Wybrane aspekty działań niepożądanych złożonej doustnej antykoncepcji hormonalnej
The first hormonal pill was approved in the 60s of the twentieth century. Since that time, oral contraception has been used worldwide by dozens of women due to its high availability, as well as relative ease and safety of taking. The main side effects of oral contraception include elevated risk for venous thromboembolism (VTE). Estrogens increase the probability of VTE development, depending on the dose in medication, and third-generation progestins increase the risk of VTE development more than older-generation progestins. Also, the coexistence of hereditary thrombophilia increases the risk of VTE development in women using oral contraceptives. Other side effects include changes in the carbohydrate and lipid economy. Progestins in oral contraceptives decrease HDL cholesterol levels but increase LDL cholesterol and total cholesterol levels. Additionally, estrogens are a recognized mitogenic factor for the epithelium of the mammary gland, acting proliferative on the glandular tissue and in the same way influence on the increased risk of breast cancer development. Patients sometimes complain about some subjective side symptoms such as headache, mood changes, nausea, back pain, breast pain and swelling, as well as decreased libido. Some patients discontinue oral contraception due to fear of side effects or temporary ailments before consulting their doctor, what may result in unintended pregnancy. The aim of the following paper was to present most frequent side effects of oral contraception, ways of their monitoring

XX wieku, kiedy do powszechnego stosowania weszły pierwsze złożone doustne hormonalne leki antykoncepcyjne, są one szeroko stosowane na całym świecie ze względu na ich dużą dostępność, łatwość oraz względne bezpieczeństwo przyjmowania. Wśród działań niepożądanych stosowania doustnej antykoncepcji wymienia się głównie wzrost ryzyka żylnej choroby zakrzepowo-zatorowej (ŻCZZ). Estrogeny zwiększają prawdopodobieństwo rozwoju ŻCZZ zależnie od ich dawki w preparacie, a progestageny III generacji, podwyższają ryzyko
rozwoju ŻCZZ w większym stopniu niż progestageny starszej generacji. Również współwystępowanie wrodzonej trombofilii zwiększa ryzyko rozwoju zakrzepicy u kobiet stosujących doustne złożone leki antykoncepcyjne. Inne działania niepożądane to zmiany w gospodarce węglowodanowo-lipidowej. Progestageny zawarte w antykoncepcji doustnej zmniejszają stężenie cholesterolu HDL, a zwiększają stężenie cholesterolu LDL i cholesterolu całkowitego. Ponadto estrogeny są uznanym czynnikiem mitogennym dla komórek nabłonkowych gruczołu piersiowego działającym proliferacyjnie na tkankę gruczołową i tym samym wpływają na wzrost ryzyka rozwoju raka piersi. Subiektywne objawy podawane przez pacjentki po rozpoczęciu stosowania doustnej hormonalnej antykoncepcji to głównie bóle głowy, zmiany nastroju, zaburzenie libido, nudności, ból pleców i brzucha, ból i obrzmienie piersi. Niepokojący wydaje się fakt, że niektóre pacjentki w obawie przed działaniami niepożądanymi, bądź doświadczając przejściowych dolegliwości związanych ze stosowaniem antykoncepcji przerywa jej stosowanie bez wcześniejszego uzgodnienia z lekarzem, co może skutkować nieplanowaną ciążą. Celem pracy jest próba omówienia najczęstszych działań niepożądanych oraz metod ich monitorowania przy stosowaniu złożonej doustnej antykoncepcji hormonalnej
Wpływ wybranych substancji roślinnych i ich chemoprewencyjne działanie w raku jajnika
A steadily growing number of studies have confirmed the beneficial effects of plant-derived substances (preparations)
on the effectiveness of pharmacotherapy for ovarian cancer. A prior or parallel application of plant-derived
substances and chemotherapy could be the way to strengthen the classic pharmacological treatment.
Our paper presents several plant-derived substances with proven antiproliferative activities, in which phenolic and
flavonoid bioactive compounds dominate, with particular emphasis on ovarian cancer cells.
We are of the opinion that our paper will contribute to better understanding of the molecular basis for the positive
interaction effect of concomitant application of the abovementioned plant substances with certain cytostatics. Also,
this work may increase the number of preclinical in vivo experiments using these and other phenolic, flavonoid-rich
plant substances to better understand their efficacy and safety and, in the future, to initiate clinical trials in this field.Znaczna liczba doniesień potwierdza korzystny wpływ preparatów pochodzenia roślinnego na efektywność
farmakoterapii raka jajnika. Obiecujące wydaje się być stosowanie preparatów roślinnych przed rozpoczęciem
chemioterapii lub jako leczenie jednoczasowe.
