Two-year clinical evaluation of one-step self-etch systems in non-carious cervical lesions.
OBJECTIVES: This randomized controlled clinical trial evaluated the 2-year clinical performance of S(3) Bond (S3) and G-Bond (GB) in 108 non-carious cervical lesions. METHODS: Twenty-three patients, 12 male and 11 female (mean age: 61.8 years, range: 30-79 years) regularly visiting the Nagasaki University Hospital of Medicine and Dentistry, participated in the study. Each patient received both materials randomly. All restorations (53 restorations for S3 and 55 restorations for GB) were placed by one dentist. The restorations were blindly evaluated by two examiners at baseline, 6 months, 1 and 2 years using modified USPHS criteria. The data were statistically analyzed using the Cochran Q test and Fisher\u27s exact test. RESULTS: One restoration of each material was lost during 2 years. The only minor clinical problem was the integrity of the enamel margin. Slight marginal staining occurred adjacent to 11 restorations of both S3 and GB. There was no significant difference in the clinical performance between S3 and GB for each variable. CONCLUSIONS: Under the protocol used in this study, S3 and GB have demonstrated an acceptable clinical performance up to 2 years
What are the risk factors for postoperative infections of third molar extraction surgery : a retrospective clinical study-?
This study aimed to identify (1) the predilection site of postoperative infection after third molar extraction surgery, (2) risk factors associated with postoperative infection, and (3) the cause of the difference between delayed- and early-onset infections. This retrospective study included 1010 patients (396 male, 614 female) who had ?1 third molars extracted (2407; 812 maxilla, 1595 mandible). The risk factors were classified as attributes, general health, anatomic, and operative. Outcome variables were delayed- and early-onset infections. Postoperative infection was completely absent in the maxilla, and all infections occurred in the mandible, with a probability of 1.94% (31/1595). Bivariate analysis for postoperative infection showed depth of inclusion and intraoperative hemostatic treatment to be significantly associated with the development of infections. Bivariate analysis for delayed- and early-onset infections showed simultaneous extraction of the left and right mandibular third molars to be prominent risk factors. Postoperative infection occurs mainly in the mandible, and that in the maxilla is very rare. The risk of postoperative infection in the mandible was found to be related to the depth of inclusion and intraoperative hemostatic treatment. Simultaneous extraction of the left and right mandibular third molars appear to increase the risk of delayed-onset postoperative infection
Evolution of superconductivity in isovalent Te-substituted KxFe2-ySe2 crystals
We report the evolution of superconductivity and the phase diagram of the
KxFe2-ySe2-zTez (z=0-0.6) crystals grown by a simple one-step synthesis. No
structural transition is observed in any crystals, while lattice parameters
exhibit a systematic expansion with Te content. The Tc exhibits a gradual
decrease with increasing Te content from Tconset = 32.9 K at z = 0 to Tconset =
27.9 K at z = 0.5, followed by a sudden suppression of superconductivity at z =
0.6. Upon approaching a Te concentration of 0.6, the shielding volume fraction
decreases and eventually drops to zero. Simultaneously, hump positions in r-T
curve shift to lower temperatures. These results suggest that isovalent
substitution of Te for Se in KxFe2-ySe2 crystals suppresses the

Prediction of common hepatic artery catheter insertion based on celiac trunk morphology
PURPOSEThis study aimed to predict the ability to insert a 4–5 French (Fr) catheter insertion with a guidewire into the common hepatic artery (CHA) based on celiac trunk morphology.METHODSThis retrospective study included 64 patients who underwent balloon-occluded transcatheter arterial chemoembolization (n = 56), transcatheter arterial chemotherapy (n = 2), or were fitted with an implantable port system (n = 6) between June 2019 and December 2019 in our institution. The morphology of the celiac trunk was classified into three types (upward, horizontal, and downward) based on celiac angiography. The aortic–celiac trunk angle was measured on sagittal images of preprocedural contrast-enhanced computed tomography (CT). We reviewed whether a 4–5-Fr shepherd’s hook catheter could advance beyond the CHA using a 0.035-inch guidewire (Radifocus® Guidewire M; Terumo). Three patients were diagnosed with median arcuate ligament syndrome (MALS) based on the characteristic hook shape of the celiac artery on sagittal images of contrast-enhanced CT. The predictive ability of celiac angiography and preprocedural CT for CHA insertion success was evaluated. In unsuccessful cases, the balloon anchor technique (BAT) was attempted as follows: (1) a 2.7/2.8-Fr microballoon catheter (Attendant Delta; Terumo) was placed beyond the proper hepatic artery, and (2) the balloon was inflated as an anchor for parent catheter advancement.RESULTSUpward, horizontal, and downward celiac trunk types were noted in 42, 9, and 13 patients, respectively. The median CT angle was 122.83° (first quartile–third quartile, 102.88°–136.55°). Insertion in the CHA using the guidewire was successful in 56 of 64 patients (87.50%), and the success rate in the downward type was significantly lower than that in the upward type [42/42 (100%) vs. 7/13 (53.85%), P < 0.001]. The CT angle was significantly larger downward in the unsuccessful group than in the successful group (121.03° vs. 140.70°, P = 0.043). Celiac angiography had a significantly higher area under the curve (AUC) than preprocedural CT (AUC = 0.91 vs. AUC = 0.72, P = 0.040). All three cases of MALS showed unsuccessful CHA insertion. In all eight patients with unsuccessful insertion, the catheter could be advanced using the BAT [8/8 (100%)].CONCLUSIONCeliac angiography and preprocedural CT could predict CHA catheter insertion using a guidewire, and celiac angiography had high predictability. CT could detect MALS, a risk factor for unsuccessful CHA insertion
Spread of Intraductal Papillary Neoplasm Arising from the Cystic Duct of the Biliary Tree
Intraductal papillary neoplasm of the bile duct (IPNB) is a variant type of the bile duct carcinoma characterized by intraductal growth. IPNB is also recognized as a precursor of invasive carcinoma. We describe herein an extremely rare case of IPNB arising from the cystic duct. A 68-year-old man was admitted to our hospital for investigation of epigastralgia and abnormal levels of biliary tract enzyme. Computed tomography and magnetic resonance imaging showed a mass lesion spreading from the cystic duct to the upper-middle bile duct. Endoscopic retrograde cholangiography demonstrated diffuse duct dilation with a grossly visible intraductal mass and amorphous blobs, suggesting the presence of mucobilia or scattered tumors. We performed extrahepatic bile duct resection with lymphadenectomy. Macroscopically, a friable papillary tumor originated from the cystic duct grows intraluminally into the bile duct. Pathologically, the tumor was found to be intramucosal adenocarcinoma spreading to the whole extrahepatic bile duct, which was compatible with IPNB. We should discuss the features and progression processes of IPNB through this precious case
