    jaw osteonecrosis risk in hip fractures

    Purpose : Antiresorptive agents, such as bisphosphonates, are useful for the prevention of the recurrence of hip fractures. However, their administration has a risk of antiresorptive agent-related osteonecrosis of the jaw (ARONJ), and risk factors include poor oral hygiene. It is difficult for an orthopedic surgeon to examine a patient’s oral condition thoroughly. This study evaluated the relationship between risk factors for ARONJ and intraoral findings in hip fracture patients. Materials and Methods : We evaluated 79 patients (average age of 82.2 years) with hip fracture surgery who underwent an oral assessment by dentists. The risk assessments of the intraoral findings were classified into four levels (levels 0-3), with levels 2 and 3 requiring dental treatment intervention. Data that could be extracted as risk factors of ARONJ were also examined. Results : Level 1 was found most frequently (54.4%), followed by level 0 (35.4%), level 2 (8.9%), level 3 (1.3%). The area under the receiver operating characteristic curve for the number of risk factors for the two groups (dental treatment intervention required and unnecessary) and oral findings were 0.732. When the cut-off value was set to two risk factors, the specificity and sensitivity was 53.5% and 87.5%. Conclusions : For hip fracture patients with a more than 2 risk factors, dental visits are recommended to prevent ARONJ. This is a useful evaluation method that can be used to screen for ONJ from data obtained from other risk factors, even if it is difficult to evaluate the oral condition in hospitals where dentists are absent

    Process of parenting a child with RB

    Background : Retinoblastoma(RB) occurs at a very young age. Since the disease is diagnosed at an early age, the family is responsible for the care of the childʼs disease acceptance. Objective : This study aims to explore the parenting process of children with RB toward disease acceptance. Methods : Parents of eleven children with RB living in Japan were interviewed, and the data were analyzed using the Modified Grounded Theory Approach of Kinoshita(M-GTA). Results : There were twenty-one concepts representing the process of parenting a child with RB while guiding him or her toward disease acceptance, and nineteen of them were classified into ten categories based on semantic similarities. The two other concepts showed similar interpretability to categories. These categories and concepts were summarized into two core categories : “Helping the child develop a positive mindset to define the disease as a part of him/herself ” and “Paving the way in advance for the child to live comfortably when his or her living space expands”. Conclusions : In a cyclical framework of parenting, consisting of two core categories described in Results, the parents coordinated these two approaches while maintaining balance by “Avoiding saying anything that does not need to be said” and established their process of parenting a child with RB while guiding him or her toward disease acceptance, according to their household situation. The results suggest the necessity of recognizing that in childhood-onset cancers, such as RB, and diseases involving genetic issues, problems tend to occur not only during the treatment period but also at the time of life events and providing support from a comprehensive perspective

    A reporter system evaluates tumorigenesis, metastasis, β-catenin/MMP regulation, and druggability

    Cancer invasion, metastasis, and therapy resistance are the crucial phenomena in cancer malignancy. The high-expression of matrix metalloproteinase 9 (MMP9) is a biomarker as well as a causal factor of cancer invasiveness and metastatic activity. However, a regulatory mechanism underlying MMP9 expression in cancer is not clarified yet. Additionally, a new strategy for anti-cancer drug discovery is becoming an important clue. In the present study, we aimed (i) to develop a novel reporter system evaluating tumorigenesis, invasiveness, metastasis, and druggability with a combination of three-dimensional (3D) tumoroid model and Mmp9 promoter and (ii) to examine pharmacological actions of anti-cancer medications using this reporter system. High expression and genetic amplification of MMP9 were found in colon cancer cases. We found that proximal promoter sequences of MMP9 in murine and human contained conserved binding sites for transcription factors β-catenin/TCF/LEF, glucocorticoid receptor (GR), and NF-κB. The murine Mmp9 promoter (-569 to +19) was markedly activated in metastatic colon cancer cells and additionally activated by tumoroid formation and by β-catenin signaling stimulator lithium chloride (LiCl). The Mmp9 promoter-driven fluorescent reporter cells enabled the monitoring of activities of MMP9/gelatinase, tumorigenesis, invasion, and metastasis in allogeneic/syngenic transplantation experiments. We also demonstrated pharmacological actions as follows. ids Dexamethasone and hydrocortisone, steroidal medications binding to GR, inhibited the Mmp9 promoter but did not inhibit tumorigenesis. On the other hand, an antimetabolite 5-fluorouracil, a golden standard for colon cancer chemotherapy, inhibited tumoroid formation but did not inhibit Mmp9 promoter activity. Notably, anti-malaria medication artesunate inhibited both tumorigenesis and the Mmp9 promoter in vitro, potentially through inhibition of β-catenin/TCF/LEF signaling.     大学院新入生を対象としたサポートグループによる支援の試み