W poniższym opracowaniu przedstawiono kilka substancji pochodzenia roślinnego o udowodnionej aktywności
antyproliferacyjnej ze szczególnym uwzględnieniem ich działania na komórki raka jajnika.
Prezentowana praca pozwoli na lepsze zrozumienie molekularnych podstaw interakcji pomiędzy substancjami
pochodzenia roślinnego a wybranymi cytostatykami. Ponadto może przyczynić do wzrostu liczby przedklinicznych
badań in vivo dotyczących powyższych preparatów, co będzie skutkowało poznaniem wydajności i bezpieczeństwa
substancji pochodzenia roślinnego w terapii onkologicznej

Pregnancy in the uterine scare after previous caesarean section is the rarest type of ectopic pregnancy. Due to
the possibility of life-threatening complications, cesarean scar pregnancy (CSP) needs rapid and proper diagnosis
and management. Hereby we present 3 cases of women with CSP, diagnosed and treated at the Department of
Gynecology and Obstetrics of Jagiellonian University Medical College, in Krakow, in 2013, as well as literature

Znaczenie współwystępowania polimorfizmów 677C>T oraz 1298A>C genu MTHFR u kobiet w populacji polskiej
Objectives: The aim of the study was to evaluate the frequency of the 677C>T and 1298A>C polymorphisms of
the methylenetetrahydrofolate reductase (MTHFR) gene, as well as the coexistence of both these genetic variants
in women from the Polish population.
Material and methods: A total of 662 women from the Polish population were enrolled in the study group. The
frequency of the investigated genotypes of the 677C>T and 1298A>C polymorphisms of the MTHFR gene was
analyzed with the use of PCR/RFLP methods.
Results: The frequency of the 677CC, 677CT and 677TT genotypes in the studied population of women was
50.60%, 39.88% and 9.52%, respectively. As to the 1298AA, 1298AC and 1298CC genotypes, the obtained
results were as follows: 42.75%, 47.88% and 9.37%, respectively (Tables II and III). Simultaneous analysis revealed
the most frequent coexistence of 677CC/1298AC (28.85%), 677CT/1298AA (20.85%) and 677CT/1298AC
(19.03%) genotypes. The coexistence of 677CC/1298AA (12.39%), 677CC/1298CC (9.37%) and 677TT/1298AA
(9.51%) genotypes was observed less frequently. In the studied population of Polish women, the coexistence of
677CT/1298CC, 677TT/1298AC and 677TT/1298CC genotypes has been not observed.
Conclusions: The frequency and coexistence of genotypes of the 677C>T and 1298A>C MTHFR gene
polymorphisms in the studied population of Polish women is similar to other North-European populations. Women
carriers of the mutated variants of both, 677C>T and 1298A>C polymorphisms of the MTHFR gene should receive
special perinatal care in order to prevent fetal defects and thrombosis-related complications during pregnancy. It is
vital to emphasize the significance of proper education of folate supplementation, especially in pregnant patients
and women of reproductive age.Cel: Określenie częstości występowania polimorfizmów 677C>T i 1298A>C genu kodującego reduktazę
metylenotetrahydrofolianową (MTHFR) oraz ocena współwystępowania obydwu wariantów genetycznych u kobiet
w populacji polskiej.
Materiał i metoda: Do grupy badawczej włączono 662 kobiety z populacji polskiej. Częstość występowania
genotypów polimorfizmów 677C>T oraz 1298A>C genu MTHFR została zbadana metodą PCR/RFLP.
Wyniki: W badanej grupie częstość występowania genotypów 677CC, 677CT oraz 677TT wynosiła odpowiednio
50,60%, 39,88% oraz 9,52%. Natomiast genotypy 1298AA, 1298AC oraz 1298CC występowały z częstością
42,75%, 47,88% oraz 9,37%. Jednocześnie analiza współwystępowania genotypów pozwoliła na określenie
najczęściej występujących genotypów – 677CC/1298AC (28,85%), 677CT/1298AA (20,85%), 677CT/1298AC
(19,03%). Współwystępowanie genotypów 677CC/1298AA (12,39%), 677CC/1298CC (9,37%), 677TT/1298AA
(9,51%) było obserwowane rzadziej. W badanej populacji nie zaobserwowano współwystępowania polimorfizmów
677CT/1298CC, 677TT/1298AC, 677TT/1298CC.
