    Medical Curriculum to Motivate Students

    Despite advancements in the pedagogy of medical education in various fields, Japan has no standardized medical English education. The U.S. Medical Licensing Examination (USMLE) Study Group of Tokushima is an extracurricular activity in which medical students and recent graduates meet every 1–2 months. The aim is to stimulate students’ curiosity ; cultivate their initiative, self-efficacy, and English learning goals ; and motivate them to be self-regulated learners. Accordingly, we conducted near-peer teaching style lectures that focused on sharing medical English-related experiences, so students could have regular opportunities to visualize the benefits of learning medical English. Following the activities, we observed increased motivation and self-study among students, resulting in a high USMLE passing rate. Furthermore, five members started their training at American hospitals and pursued careers in English-speaking environments. Thus, near-peer teaching style leads to shared medical English-related experiences that help students to visualize English-related opportunities. This education style taught by similar generations aids in setting a specific goal by providing access to role models, cultivating their initiative and self-efficacy, motivating them to learn English, and producing positive outcomes. Modifying the curriculum to actively create opportunities for students to visualize themselves in an international environment can motivate them to continue learning English

    1006-44 A Prognostic Factor in Coronary Artery Disease (CAD): Platelet-Dependent Thrombin Generation in Patients with CAD

    We examined platelet-dependent thrombin generation in patients with coronary artery disease (CAD). Thrombin generation was measured according to the method of Aronson et al (Circulation, 1992). 0.5ml of platelet rich plasma (PRP, 15×104/ml) was prepared, and 40mM of CaCl, was added to start clotting. 0.5mM of S-2238 was added to each sample in a microtiter plate every 10min, and the plate was read kinetically at a wavelength of 405nm on a microtiter plate reader. AMI patients with severe coronary artery disease (Group B) showed higher levels of thrombin generation (Group A, Gensini score<32: 382±248 mOD vs Group B, Gensini score> 31: 578±238, P<0.05). LVEF of group A is significantly higher than that of group B (P < 0.05). These findings indicate that patients with UAP and AMI have an evidence of hypercoagulable states and that platelet-dependent thrombin generation may play an important role in pathophysiology of UAP or AMI, and may be a prognostic factor in CAD

    Psychological characteristics of Japanese patients with chronic pain assessed by the Rorschach test

    <p>Abstract</p> <p>Background</p> <p>The increasing number of patients with chronic pain in Japan has become a major issue in terms of the patient's quality of life, medical costs, and related social problems. Pain is a multi-dimensional experience with physiological, affective, cognitive, behavioral and social components, and recommended to be managed via a combination of bio-psycho-social aspects. However, a biomedical approach is still the dominant method of pain treatment in Japan. The current study aimed to evaluate comprehensive psychological functions and processes in Japanese chronic pain patients.</p> <p>Methods</p> <p>The Rorschach Comprehensive System was administered to 49 in-patients with non-malignant chronic pain. Major variables and frequencies from the test were then compared to normative data from non-patient Japanese adults by way of the t-test and chi-square test.</p> <p>Results</p> <p>Patients exhibited high levels of emotional distress with a sense of helplessness with regard to situational stress, confusion, and ambivalent feelings. These emotions were managed by the patients in an inappropriate manner. Cognitive functions resulted in moderate dysfunction in all stages. Information processing tended to focus upon minute features in an inflexible manner. Mediational dysfunction was likely to occur with unstable affective conditions. Ideation was marked by pessimistic and less effective thinking. Since patients exhibited negative self-perception, their interpersonal relationship skills tended to be ineffective. Originally, our patients displayed average psychological resources for control, stress tolerance, and social skills for interpersonal relationships. However, patient coping styles were either situation- or emotion-dependent, and patients were more likely to exhibit emotional instability influenced by external stimuli, resulting in increased vulnerability to pain.</p> <p>Conclusions</p> <p>Data gathered from the Rorschach test suggested psychological approaches to support chronic pain patients that are likely to be highly beneficial, and we thus recommend their incorporation into the course of current pain treatments.</p

    Absorption of Scintillation Light in a 100 \ell Liquid Xenonγ\gamma Ray Detector and Expected Detector Performance

    An 800L liquid xenon scintillation γ\gamma ray detector is being developed for the MEG experiment which will search for μ+e+γ\mu^+\to\mathrm{e}^+\gamma decay at the Paul Scherrer Institut. Absorption of scintillation light of xenon by impurities might possibly limit the performance of such a detector. We used a 100L prototype with an active volume of 372x372x496 mm3^3 to study the scintillation light absorption. We have developed a method to evaluate the light absorption, separately from elastic scattering of light, by measuring cosmic rays and α\alpha sources. By using a suitable purification technique, an absorption length longer than 100 cm has been achieved. The effects of the light absorption on the energy resolution are estimated by Monte Carlo simulation.Comment: 18 pages, 10 figures (eps). Submitted to Nucl. Instr. and Meth.

