Transcranial ultrasound monitoring in patients with acute stroke
Przezczaszkowa ultrasonografia doplerowska (TCD, transcranial Doppler) i/lub przezczaszkowa ultrasonografia doplerowska
z zakodowanym w kolorze przepływem (TCCD, transcranial color-coded duplex) jest znakomitą techniką szybkiej, nieinwazyjnej
i przyłóżkowej diagnostyki miejsca oraz stopnia niedrożności (w znaczeniu wielkości przepływu rezydualnego) naczyń
mózgowych u pacjentów w ostrym okresie udaru niedokrwiennego mózgu, a także monitorowania zmian drożności naczyń
pod wpływem leczenia trombolitycznego oraz w naturalnym przebiegu choroby. W ostatnich latach wypracowano protokół
tak zwanej szybkiej ścieżki diagnostycznej TCD, umożliwiający błyskawiczne rozpoznanie miejsca niedrożności naczynia
mózgowego, stworzono skalę, która pozwala określić stopień przepływu rezydualnego w naczyniu - Thrombolysis in Brain
Ischemia (TIBI), sklasyfikowano oznaki rekanalizacji naczynia oraz określono rokowanie dotyczące szans na skuteczną
trombolizę i poprawę kliniczną zależnie od umiejscowienia niedrożności, wielkości przepływu rezydualnego, szybkości
i efektywności rekanalizacji. Wstępne wyniki badań wskazują na możliwość terapeutycznego zastosowania ultradźwięków
o niskiej częstotliwości. Ograniczeniem tej metody jest nieodpowiednie okno kostne skroniowe u niektórych osób oraz duża
zależność wyników badania od doświadczenia osoby badającej i od jakości sprzętu.Both transcranial doppler and/or transcranial color-coded duplex combined with extracranial duplex are excellent fast, noninvasive and bedside methods to diagnose the site and degree of arterial occlusion in patients with acute stroke. By these
methods the changes in vessel patency during thrombolytic treatment and in the natural course of the disease can be
followed. Recently the protocol of the fast track TCD was developed, allowing very quick diagnosis of the site of arterial
occlusion. The grades of residual flow were classified in the Thrombolysis of Brain Ischemia Scale and the TCD symptoms of
recanalisation were described. The chances to successful thrombolysis and clinical improvement were determined based on
TCD diagnosis and recanalisation monitoring. The first studies were published on the therapeutic use of low frequency
ultrasound in assisting the treatment with tissue plasminogen activator. The both methods are easy to use and non-invasive,
howevere the thick bone window in some patients and strong dependency of the results on the examiner experience and
quality of the equipment limit their use
Obustronny udar wzgórza z dysfunkcją podwzgórza
Unilateral thalamic lesions cause transient or permanent behavioral, sensory and oculomotor disturbances; bilateral lesions of thalamus result in more severe and longer lasting symptoms. We present an atypical case of bilateral paramedian thalamic infarct with concomitant hypothalamic dysfunction. The only risk factor of ischaemic stroke found in the patient was a short lasting episode of atrial fibrillation. Bilateral paramedian thalamic infarcts may result from occlusion of one paramedian thalamic artery, which arises from the posterior cerebral artery, either with separated or with a common trunk, thus supplying the thalamus bilaterally. Independently of anatomical variants of thalamus blood supply, the most probable cause of infarct in our patient was unilateral or bilateral occlusion of the posterior cerebral artery by cardioembolism, probably in the course of basilar artery occlusion. Hypothalamic dysfunction may accompany thalamic infarcts; thus hypothalamo-pituitary function should be routinely assessed in bithalamic infarcts.Jednostronne udary w obrębie wzgórza mogą być przyczyną przemijających lub trwałych zaburzeń zachowania, objawów czuciowych lub zaburzeń gałkoruchowych. Obustronne udary wzgórza skutkują zwykle bardziej nasilonymi i dłużej trwającymi objawami. W niniejszym artykule zaprezentowano przypadek obustronnego udaru wzgórza z jednoczesną dysfunkcją podwzgórza. Jedynym czynnikiem ryzyka udaru niedokrwiennego mózgu, jaki stwierdzono u pacjenta, był krótkotrwały epizod migotania przedsionków. Obustronne udary wzgórza mogą być wynikiem zamknięcia jednej tętnicy przyśrodkowej