side test is clinically feasible, simple
and can
be completed within 30 minutes. Further studies are encouraged
to
compare above method to other tests in a larger, stratified
cardiac
surgery populatio
The assessment of neural injury following open heart surgery by physiological tremor analysis
INTRODUCTION: The appearance of post-operative cognitive dysfunction as a result of open heart surgery has been proven by several studies. Focal and/or sporadic neuron damage emerging in the central nervous system may not only appear as cognitive dysfunction, but might strongly influence features of physiological tremor. MATERIAL AND METHODS: We investigated 110 patients (age: 34-73 years; 76 male, 34 female; 51 coronary artery bypass grafting (CABG), 25 valve replacement, 25 combined open heart surgery, 9 off-pump CABG) before surgery and after open-heart surgery on the 3(rd) to 5(th) post-operative day. The assessment of the physiological tremor analysis was performed with our newly developed equipment based on the Analog Devices ADXL 320 JPC integrated accelerometer chip. Recordings were stored on a PC and spectral analysis was performed by fast Fourier transformation (FFT). We compared power integrals in the 1-4 Hz, 4-8 Hz and 8-12 Hz frequency ranges and these were statistically assessed by the Wilcoxon rank correlation test. RESULTS: We found significant changes in the power spectrum of physiological tremor. The spectrum in the 8-12 Hz range (neuronal oscillation) decreased and a shift was recognised to the lower spectrum (p < 0.01). The magnitude of the shift was not significantly higher for females than for males (p < 0.157). We found no significant difference between the shift and the cross-clamp or perfusion time (p < 0.6450). CONCLUSIONS: The assessment of physiological tremor by means of our novel, feasible method may provide a deeper insight into the mechanism of central nervous system damage associated with open heart surgery

Bevezetés: A mitralis anulus (MA) morfológiája és funkciója
számos valvularis (például mitralis regurgitatióban) és nem valvularis
betegségben (például bizonyos cardiomyopathiákban, cardialis amyloidosisban)
eltéréseket mutathat. Célkitűzés: A jelen vizsgálat célja az MA
morfológiai és funkcionális jellemzői és a háromdimenziós speckle-tracking
echokardiográfiával (3DSTE) számított bal kamrai (BK) ejekciós frakció (EF)
összefüggéseinek vizsgálata volt normális és határérték-BK-EF-fel bíró
esetekben. Módszer: A jelen vizsgálatba 146 olyan önkéntes
eredményeit válogattuk be (átlagos életkor 32,0 ± 11,4 év; 74 férfi), akiknél
teljes körű kétdimenziós Doppler-echokardiográfiás vizsgálat történt negatív
eredménnyel, melyet 3DSTE-vel egészítettünk ki. A vizsgált populációt két
további alcsoportra bontottuk a 3DSTE-vel számított BK-EF-nek megfelelően
(határérték 50–54% versus ≥55%). Eredmények: A
határérték-BK-EF-fel bíró esetekben magasabb BK-i végszisztolés térfogatot és
alacsonyabb BK-i longitudinális straint lehetett mérni. A végszisztolés és
végdiasztolés MA-átmérő-, -area- és -kerület-értékek nagyobbnak bizonyultak a
határérték-BK-EF-fel bíró esetekben, ekkor az MA funkcionális paraméterek is
kisebbek voltak. A fenti összefüggések ellenére a BK-EF nem mutatott korrelációt
sem a végszisztolés és végdiasztolés MA-méretekkel, sem az MA funkcionális
paraméterekkel. Következtetések: A 3DSTE-vel meghatározott
határérték-BK-EF együtt jár az MA tágulásával és funkciójának romlásával. Orv

