    Realities of participatory community-based environmental rehabilitation projects: a case study of soil erosion countermeasures in western Kenya

    This study attempts to describe the realities of participatory development projects in western Kenya, where severe soil degradation has occurred, using participatory observation methods. Six aid organizations administered development projects for environmental rehabilitation in the area; however, less than 10% of study area residents participated in project activities. This article argues that this lack of participation was fuelled by a disjuncture of understanding between aid organizations, community-based development organizations, and area residents about project purposes and goals. It suggests that in order for participatory development projects to be successful all stakeholders must understand and take ownership of the project. People who the aid organization called “community” often turned out to be a collection of unconnected people. In addition, not everyone who was involved participated out of a desire to stop soil erosion. Many people were not directly impacted by the gullies and their motivation for participation was purely for economic reasons. Thus, growing seedlings and planting them was seen more as a source of income than as a way to repair and restore the local environment.     Initial Surgical Versus Conservative Strategies in Patients With Asymptomatic Severe Aortic Stenosis

    AbstractBackgroundCurrent guidelines generally recommend watchful waiting until symptoms emerge for aortic valve replacement (AVR) in asymptomatic patients with severe aortic stenosis (AS).ObjectivesThe study sought to compare the long-term outcomes of initial AVR versus conservative strategies following the diagnosis of asymptomatic severe AS.MethodsWe used data from a large multicenter registry enrolling 3,815 consecutive patients with severe AS (peak aortic jet velocity >4.0 m/s, or mean aortic pressure gradient >40 mm Hg, or aortic valve area <1.0 cm2) between January 2003 and December 2011. Among 1,808 asymptomatic patients, the initial AVR and conservative strategies were chosen in 291 patients, and 1,517 patients, respectively. Median follow-up was 1,361 days with 90% follow-up rate at 2 years. The propensity score–matched cohort of 582 patients (n = 291 in each group) was developed as the main analysis set for the current report.ResultsBaseline characteristics of the propensity score–matched cohort were largely comparable, except for the slightly younger age and the greater AS severity in the initial AVR group. In the conservative group, AVR was performed in 41% of patients during follow-up. The cumulative 5-year incidences of all-cause death and heart failure hospitalization were significantly lower in the initial AVR group than in the conservative group (15.4% vs. 26.4%, p = 0.009; 3.8% vs. 19.9%, p < 0.001, respectively).ConclusionsThe long-term outcome of asymptomatic patients with severe AS was dismal when managed conservatively in this real-world analysis and might be substantially improved by an initial AVR strategy.     Statistical Analysis of Stevens-Johnson Syndrome Caused by Mycoplasma pneumonia Infection in Japan

    Background: Stevens-Johnson syndrome (SJS) associated with Mycoplasma pneumoniae (M. pneumoniae) infection is mainly observed in children. In adults, drugs are a major cause of SJS, but some adult patients with SJS are infected with M. pneumoniae. We analyzed patients with SJS associated with M. pneumoniae infection to elucidate the differences between drug-induced SJS and M. pneumoniae-associated SJS and also to study differences between M. pneumoniae-associated SJS in children and adults. Methods: This is a retrospective review of Japanese patients who have been reported as M. pneumoniae-associated SJS in medical Journals published from 1981 to 2009, compared with data of Japanese patients with drug-induced SJS reported from 2000 to 2009. Results: Thirty-eight cases of M. pneumoniae-associated SJS and 78 cases of drug-induced SJS were analyzed in this study. Ocular lesions were observed more frequently in M. pneumoniae-associated SJS than in drug-induced SJS (p < 0.01), and adult patients showed a higher ratio of sequelae in their eyes than did patients under 20 years of age (p < 0.01). Sixty-six percent of adult patients with M. pneumoniae-associated SJS developed fever/respiratory symptoms and mucocutaneous lesions on the same day. In contrast, most of the patients under 20 years of age developed fever/respiratory symptoms before mucocutaneous involvement. This means that these adult patients were infected and immunized previously and developed allergic reactions to M. pneumoniae soon after the later infection. Conclusions: In order to prevent ocular sequelae in adult patients when M. pneumoniae infection is suspected, more intensive treatment may be needed in adult patients than in younger patients

    Agricultural production and food consumption of mountain farmers in Tanzania: a case study of Kiboguwa village in Uluguru Mountains

    Abstract Background The present study investigates the agricultural practices and food security of rural areas in Africa by observing the farming and food consumption patterns in Kiboguwa village, on the eastern slopes of the Uluguru Mountains, Tanzania. Results The participant observation method was employed to collect and analyze the data of four village divisions: Kiseneke, Changa, Ludewa, and Mungi. Empirical research was conducted on the agricultural and food consumption patterns of the villagers using firsthand information. The production of two main types of cereals, maize and rice, was insufficient owing to low productivity or low production. Many households sell bananas grown in home gardens to compensate for cereal food shortages. Therefore, as a commercial crop, bananas play a vital role in achieving food security for the villagers. Conclusion The present study demonstrates the importance of different dimensions of agriculture and staple food crop productivity. It also describes the specific situation of rural farming in Africa. The newly collected information could help policymakers enhance food security in the areas under investigation

