    Influence of antiviral therapy on survival of patients with hepatitis B-associated hepatocellular carcinoma undergoing transarterial chemoembolization

    Purpose: To examine the prognostic value of antiviral therapy among hepatocellular carcinoma (HCC) patients undergoing transarterial chemoembolization (TACE).Method: A total of 356 patients with HCC undergoing TACE were recruited for the purpose of current study. All the patients were categorized into two groups; antiviral (n = 132) and non-antiviral group (n = 224). All the clinical and laboratory parameters were noted at baseline. Patients were then followed up for five years. The mortality rates in two groups were evaluated with Kaplan-Meier estimate.Results: The average age of the participants was 51.2 ± 6.17 years. A majority (329; 92.4 %) of the patients were male while females constituted only 7.6 %. During five years follow-up period, a total of 274 (76.9 %) died, with 89 patients belonging to the antiviral group while the remaining 185 patients were in the non-antiviral group. Mortality rate significantly differed between the antiviral and non-antiviral groups (67.4 % versus 82.5 %, p = 0.028). The results of Cox regression demonstrated that being a smoker, low serum albumin, local ablation and resection decreased overall survival while female gender, antiviral therapy, and early tumor site-node-metastatis (TNM) staging increased overall survival.Conclusion: Antiviral therapy for underlying hepatitis B in HCC patients undergoing TACE prolongs overall survival and prevents or delays reactivation of tumor.Keywords: Cancer, Chemoembolization, Hepatitis, Hepatocellular carcinoma, Transarterial chemoembolization (TACE

    Expression and biological significance of c-FLIP in human hepatocellular carcinomas

    <p>Abstract</p> <p>Background</p> <p>c-FLIP can be considered as a tumor-progression factor in regard to its anti-apoptotic functions. In the present study, we intended to investigate the expression of c-FLIP in human HCC tissues, and its relation with drug-induced cell apoptosis through the specific inhibition of c-FLIP expression by siRNA in 7721 cells.</p> <p>Methods</p> <p>c-FLIP expression was quantified immunohistochemically in HCC tissues(eighty-six cases), and corresponding noncancerous tissues (fifty-seven cases). Patients with HCC were followed up for cancer recurrence. Then, the c-FLIP gene was silenced with specific siRNA in 7721 HCC cells. c-FLIP expression was detected by RT-PCR, Western Blot and immunocytochemical staining. The cellular viability and cell apoptosis were assayed <it>in vitro </it>with cells treated with doxorubicin.</p> <p>Results</p> <p>Positive immunostaining was detected for c-FLIP in 83.72% (72/86) human HCC tissues, 14.81% (4/27) hepatic cirrhosis, 11.11% (2/18) hepatic hemangioma tissues, and absent in normal hepatic tissues. The overexpression(more than 50%) of c-FLIP in HCC adversely affected the recurrence-free survival. Through c-FLIP gene silencing with siRNA, the expressions of c-FLIP mRNA and protein were remarkably down-regulated in 7721 HCC cells. And doxorubicin showed apparent inhibition on cell proliferations, and induced more apoptosis.</p> <p>Conclusion</p> <p>These results indicate that c-FLIP is frequently expressed in human HCCs, and its overexpression implied a lesser probability of recurrence-free survival. The specific silencing of c-FLIP gene can apparently up-regulate drug-induced HCC cell apoptosis, and may have therapeutic potential for the treatment of human HCC.</p

    Clinicopathological significance of SOX4 expression in primary gallbladder carcinoma

