    The influences of sibling relationships in adolescence

    Most of the previous research has been focused mainly on the effects of parent-child relationships and marital relationships on adolescent development. The purpose of the present study is to investigate the potential impacts of the sibling relationship, another important family subsystem that also has significant influences on adolescent development, across early and middle adolescence.;Three theoretically interested models, the stress-suppressing model, the counteractive model, and the buffering model, were tested by structural equation modeling (LISREL VIII), using the three-wave data collected from multiple informants (fathers, mothers, target children, and siblings). Positive sibling relationships in adolescence were hypothesized to predict fewer adolescent developmental problems later, directly and indirectly through adolescent friendships, academic achievement, and self-esteem. Positive sibling relationships in adolescence were also hypothesized to have buffering effects on later adolescent development. Adolescents with higher level of perceived positive sibling relationships should have fewer developmental problems than those who face the same risk factors but do not have positive sibling relationships.;Results showed that early positive sibling relationships influence adolescent development only indirectly through adolescent friendship quality and self-esteem. Adolescents who perceived more positive sibling relationships at Time 1 tend to have better friendships and higher self-esteem at Time 2, which in turn lead to higher self-esteem, less loneliness, less depression, and fewer delinquent behaviors and substance uses later at Time 3. Moreover, a bidirectional relationship was found between adolescent self-esteem and their sibling relationship quality, suggesting that higher level of positive sibling relationships help to enhance adolescent self-esteem, and higher adolescent self-esteem predicts higher level of positive sibling relationships.;Furthermore, a significant positive relationship was found from adolescent positive sibling relationships at Time 1 to adolescent friendships at Time 2, not from adolescent friendships at Time 1 to adolescent positive sibling relationships at Time 2. It suggests that earlier sibling relationship quality is more predictive of later friendship quality for adolescents, rather than the other way around. The implications and limitations of the present study were discussed to explain and emphasize the importance of positive sibling relationships on adolescent development

    This article describes the development, implementation, and exploratory evaluation of a professional development series that addressed educators\u27 knowledge and use of the terms research-based and evidence-based within Human Sciences Extension and Outreach at one university. Respondents to a follow-up survey were more likely to select correctly the commonly accepted standard for each term, and they reported asking more questions, talking with others, examining programs\u27 evidence bases, and placing more value on fidelity and evaluation following participation in the professional development series. Educator reactions to the series were generally positive, although researchers interested in designing like programs might consider engaging educators within the context of their preexisting knowledge levels

    Sliding and pressure evaluation on conventional and V-shaped seats of reclining wheelchairs for stroke patients with flaccid hemiplegia: a crossover trial

    <p>Abstract</p> <p>Background</p> <p>Reclining wheelchairs are commonly used to transport elderly stroke patients in Taiwan. However, there is concern that the patient's body in the wheelchair often slides forward when they return to a seated position, increasing the sitting pressure. Therefore, a novel reclining wheelchair with an ergonomic "V-Seat" was designed to prevent forward sliding and pressure sores. The use of these reclining chairs by stroke patients has not yet been studied. Thus, we investigated the effects of V-shaped and conventional seats in reclining wheelchairs on the extent of forward sliding and on the sitting pressure of stroke patients with flaccid hemiplegia and of able-bodied elders.</p> <p>Methods</p> <p>We recruited 13 able-bodied elders and 11 stroke patients with flaccid hemiplegia and performed 5 reclining cycles in both types of wheelchair. The amount of sliding along the backrest (BS) plane and the seat (SS) plane, the mean sitting pressure (MP), and the sacral peak pressure (SPP) of the subjects were recorded. We used the Wilcoxon signed-rank test to compare the BS, SS, MP, and SPP in wheelchairs with conventional and V-shaped seats, and we used the Wilcoxon rank sum test to compare the differences in BS and SS between stroke patients and able-bodied elders in both types of reclining wheelchair.</p> <p>Results</p> <p>The BS, SS, and SPP of stroke patients were significantly lower in the wheelchairs with V-shaped seats than in conventional wheelchairs in most comparisons; however, the BS of able-bodied elders was higher in V-shaped seats than in conventional seats. The SS and SPP of stroke patients were significantly higher than those of able-bodied elders in both types of reclining wheelchair, and the BS of stroke patients was significantly higher than that of able-bodied elders only in conventional reclining wheelchairs.</p> <p>Conclusions</p> <p>The use of V-shaped seats in reclining wheelchairs can help reduce the forward sliding and sacral peak pressure of stroke patients with flaccid hemiplegia. The back displacement of able-bodied subjects when using both conventional and V-shape seats in reclining positions differs from the back displacement of stroke patients with flaccid hemiplegia when using such seats. These results are of paramount value and should be considered when prescribing the use of reclining wheelchairs to subjects with flaccid hemiplegia.</p

