    The shielding effect of HTS power cable based on E-J power law

    A method for analysing the current distribution in high-T/sub c/ superconducting (HTS) power cable is examined by the aid of the novel use of anisotropic conductivity and 3-D finite element method considering E-J power law characteristic. The detailed current distribution in the cable is illustrated and the shielding effect of HTS shield layer with intervals is also examined.     THE INCIDENCE OF PEPTIC ULCER SURGERY BEFORE AND AFTER THE INTRODUCTION OF H2-RECEPTOR ANTAGONISTS

    Since the introduction of H2-receptor antagonists, the incidence of ulcer surgery has decreased markedly. Based on the number of patients who underwent ulcer surgery and upper gastrointestinal endoscopy in our hospital, the number of operations for peptic ulcer disease has decreased, although the number of peptic ulcer patients diagnosed by endoscopy has increased after the introduction of cimetidine (the first H2-receptor antagonist in Japan). The decreasing rate of surgical therapy before and after the administration of H2-receptor antagonists was 49.1%. The number of emergency operations has also decreased among ulcer patients diagnosed by endoscopy although the number of perforated and bleeding ulcer patients has increased. It is also reported that the most common operative procedure for both gastric and duodenal ulcer was distal gastrectomy reconstructed by gastroduodenostomy (Billroth Ⅰ)

    Investigation of Enhanced Tacotron Text-to-speech Synthesis Systems with Self-attention for Pitch Accent Language

    End-to-end speech synthesis is a promising approach that directly converts raw text to speech. Although it was shown that Tacotron2 outperforms classical pipeline systems with regards to naturalness in English, its applicability to other languages is still unknown. Japanese could be one of the most difficult languages for which to achieve end-to-end speech synthesis, largely due to its character diversity and pitch accents. Therefore, state-of-the-art systems are still based on a traditional pipeline framework that requires a separate text analyzer and duration model. Towards end-to-end Japanese speech synthesis, we extend Tacotron to systems with self-attention to capture long-term dependencies related to pitch accents and compare their audio quality with classical pipeline systems under various conditions to show their pros and cons. In a large-scale listening test, we investigated the impacts of the presence of accentual-type labels, the use of force or predicted alignments, and acoustic features used as local condition parameters of the Wavenet vocoder. Our results reveal that although the proposed systems still do not match the quality of a top-line pipeline system for Japanese, we show important stepping stones towards end-to-end Japanese speech synthesis.Comment: to be appeared at ICASSP 201

    A SCINTIGRAPHIC STUDY OF MASS PERISTALSIS IN HUMAN COLON

    Although many attempts have been made to study human colonic motility, the colonic transit is still poorly understood. Both spontaneous and neostigmine-induced peristalsis of the colon were studied with scintigraphy. A polythene tube was inserted into the cecum through a colonofiberscope. 37 MBq of ⁹⁹ᵐTc-DTPA and 75 ml of saline were instilled and dynamic scan was begun. Eight healthy volunteers were examined by the method above mentioned. The sampling time was set at fifteen seconds in six persons and three seconds in the rest. 0.5 mg of neostigmine was injected intravenously to stimulate the paristalsis when no peristalsis occurred within thirty minutes after the study was begun. Dynamic scanning was performed for sixty to ninety minutes. This scintigraphic study revealed that the spontaneous and induced peristalsis were almost identical on colonogram. ⁹⁹ᵐTc-DTPA solution was propelled from the cecum and ascending colon to the sigmoid colon or the rectum for about fifteen seconds during mass peristalsis. Colonogram (time-activity curve) enables us to analyze mass peristalsis easily and more objectively than colonoscintigram.     A Successfully Treated Case of Intrahepatic Cholangiocarcinoma with Exacerbation of Dermatomyositis

    Dermatomyositis (DM) is often found in conjunction with malignant tumors such as lung, cervical, and breast cancer. However, the association with intrahepatic cholangiocarcinoma (ICC) is extremely rare. Moreover, to our knowledge, there have been no previous reports of DM discovered because of exacerbation of DM. Our case was a 44-year-old female with dry cough, myalgia, and arthralgia. We performed hepatic resection for intrahepatic ICC. She was diagnosed with DM, and combination treatment with prednisolone and tacrolimus was started. During outpatient visits, her symptoms worsened, and she was hospitalized due to deterioration of her primary disease. On detailed examination, a malignant lesion in the liver was discovered. After operation, the symptoms of DM remain stable by taking prednisolone and tacrolimus. The patient was suspected to have paraneoplastic syndrome, which was discovered due to the exacerbation of the DM that was caused by the intrahepatic ICC

    ヒト大腸癌におけるHVEM発現は腫瘍の進行と予後に影響する

    Background: Herpesvirus entry mediator (HVEM) has been recently suggested to play certain roles in cancer biology. We examined HVEM expression in human colorectal cancer (CRC) to reveal its clinical importance. Materials and Methods: Immunohistochemical staining was carried-out in normal epithelium, benign and malignant lesions. Results: While intense HVEM expression was not observed in normal epithelium and hyperplastic polyps, 24% of adenoma and more than half of CRCs had high HVEM expression. In 234 CRCs, HVEM expression was significantly associated with tumor status and pathological stage. Patients with high HVEM expression had a significantly poorer prognosis than those with low expression. Importantly, HVEM status had an independent prognostic value in CRC. Furthermore, HVEM status was inversely corrected with the presence of tumor-infiltrating T-cells.     Roles of mTOR in Diabetic Kidney Disease.

    Diabetic kidney disease (DKD) is the leading cause of end-stage renal disease and the number of patients affected is increasing worldwide. Thus, there is a need to establish a new treatment for DKD to improve the renal prognosis of diabetic patients. Recently, it has shown that intracellular metabolic abnormalities are involved in the pathogenesis of DKD. In particular, the activity of mechanistic target of rapamycin complex 1 (mTORC1), a nutrient-sensing signaling molecule, is hyperactivated in various organs of diabetic patients, which suggests the involvement of excessive mTORC1 activation in the pathogenesis of diabetes. In DKD, hyperactivated mTORC1 may be involved in the pathogenesis of podocyte damage, which causes proteinuria, and tubular cell injury that decreases renal function. Therefore, elucidating the role of mTORC1 in DKD and developing new therapeutic agents that suppress mTORC1 hyperactivity may shed new light on DKD treatments in the future
