    <i>Actinomyces europaeus</i> Isolated from a Breast Abscess in a Penicillin-Allergic Patient.

    This is a case of Actinomyces europaeus in the breast abscess of a penicillin-allergic woman. The mainstay of treatment for actinomycosis is penicillin, and there is a lack of literature describing nonpenicillin treatment options. A 69-year-old woman presented acutely with a breast abscess which was managed with incision and drainage and antibiotic therapy to good response. 21 days after presentation, Actinomyces were grown from the culture of pus, so the patient was recalled and more rigorous treatment and follow-up were initiated. The penicillin allergy led to difficulty in the identification of an appropriate antimicrobial agent that was also logistically feasible to be given on an outpatient IV basis. IV tigecycline followed by oral clarithromycin was found to be effective treatment

    Neuropsychological Findings in Pediatric Maltreatment: Relationship of PTSD, Dissociative Symptoms, and Abuse/Neglect Indices to Neurocognitive Outcomes

    Maltreated (n=38), maltreated+posttraumatic stress disorder (PTSD)(N=60), and control youth (N=104) underwent comprehensive neuropsychological testing. The two maltreated groups performed significantly lower on IQ, Academic Achievement, and nearly all of the neurocognitive Domains than controls. Maltreated+PTSD performed significantly worse than maltreated youth without PTSD on a task in the Visuospatial Domain that assessed higher-order visuoconstructive abilities. No group differences were evident on the Fine-Motor Domain. PTSD diagnosis duration negatively correlated with the Visuospatial, and dissociation negatively correlated with the Attention Domain. Cumulative lifetime maltreatment types experienced negatively correlated with Academic Achievement. Sexual abuse negatively correlated with Language and Memory functions after controlling for other maltreatment types. These data support the adverse effects of maltreatment on neuropsychological functions in youth, and suggest that all child protective services identified youth should be comprehensively examined for the integrity of their neuropsychological functioning and academic skills, regardless of the presence or absence of mental health symptoms

    The relationship between superior longitudinal fasciculus microstructural integrity and neuropsychological functions were examined in 49 healthy children (range: 5–17 years) using diffusion tensor imaging. Seven major cognitive domains (intellegience, fine-motor, attention, language, visual-spatial, memory, executive function) were assessed. Data analyses utilized correlational methods. After adjusting for age and gender, fractional anisotropy and axial diffusivity values in the superior longitudinal fasciculus were positively correlated with executive functions of set-shifting; while left superior longitudinal fasciculus fractional anisotropy values correlated with attention and language. Apparent diffusion coefficient values in the left superior longitudinal fasciculus negatively correlated with inhibitory control. In the left arcuate fasciculus, fractional anisotropy correlated with IQ and attention; while radial diffusivity values negatively correlated with IQ, fine-motor skills, and expressive language. Findings from this study provide an examination of the relationship between superior longitudinal fasciculus integrity and children’s neuropsychological abilities that can be useful in monitoring pediatric neurological diseases

    In April 2017, a 56-year-old HIV-positive man was transferred from another institution to our regional infectious disease unit having presented with 2 weeks of profuse, watery diarrhea accompanied by intermittent, fresh bleeding of the rectum. Symptoms had started during a 2-month vacation to Indonesia, Vietnam, and Malaysia. While on vacation, he stayed in hotels in urban areas, had no rural travel, and drank only bottled water. He reported that he was not a man who has sex with men (MSM) and preferred to discuss his further sexual history confidentially with the genitourinary medicine team. Two months prior to admission, his CD4 count was 194 cells/mm3, and while taking an antiretroviral therapy regimen consisting of tenofovir, emtricitabine, and nevirapine, his HIV RNA had been undetectable in his plasma for 3 years

    Posterior structural brain volumes differ in maltreated youth with and without chronic posttraumatic stress disorder