Abstract:
Introduction: Morphology and function of the mitral annulus (MA)
shows alterations in different valvular (for instance in mitral regurgitation)
and non-valvular disorders (for instance in certain cardiomyopathies, cardiac
amyloidosis). Aim: The aim of the present study was to examine
the relationship between MA morphologic and functional properties and
three-dimensional speckle-tracking echocardiography- (3DSTE) derived left
ventricular (LV) ejection fraction (EF) in subjects with normal
versus borderline LV-EF. Method: The
present study comprised 146 volunteers (mean age: 32.0 ± 11.4 years; 74 males)
in whom complete two-dimensional Doppler echocardiography was performed with a
negative result extended with 3DSTE. The population was further divided into two
groups according to their 3DSTE-derived LV-EF (borderline 50–54%
versus ≥55%). Results: In cases with
borderline LV-EF, higher LV end-systolic LV volumes and lower LV longitudinal
strain could be measured. All end-systolic and end-diastolic MA diameter, area
and perimeter data proved to be higher in cases with borderline LV-EF. In these
subjects, MA functional parameters proved to be lower as well. In contrast,
LV-EF showed correlations neither with end-systolic and end-diastolic MA
dimensions nor with MA functional parameters. Conclusions:
3DSTE-derived borderline LV-EF is associated with MA dilatation and functional

Early post-operative thrombosis of the prosthetic mitral valve in patient with heparin-induced thrombocytopenia
Heparin-induced thrombocytopenia (HIT) is one of the most common immune-mediated adverse drug reactions, with frequencies as high as 2-3% for certain groups of post-cardiac surgery patients. We report on an 50-year-old woman with early post-operative thrombosis of the prosthetic mitral valve due to heparin-induced thrombocytopenia. Non-invasive imaging (two-dimensional transesophageal echocardiography; 2D-TEE) allowed the exact localisation of thrombotic masses and revealed the increase of the mean diastolic mitral gradient. The HIT diagnosis was proved by the clinical scoring system, and with the identification of heparin platelet factor 4-induced antibodies. After the withdrawal of LMWH therapy and the start of intravenous lepirudin treatment, the patient's medical condition improved continuously. Follow-up echocardiography showed a step-wise decrease in the severity of the mean diastolic mitral valve gradient and a complete resolution of thrombus formations. Perhaps we may remind ourselves that, whilst HIT is one of the most common immune-mediated adverse drug reactions for certain groups of post-cardiac surgery patients, it can be managed successfully. We would also stress the importance of serial 2D-TEE examinations in the early post-operative period
Left atrial dysfunction in light-chain cardiac amyloidosis and hypertrophic cardiomyopathy - a comparative three-dimensional speckle-tracking echocardiographic analysis from the MAGYAR-Path Study
INTRODUCTION: While cardiac amyloidosis (CA) is a rare systemic disease characterized by extracellular deposition of protein-derived fibrils, hypertrophic cardiomyopathy (HCM) is histopathologically characterized by myocyte hypertrophy and disarray, interstitial fibrosis, and small intramural coronary arteriole dysplasia. The aim of the present study was to compare left atrial (LA) volumetric and functional characteristics between light-chain (AL) CA and HCM by three-dimensional (3D) speckle-tracking echocardiography (STE). METHODS: The AL-CA group initially consisted of 17 patients with AL-CA, but one patient was excluded due to inadequate image quality, and so the study population consisted of 16 patients (mean age: 64.0+/-9.6 years, five men). Their results were compared with data on 20 age-matched HCM patients (mean age: 59.8+/-5.2 years, 10 men) and on 16 age-matched healthy controls (mean age: 58.2+/-7.2 years, six men). Complete two-dimensional Doppler echocardiography and 3D-STE were performed in all cases. RESULTS: Significantly increased LA volumes were observed in both AL-CA and HCM compared with the control group. Only active atrial emptying fraction was found to be significantly reduced in AL-CA patients compared to controls. Peak global and mean segmental circumferential, longitudinal and area strains showed significant reductions in AL-CA patients compared with controls, but only peak mean segmental longitudinal strain differed significantly between HCM patients and controls. While no differences were demonstrated in global and mean segmental strain at atrial contraction between HCM patients and controls, AL-CA patients showed reductions in certain strain parameters compared to controls and HCM patients. CONCLUSIONS: Different patterns of LA functional characteristics were demonstrated in AL-CA and HCM patients by 3D-STE
