    The separation of the East Australian Current: A Lagrangian approach to potential vorticity and upstream control

    The East Australian Current (EAC) is the western boundary current flowing along the east coast of Australia separating from the coast at approximately 34°S. After the separation two main pathways can be distinguished, the eastward flowing Tasman Front and the extension of the EAC flowing southward. The area south of the separation latitude is eddy-rich and the separation latitude of the EAC is variable. Little is known of the properties of the water masses that separate at the bifurcation of the EAC. This paper presents new insights from the Lagrangian perspective, where the water masses that veer east and those that continue south are tracked in an eddy-permitting numerical model. The transport along the two pathways is computed, and a 1:3 ratio between transport in the EAC extension and transport in the Tasman Front is found. The results show that the "fate" of the particles is to first order already determined by the particle distribution within the EAC current upstream of the separation latitude, where 85% of the particles following the EAC extension originate from below 460 m and 90% of the particles following the Tasman Front originate from the top 460 m depth at 28°S. The separation and pathways are controlled by the structure of the isopycnals in this region. Analysis of anomalies in potential vorticity show that in the region where the two water masses overlap, the fate of the water depends on the presence of anticyclonic eddies that push isopycnals down and therefore enable particles to travel further south

    Productivity Levels in Distributive Trades:A New ICOP Dataset for OECD Countries

    This study provides a new dataset for international comparisons of labour productivity levels in distributive trade (retail and wholesale trade) between OECD countries. The productivity level comparisons are based on a harmonised set of Purchasing Power Parities (PPPs) for 1997 using the industry-of-origin approach as developed in the International Comparisons of Output and Productivity (ICOP) project. The methodology mimics current national accounts practice in measuring real output over time. The comparative estimates are extrapolated from the benchmark year using those national accounts series. The main finding of this study is that there is still a wide variety in labour productivity levels in the distribution sector across the OECD area. In 2002, the Germany, the Benelux and Scandinavian countries (except Sweden) were leading in terms of PPP-converted value added per hour worked with higher levels than in the U.S.. In Asia, the comparative labour productivity level is on average 39% of the U.S. level, whereas it is 48% on average in Eastern Europe. Within the “old” EU-15, countries like Italy, Portugal, Spain and the U.K. had relative levels less than 70% of the U.S.. There is no clear sign of convergence in productivity levels among OECD countries during the past two decades

    IT in the European Union:driving productivity divergence?

    This paper analyses the contributions of IT-capital deepening and total factor productivity growth (TFP) in IT-production on aggregate labour productivity growth patterns within the European Union in comparison with the US. We find that differences in the direct effects of IT almost fully explain the US lead in labour productivity growth over the EU aggregate over the period 1995-2001. However differences in the direct effects of IT are by no means the sole determinants of the widening of the "Atlantic Divide", neither the main cause of divergent labour productivity growth patterns within Europe. Non-IT capital deepening and non-IT TFP growth were major contributors to continued or even accelerating growth in small economies such as Austria, Finland, Greece, Ireland, Portugal and Sweden. In Finland, Sweden and especially Ireland this was augmented by high contributions from IT, which were even higher than in the US. At the same time, decelerating labour productivity growth in major European countries such as France, Germany, Italy and the UK was mainly due to declining contributions of non-IT capital deepening and non-IT TFP growth compared to the period 1980-1995

    SummaryBackgroundIn The Netherlands, efforts to control meticillin-resistant Staphylococcus aureus (MRSA) in hospitals have been largely successful due to stringent screening of patients on admission and isolation of those that fall into defined risk categories. However, Dutch hospitals are not free of MRSA, and a considerable number of cases are found that do not belong to any of the risk categories. Some of these may be due to undetected nosocomial transmission, whereas others may be introduced from unknown reservoirs.AimIdentifying multi-institutional clusters of MRSA isolates to estimate the contribution of potential unobserved reservoirs in The Netherlands.MethodsWe applied a clustering algorithm that combines time, place, and genetics to routine data available for all MRSA isolates submitted to the Dutch Staphylococcal Reference Laboratory between 2008 and 2011 in order to map the geo-temporal distribution of MRSA clonal lineages in The Netherlands.FindingsOf the 2966 isolates lacking obvious risk factors, 579 were part of geo-temporal clusters, whereas 2387 were classified as MRSA of unknown origin (MUOs). We also observed marked differences in the proportion of isolates that belonged to geo-temporal clusters between specific multi-locus variable number of tandem repeat analysis (MLVA) clonal complexes, indicating lineage-specific transmissibility. The majority of clustered isolates (74%) were present in multi-institutional clusters.ConclusionThe frequency of MRSA of unknown origin among patients lacking obvious risk factors is an indication of a largely undefined extra-institutional but genetically highly diverse reservoir. Efforts to understand the emergence and spread of high-risk clones require the pooling of routine epidemiological information and typing data into central databases

    Carfilzomib, Pomalidomide, and Dexamethasone As Second-line Therapy for Lenalidomide-refractory Multiple Myeloma

    This phase 2 trial investigated reinduction with carfilzomib, pomalidomide, and dexamethasone (KPd) and continuous pomalidomide/dexamethasone in patients at first progression during lenalidomide maintenance. The second objective was to evaluate high-dose melphalan with autologous stem cell transplantation (HDM/ASCT) at first progression. Patients were eligible who had progressive disease according to International Myeloma Working Group (IMWG) criteria. Treatment consisted of 8 cycles carfilzomib (20/36 mg/m2), pomalidomide (4 mg) and dexamethasone. Patients without prior transplant received HDM/ASCT. Pomalidomide 4 mg w/o dexamethasone was given until progression. One hundred twelve patients were registered of whom 86 (77%) completed 8 cycles of KPd. Thirty-five (85%) eligible patients received HDM/ASCT. The median time to discontinuation of pomalidomide w/o dexamethasone was 17 months. Best response was 37% ≥ complete response, 75% ≥ very good partial response, 92% ≥ partial response, respectively. At a follow-up of 40 months median PFS was 26 and 32 months for patients who received KPd plus HDM/ASCT and 17 months for patients on KPd (hazard ratio [HR] 0.61, 95% confidence interval [CI] 0.37-1.00, P = 0.051). PFS was better after longer duration of prior lenalidomide (HR 3.56, 95% CI 1.42-8.96, P = 0.035). Median overall survival (OS) was 67 months. KPd-emerging grade 3 and 4 adverse events included hematologic (41%), cardiovascular (6%), respiratory (3%), infections (17%), and neuropathy (2%). KPd followed by continuous pomalidomide is an effective and safe triple drug regimen in second-line for patients previously exposed to bortezomib and/or refractory to lenalidomide