    Pairing competition in a quasi-one-dimensional model of organic superconductors (TMTSF)2X_{2}X in magnetic field

    We microscopically study the effect of the magnetic field (Zeeman splitting) on the superconducting state in a model for quasi-one-dimensional organic superconductors (TMTSF)2X_{2}X. We investigate the competition between spin singlet and spin triplet pairings and the Fulde-Ferrell-Larkin-Ovchinnikov(FFLO) state by random phase approximation. While we studied the competition by comparison with the eigenvalue of the gap equation at a fixed temperature in our previous study (Phys. Rev. Lett. \textbf{102} (2009) 016403), here we obtain both the TcT_c for each pairing state and a phase diagram in the TT(temperature)-hzh_z(field)-VyV_y(strength of the charge fluctuation) space. The phase diagram shows that consecutive transitions from singlet pairing to the FFLO state and further to Sz=1S_z=1 triplet pairing can occur upon increasing the magnetic field when 2kF2k_{F} charge fluctuations coexist with 2kF2k_{F} spin fluctuations. In the FFLO state, the singlet d-wave and Sz=0S_{z}=0 triplet ff-wave components are strongly mixed especially when the charge fluctuations are strong.Comment: 11 pages, 9 figure

    Periodontal disease adversely affects the survival of patients with end-stage renal disease

    Periodontal disease is associated with cardiovascular disease and is thought to accelerate systemic atherosclerosis. Here we examined the relationship between periodontitis and cardiovascular disease mortality in outpatients on hemodialysis using a retrospective analysis of 168 adult patients in New York City and North Carolina. During 18 months of follow-up, cardiovascular disease and all-cause mortality were determined from a centralized dialysis registry. One hundred patients had mild or no periodontal disease but the remaining 68 had moderate-to-severe disease defined as 2 or more teeth with at least 6 mm of inter-proximal attachment loss. At baseline, the proportion of males was significantly lower in the moderate-to-severe group. Compared with mild or no periodontal disease, moderate-to-severe disease was significantly associated with death from cardiovascular causes. Adjustment for age, gender, center and dialysis vintage, smoking status, and history of diabetes mellitus or hypertension did not diminish the strength of this association. Our findings suggest a need for larger studies to confirm this connection, along with intervention trials to determine if treating periodontitis reduces cardiovascular disease mortality in dialysis patients

    他言語要約 (日本語) 背景 日本において、外国人住民は日本人に比べて保健医療へのアクセスバリアがあり、健康のアウトカムもよくないことが知られている。しかし、外国人住民の保健医療に関する状況は大きく変化し続けており、「取り残された」人を特定することは困難である。そこで本研究では、NGOの無料健康相談を受けた外国人住民を対象に、「医療機関を受診するように勧められた」「医療機関を紹介された」ことを保健医療へのアクセスバリアを示唆する代理指標として、これらに関連する要因を明らかにすることを目的とした。. 方法 愛知県内の複数の会場で外国人住民を対象とした無料健康相談会を行っているNGOの活動記録を用いて二次データ分析を行った。保険加入を含む参加者特性を明らかにした上で、二変量分析(χ2検定) および多変量分析 (log-binomial回帰分析)を行い保健医療へのアクセスバリアと関連する要因を明らかにした。. 結果 抽出された608例のうち、相談時に医療機関への受診を勧められたのは164例(27.5%)、医療機関への紹介は72例(11.8%)であった。公的保険に加入していない人は、公的保険に加入している人に比べて、医療機関への受診を勧められる割合 (prevalence)が1.56倍(95%信頼区間 [CI]:1.19–2.05)高かった。仕事をしていない人および学生は、専門職者に比べて医療施設への紹介を有意に多く受けており、prevalence ratioはそれぞれ3.28(95%信頼区間 [CI]:1.64–6.57) および2.77(95%CI,1.18–6.46) であった。. 結論 大多数は保険に加入していたが、無料健康相談を利用する前に正式な医療制度へのアクセスが制限されていたことを示唆する、医療機関を受診するよう勧められた参加者が30%近くいた。本研究結果は、保険に加入していない人、仕事をしていない人、学生など、保健医療へのアクセスが脆弱だと考えられる人たちを示した。誰もが「取り残されない」ようにするために、エビデンスを更新しながら、脆弱な人々へ必要な支援を提供することが肝要である。

    Bone Tissue Regeneration by Collagen Scaffolds with Different Calcium Phosphate Coatings: Amorphous Calcium Phosphate and Low-Crystalline Apatite

    Surface-mineralized collagen sponges have attracted much attention as scaffolds for bone tissue engineering. Recently, we developed amorphous calcium phosphate (ACP) and low-crystalline apatite coating processes on collagen sponges. In the present study, we applied these coating processes to granular collagen sponges (referred to as Col) to compare the bone tissue regeneration capabilities of ACP-coated and apatite-coated Col (referred to as Col-ACP and Col-Ap, respectively) using a rat cranial bone defect model. According to micro-CT and histological analyses, Col-Ap enhanced bone tissue regeneration compared to Col, whereas Col-ACP did not. These results not only demonstrated the superior bone tissue regeneration capability of Col-Ap, but also indicated limitations of the in vitro simulated body fluid (SBF) test used in our previous study. Despite the apatite-forming ability of Col-ACP in SBF, it was ineffective in improving bone tissue regeneration in vivo, unlike Col-Ap, most likely due to the quick resorption of the ACP coating in the defect site. The present results clarified the importance of the coating stability in vivo and revealed that the low-crystalline apatite coating was more beneficial than the ACP coating in the fabrication of surface-mineralized collagen sponges for use as bone tissue engineering scaffolds
