The purpose of study was to determine muscle activity levels while walking with baggage. Subjects were 5 male university students in physical education. Electromyograms (EMG) recordings obtained from rectus abdominis and erector spinae during 1-minute walk with baggage were analyzed for muscle activity. The results of this study were as follows. (1) Muscle activity of rectus abdominis was found to be high nearer to the center of gravity. (9.8-19.7%) (2) Muscle activity of erector spinae was comparatively high throughout the duration. (48.4-57.3%) (3) These results indicate that it is more desirable for you to carry a handbag on the shoulder, and to wear a kit, shoulder or drum bag on the back and by crossing the straps across the chest

筋活動レベルから見たアイソメトリクス運動の特徴

The purpose of this study was to observe the muscle activity levels during isometric contraction. Subjects were 5 male university students of physical education. Electromyograms (EMG) during isotonic contraction and isometric contraction were obtained from rectus abdominis and vastus medialis. The results of this study were as follows. (1) Muscle activity levels of rectus abdominis were found to be higher than those of other muscles. (86.0%) (2) Muscle activity of vastus medialis was lower than that of other muscles. (35.1%) (3) These results indicate that for rectus abdominis the muscle activity level during isometric contraction may be enough to obtain the desired training effect

The purpose of this study was to analyze walking in middle age, old age and youth and explain characteristics of each throug a comparison by age. The subjects were 107 middle-age and elderly people (47 male and 60 female) and 17 university students (9 male and 8 female) in the physical education couese. Experiment was to film walking from side with digital video camera. We then measured pace and pitch, and obtained the following results. (1) Pitch was fastest in those in their twenties, and slowed more and more with age. Females were faster than males. Especially, in their seventies were faster than males in their sixties. 中高齢者の皮下脂肪厚と筋厚の特徴

The purpose of this study was to observe the skin fat thickness and muscle thickness in middle-age and elderly people. The subjects were 109 people (47 male and 62 female). Skin fat thickness and muscle thickness were measured using an ultrasound diagnostic device. The measurement points were the biceps brachii, triceps brachii, rectus abdminis, erector spinae, vastus medials and biceps femoris. Measurement measure results were competed with standard values and daily exercise program. The results are as follows. 1) Skin fat thickness was barely over the standard value in every generation. Comparative girth was high. 2) Muscle thickness was barely below the standard value in every generation. The vastus mediali and biceps femoris were below the standard value. 3) Most subject (?) did aerobic exercise. Therefore, skin fat thickness was below the standard value at most points. But muscle thickness was over the standard value at a few points. Compared with aerobics, time spent doing, muscle exercise was short

Trastuzumab/Capecitabine/Cisplatin HXP リョウホウ ニヨル 2ジ チリョウ ガ ユウコウ デアッタ シンコウ イガン ノ 1レイ

The case was a ６９-year-old man, and gastric cancer （papillary adenocarcinoma） was diagnosed by a close inspection of the abdominal pain in ２００８, and it was our department introduction. Show the type ０‐IIa＋IIc like advanced lesion of gastric middle body gastrointestinal endoscope, and CT showed no regional lymph node and distant metastases, and we diagnosed it as c-T２ （MP）, N０, M０, stageIB. We performed laparoscopic distal gastrectomy with D２ dissection, and in pathology, acknowledgment of severe lymph node metastases was obtained, and diagnosed it as T３ （SE）, N２, M０, stageIIIB. We gave adjuvant chemotherapy with S-１and Cisplatin for ５ cycles, and maintenance S-１alone up to ２４ months after surgery. There was no recurrence for ３９ months after surgery. But multiple metastases to lung recurrence was acknowledged and we performed S-１/Docetaxel therapy for ６ cycles, which lead to progressive disease. Then we performed Trastuzumab/Capecitabine/Cisplatin chemotherapy （HXP）, the anti-tumor effect was good, and after １４ months later, showed a liver metastasis, but the tolerability was good. Generally, it is said that the chemotherapy is poor at an effect after the second line treatment, but HXP may be useful even in the second line chemotherapy

Omentin Prevents Myocardial Ischemic Injury Through AMP-Activated Protein Kinase- and Akt-Dependent Mechanisms