Wnioski: Częstość występowania i współwystępowania polimorfizmów 677C>T i 1298A>C u kobiet w populacji
polskiej jest porównywalna do częstości występowania tych wariantów w innych populacjach północnoeuropejskich.
Kobiety, nosicielki zmutowanych genotypów polimorfizmów 677C>T i 1298A>C genu kodującego MTHFR, powinny
zostać objęte specjalną opieką perinatalną, z powodu wzrostu ryzyka wystąpienia wad płodu oraz powikłań
zakrzepowych w czasie ciąży. Należy zwrócić uwagę na właściwą edukację i suplementację folianami kobiet,
szczególnie w wieku rozrodczym oraz w czasie ciąży
Rola szlaku sygnałowego Wnt/β-katenina oraz białka LRP5 w metabolizmie tkanki kostnej oraz etiologii osteoporozy
Osteoporosis is a metabolic bone disease, manifested by decreased bone mineral density, microarchitectural
disturbances of bone tissue, and increased risk of bone fractures. Owing to large-scale morbidity, particularly
among postmenopausal women, nowadays osteoporosis constitutes a significant global health problem. In
recent years, much attention has been paid to the role of signaling Wnt/β-catenin pathway and LRP protein in the
pathomechanism of osteoporosis, indicating a possible contribution of polymorphic variants of the candidate LRP5
gene to disease development.
The goal of our study is to present contemporary research on signaling Wnt/β-catenin pathway and mechanism of
LRP protein action in the process of bone tissue metabolism and etiology of osteoporosis.Osteoporoza jest metaboliczną chorobą szkieletu kostnego, objawiającą się zmniejszeniem gęstości mineralnej
kości, zaburzeniami wewnętrznej mikroarchitektury tkanki kostnej i zwiększeniem ryzyka wystąpienia złamań.
Z uwagi na olbrzymią skalę zachorowań, szczególnie wśród kobiet po menopauzie, osteoporoza stanowi obecnie
istotny problem zdrowotny na całym świecie. Od kilku lat duże zainteresowanie wzbudzają badania dotyczące roli
szlaku sygnałowego Wnt/β-katenina oraz białka LRP5 w patomechanizmie osteoporozy, wskazujące również na
możliwy udział wariantów polimorficznych genu kandydującego LRP5 w rozwoju tej choroby.
Celem pracy jest przedstawienie najnowszych badań dotyczących szlaku sygnałowego Wnt/β-katenina oraz
mechanizmu działania białka LRP5 w procesie metabolizmu tkanki kostnej oraz etiologii osteoporozy

Vertical transmission of HPV in pregnancy. A prospective clinical study of HPV-positive pregnant women
Introduction: Human papillomavirus (HPV) is the most common sexually transmitted infection. Data reporting vertical transmission of HPV from the mother to the fetus are inconsistent and scant. Vertical transmission may occur by hematogenic route (transplacental), or by ascending contamination, or through the birth canal, which may result in the dreaded and rare laryngeal papillomatosis. Infected sperm at fertilization is a potential route of infection, too. Objective: The objective of the study was to evaluate the rate of vertical transmission of HPV in HPV-positive pregnant women to their newborn infants, as well as the risk factors of HPV vertical transmission. Material and methods: The clinical material was provided by 136 pregnant women, aged 18-45 years. Out of this group, 30 (22.05%) women with abnormal Pap test and positive DNA HPV test were prospectively observed. Neonatal status, i.e. DNA HPV from the nasopharyngeal smear, was recorded in all infants during the perinatal period. The conventional Pap test was performed with the cervix brush in all women. The Bethesda 2011 classification system was applied. Results: An average C Reactive Protein (CRP) concentration in the studied pregnant women was 11.6083 (Std Dev – 12.93). The most frequent cytological findings in the cervical smears from the examined women were ASCUS, n=13 (43.3%), then – LSIL, n=10 (33.3%), HSIL- n=5 (16.7%) and AGC- n=2 (6.7%). In the neonates, the presence of LR HPV DNA was detected in 9 cases (30.0%) and HR HPV DNA in 7 cases (23.3%). Fourteen neonates (46.7%) tested HPV DNA negative in the perinatal period. Conclusions: HPV infection (incidental or chronic) is observed in approximately 22% of pregnant women from the Małopolska province. Neonatal HPV infection in HPV-positive women was observed in 53.3% of the subjects. CRP concentration > 10 mg/dl in the serum of pregnant women statistically significantly (p 0.001) reduces the risk of vertical transmission of HPV from the mother to the fetus
Udział polimorfizmu Hind III C>G genu PAI-1 w etiologii poronień nawracających
Objectives: The goal of the study was to assess the relationship of HindIII C>G PAI-1 gene polymorphism with
increased risk of recurrent miscarriages.
