    DergiPark: 497928ejovocEndüstri 4.0,geleneksel sanayinin dijitalleşme yönünde teşvik edilmesi ve yüksekteknolojiyle donatılması projesidir. Bununla birlikte, çalışma ortamında herbir verinin toplanmasına ve efektif bir şekilde izlenip analiz edilmesineolanak sağladığı için daha verimli iş modellerini ortaya çıkarmıştır.  Bu çalışmada, yüksek teknoloji donanımı ileverimliliği arttırmayı hedefleyen Endüstri 4.0’ ın treyler üzerindekiuygulamaları ve lojistik sektörü üzerine etkileri araştırılmıştır. Bu kapsamdafilo yönetim operasyonları, treyler arıza ve bakım süreçleri, lojistikoperasyonları ve şoför davranışları ele alınmıştır. Ele alınan mevcut süreçlerile Endüstri 4.0’a uygun treyler uygulamaları verimlilik yönündenkarşılaştırılmıştır. Yapılan çıkarımlardan treyler üzerinden elde edilenveri  miktarı arttıkça verimliliğinarttığı görülmüştür ve ürünün ilk üreticiden son tüketiciye ulaşıncaya kadarolan tüm süreçlerini karşılayan lojistik sektörü için önemli fırsatlar sunduğuortaya koyulmuştur.  Elde edilen veriler incelendiğinde, lojistiksektöründeki verimlilik beklentisi ile birlikte lojistik sektörüne hizmet verenfirmalarında teknolojik gelişimi kaçınılmazdır. Lojistik sektöründe ihtiyaçduyulacak bu teknolojik gelişimi sağlayacak olan da treylerlerdir. Gelişen teknolojikimkanlarla birlikte treyler üreticileri “ürüne dayalı” modelden “ürün ve çözümodaklı” modele geçiş için çalışmalar yapmalıdır. Treyler sektöründekigelişmeler treyleri “ticari bir ürün” olmaktan “nakliye çözümüne geçirecektir.Bu çözümler için Endüstri 4.0’ın getirdiği nesnelerin internetine dayalı,komponentlerin birbirleri ile haberleşerek dijitalleşen, arıza tehşisiyapabilen ve insan gücünün azaltıldığı makine odaklı sistemleryaygınlaşacaktır.Industry4.0 is a project of encouraging traditional industry to digitalize andequipping it with high technology. However, it has created more efficientbusiness models, as it allows each data to be collected and analyzed in aneffective way in the working environment.Inthis study, the applications of Industry 4.0, which aims to increase efficiencywith high technology equipment of the trailers and it’s effects on thelogistics sector were researched. In this context, fleet management operations,trailer failures and maintenance processes, logistics operations and behaviorsof the drivers were researched. These existing processes which we researchedand Industry 4.0 applications which is suitable to trailer sector werecompared. After these inference we obtained, the efficiency increases asequally as amount of data which is acquired from trailer and tremendousadvantages were revealed on the logistics sector which contains all theprocesses from the manufacturer to the end user. Whenthe obtained data was analyzed, technological advancements are inevitable inlogistics companies which endeavours with efficiency expectations. Thesemuch-needed technological advancements will be provided by trailers. Togetherwith this technological opportunities, trailer companies needs to start studieson transition between product based model and product and solution based model.These advancements on the trailer sector will turn commercial product intotransportation solution. Based upon Internet of Things which is brought byIndustry 4.0, digitalizating with communication between the components,enabling to troubleshoot and machine oriented systems with less human effortwill become widespread

    Patient’s Refusal of Early Treatment of a Cesarean Scar Pregnancy Resulting in Life - Saving Hysterectomy at 23 Weeks Gestation: Report of a Case

    Cesarean scar pregnancy (CSP) is a rare life-threatening form of ectopic pregnancy embedded in the myometrium of a previous cesarean scar. Pathogenesis is suggested to be related with an existing scar defect or a microscopic dehiscent tract generated between the prior cesarean scar and the endometrial canal. The most common risk factor is a history of previous cesarean section. Sonography is the firstline diagnostic tool for the diagnosis of CSP. It is possible to make the diagnosis in the early weeks of pregnancy and early diagnosis would let the patient retain her future fertility. Treatment options prior to rupture include expectant management, dilatation&curettage, conservative medical treatment, hysteroscopic- laparoscopic-primary open surgical removal or hysterectomy.     Feasibility of basket technique compared to the office hysteroscopy to treat endometrial polyps in patients undergoing in vitro fertilization

    Objective: The authors aim to compare the results of office hysteroscopic basket polypectomy with operative hysteroscopy in endometrial polyps detected prior to in vitro fertilization (IVF). Material and Methods: Infertile patients, who underwent polypectomy before treatment, were analyzed retrospectively. Patients who underwent basket polypectomy as office hysteroscopy (Group 1, n=55) and hysteroscopic polypectomy (Group 2, n=49) were included in the study. Demographic data, operation time, success rates, and pregnancy rates after operation were compared between the two groups. Results: The mean operating time for Group 1 was 14.98 ±3.67 min, while that for Group 2 was 18.84±3.30 min (p=0.001). The success rates between the groups were similar (96.3% vs. 100%; p=0.652), without any major complications. Further, the biochemical (52.7% vs. 48.9%, OR 1.19; 95% CI: 0.55-2.59; p=0.696), clinical (43.6% vs. 40.8%, OR 1.16; 95% CI: 0.52-2.59; p=0.712), ongoing pregnancy (38.1% vs. 34.6%, OR 1.21; 95% CI: 0.52- 2.81; p=0.647), live births (34.5% vs. 28.6%, OR 1.3116; 95% CI:0.57-3.03; p=0.514), early spontaneous miscarriage (5.5% vs. 6.1, OR 0.88; 95% CI: 0.17-4.60; p=0.884) and spontaneous pregnancy (5.5 vs. 4.1, OR 0.745;%95 CI: 0.21-8.4; p=0.745) rates after polypectomy were observed to be similar in both the groups. Conclusion: Hysteroscopic basket polypectomy was not inferior to operative hysteroscopy regarding success rates and was found to be superior in terms of operating time in patients planning in vitro fertilization.
