    Analysis on the Influence of Director Leadership Style to Patient Safety Culture in Hermina Pandanaran Hospital

    Hermina Pandanaran hospital had established a policy regarding patient safety program based onseven steps of patient safety. However, patient safety culture had not adequately performed. Objectiveof this study was to explain transformational leadership style of a director according to seven steps ofhospital patient safety in order to implement patient safety culture in Hermina Pandanaran hospital.This was a qualitative study. Main informants were a head and team member of hospital patientsafety; it included a hospital medical deputy director, a medical support manager, a nursing manager,a domestic affair manager, and PPI nurses. Triangulation informants were people who involved in the hospital patient safety program. Data management was done by applying content analysis method.Results of the study showed that transformational leadership style of a director involved staffs inthe implementation of patient safety culture especially none blaming culture and learning fromincident culture. However, reporting culture was not implemented properly. Seven steps of hospitalpatient safety were not done completely; staffs had not been motivated optimally, steps to managepatient safety incident were not described directly in the field, policy regarding patient safety wasnot established, risk management system was not developed, RCA training in the hospital was notimplemented.Suggestions for hospital director are to do more frequent socialization on reporting culture withstressing on non-blaming culture to staffs; to describe steps on the management of patient safetyincident and conducting risk assessment. Director is expected to go directly to the field and involvedin the patient safety round, and to establish a policy regarding patient safety education and trainingincluding RCA training. It could be in the form of decree in order the implementation of policy wasdone properly, and the results could be evaluated

    Evaluation on Rooming in Mardi Rahayu, Kudus

    Infant mortality rate in Indonesia was high. Scientific evidence showed that infants with formula milk had higher probability to die than infant with exclusive breastfeeding. Implementation of ‘Sayang Ibu dan Anak\u27 hospital movement was done by performing rooming in program. Objective of this study was to evaluate the implementation of rooming-in program in Mardi Rahayu hospital Kudus. This was a descriptive-qualitative study. Study subjects were five main informants as policy makers, and five triangulation informants as executors of the program. Data were collected through in- depth interview. Content analysis method was applied in the data analysis. Results of the study showed that output aspect was not good; rooming in for mother and child were conducted ± 2 hours/day; formula milk was still given. Input aspect: human resource was not standardized to the type B hospital worker standard. Budget for each program was not equal; availability, sufficiency, and feasibility of facility were good. Third class post delivery room was not complete and uncomfortable. Not all standard operating procedure was performed. Marketing was not done. Process aspect was inadequate. Leader\u27s policy was made; socialization, actuating, and coordination were not done; monitoring and evaluation were not done; no commitment from the leaders and executors in the implementation of rooming in. In conclusion, Mardi Rahayu hospital performed partial rooming in; formula milk was still given. Human resource aspects were not suitable with type B hospital standard; budgeting program was not equal; facilities for third class were not suitable with the standard. Not all SOP was performed; marketing was insufficient, and no collective commitment was done. It was suggested to do total rooming-in, and to give formula milk according to the indication; to do lactation management training, breast milk counselor and supporter training, to conduct monitoring and evaluation

The purpose of this study is to analyze the internal and external environment RSISA SEC unit based marketing mix is used as the basis for the preparation of marketing plans startegik SEC RSISA Unit.This research uses descriptive qualitative research approach. Subjects of the study were taken by purposive sampling techniques include nursing services and medical director, head of the division units SEC, marketing head of the division, in charge of field units SEC, two eye specialist doctors, nurses, medical and non-medical support personnel as the main respondents while two SEC service users as respondents triangulation. Analysis of the data using content analysis. The results showed the power that is the latest medical technology supporting facilities, strategic location, affordable rates and nonmedical medics good.Weaknesses are inadequate waiting area, promotion is not maximized, nurse jobs overlap. Opportunity that is a one-stop service, has partnerships with government and private institutions, and the provision of social activities. Threats owned unit layout SEC is located alongside the town, numbering queue is still manual. Strategic advice includes working with fendor-fendor medical devices, a partnership with both the private and government institutions, a larger installation of billboards, to enforce the one-stop service.     Evaluasi Program Inisiasi Menyusu Dini oleh Bidan di Rumah Sakit Panti Wilasa Citarum Semarang

