    Fraction of clear skies above astronomical sites: a new analysis from the GOES12 satellite

    Comparing the number of clear nights (cloud free) available for astronomical observations is a critical task because it should be based on homogeneous methodologies. Current data are mainly based on different judgements based on observer logbooks or on different instruments. In this paper we present a new homogeneous methodology on very different astronomical sites for modern optical astronomy, in order to quantify the available night time fraction. The data are extracted from night time GOES12 satellite infrared images and compared with ground based conditions when available. In this analysis we introduce a wider average matrix and 3-Bands correlation in order to reduce the noise and to distinguish between clear and stable nights. Temporal data are used for the classification. In the time interval 2007-2008 we found that the percentage of the satellite clear nights is 88% at Paranal, 76% at La Silla, 72.5% at La Palma, 59% at Mt. Graham and 86.5% at Tolonchar. The correlation analysis of the three GOES12 infrared bands B3, B4 and B6 indicates that the fraction of the stable nights is lower by 2% to 20% depending on the site

    Walking the last mile on the long road to evidence-informed development: building capacity to use research evidence

    The systematic application of rigorous evidence to inform the design and implementation of development policies and programmes has the potential to positively influence development outcomes. To achieve such evidence-informed development, a process of generating, transmitting, and using high-quality, policy-relevant evidence of development effectiveness is required. This article focuses on the final step in this casual chain – the use of evidence by national development policymakers. It proposes a people- and demandfocused approach to capacity building for the use of research evidence by policymakers. This support in building personal as well as team capacity and demand is assumed to encourage a growing institutionalisation of evidence use. The article integrates these capacity-building efforts into the wider theory of change for evidence-informed development, highlighting the importance of effective mechanisms to encourage research use in order to achieve the objective of improving lives through research evidence

    Rare Mutations of Peroxisome Proliferator-Activated Receptor Gamma: Frequencies and Relationship with Insulin Resistance and Diabetes Risk in the Mixed Ancestry Population from South Africa

    Background. Genetic variants in the nuclear transcription receptor, PPARG, are associated with cardiometabolic traits, but reports remain conflicting. We determined the frequency and the clinical relevance of PPARG SNPs in an African mixed ancestry population. Methods. In a cross-sectional study, 820 participants were genotyped for rs1800571, rs72551362, rs72551363, rs72551364, and rs3856806, using allele-specific TaqMan technology. The homeostatic model assessment of insulin (HOMA-IR), β-cells function (HOMA-B%), fasting insulin resistance index (FIRI), and the quantitative insulin-sensitivity check index (QUICKI) were calculated. Results. No sequence variants were found except for the rs3856806. The frequency of the PPARG-His447His variant was 23.8% in the overall population group, with no difference by diabetes status (). The His447His allele T was associated with none of the markers of insulin resistance overall and by diabetes status. In models adjusted for 2-hour insulin, the T allele was associated with lower prevalent diabetes risk (odds ratio 0.56 (95% CI 0.31–0.95)). Conclusion. Our study confirms the almost zero occurrences of known rare PPARG SNPs and has shown for the first time in an African population that one of the common SNPs, His447His, may be protective against type 2 diabetes

    A comparison of five glucometers in South Africa

    Objective: To assess the accuracy and precision of five currently available blood glucose meters in South AfricaBackground: Since the introduction of glucometers, there has been an ongoing, competition-driven development in both meter and strip technology, which has allowed for greater accuracy and reliability of results. Despite the advances in technology, there is significant variation amongst these glucometers necessitating a proper evaluation before use.Methods: Glucose levels in capillary blood samples from 115 patients attending the diabetic clinic at Tygerberg Hospital were measured witheach meter, and compared with the laboratory reference method.Results: The coefficients of variation (CVs) (imprecision) of most meters were acceptable at less than 5%, with a bias ranging from 1.7 to 6.8%. None of the glucometers satisfied the American Diabetes Association (ADA) recommendation of less than 5% bias.Conclusions: The study highlights the need for an objective and independent comparison of all glucometers in South Africa, as the variability observed can impact on patient care.Keywords: glucometer; diabetes; self monitoring (SMBG); analytical error; Clarke error gri

    Extreme heat events, high ambient temperatures and human morbidity and mortality in Africa: A systematic review

    Temperature extremes vary across Africa. A continent-wide examination of the impacts of heat on health in Africa, and a synthesis of Africa-informed evidence is, however, lacking. A systematic review of articles published in peer-reviewed journals between January 1992 and April 2019 was conducted. To be eligible, articles had to be Africa-specific, in English, and focused on how heatwaves and high ambient temperatures affect morbidity and mortality. A secondary systematic analysis on policies and interventions comprising 17 studies was also conducted, and the findings synthesised together with those of the 20 primary studies. Eleven studies showed that high ambient temperatures and heat waves are linked with increased mortality rates in Africa. These linkages are characterised by complex, linear and non-linear (J or U) relationships. Eight of the nine primary studies of morbidity outcome reported that an increase in temperature was accompanied by raised disease incidence. Children and the elderly were the population groups most vulnerable to extreme heat exposure. Location-specific interventions and policy suggestions include developing early warning systems, creating heat-health plans, changing housing conditions and implementing heat-health awareness campaigns. In summary, this review demonstrates that, while heat-health relationships in Africa are complex, extreme temperatures are associated with high mortality and morbidity, especially amongst vulnerable populations. As temperatures increase across Africa, there is an urgent need to develop heat-health plans and implement interventions. Future studies must document intervention effectiveness and quantify the costs of action and inaction on extreme heat-related mortality and morbidity. Significance: Empirical evidence shows that the relationship between heat and human health is complex in the African context. This complexity has implications for the development of interventions and policies for heathealth on the continent. This review is important for African policymakers, practitioners and others who support Africa’s adaptation to climate change. Through this review, a compendium of Africa-specific and relevant empirical information is aggregated and made readily available to various interested and affected parties