wzgórza, odchodzącej od tętnicy tylnej mózgu albo w postaci dwóch osobnych gałęzi, albo też jednego wspólnego pnia, zaopatrującego jednak wzgórze obustronnie. Niezależnie jednak od wariantów anatomicznych unaczynienia wzgórza najbardziej prawdopodobną przyczyną udaru niedokrwiennego u przedstawionego pacjenta było jednostronne lub też obustronne zamknięcie tętnicy tylnej mózgu przez materiał zatorowy, prawdopodobnie w przebiegu zamknięcia szczytu tętnicy podstawnej. Dysfunkcja podwzgórza może towarzyszyć udarom wzgórza, autorzy sugerują więc, aby funkcja układu podwzgórzowo-przysadkowe-go była rutynowo oceniana w przypadku obustronnych udarów wzgórza

Predictors of intracranial cerebral artery stenosis in patients before cardiac surgery and its impact on perioperative and long-term stroke risk
Background
The aim of this prospective study was to determine the prevalence of stenosis within intracranial and extracranial arteries in patients before coronary artery bypass surgery (CABG), to evaluate the influence of intracranial artery stenosis on neurological outcome and to identify preoperative risk factors for these patients.
Methods
One hundred and seventy-five patients (71% males, mean age=66.1) scheduled for CABG were enrolled for extracranial Doppler duplex sonography, transcranial color-coded duplex sonography (TCCS) and transcranial Doppler (TCD) examination.
Results
Twenty-six patients (14.7%) had extracranial stenosis or occlusion and 13 patients (7.3%) intracranial vascular disease. Six patients (3.5%) had both extra- and intracranial artery disease. The presence of peripheral artery disease and diabetes mellitus was a strong risk factor for extracranial artery stenosis but not for intracranial artery stenosis, which occurred independently also of typical atherosclerotic risk factors like age >70, male sex, hypertension, hyperlipidemia, hyperhomocysteinemia, smoking habit, obesity (BMI>30) and waist to hip ratio >1. Functional neurological outcome of the patients with intracranial arterial disease evaluated 7 days after CABG was the same as the patients without extra- and intracranial stenosis. However, 12-months neurological follow-up revealed significantly more ischemic strokes in patients with intracranial artery stenosis compared to patients without intracranial stenosis (p=0.015).
Conclusion
The occurrence of intracranial artery stenosis in CABG patients cannot be predicted by well-known atherosclerotic risk factors and seems not to be associated with perioperative stroke

Quantitative evaluation of crossed cerebellar diaschisis, using voxel-based analysis of Tc-99m ECD brain SPECT
BACKGROUND: In the recent literature there is no consensusregarding the relationships between crossed cerebellar diaschisis(CCD) with the primary lesion size, severity or location.Thus, the aim of the present study was to investigate relationshipbetween the size and severity of cerebral lesions and CCD in patientswith chronic stroke, using voxel-based analysis of Tc-99mECD single-photon emission computed tomography (SPECT).MATERIAL AND METHODS: We retrospectively reviewed dataof 57 patients with chronic ischemic lesions localized unilaterallyin the cerebral hemisphere. SPECT evaluation was performedwith the voxel-based analysis. The percentage inter-hemisphericasymmetry index (AI) and the volume of abnormal clusters ofvoxels (CV) were ascertained for hypoperfusion in the supratentoriallesion and contralateral cerebellum.RESULTS: CCD was present in 35.1% cases. In Group CCD(+), the CV and AI of supratentorial hypoperfusion (median128.1 ml and 21.9%, respectively) were significantly highercompared with Group CCD(–) (median 41.4 ml and 18.0%,respectively). Statistically significant correlation was foundbetween CV of supratentorial and cerebellar perfusion defects(r = 0.4; p < 0.05), between AI of supratentorial and cerebellarperfusion defects (r = 0.6; p < 0.05) and between CV of supratentorial defect and AI of cerebellar perfusion defects (r = 0.6; p < 0.05).CONCLUSIONS: Our data suggest, that in the chronic stageof stroke, the size and severity of the supratentorial lesion aredeterminants of CCD, correlating with the degree of cerebellarhypoperfusion