    大学院生は在学年数が短いため,入学初期に早く環境に適応することが重要となる。近年,大学院新入生に「他大学出身者」,「社会人」,「留学生」の割合が増え,大学の環境に適応するには課題が多い。本学では,ピアサポートルームの活動の一環として,大学院生の早期適応を支援するためのサポートグループ活動を実施した。本研究では,その実践活動の内容と効果について検討した。その結果,サポートグループに参加した大学院新入生は,緊張-不安,疲労,混乱,抑うつ-落ち込みなどのネガティブな感情(日本語版POMS 短縮版)が軽減された。また,他大学出身の新入生において,抑うつ-落ち込みの程度が低減するとともに,大学への所属感が高まったことが示唆された。Graduate school programs are short term, so it is important for first-year graduate students to adapt to the environment as soon as they begin their first semester. As part of peer support activities, we conducted support group activities to assist in early adaptation of new graduate students. In this paper, we examined the effect of the support group. We found that the first-year graduate students who joined the support group had reduced negative emotions (POMS shortened version) such as anxiety, fatigue, confusion, and depression. In addition, depression in students transferring from other universities was reduced and the sense of belonging to the university had increased

    The purpose of this study was to clarify the current status of oncology certified nurse specialist(OCNS) involvement in cancer genome medicine. A survey of 235 OCNSs who consented to participate in the study revealed that those involved in cancer genome medicine had a better understanding of hereditary tumors. Through descriptive content analysis, 8 learning needs were identified in relation to cancer genome medicine. Among these needs,[ basic knowledge of hereditary tumors and cancer genome medicine], [regular acquisition of up-to-date information on genetic medicine], and[ genetic counseling in actual clinical settings]revealed no differences between OCNSs involved and not involved in genetic medicine. On the other hand,[ role of an OCNS in genetic medicine], [nursing care for patients and families with hereditary diseases], [systems and collaboration to incorporate genetic medicine into organizations], [information on genetic medicine for facilities other than urban/designated hospitals], and[ programmed learning as a systematic approach to genetic medicine provided by academic societies] differed between the groups in terms of descriptive contents

    RSV replication is attenuated by counteracting expression of the suppressor of cytokine signaling (SOCS) molecules

    AbstractHuman RSV causes an annual epidemic of respiratory tract illness in infants and in elderly. Mechanisms by which RSV antagonizes IFN-mediated antiviral responses include inhibition of type I IFN mRNA transcription and blocking signal transduction of JAK/STAT family members. The suppressor of cytokines signaling (SOCS) gene family utilizes a feedback loop to inhibit cytokine responses and block the activation of the JAK/STAT signaling pathway. To evaluate the potential of SOCS molecules to subvert the innate immune response to RSV infection, eight SOCS family genes were examined. RSV infection up-regulated SOCS1, SOCS3, and CIS mRNA expression in HEp-2 cells. Suppression of SOCS1, SOCS3 and CIS by short interfering ribonucleic acid (siRNA) inhibited viral replication. Furthermore, inhibition of SOCS1, SOCS3, or CIS activated type I IFN signaling by inducing STAT1/2 phosphorylation. These results suggest that RSV infection escapes the innate antiviral response by inducing SOCS1, SOCS3 or CIS expression in epithelial cells
