Administration of 1α-OH vitamin D3 and calcium prevents bone mass loss in patients with advanced prostatic carcinoma after orchidectomy treated with complete androgenic blockade
Całkowita blokada androgenowa stosowana w leczeniu zaawansowanego raka stercza osiągana jest przez podawanie pacjentom po orchidektomii preparatów antyandrogenowych lub łączne stosowanie analogu LH-RH wraz z lekiem antyandrogennym. Następstwem tej terapii może być jednak przyspieszony ubytek masy kostnej oraz zwiększenie ryzyka złamań szkieletu, wynikające z ograniczenia wpływu zarówno androgenów, jak i estrogenów na tkankę kostną.
Celem pracy było określenie wpływu całkowitej blokady androgenowej na metabolizm i masę tkanki kostnej u pacjentów z zaawansowanym rakiem stercza oraz ocena czy podawanie 1α-OH witaminy D3 (1α-OHD3) wraz z suplementacją wapnia może zapobiec ubytkowi masy kostnej u mężczyzn leczonych całkowitą blokadą androgenową.
Trwającymi 12 miesięcy, prospektywnymi badaniami objęto 51 pacjentów z zaawansowanym rakiem stercza, z przerzutami do układu kostnego, w wieku 44-86 lat, średnio 68 lat. Wszyscy chorzy poddani byli orchidektomii z następową terapią flutamidem w dawce 750 mg/dobę. 26 pacjentów otrzymywało ponadto 1α-OHD3 w dawce 0,5 µg/dobę oraz węglan wapnia w dawce początkowej 1g/dobę.
Wykazano, że 12-miesięczne leczenie całkowitą blokadą androgenową spowodowało obniżenie gęstości mineralnej kości (BMD) o 8,1% w odcinku lędźwiowym kręgosłupa, o 6,3% w szyjce kości udowej i o 3,5% sumarycznie w całym szkielecie.
Terapia 1α-OHD3 i CaCO3 całkowicie zahamowała ubytek tkanki kostnej w kręgosłupie lędźwiowym oraz spowodowała wzrost BMD o 2,2% w szyjce kości udowej i o 1,9% sumarycznie w całym szkielecie. U żadnego z badanych pacjentów nie stwierdzono złamań szkieletu.
W obu grupach pacjentów obserwowano szybki spadek aktywności fosfatazy zasadowej - markera aktywności osteoblastów oraz wzrost wydalania wapnia z moczem, w przeliczeniu na kreatyninę wskazujące na przyspieszenie resorpcji tkanki kostnej.
Wnioski: u pacjentów z zaawansowanym rakiem stercza leczonych całkowitą blokadą androgenową dochodzi do przyspieszonego ubytku tkanki kostnej; stosowanie 1α-OHD3 i CaCO3 zapobiega ubytkowi zarówno kości beleczkowej, jak i korowej.Complete androgenic blockade used in the treatment of advanced prostatic carcinoma can be attained by administration of antiandrogens in orchidectomized patients or by combined therapy with LH-RH analogs and antiandrogens. The treatment, however, decreases the influence of both androgens end estrogens on bone tissue and may result in bone mass loss and increased propensity to fractures. The purpose of the study was to determine the influence of complete androgenic blockade on bone mass and skeletal metabolism in men with advanced prostatic carcinoma and to assess whether 1α-OH vitamin D3 (1α-OHD3) together with calcium supplementation is able to prevent bone mass loss in men treated with complete androgenic blockade. 51 patients with advanced prostatic carcinoma, with skeletal metastases, aged 44-86, mean 68 ys were included into a 12-month prospective study. All patients were treated with orchidectomy followed by therapy with flutamide in a dose of 750 mg daily. 26 patients were additionally given 1α-OHD3 in a dose of 0.5 µg/d and calcium carbonate in an initial dose of 1 g daily. It was found that the 12-month treatment with complete androgenic blockade resulted in a decrease in bone mineral density (BMD) by 8.1% in the lumbar spine, by 6.3% in the femoral neck and by 3.5% in the total skeleton. Therapy with 1α-OHD3 and CaCO3 caused complete inhibition of bone tissue loss in the lumbar spine and resulted in an increase in BMD by 2.2% in femoral neck and by 1.9% in the total skeleton. None of the examined patients experienced any skeletal fractures. In both groups of patients a prompt decrease in serum alkaline phosphatase activity - a marker of osteoblast activity and an increase in fasting urine calcium creatinine ratio indicating acceleration of bone resorption were found. Conclusions: in patients with advanced prostatic carcinoma treated with complete androgenic blockade acceleration of bone mass loss is observed; treatment with 1α-OHD3 and CaCO3 is able to prevent both trabecular and compact bone loss

Landscapes of Polish memory: conflicting ways of dealing with the communist past in a Polish town
This research centres on the local acts of national memory politics and retributive justice performed
in a Polish town during the years 2006‐2008, when a conservative Law and Justice Party government
was in power. Looking at the political processes of conventionalizing and objectifying the communist
past in authoritative settings (commemorative rituals, unveiling of monuments, courtrooms,
associational meetings, Catholic masses) the author searched for patterns of inclusion and exclusion
of political subjects into/from the commemorative landscape of the Polish historicized national
community.
The thesis concerns the two broadly defined categories of people who became politically engaged
during communism: the anti‐communist activists and the ex‐officers of the Ministry of Interior
Affairs. The author combines participant observation in various institutional settings with archival
work, Critical Discourse Analysis of institutionalized discourses, analysis of material culture of
commemoration and an in‐depth work on personal narratives, in order to address the mediating role
which state and religious institutions, their agents, and the representations produced by them have
played in individual processes of remembering, commemorating, and recalling.
The thesis describes the conflicting ways in which the two groups of subjects have actively engaged
in an interactive production of representations and claims concerning the communist past. It
examines ways in which the institutionalization of memory has generated a cultural form of self-perception
relying on a sequential and repeatable way of experiencing the self in a collectively
achieved framework of ‘hero/victim’ or ‘perpetrator’.
The author approaches the topic of memory by imagining it as a multidimensional figure,
understanding the processes of individual remembering and constructing the self as located at the
intersection of processes of perception, development of self-schemata, group interaction, and
collective instances of objectification and conventionalizing