Material and methods: A whole of 152 women with a history of at least two miscarriages were classified into
analysis. The study group was divided twice (114 subjects with 2 miscarriages and 38 subjects with >3 miscarriages,
123 subjects with miscarriages at 3 miscarriages (50.0% vs. 42.2% in controls, OR=1.37, p=0.13). A tendency
of higher frequency of HindIII GG genotype and HindIII G allele was also noted in the subgroup of patients with
miscarriages in the first and second trimester (HindIII GG: 31.0% vs. 20.0% in controls, OR=1.80, p=0.14, HindIII
G: 51.7% vs. 42.2% in controls, OR=1.47, p=0.11).
Conclusions: Mutated HindIII G allele and HindIII GG genotype of HindIII C>G polymorphism probably augment
the risk of recurrent miscarriages.Cel pracy: Ocena związku polimorfizmu HindIII C>G genu PAI-1 z występowaniem poronień nawracających.
Metody: W badaniu uwzględniono dwie grupy kobiet: 152 pacjentki z obciążonym wywiadem w kierunku
występowania poronień (dwóch lub więcej) oraz 180 pacjentek, u których potwierdzono w wywiadzie co najmniej
jedną ciążę o niepowikłanym przebiegu, zakończoną urodzeniem zdrowego, donoszonego noworodka oraz
wykluczono wystąpienie poronień. W grupie kobiet z poronieniami nawracającymi dokonano podziału na podgrupy
(114 kobiet z dwoma poronieniami w wywiadzie oraz 38 kobiet z >3 poronieniami w wywiadzie, 123 pacjentki,
u których poronienia występowały poniżej 13 tc. oraz 29 kobiet, u których poronienia miały miejsce poniżej 21 tc.).
Analizę genetyczną przeprowadzono przy zastosowaniu metody PCR/RFLP.
Wyniki: Zmutowany genotyp HindIII GG w grupie badanej występował z częstością 25,7%, natomiast wśród kobiet
z grupy kontrolnej z częstością 20,0% (OR=1,38, p=0,14). Podobna obserwacja dotyczyła częstości występowania
zmutowanego allela HindIII G (45,7% vs. 42,2% w grupie kontrolnej, OR=1,15, p=0,20). W podgrupie pacjentek
z >3 poronieniami frekwencja genotypu HindIII GG była wyraźnie wyższa niż w grupie kontrolnej (31,6% vs. 20,0%
w grupie kontrolnej, OR=1,85, p=0,09). Również frekwencja allela HindIII G była wyższa w tej podgrupie (50,0%
vs. 42,2% w grupie kontrolnej, OR=1,37, p=0,13). Tendencja do częstszego występowania genotypu HindIII GG
i allela HindIII G była także obserwowana u pacjentek z poronieniami G genu PAI-1 prawdopodobnie zwiększają ryzyko występowania poronień nawracających
Chorionic thickness and PlGF concentrations as early predictors of small-for-gestational age birth weight in a low risk population
Objectives: SGA is associated with higher incidence of postnatal complications, including suboptimal neurodevelopment and increased cardiovascular risk. Screening for SGA, carried out at 11–13 (+ 6d) gestational weeks enables to reduce or completely eliminate the above mentioned complications. The aim of this study was to assess the correlation between chorionic thickness, concentration of PIGF protein and foetal birth weight in a single low-risk pregnancy.
Material and methods: The study included 76 patients at 11–13 (+ 6d) gestational weeks, monitored throughout pregnancy. Ultrasound examinations identified the location and thickness of the chorion by measuring it in its central part at its widest point in a sagittal section. Additionally, at each visit venous blood was collected to determine the level of PlGF, PAPP-A, and BhCG.
Results: A significant positive correlation (r = 0.37) was found between the foetal weight and chorionic thickness. This correlation was affected by the location of the chorion and a significant negative correlation was observed between the level of PLGF, FHR, weight and length of the newborn. Maternal early-pregnancy BMI did not affect neonatal weight and body length, FHR, chorionic thickness, and the levels of PlGF, PAPP-A, and BhCG.
Conclusions: The preliminary analysis indicates an association between chorionic thickness assessed during ultrasound at 11–13 (+ 6d) gestational weeks, PIGF levels assayed at the same time and birth weight. Increasing chorion thickness was accompanied by increasing foetal birth weight. PlGF level showed an inversely proportional effect on the foetal weight.
