    Closed-Eye Visualizations in the Setting of Hyponatremia.

    Purpose: To report a case of closed-eye visualizations and to clarify the different types of hallucinations and their etiologies. Methods: Retrospective case report of a patient with closed-eye visualizations secondary to hyponatremia. Clinical findings, physical exam, laboratory assessment, treatment, and disease course from the patient\u27s hospitalization were used in creating this report. Follow-up data after discharge were also obtained. Results: Closed-eye visualizations were diagnosed as secondary to hyponatremia, as they did not occur with the eyes open, and potential alternate causes were excluded. Serum sodium nadir was 119 mEq/L. Symptoms resolved with correction of hyponatremia via fluid resuscitation and electrolyte replenishment. There has been no recurrence of the symptoms. Conclusion: This patient had hallucinations exclusively with the eyes closed, which must be differentiated from the release hallucinations seen with the eyes open in Charles Bonnet syndrome. This patient had no visual loss or retinal disease, which should be suspected in open eye hallucinations

    The architecture of the United States life-saving stations

    The study of the architecture of the United States Life-Saving stations was undertaken to trace the development of a single building type: the coastal rescue stations which housed men and equipment engaged in bringing ashore victims of shipwrecks. Formed in 1848, the United States Life-Saving Service was the only government supported lifesaving.system in the world, all other organizations being either volunteer or private societies. The Service began its activities with the building of eight small, unmanned volunteer boathouses along the New Jersey coast during 1849, and the following year it was extended to the south shore of Long Island. After the Reorganization of 1871 established paid crews to live in the stations, the Service embarked on a building program which eventually led to the construction of stations on the Atlantic, Gulf, Great Lakes and Pacific coasts. In 1915 the United States Coast Guard was formed to take over the Service. Nearly all stations were built from standardized plans and specifications designed by architects working directly for the Lifesaving Service. Most reflected current architectural trends. This study was undertaken to determine how many different plans were drawn, how many stations of each type were constructed, where they were located and the architects responsible for their designs. Using architectural plans, photographs and secondary sources a history of the Service was written focusing on the design and construction of the stations. Lists were then compiled of each type built

    Cognitive Rehabilitation for Executive Dysfunction in Parkinson's Disease: Application and Current Directions

    Cognitive dysfunction in Parkinson's disease contributes to disability, caregiver strain, and diminished quality of life. Cognitive rehabilitation, a behavioral approach to improve cognitive skills, has potential as a treatment option to improve and maintain cognitive skills and increase quality of life for those with Parkinson's disease-related cognitive dysfunction. Four cognitive rehabilitation programs in individuals with PD are identified from the literature. Characteristics of the programs and outcomes are reviewed and critiqued. Current studies on cognitive rehabilitation in PD demonstrate feasibility and acceptability of a cognitive rehabilitation program for patients with PD, but are limited by their small sample size and data regarding generalization of effects over the long term. Because PD involves progressive heterogeneous physical, neurological, and affective difficulties, future cognitive rehabilitation programs should aim for flexibility and individualization, according to each patient's strengths and deficits

    The First High Redshift Quasar from Pan-STARRS

    We present the discovery of the first high redshift (z > 5.7) quasar from the Panoramic Survey Telescope and Rapid Response System 1 (Pan-STARRS1 or PS1). This quasar was initially detected as an i dropoutout in PS1, confirmed photometrically with the SAO Widefield InfraRed Camera (SWIRC) at Arizona's Multiple Mirror Telescope (MMT) and the Gamma-Ray Burst Optical/Near-Infrared Detector (GROND) at the MPG 2.2 m telescope in La Silla. The quasar was verified spectroscopically with the the MMT Spectrograph, Red Channel and the Cassegrain Twin Spectrograph (TWIN) at the Calar Alto 3.5 m telescope. It has a redshift of 5.73, an AB z magnitude of 19.4, a luminosity of 3.8 x 10^47 erg/s and a black hole mass of 6.9 x 10^9 solar masses. It is a Broad Absorption Line quasar with a prominent Ly-beta peak and a very blue continuum spectrum. This quasar is the first result from the PS1 high redshift quasar search that is projected to discover more than a hundred i dropout quasars, and could potentially find more than 10 z dropout (z > 6.8) quasars.Comment: 8 pages, 7 figure