    APOL1 genetic variants, chronic kidney diseases and hypertension in mixed ancestry South Africans

    BackgroundThe frequencies of apolipoprotein L1 (APOL1) variants and their associations with chronic kidney disease (CKD) vary substantially in populations from Africa. Moreover, available studies have used very small sample sizes to provide reliable estimates of the frequencies of these variants in the general population. We determined the frequency of the two APOL1 risk alleles (G1 and G2) and investigated their association with renal traits in a relatively large sample of mixed-ancestry South Africans. APOL1 risk variants (G1: rs60910145 and rs73885319; G2: rs71785313) were genotyped in 859 African mixed ancestry individuals using allele-specific TaqMan technology. Glomerular filtration rate (eGFR) was estimated using the Modification of Diet in Renal Disease (MDRD) and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations.ResultsThe frequencies of rs73885319, rs60910145 and rs71785313 risk alleles were respectively, 3.6%, 3.4%, and 5.8%, resulting in a 1.01% frequency of the APOL1 two-risk allele (G1:G1 or G1:G2 or G2:G2). The presence of the two-risk allele increased serum creatinine with a corresponding reduction in eGFR (either MDRD or CKD-EPI based). In dominant and log-additive genetic models, significant associations were found between rs71785313 and systolic blood pressure (both p ≤ 0.025), with a significant statistical interaction by diabetes status, p = 0.022, reflecting a negative non-significant effect in nondiabetics and a positive effect in diabetics.ConclusionsAlthough the APOL1 variants are not common in the mixed ancestry population of South Africa, the study does provide an indication that APOL1 variants may play a role in conferring an increased risk for renal and cardiovascular risk in this population

There has been increased emphasis over the last 15 years on the use of evidence to inform decision-making at policy and practice levels but the conceptual thinking has not kept pace with practical developments in the field. We have developed a new demand-side model, with multiple dimensions to conceptualise support for the use of evidence in decision-making. This model emphasises the need for multiple levels of engagement, a combination of interventions, a spectrum of outcomes and a detailed consideration of context

    Practical reflections on combining workshops and mentorships to build capacity in demand and use of evidence in government organisations

    Decision makers’ capacity to use evidence is a key component of evidence-informed decision making (EIDM) at a policy level. This paper describes a unique combination of EIDM workshops and mentoring to build decision makers’ capacity to use evidence. It reflects how the deliberate design of sequential workshop and mentoring capacity-building activities enhanced programme effects and reports on the design features that we believe have most contributed to the observed effects. We have found that this combined approach is most effective when it is underpinned by a relationship-building theory of change and remains flexible and responsive to delivering on the needs of participants, and when it is delivered in a timely manner by partners who have been carefully paired with participants based on required expertise

    Association of the ENPP1 rs997509 polymorphism with obesity in South African mixed ancestry learners

    Background: The Ectonucleotide Pyrophosphatase Phosphodiesterase1 (ENPP1) polymorphisms have been associated with metabolic traits. There is no data on the effect of ENPP1 in South African children or adults. Objective: To investigate the role of K121Q (rs1044498), rs997509 and rs9402349 in obesity and other components of the metabolic syndrome. Design: A case-control study. Subjects: Sixty four obese and 64 lean mixed ancestry learners. Setting: Western Cape, South Africa. Main outcome measure: The EN PP1 rs997509T allele is independently associated with obesity in children of mixed ancestry from South Africa. Results: The T allele frequency of the rs997509 differed significantly between obese and controls, p=0.0100 and increased the risk of being obese, p = 0.0238. Furthermore, the estimated effect of the T allele was an increase of 8.6 cm in waist circumference, 10.2 kg in weight and a corresponding 4.9 kg/m2 in BMI. Individuals carrying both the 121Q and the T allele of rs997509 were more associated with obesity (odds ratio = 3.85, 95% CI: 1.13 to 13.09) whilst those carrying the C allele of rs997509 in the presence of 121Q were likely to be lean with odds ratio of obesity 0.41 (95% CI: 0.19 to 0.87). Conclusion: Our findings suggest that ENPP1 polymorphisms may contribute to different metabolic characteristics, all of which are associated with insulin resistance in mixed ancestry children of South Africa.     Reflections on per diems in international development projects: Barriers to and enablers of the project cycle

    The concept of what a per diem is and who should receive one is a complex idea that, within the development context, can either support or hinder the achievement of development projects’ goals. This paper seeks to explain the evolving nature of per diems and their use within the development context; explore how they serve as barriers or enablers in achieving project goals; and touch on their impact on the development project cycle. Through a 3-year-long internationally funded development programme in Malawi and South Africa, the authors compare lessons drawn from their experience with existing literature to determine the practicalities of paying per diems and address the question: To what extent do per diems support or hinder international development projects?