    The effectiveness of different health education strategies in people with pre-diabetes: A randomized controlled trial

    Background. People with pre-diabetes often lack knowledge of their risks of developing diabetes. In one of our previous study, Multi-Approach Health Education was shown evidence to be effective on health behavior of reducing risks of developing diabetes. However, which one approach is really effective and efficient need further investigation. Purpose. To examine the effects of different intervention strategies on diabetes prevention knowledge, exercise, dietary behavior, and physiological indicators for people with pre-diabetes. Methods. This was a randomly controlled trial. People who received health examination and were found fasting blood glucose higher than normal, between 100 - 125 mg/dl in 2011 were recruited. Three types of intervention were randomly assigned to 3 groups respectively. The control group (n=51) received a health education lecture. One experimental group (n=48) received the identical lecture plus telephone encouragement. The second experimental group (n=41) received the identical lecture plus a health reminder poster in their daily life. The outcomes were evaluated for the change in knowledge of diabetes prevention, exercise behavior, dietary behavior, and physiological outcomes at 6 and 12 weeks after the lecture of three groups, respectively. Results. Three intervention strategies were equally efficacious at inducing positive behavioral changes but overall the magnitudes of physiological changes were the same. In general, the maximum change in parameters was achieved after 6 weeks and maintained in the second 6 weeks of the study. Conclusions. Educating people with pre-diabetes about their condition can have a positive effect upon their health behaviors. However, education lecture coupled with a telephone follow up or plus educational posters were found no more effective than lecture alone. The lecture alone of health education may be enough for people with pre-diabetes, but the long term effect needs further investigation

    Rural-urban differences in substance use among African-American adolescents

    Purpose: To examine substance use differences among African‐American adolescents living in rural and more urban areas in Iowa and Georgia and factors thought to be related to those differences. Specifically, negative affect and perceived availability were examined as mediators of the relation between community size and alcohol, tobacco, and drug use. Methods: In‐home interviews with the adolescents (Time 1: N = 897, Mean age = 10.5) assessed their use, perceived substance availability, and negative affect across 3 waves. Their parents’ use was also assessed. Census data were used to determine community size (rural ≤ 2,500; urban ≥ 2,500). Findings: Perceived substance availability and use were both higher among the more urban adolescents. As expected, negative affect was a primary antecedent to use at each wave. Structural Equation Modeling indicated that the relation between population and use was mediated by perceived availability of the substances. Additional multigroup analyses indicated that the relations between negative affect and use were significantly stronger among the urban adolescents at all waves. Conclusions: Results suggest that stress or negative affect is an important antecedent to use among African‐American adolescents, especially when it occurs at an early age, but living in rural areas may be a buffer for both problems, in part, because exposure to this type of risk is lower in these environments

    Optimizing Human Synovial Fluid Preparation for Two-Dimensional Gel Electrophoresis

    <p>Abstract</p> <p>Background</p> <p>Proteome analysis is frequently applied in identifying the proteins or biomarkers in knee synovial fluids (SF) that are associated with osteoarthritis and other arthritic disorders. The 2-dimensional gel electrophoresis (2-DE) is the technique of choice in these studies. Disease biomarkers usually appear in low concentrations and may be masked by high abundant proteins. Therefore, the main aim of this study was to find the most suitable sample preparation method that can optimize the expression of proteins on 2-DE gels that can be used to develop a reference proteome picture for non-osteoarthritic knee synovial fluid samples. Proteome pictures obtained from osteoarthritic knee synovial fluids can then be compared with the reference proteome pictures obtained in this study to assist us in identifying the disease biomarkers more correctly.</p> <p>Results</p> <p>The proteomic tool of 2-DE with immobilized pH gradients was applied in this study. A total of 12 2-DE gel images were constructed from SF samples that were free of osteoarthritis. In these samples, 3 were not treated with any sample preparation methods, 3 were treated with acetone, 3 were treated with 2-DE Clean-Up Kit, and 3 were treated with the combination of acetone and 2-D Clean-Up Kit prior to 2-DE analysis. Gel images were analyzed using the PDQuest Basic 8.0.1 Analytical software. Protein spots that were of interest were excised from the gels and sent for identification by mass spectrometry. Total SF total protein concentration was calculated to be 21.98 ± 0.86 mg/mL. The untreated SF samples were detected to have 456 ± 33 protein spots on 2-DE gel images. Acetone treated SF samples were detected to have 320 ± 28 protein spots, 2-D Clean-Up Kit treated SF samples were detected to have 413 ± 31 protein spots, and the combined treatment method of acetone and 2-D Clean-Up Kit was detected to have 278 ± 26 protein spots 2-DE gel images. SF samples treated with 2-D Clean-Up Kit revealed clearer presentation of the isoforms and increased intensities of the less abundant proteins of haptoglobin, apolipoprotein A-IV, prostaglandin-D synthase, alpha-1B-glycoprotein, and alpha-2-HS-glycoprotein on 2-DE gel images as compared with untreated SF samples and SF samples treated with acetone.</p> <p>Conclusions</p> <p>The acetone precipitation method and the combined treatment effect of acetone and 2-DE Clean-Up Kit are not preferred in preparing SF samples for 2-DE analysis as both protein intensities and numbers decrease significantly. On the other hand, 2-D Clean-Up Kit treated SF samples revealed clearer isoforms and higher intensities for the less abundant proteins of haptoglobin, apolipoprotein A-IV, prostaglandin-D synthase, alpha-1B-glycoprotein, and alpha-2-HS-glycoprotein on 2-DE gels. As a result, it is recommended that SF samples should be treated with protein clean up products such as 2-D Clean-Up Kit first before conducting proteomic research in searching for the relevant biomarkers associated with knee osteoarthritis.</p