    Radial Movement of Minerals in the Trunks of Standing Japanese Cedar (Cryptomeria Japonica D. Don) Trees in Summer by Tracer Analysis

    The radial movement of minerals in tree trunks is a widely accepted function of ray parenchyma cells, but there is little experimental evidence for this. We previously obtained experimental data showing that the parenchyma cells were the site of the radial mineral movement in Japanese cedar (Cryptomeria japonica D. Don) trunks in winter. Therefore, the aim of this study was to answer two remaining questions: do parenchyma cells move minerals via active transport or passive diffusion and how do seasonality and the injection duration affect the radial movement of minerals. To analyze this, we compared mineral movement in living standing Japanese cedar trees with heartwood in which the trunk had been left untreated or freeze&ndash;thawed with liquid nitrogen to kill the living cells. A solution of a stable isotope of cesium (Cs), as a tracer of mineral movement, was continuously injected into the outer sapwood of these normal and freeze&ndash;thaw-treated trees for an objective period, following which the trunk was freeze-fixed with liquid nitrogen. The Cs distribution in frozen samples was then analyzed by cryo-scanning electron microscopy/energy-dispersive X-ray spectroscopy. After 1 and 5 days of injection, the Cs detection area was almost the same among parenchyma cells and tracheid cell walls in the freeze&ndash;thaw-treated samples (without living cells) but was further toward the inner xylem in the parenchyma cells than the tracheids in the normal samples (with living cells), indicating that living parenchyma cells move Cs. Furthermore, after 5 days of injection, Cs in the tracheid cell walls was detected further toward the inner xylem in the normal samples than in the freeze&ndash;thaw-treated samples, indicating that Cs is exuded from the parenchyma cells into the tracheid cell walls. Together, these results suggest that the radial movement of minerals in standing Japanese cedar trees occurs through a combination of active transport by parenchyma cells and diffusion in the cell walls

    In Planta Analysis of the Radial Movement of Minerals from Inside to Outside in the Trunks of Standing Japanese Cedar (<i>Cryptomeria japonica</i> D. Don) Trees at the Cellular Level

    Although the radial movement of minerals in tree trunks is a widely accepted phenomenon, experimental evidence of their movement in standing trees and underlying mechanisms is very limited. Previously, we clarified that cesium (Cs) artificially injected into the outer part of the sapwood of standing Japanese cedar (Cryptomeria japonica D. Don) trunks moved to the inner part of the sapwood, including the intermediate wood, via active transport by xylem parenchyma cells and diffusion through cell walls and then moved into the heartwood by diffusion. To understand the mechanism underlying the radial movement of minerals in the standing tree trunk, it is necessary to clarify their movement in the opposite direction. Therefore, the present study aimed to determine the radial movement of minerals from inside to outside in the trunks of standing trees at the cellular level. For this, a long hole across the center part of the trunk, which reached the heartwood, intermediate wood, and sapwood, was made in standing Japanese cedar trunks, and a solution of stable isotope Cs was continuously injected into the hole for several days as a tracer. The injected part of the trunk was collected after being freeze-fixed with liquid nitrogen, and the frozen sample was subjected to analysis of Cs distribution at the cellular level using cryo-scanning electron microscopy/energy-dispersive X-ray spectroscopy. The Cs injected into the inner sapwood or intermediate wood rapidly moved toward the outer sapwood via xylem ray parenchyma cells together with diffusion through the cell walls. In contrast, the Cs injected into the heartwood barely moved to the sapwood, although it reached a part of the inner intermediate wood. These results suggest that minerals in xylem ray parenchyma cells in the sapwood are bidirectionally supplied to each other; however, the minerals accumulated in the heartwood may not be supplied to living cells

    Diversity of Cropping Patterns and Factors Affecting Homegarden Cultivation in Kiboguwa on the Eastern Slopes of the Uluguru Mountains in Tanzania

    This study investigated what kind of diversities of cropping patterns observed in home gardens distributed on the eastern slopes of the Uluguru Mountains in central Tanzania, and how the diversity come into occurred. The major focus included the differences in ecological environment due to elevation, the impacts of the Ujamaa policy, and the characteristics of household members. Participatory observation with a one year stay in the study village was conducted to collect comprehensive information and to detect specific factors about formation of diversity cropping patterns of homegardens. The features of cropping patterns of the homegardens were assessed in an area distributed at altitudes of 650&ndash;1200 m. Many of the tree crops in this village originated from outside regions around the period of Tanzanian independence, and their cultivation spread throughout the village after the implementation of the Ujamaa policy. At present, village districts with many distributed homegardens with numerous tree crops are those that were confiscated from clans by the village government at the time of the Ujamaa policy and then redistributed to individuals. Cultivation of trees crops was very few at altitude of 900 m or more, because of cultivation characteristics of tree crops in this village were suitable for low altitude. In addition, since homegardens are considered to be abandoned for one generation only, their cropping patterns tended to easily reflect the ages and preferences of the members of the households living on them. The cropping patterns of the homegardens differed remarkably even between neighboring households owing to the cumulative effects of these multiple factors.
