Optimization of the extent of surgical treatment in patients with stage I in cervical cancer
The study included 26 patients with FIGO stage Ia1–Ib1 cervical cancer who underwent fertility-sparing surgery (transabdominaltrachelectomy). To visualize sentinel lymph nodes, lymphoscintigraphy with injection of 99mTc-labelled nanocolloid was performed the day before surgery. Intraoperative identification of sentinel lymph nodes using hand-held gamma probe was carried out to determine the radioactive counts over the draining lymph node basin. The sentinel lymph node detection in cervical cancer patients contributes to the accurate clinical assessment of the pelvic lymph node status, precise staging of the disease and tailoring of surgical treatment to individual patient

РЕФЛЕКТОМЕТРИЧЕСКОЕ И ВИЗУАЛЬНО-ТЕСТОВОЕ ОПРЕДЕЛЕНИЕ PD(II) В ОБЪЕКТАХ СЛОЖНОГО СОСТАВА С ИСПОЛЬЗОВАНИЕМ ИНДИКАТОРНОЙ БУМАГИ
RIP-Pd-Test reagent indicator paper based on 1-phenyl-5-(3,4-dihydro-6-methyl-4-oxo-3H-pyrimidin-2-yl)-formazan-6-cellulose has been proposed for determining the palladium content using the sorption-reflectometry and visual-test methods in objects of complex composition. The RIP synthesis technique was described. The estimated composition of the metal complex formed as a result of the heterogeneous indicator reaction at pH 1.5–2.5 was indicated, and the forced coordination of palladium : reagent = 1:1 was ensured. When the RIP interacted with palladium (II) ions, the yellow indicator strip became green with different intensities. Under the selected conditions, the high selectivity for the detection and determination of Pd(II) was shown. With the help of the «Ecotest-2040» portable reflectometer, the reflectometry measurements were carried out using the red LED with the radiation wavelength of 660 nm. For the visual testing, the color scales were used. The indicator reaction was proposed to be carried out in three modes: without concentration and with concentration of the test solution in 100 and 1000 times on the reaction indicator zone of the paper strip (using the pocket pumping device). By the statistical method, the limit of Pd(II) determination for the visual test method (40, 0.47, 0.042 mg/l) was estimated. The palladium detection limit using the reflectometer (19, 0.22, 0.018 mg/l) for the three modes, respectively, was calculated by 3s criterion. The found values of the limiting concentrations were comparable with the metrological characteristics of the known analysis methods in which solid-phase reagents were used. The trueness of the palladium determinations in technological objects of complex composition and drugs has been proven by the independent methods - atomic emission, spectrophotometric and gravimetric methods of analysis. Изложена методика синтеза РИБ, указан предполагаемый состав металлокомплекса, образующегося в результате гетерогенной индикаторной реакции при рН = 1.5-2.5, с принудительной координацией палладий : реагент = 1 : 1. При взаимодействии РИБ с ионами палладия(II) желтая индикаторная полоса становилась зеленого цвета разной интенсивности. В выбранных условиях показана высокая селективность обнаружения и определения Pd(II). Рефлектометрические измерения проводили с помощью портативного рефлектометра «Экотест-2040», используя красный светодиод с длиной волны излучения 660 нм; для визуального тестирования применяли цветовые шкалы. Индикаторную реакцию предложено проводить в трех режимах: без концентрирования и с концентрированием исследуемого раствора в 100 и 1000 раз на реакционной индикаторной зоне бумажной полосы (применяя карманное прокачивающее устройство). Статистическим методом оценены значения предела определения Pd(II) для визуально-тестовой методики (40, 0.47, 0.042 мг/л) и по 3s-критерию вычислены значения предела обнаружения палладия с использованием рефлектометра (19, 0.22, 0.018 мг/л) для трех режимов, соответственно. Найденные значения предельных концентраций сопоставимы с метрологическими характеристиками известных методик анализа, в которых применяли твердофазные реагенты. Правильность определений палладия в технологических объектах сложного состава и лекарственных препаратах доказана независимыми методами – атомно-эмиссионным, спектрофотометрическим и гравиметрическим методами анализа. Features of detection of latent tuberculosis infection and tuberculosis in children from household contacts within a household (clinical case)
Aim: to demonstrate the features of detection and diagnosis of latent tuberculosis infection and pulmonary tuberculosis in children from household contacts based on own clinical observation.
Materials and methods. A clinical case of own observation of pulmonary tuberculosis and latent tuberculosis infection in children from household contacts who were treated in the paediatric department of the clinical base of the Phthysiatry and Pulmonology Department of Zaporizhzhia State Medical and Pharmaceutical University on Public Non-Profit Enterprise of the “Zaporizhzhia Regional Clinical and Diagnostic Center of Phthysiatry and Pulmonology” of Zaporizhzhia Regional Council.