    The influence of over-distraction on biomechanical response of cervical spine post anterior interbody fusion: a comprehensive finite element study

    Introduction: Anterior cervical discectomy and fusion (ACDF) has been considered as the gold standard surgical treatment for cervical degenerative pathologies. Some surgeons tend to use larger-sized interbody cages during ACDF to restore the index intervertebral disc height, hence, this study evaluated the effect of larger-sized interbody cages on the cervical spine with ACDF under both static and cyclic loading.Method: Twenty pre-operative personalized poro-hyperelastic finite element (FE) models were developed. ACDF post-operative models were then constructed and four clinical scenarios (i.e., 1) No-distraction; 2) 1 mm distraction; 3) 2 mm distraction; and 4) 3 mm distraction) were predicted for each patient. The biomechanical responses at adjacent spinal levels were studied subject to static and cyclic loading. Non-parametric Friedman statistical comparative tests were performed and the p values less than 0.05 were reflected as significant.Results: The calculated intersegmental range of motion (ROM) and intradiscal pressure (IDP) from 20 pre-operative FE models were within the overall ranges compared to the available data from literature. Under static loading, greater ROM, IDP, facet joint force (FJF) values were detected post ACDF, as compared with pre-op. Over-distraction induced significantly higher IDP and FJF in both upper and lower adjacent levels in extension. Higher annulus fibrosus stress and strain values, and increased disc height and fluid loss at the adjacent levels were observed in ACDF group which significantly increased for over-distraction groups.Discussion: it was concluded that using larger-sized interbody cages (the height of ≥2 mm of the index disc height) can result in remarkable variations in biomechanical responses of adjacent levels, which may indicate as risk factor for adjacent segment disease. The results of this comprehensive FE investigation using personalized modeling technique highlight the importance of selecting the appropriate height of interbody cage in ACDF surgery

    Endoscopic Submucosal Dissection for Gastric Epithelial Tumors: A Multicenter Study in Taiwan

    Background/PurposeEndoscopic submucosal dissection (ESD) is an advanced endoscopic procedure to resect early gastric cancer (EGC). The purpose of this study was to determine the effectiveness and complications of ESD for gastric epithelial tumors in Taiwan.MethodsWe retrospectively analyzed the efficacy and outcome of ESD in patients who received ESD for gastric epithelial tumors between June 2004 and August 2007.ResultsA total of 70 patients with gastric epithelial tumors were treated by ESD. The mean age was 66.5 ±12.9 years (range, 35–84 years). The mean size of the gastric epithelial tumors was 1.85 ± 0.81 cm. The mean size of resected specimens was 3.26 ± 1.39 cm. The one-piece resection rate was 91.4% (64/70). The median operation time was 92.4 minutes. The complicating bleeding and perforation rates were 5.7% (4/70) and 4.3% (3/70), respectively. Emergency surgery was performed for three patients with perforations. The local recurrence rate of gastric cancer was 2.8%. Except for one patient who died of congestive heart failure and another who died of stroke, the remaining 68 patients (97.1%) survived.ConclusionESD is a promising local curative treatment option for EGC in Taiwan but it still carries risks of perforation and bleeding. The education and learning curve of endoscopists will improve the outcome of this procedure