    Sesuai target Millenium Development Goals (MDGs) (AKI) di Indonesia sampai 2015 adalah 102 per 100.000. Dalam rangka mengatasi masalah AKI dan AKB, Departemen Kesehatan mengeluarkan kebijakan yang mengacu kepada intervensi strategis dalam upaya safe motherhood yang salah satunya yaituInisiasi menyusu dini. Rumah Sakit Panti Wilasa Citarum Semarang merupakan Rumah Sakit Sayang Ibu dan Bayi yang melakukan program IMD. Data tahun 2010 terdapat 43,1% persalinan normal dengan IMD dan tahun 2011 sebanyak 34.6%. Berdasarkan studi pendahuluan yang dilakukan pada 3 ibu bidan diketahui bahwa 2 dari 3 pelaksana IMD belum melaksanakan IMD sesuai dengan pedoman pelaksanaan IMD dan Rumah Sakit Panti Wilasa Citarum Semarang belum melaksanakan evaluasi selama 5 tahun terakhir. Tujuan penelitian untuk mengukur pelaksanaan program IMD oleh bidan di Rumah Sakit Panti Wilasa Citarum Semarang. Penelitian dilaksanakan di ruang bersalin dengan menggunakan pendekatan kualitatif deskriptif dengan 8 informan utama dan 11 informan triangulasi. Pengumpulan data dilakukan dengan indepth interview serta teori system Azrul Azwar digunakan sebagai kerangka konsep dengan analisis data secara induktif. Dari hasil penelitian dapat disimpulkan bahwa bidan sudah cukup berkulitas dan berkompeten dalam pelaksanaan IMD, sarana dan prasarana sudah cukup baik namun tidak tersedia Klinik Laktasidan KP-ASI sebagai sarana pendukung program, IMD, perencanaan anggaran IMD sudah cukup yaitu diperoleh dari rumah sakit dan dikelola oleh TIM RSSIB, protap IMD sudah tersedia di rumah sakit dan disahkan oleh Direktur sebagai petunjuk kerja IMD, perencanaan kegiatan IMD sudah cukup baik yaitu perencanaan tenaga IMD, dana dan sarana dan prasarana. Pelaksanaan kegiatan IMD belum cukup baik, bidan tidak melaksanakan IMD sesuai dengan protap IMD, dan pengawasan dilakukan oleh TIM RSSIB yaitu melakukan monitoring pelaksanaan program IMD di ruang VK. Dapat disimpulkan bidan belum mematuhi protap IMD sehingga perlunya diberikan motivasi kepada bidan pelaksana IMD agar bidan melaksanakan IMD sesuai dengan protap IMD dan evaluasi keseluruhan program IMD oleh Rumah Sakit Panti Wilasa Citarum Semaran

    Factors That Affect the Performance of the Village Midwife in the Standard Implementation of Midwifery Birthing Mothers and Orphans in Semarang

    In Semarang district, maternal mortality rate (AKI) increased from 101.92/ 100000 live-births in 2010 to 146.24/100000 live-births in 2011. The majority of death patients were caused by referral delay and a delay in detection of in labour emergency signs. This indicated that the implementation of obstetric standard care in Semarang district was not done properly. Objective of the study was to analyze factors affecting work performance of village midwives in the application of obstetric standard care for in labour mothers in Semarang district. This was an observational analytical study with cross sectional approach. Data were collected through interview guided by questionnaire. Study population was 311 village midwives in Semarang district. The number of respondents was 76 midwives selected purposively according to inclusion and exclusion criteria in each primary healthcare center. Product moment test and linear regression were applied in the data analysis. Results of the study showed that average age of respondents was 33 years old, average working period was 11 years old, majority of respondent\u27s education background was D III in midwifery (72.36%). Work performance of village midwives in the implementation of obstetric standard care for physiological mother in labour was insufficient (83%), work performance of village midwives in the implementation of obstetric standard care for pathological mother in labour was insufficient (87%), respondents with insufficient knowledge was 57%, respondents with high motivation was 51%, respondent\u27s perception on supervision was inadequate (55%), respondent\u27s perception on workload was low (57%), respondent\u27s perception on incentive was insufficient (57%), and respondent\u27s perception on sanction was inadequate (55%). Factors related to work performance of village midwives were knowledge (p= 0.000), motivation (p= 0.007), perception on supervision (p= 0.008), perception on workload (p= 0.012), perception on incentive (p= 0.000), perception on sanction (p= 0.000). Factors affecting work performance of village midwives in the implementation of obstetric standard care for in labour mothers were knowledge (p= 0.000), perception on workload (p= 0.027), perception on sanction (p= 0.000). Suggestions for Semarang district health office are to conduct refreshing on maternity services according to obstetric standard care to all village midwives, to improve supervision technique by implementing facilitative supervision