    <p>Abstract</p> <p>Aim</p> <p>SOX4, as a member of the SRY-related HMG-box (SOX) transcription factor family, has been demonstrated to be involved in tumorigenesis of many human malignancies; however, its role in primary gallbladder carcinoma (PGC) is still largely unknown. The aim of this study was to investigate SOX4 expression in PGC and its prognostic significance.</p> <p>Methods</p> <p>From 1997 to 2006, 136 patients underwent resection for PGC. The median follow-up was 12.8 months. Immunostainings for SOX4 were performed on these archival tissues. The correlation of SOX4 expression with clinicopathological features including survival was analyzed.</p> <p>Results</p> <p>SOX4 was expressed in 75.0% (102/136) of PGC but not in the normal epithelium of the gallbladder. In addition, the over-expression of SOX4 was significantly associated with low histologic grade (<it>P </it>= 0.02), low pathologic T stage (<it>P </it>= 0.02), and early clinical stage (<it>P </it>= 0.03). The levels of SOX4 immunostainings in PGC tissues with positive nodal metastasis were also significantly lower than those without (<it>P </it>= 0.01). Moreover, Kaplan-Meier curves showed that SOX4 over-expression was significantly related to better overall (<it>P </it>= 0.008) and disease-free survival (<it>P </it>= 0.01). Furthermore, multivariate analyses showed that SOX4 expression was an independent risk factor for both overall (<it>P </it>= 0.03, hazard ratio, 3.682) and disease-free survival (<it>P </it>= 0.04, hazard ratio, 2.215).</p> <p>Conclusion</p> <p>Our data indicate for the first time that the over-expression of SOX4 in PGC was significantly correlated with favorable clinicopathologic features and was an independent prognostic factor for better overall and disease-free survival in patients.     Targeting bone homeostasis regulation: potential of traditional Chinese medicine flavonoids in the treatment of osteoporosis

    Osteoporosis is a systemic metabolic disease characterized by disrupted bone formation/resorption and homeostasis. Flavonoids extracted from traditional Chinese medicinal plants regulate bone homeostasis by intervening in differentiating bone marrow mesenchymal stem cells, balancing the bone immune system, inhibiting oxidative stress response, and reversing iron overload. The target molecules and signaling pathways, such as Wnt/β-catenin and OPG/RANKL/RANK, directly affect osteoblast/osteoclast activity, exhibiting significant potential in the treatment of OP. Therefore, this study presents a systematic review of the recent literature to provide comprehensive information on the traditional Chinese medicine flavonoids involved in the regulation of bone homeostasis. Also, the molecular mechanisms and pharmacological uses of these metabolites are summarized, and their clinical translation and development potential are discussed

    Effect of sample temperature on laser-induced plasma of silicone rubber

    Silicone rubber in power transmission and transformation equipment is subjected to considerable temperature changes under different application environment conditions and in different operational states. In tropical areas and the Turpan region of China, surface temperatures of silicone rubber insulators may reach or exceed 70°C. During in situ testing of silicone rubber, the spectral signal may fluctuate or even be distorted when the temperature changes, and consequently, the accuracy of the analysis may be affected. Therefore, we performed a LIBS-based investigation into the dependence of the spectral signal of rubber silicone on the sample temperature. Using high-temperature vulcanized silicone rubber as the experimental material, we determined the trends in spectral line intensity for different elements, plasma temperature, and electron density with temperature when the sample temperature was increased from 25°C to 310°C. The results indicated that the intensities of the Al I 394.40 nm, Al I 396.15 nm, and Si I 390.55 nm lines in the LIBS spectra underwent a gradual decrease as the temperature was increased, whereas the intensity of the Al I 309.27 nm spectral line was essentially stable. However, the spectral line intensity, plasma temperature, and electron density all exhibited a spike at approximately 260°C, which occurred because of the decomposition of aluminum hydroxide. The results of the present study should prove to be of significance in further increasing the accuracy of LIBS analysis as applied to silicone rubber surface monitoring in high-temperature environments

    Regulatory Feedback Loop of Two phz Gene Clusters through 5′-Untranslated Regions in Pseudomonas sp. M18