ObjectivesThis study examined the impact of omentin on myocardial injury in a mouse model of ischemia/reperfusion (I/R) and explored its underlying mechanisms.BackgroundObesity is a major risk factor for ischemic heart disease. Omentin is a circulating adipokine that is down-regulated by obesity.MethodsIn patients who underwent successful reperfusion treatment after acute myocardial infarction, cardiac function and perfusion defect were assessed by using scintigraphic images. Mice were subjected to myocardial ischemia followed by reperfusion.ResultsThis study found that high levels of plasma omentin were associated with improvement of heart damage and function after reperfusion therapy in patients with acute myocardial infarction. Systemic administration of human omentin to mice led to a reduction in myocardial infarct size and apoptosis after I/R, which was accompanied by enhanced phosphorylation of AMP-activated protein kinase (AMPK) and Akt in the ischemic heart. Fat-specific overexpression of human omentin also resulted in reduction of infarct size after I/R. Blockade of AMPK or Akt activity reversed omentin-induced inhibition of myocardial ischemic damage and apoptosis in mice. In cultured cardiomyocytes, omentin suppressed hypoxia/reoxygenation-induced apoptosis, which was blocked by inactivation of AMPK or Akt.ConclusionsOur data indicate that omentin functions as an adipokine that ameliorates acute ischemic injury in the heart by suppressing myocyte apoptosis through both AMPK- and Akt-dependent mechanisms

Short-Term Outcomes of Laparoscopic Distal Gastrectomy for Advanced Gastric Cancer

The purpose of this study was to investigate the oncologic outcomes of laparoscopic distal gastrectomy (LDG) for advanced gastric cancer (AGC). Between April 2003 and March 2014, LDG was performed for 392 patients, 91 patients (23.2%) had histopathologically diagnosed AGC beyond T2 depth. The clinicopathological features, postoperative outcomes, mortality, morbidity, recurrence rate, and survivals of those patients were reviewed. The TNM stages of the tumor were IB in 26 patients (28.5%), IIA in 20 (21.9%), IIB in 18 (19.7%), IIIA in 13 (14.2%), IIIB in 6 (6.5%), IIIC in 6 (6.5%), and IV in 2 (2.1%). Major morbidity occurred in 14 patients (15.3%), with no postoperative mortality. Median follow-up was 24.5 months ; 10 patients developed recurrence during the follow-up period, and 10 patients died, including 6 cancer deaths. The 5-year overall and disease-free survival rates were 76.8% and 72.6%, respectively. By stage, OS/DFS was 92.3%/91.8% in stage IB, 85.4%/85.4% in stage II, and 49.3%/26.9% in stage III. Oncologic outcomes were good in patients with AGC, especially with stage IB-IIB, who underwent LDG. LDG appears to be an effective approach for treating stage IB and II gastric cancer

The patient was a４５-year-old man. He had suffered from nephrotic syndrome at time of his twenties and had steroid salvage treatment. But he retired the treatment by himself. Esophageal tumor was suspected at the screening, and he was referred to our hospital. Preoperative diagnosis was the adenocarcinoma of the esophagogastric junction（cT２N０M０ stage Ⅱ）． Thoracoscopy assisted subtotal esophagectomy in prone position with D２dissection was performed. Gastric role was prepared in laparoscopic approach, and pulled up to the neck via posterior mediastinal route. Although early postoperative course was uneventful and esophageal fluoroscopy on the７th day showed no leakage, sudden dyspnea appeared on the８th day. CT examination and Bronchoscopy showed tracheoesophageal fistula. Unfortunately, the fistula didn’t get well, and we considered that it was difficult to close the fistula by only conservative treatment. Esophageal covered stent was inserted on the５６th day. After that, he could start ingestion intake and was discharged from hospital on the８５th day. Now, he is being followed up in our hospital

A５２-year-old man visited our hospital because of epigastralgia. The colonoscopic examination revealed an about ４cm-protruded lesion like SMT on the appendix and findings of the biopsy specimen were compatible with the disgnosis of signet ring cell carcinoma. The primary lesion was unknown by upper gastrointestinal endoscopy, CT and PET, and the tumor markers were normal revel. At laparotomy, severe peritoneal metastasis was revealed in the abdominal cavity, especially appendix. Severe stenosis of ileocecum was found, so we conducted ileocecal resection. The histopathological diagnosis was primary signet ring cell caicinoma of appendix, SE, N２, M０, P３, pStage Ⅳ. Postoperatively mFOLFOX was started, but allergic reaction was seen after１cycle. We started Panitumumab/CPT-１１and the patient attended our emergency department with shivering chill and fever on treatment day１０. The next day he became shock state and CT revealed free air. Operation might not save his life and we started supportive care. He died on the day. The cause of his death was peritonitis by cancer perforation