    Abstract Magnetic resonance imaging studies of maltreated children with posttraumatic stress disorder (PTSD) suggest that maltreatment-related PTSD is associated with adverse brain development. Maltreated youth resilient to chronic PTSD were not previously investigated and may elucidate neuromechanisms of the stress diathesis that leads to resilience to chronic PTSD. In this cross-sectional study, anatomical volumetric and corpus callosum diffusion tensor imaging measures were examined using magnetic resonance imaging in maltreated youth with chronic PTSD ( N = 38), without PTSD ( N = 35), and nonmaltreated participants ( n = 59). Groups were sociodemographically similar. Participants underwent assessments for strict inclusion/exclusion criteria and psychopathology. Maltreated youth with PTSD were psychobiologically different from maltreated youth without PTSD and nonmaltreated controls. Maltreated youth with PTSD had smaller posterior cerebral and cerebellar gray matter volumes than did maltreated youth without PTSD and nonmaltreated participants. Cerebral and cerebellar gray matter volumes inversely correlated with PTSD symptoms. Posterior corpus callosum microstructure in pediatric maltreatment-related PTSD differed compared to maltreated youth without PTSD and controls. The group differences remained significant when controlling for psychopathology, numbers of Axis I disorders, and trauma load. Alterations of these posterior brain structures may result from a shared trauma-related mechanism or an inherent vulnerability that mediates the pathway from chronic PTSD to comorbidity

    Untangling free carrier and exciton dynamics in layered hybrid perovskites using ultrafast optical and terahertz spectroscopy

    Layered hybrid perovskites (LPKs) are promising as alternatives or additives to 3D metal halide perovskites for optoelectronic applications including photovoltaic cells, LEDs and lasers due to their increased stability. However, high exciton binding energies in these materials mean that excitons are the majority species under the operating conditions of many devices. Although the efficiency of devices that incorporate LPKs has been increasing, much is still unknown about the interplay of excitons and free charge-carriers in these materials, which is vital information for understanding how optoelectronic properties dictate device efficiency. In this work, we employ optical pump/THz probe spectroscopy (OPTP) and visible transient absorption spectroscopy (TAS) to analyse the optoelectronic properties and charge-carrier dynamics of phenylethylammonium lead iodide (PEA)2PbI4. By combining these techniques, we are able to disentangle the contributions from excitons and free charge-carriers. We observe fast cooling of free charge-carriers and exciton formation on a timescale of ∼400 fs followed by slower bimolecular recombination of residual free charge-carriers with a rate constant k 2 ∼ 109 cm3s−1. Excitons recombine via two monomolecular processes with lifetimes t 1 ∼ 11 ps and t2 ∼ 83 ps. Furthermore, we detect signatures of exciton–phonon coupling in the transient absorption kinetic traces. These findings provide new insight into the interplay between free charge-carriers and excitons as well as a possible mechanism to further understand the charge-carrier dynamics in LPKs

    The 2022 monkeypox outbreak: A UK military perspective.

    With the emergence of SARS-CoV-2 and now monkeypox, the UK Defence Medical Services have been required to provide rapid advice in the management of patients with airborne high consequence infectious diseases (A-HCID). The Defence Public Health Network (DPHN) cadre, consisting of closely aligned uniformed and civilian public health specialists have worked at pace to provide evidence-based recommendations on the clinical management, public health response and policy for monkeypox, with military medicine and pathology clinicians (primarily infectious disease physicians and medical microbiologists). Military environments can be complicated and nuanced requiring specialist input and advice to non-specialists as well as unit commanders both in the UK and overseas. DPHN and military infection clinicians have close links with the UK National Health Service (NHS) and the UK Health Security Agency (UKHSA), allowing for a dynamic two-way relationship that encompasses patient management, public health response, research and development of both UK military and national guidelines. This is further demonstrated with the Royal Air Force (RAF) Air Transport Isolator (ATI) capability, provided by Defence to support the UK Government and UKHSA. Military infectious disease clinicians are also embedded within NHS A-HCID units. In this manuscript we provide examples of the close interdisciplinary working of the DPHN and Defence clinicians in managing military monkeypox patients, co-ordinating the public health response, advising the Command and developing monkeypox policy for Defence through cross-government partnership. We also highlight the co-operation between civilian and military medical authorities in managing the current outbreak