    BACKGROUND: Phenazines are important compounds produced by pseudomonads and other bacteria. Two phz gene clusters called phzA1-G1 and phzA2-G2, respectively, were found in the genome of Pseudomonas sp. M18, an effective biocontrol agent, which is highly homologous to the opportunistic human pathogen P. aeruginosa PAO1, however little is known about the correlation between the expressions of two phz gene clusters. METHODOLOGY/PRINCIPAL FINDINGS: Two chromosomal insertion inactivated mutants for the two gene clusters were constructed respectively and the correlation between the expressions of two phz gene clusters was investigated in strain M18. Phenazine-1-carboxylic acid (PCA) molecules produced from phzA2-G2 gene cluster are able to auto-regulate expression itself and activate the expression of phzA1-G1 gene cluster in a circulated amplification pattern. However, the post-transcriptional expression of phzA1-G1 transcript was blocked principally through 5'-untranslated region (UTR). In contrast, the phzA2-G2 gene cluster was transcribed to a lesser extent and translated efficiently and was negatively regulated by the GacA signal transduction pathway, mainly at a post-transcriptional level. CONCLUSIONS/SIGNIFICANCE: A single molecule, PCA, produced in different quantities by the two phz gene clusters acted as the functional mediator and the two phz gene clusters developed a specific regulatory mechanism which acts through 5'-UTR to transfer a single, but complex bacterial signaling event in Pseudomonas sp. strain M18

    Potential candidates for liver resection in liver-confined advanced HCC: a Chinese multicenter observational study

    BackgroundAdvanced hepatocellular carcinoma (HCC) is characterized as symptomatic tumors [performance status (PS) score of 1-2], vascular invasion and extrahepatic spread, but patients with PS1 alone may be eliminated from this stage. Although liver resection is used for liver-confined HCC, its role in patients with PS1 alone remains controversial. Therefore, we aimed to explore its application in such patients and identify potential candidates.MethodsEligible liver-confined HCC patients undergoing liver resection were retrospectively screened in 15 Chinese tertiary hospitals, with limited tumor burden, liver function and PS scores. Cox-regression survival analysis was used to investigate the prognostic factors and develop a risk-scoring system, according to which patients were substratified using fitting curves and the predictive values of PS were explored in each stratification.ResultsFrom January 2010 to October 2021, 1535 consecutive patients were selected. In the whole cohort, PS, AFP, tumor size and albumin were correlated with survival (adjusted P&lt;0.05), based on which risk scores of every patient were calculated and ranged from 0 to 18. Fitting curve analysis demonstrated that the prognostic abilities of PS varied with risk scores and that the patients should be divided into three risk stratifications. Importantly, in the low-risk stratification, PS lost its prognostic value, and patients with PS1 alone achieved a satisfactory 5-year survival rate of 78.0%, which was comparable with that PS0 patients (84.6%).ConclusionSelected patients with PS1 alone and an ideal baseline condition may benefit from liver resection and may migrate forward to BCLC stage A

    Identify optimal HAP series scores for unresectable HCC patients undergoing TACE plus sorafenib: A Chinese multicenter observational study

    BackgroundHepatoma arterial-embolization prognostic (HAP) series scores have been proposed for prognostic prediction in patients with unresectable hepatocellular carcinoma (uHCC) undergoing transarterial chemoembolization (TACE). However, their prognostic value in TACE plus sorafenib (TACE-S) remains unknown. Here, we aim to evaluate their prognostic performance in such conditions and identify the best model for this combination therapy.MethodsBetween January 2012 and December 2018, consecutive patients with uHCC receiving TACE-S were recruited from 15 tertiary hospitals in China. Cox regression analyses were used to investigate the prognostic values of baseline factors and every scoring system. Their prognostic performance and discriminatory performance were evaluated and confirmed in subgroup analyses.ResultsA total of 404 patients were enrolled. In the whole cohort, the median follow-up period was 44.2 (interquartile range (IQR), 33.2–60.7) months, the median overall survival (OS) time was 13.2 months, and 336 (83.2%) patients died at the end of the follow-up period. According to multivariate analyses, HAP series scores were independent prognostic indicators of OS. In addition, the C-index, Akaike information criterion (AIC) values, and time-dependent area under the receiver operating characteristic (ROC) curve (AUC) indicated that modified HAP (mHAP)-III had the best predictive performance. Furthermore, the results remained consistent in most subsets of patients.ConclusionHAP series scores exhibited good predictive ability in uHCC patients accepting TACE-S, and the mHAP-III score was found to be superior to the other HAP series scores in predicting OS. Future prospective high-quality studies should be conducted to confirm our results and help with treatment decision-making

    Robust estimation of bacterial cell count from optical density

    Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals &lt;1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data