    Factors Associated to the Performance of Village Midwives in the Institutionalization of Local Area Monitoring in Maternal and Child Health (MCH- LAM) at Majalengka District West Java Province

    Monitoring effort on maternal and child health (KIA) service coverage known as local area monitoring of maternal and child health (PWS-KIA) was one of midwives duties. Good implementation of PWS-KIA could be used to follow up and improve KIA service. However, based on PWS-KIA implementation data in 2011, only 40% midwives who performed complete steps of PWS-KIA activities. Objective of this study was to analyze factors related to work performance of midwives to institutionalize PWS-KIA in Majalengka district area of West Java province, 2011. This was an observational study, and data was collected cross sectionally. Study population was all village midwives who resided in the work area of Majalengka district health office. These 360 midwives were distributed in 31 primary healthcare centers. Samples were 190 midwives. They were selected using simple random sampling method. Data collection was done through interview and observation using questionnair and observation form. Univariate, bivariate, and multivariate analysis were applied in the data analysis. Results of the study showed that work performance of less than a half (42.1%) of village midwives who institutionalized PWS-KIA was inadequate. Variables related to work performance of midwives who institutionalized PWS-KIA in Majalengka district area in 2011 were knowledge, availability of PWS- KIA facility, supervision of the head of puskesmas, and village leader support (p< 0.05). The most influencing variables was support/ the availability of PWS-KIA facilities (exp (â)= 9.784, p= 0.000). Suggestions for Majalengka district health office or primary healthcare centers are to provide PWS-KIA facility support to improve work performance of village midwives who institutionalized PWS-KIA; the support is in the form of providing PWS-KIA guideline books, stationery, computer devices; to conduct training on computer based PWS-KIA management data; to provide training for village midwives in their first duty; to conduct supervision by the head of primary healthcare center to village midwives; .to conduct coordination with village leader and sub district leader for PWS-KIA institutionalization

    Analysis on the Implementation of Delivery Assurance Programme by Village Midwives in Lumajang District

    Implementation of delivery assurance (Jampersal) program in Lumajang district was not doneaccording to the technical guideline; it was not done by way of the implementation of KB afterdelivery program was done. Objective of this study was to explain the implementation of deliveryassurance program by village midwives in Lumajang district.This was an explanatory study. Main informants were two village midwives from PONED and nonPONED primary healthcare centers (puskesmas) with the highest delivery coverage, and two villagemidwives from each puskesmas with the lowest delivery coverage. Triangulation informants were 12 people. Those people were one jampersal manager from district health office, one jampersalverification person, two heads of puskesmas, and 8 jampersal patients. Data were collected throughin-depth interview and observation to polindes (village polyclinic) facility. Content analysis methodwas applied in the data analysis.Results of the study showed that activities of delivery assurance service were conducted usingminimal standard service. Delivery assurance policy was a right policy to solve health cost problems.Executors of the policy were village midwives who had competency that suited with programpurposes. Target and environment supported the program implementation. Basic indicator andpurpose of the policy were clear and understandable for village midwives. However, funding wasnot maximal, and it caused many complaints from midwives. Communication and coordinationwere adequate; however social condition and community economy did not completely support theimplementation of post-delivery KB (family planning). Funding for jampersal was very small forboth delivery assistance and consumable materials. This condition made the implementation ofprogram was only for fulfilling the responsibility.Recommended suggestions for Lumajang district health office are to provide regulations related topost-delivery KB, to do proper planning regarding resources requirement to support serviceimplementation. Monitoring on the USAge of KIA book should be done by puskesmas to minimizetechnical problems such as double claim due to using double KIA book