Results. Children from household contacts were examined at ages: 8 months, 2, 3, 5 and 7 years, after their mother was diagnosed with tuberculosis (TB). All children showed an increase in tuberculin skin test, at the X-ray of the thoracic cavity organs (TCO) no changes were detected in any child. Taking into account the age of the children and their constant residence with the index patient, a decision was made to conduct an additional examination using CT scan of the TCO. Thus, three children were diagnosed with pulmonary tuberculosis – primary tuberculosis complex, and two were infected. After receiving the results of the culture test in the gastric lavage, the youngest child was found MBT, which was resistant to first-line drugs, namely isoniazid and rifampicin, which required a treatment regimen adjustment. It should be noted that the youngest girl, who was not vaccinated, had the most widespread process in the lungs compared to other children.
Conclusions. Monitoring and screening of children who have been in contact with the index patient within the household maximises the detection of additional cases of latent tuberculosis infection and pulmonary TB. The most at-risk groups are those who not vaccinated with BCG vaccine and children under 5 years of age. Early detection and timely prescription of anti-TB therapy is the key not only to effective treatment, but also to preventing the transmission of TB infection
The use of {99m}Tc-Al[2]O[3] for detection of sentinel lymph nodes in cervical cancer patients
The purpose of the study was to evaluate the feasibility of using {99m}Tc-Al[2]O[3]- based radiopharmaceutical, a novel molecular imaging agent for sentinel lymph node detection in patients with invasive cervical cancer. The study included 23 cervical cancer patients (TlaNxMx- T[2]bNxMx) treated at the Tomsk Cancer Research Institute. At 18 hours before surgery, 80 MBq of the {99m}Tc-Al[2]O[3] were injected peritumorally, followed by single-photon emission computed tomography (SPECT) of the pelvis and intraoperative SLN identification. Twenty-seven SLNs were detected by SPECT, and 34 SLNs were identified by intraoperative gamma probe. The total number of identified SLNs per patient ranged from 1 to 3(the mean number of SLNs was 1.4 per patient). The most common site for SLN detection was the external iliac region (57.2%), followed by the internal iliac, obturator, presacral and retrosacral regions (they amounted to 14%, respectively),and the parametrial region (1%). Sensitivity in detecting SLNs was 100% for intraoperative SLN identification and 79% for SPECT image

The paper studies the clinical and epidemiological features of one of the acute clinical forms of infection caused by the human herpes virus type 6 — «sudden exanthema » — in children of St. Petersburg. The study involved 149 children hospitalized aged 6 months to 15 years in the period from 2012 to 2014. Sudden rash confirmed by molecular genetic studies in 38 children. The disease is mainly observed in patients of the first two years of life. Rash and toxic syndromes dominated in clinical picture.Статья посвящена изучению клинических и эпидемиологических особенностей одной из острых клинических форм инфекции, вызванной вирусом герпеса человека 6 типа — экзантемы внезапной — у детей Санкт-Петербурга. Проведено обследование 149 детей, госпитализированных в возрасте от 6 месяцев до 15 лет в период с 2012 г. по 2014 г. Этиология внезапной экзантемы подтверждена молекулярно-генетическим исследованием у 38 детей. Заболевание преимущественно отмечалось у пациентов первых двух лет жизни. В клинической картине доминировали синдромы интоксикации и экзантемы
Clinical Significance of Sentinel Lymph Node Detection in Patients with Invasive Cervical Cancer
The clinical significance of determining sentinel lymph nodes (SLN) in patients with invasive cervical cancer was studied. From 2013 to 2014, 30 cervical cancer patients (T1a1NxM0-T1b1NxM0) were treated at the Gynecological Oncology Department of the Cancer Research Institute. The day before surgery, four submucosal injections of 99mTc Al2O3 at a total dose of 80 MBq were made in each quadrant around the cervical tumor. Patients were submitted to preoperative lymphoscintigraphy and intraoperative SLN detection. The feasibility of preserving the reproductive potential in patients after radical abdominal trachelectomy was assessed. The 3-year, overall, disease-free and metastasis-free survival rates were analyzed. Thirty-four SLNs were detected by single-photon emission computed tomography (SPECT) and 42 SLNs were identified by intraoperative gamma probe. The sensitivity in detecting SLNs was 100% for intraoperative SLN identification and 80% for SPECT image. The reproductive potential was preserved in 86% of patients. The 3-year overall and metastases-free survival rates were 100%. Recurrence occurred in 8.6% of cases
Full Multiple Scattering Analysis of XANES at the Cd L 3- and O K- Edges in CdO Films Combined with a Soft-X-Ray Emission Investigation
X-ray absorption near edge structure (XANES) at the cadmium L3 and oxygen K edges for CdO thin films grown by pulsed laser deposition method, is interpreted within the real-space multiple scattering formalism, FEFF code. The features in the experimental spectra are well reproduced by calculations for a cluster of about six and ten coordination shells around the absorber for L3 edge of Cd and K edge of O, respectively. The calculated projected electronic density of states is found to be in good agreement with unoccupied electronic states in experimental data and allows to conclude that the orbital character of the lowest energy of the conductive band is Cd 5s-O 2p[sigma]*. The charge transfer has been quantified and not purely ionic bonding has been found. Combined XANES and resonant inelastic x-ray scattering measurements allow us to determine the direct and indirect band gap of investigated CdO films to be ~2.4 eV and ~0.9 eV, respectively
