    Part 2: Preparing Entry-Level Occupational and Physical Therapy Students to Promote Healthy Lifestyles Emphasizing Healthy Eating with Individuals with Disabilities

    Rehabilitation professionals including occupational therapists (OT) and physical therapists (PT) are increasingly called upon to incorporate health promotion of lifestyle behaviors including physical activity and healthy eating into routine clinical care. While OTs and PTs may be comfortable promoting activity-related behaviors, many are less comfortable with nutrition behaviors. To address entry-level OT and PT students’ perceived discomfort with discussing diet-related behaviors, faculty developed a healthy eating module for students to use during a community-based service learning program. The purpose of this paper is to describe the formative evaluation process of developing the healthy eating module, and to discuss results of a pilot trial of this module. The formative assessment of the healthy eating module consisted of four steps: focus groups with students, key informant interviews with community partners, expert panel round table, and expert panel review of materials. Students (n=117) completed questionnaires at the end of the service learning program to assess how much they used the new resources, and how useful they found the resources. The final healthy eating module consisted of an on-line training session and a healthy eating toolkit, including resources for assessments, treatment activities and additional nutrition information.     To address accreditation standards for health and wellbeing within entry-level occupational therapy (OT) and physical therapy (PT) programs, the OT, PT, and Human Studies Departments at the University of Alabama at Birmingham (UAB) collaborated with community partners to conduct an interdisciplinary service learning activity based on the I Can Do It, You Can Do It Program (ICDI). This program is a structured community health program where individuals without disabilities are partnered with individuals with disabilities to enhance physical activity, healthy eating, and community participation. The purpose of this paper is to describe a formative evaluation of ICDI at UAB, and to discuss revisions to the program made as a result of the evaluation. Faculty used a qualitative design to collect feedback on perceived benefits and challenges of the program. Focus groups were conducted with students who completed the program, and key informant interviews were conducted with site coordinators from each of the three partnering community sites. Two themes emerged from student focus groups: (1) Program benefits, with sub-themes of hands-on application and interaction, and (2) Challenges with suggestions for change, with sub-themes of preparation, communication, and expectations. Four themes emerged from key informant interviews: (1) Students, (2) Logistics, (3) Program benefits, and (4) Transference. Results of this evaluation led to a number of revisions for the 2016 cohort. Future evaluations will include objective measures of change in student knowledge over time, as well as health and behavioral outcomes of community members who participated in the ICDI program at UAB

    The Heritability of Amyotrophic Lateral Sclerosis in a Clinically Ascertained United States Research Registry

    The genetic basis of amyotrophic lateral sclerosis (ALS) is not entirely clear. While there are families with rare highly penetrant mutations in Cu/Zn superoxide dismutase 1 and several other genes that cause apparent Mendelian inheritance of the disease, most ALS occurs in families without another affected individual. However, twin studies suggest that all ALS has a substantial genetic basis. Herein, we estimate the genetic contribution to ALS in a clinically ascertained case series from the United States.We used the database of the Emory ALS Center to ascertain individuals with ALS along with their family histories to determine the concordance among parents and offspring for the disease. We found that concordance for all parent-offspring pairs was low (<2%). With this concordance we found that ALS heritability, or the proportion of the disease explained by genetic factors, is between 40 and 45% for all likely estimates of ALS lifetime prevalence.We found the lifetime risk of ALS is 1.1% in first-degree relatives of those with ALS. Environmental and genetic factors appear nearly equally important for the development of ALS

    Quantitative measurement of thyroglobulin mRNA in peripheral blood of patients after total thyroidectomy

    Previous studies have reported the clinical usefulness of reverse transcription-polymerase chain reaction (RT-PCR) detection of thyroglobulin (TG) mRNA in the peripheral blood of patients with differentiated thyroid carcinoma. To evaluate this usefulness, we measured TG mRNA in the peripheral blood of patients diagnosed with thyroid carcinoma after total thyroidectomy by real-time quantitative RT-PCR using glyceraldehyde-3-phosphate dehydrogenase (GAPDH) mRNA as an internal control. Surprisingly, we detected TG mRNA in all samples obtained after total thyroidectomy, including those from 4 medullary carcinomas. Further, there was no statistical difference in expression levels of TG mRNA in the patients with or without metastasis, and no significant correlation was found between serum TG concentrations and the expression levels of TG mRNA. These results give rise to a question regarding the clinical applications of not only RT-PCR detection but also quantitative measurement of TG mRNA in peripheral blood.     Reliability and validity study of Persian modified version of MUSIC (musculoskeletal intervention center) – Norrtalje questionnaire

    <p>Abstract</p> <p>Background</p> <p>Musculoskeletal disorders (MSDs) are a major health problem in the world. Self-reported questionnaires are a known method for estimating the prevalence of MSDs among the population. One of the studies concerning MSDs and their relation to work-related physical and psychosocial factors, as well as non-work-related factors, is the MUSIC-Norrtalje study in Sweden. In this study, the research group developed a questionnaire, which has been validated during its development process and is now considered a well-known instrument. The aim of this study is to validate the Persian version of this questionnaire.</p> <p>Methods</p> <p>The first step was to establish two expert panel groups in Iran and Sweden. The Focus Group Discussion (FGD) method was used to detect questionnaire face and content validity. To detect questionnaire reliability, we used the test-retest method.</p> <p>Results</p> <p>Except for two items, all other questions that respondents had problems with in the focus group (20 of 297), had unclear translations; the ambiguity was related to the stem of the questions and the predicted answers were clear for the participants. The concepts of 'household/spare time' and 'physical activity in the workplace' were not understood by the participants of FGD; this has been solved by adding further descriptions to these phrases in the translation. In the test-retest study, the reliability coefficient was relatively high in most items (only 5 items out of 297 had an ICC or kappa below 0.7).</p> <p>Conclusion</p> <p>The findings from the present study provide evidence that the Persian version of the MUSIC questionnaire is a reliable and valid instrument.</p

    Allelic losses on chromosome 3p are accumulated in relation to morphological changes of lung adenocarcinoma

    We performed allelotyping analysis at nine regions on chromosome 3p using 56 microdissected samples from 23 primary lung adenocarcinomas to examine the process of progression within individual lung adenocarcinoma with various grades of differentiation. Identical allelic patterns among various grades of differentiation were found in eight cases. Accumulation of allelic losses from high to lower differentiated portions was found in seven cases and accumulation of allelic losses from low to higher differentiated portions was found in five cases. Various allelic patterns among various grades of differentiation were found in three cases. These results suggested that allelic losses on 3p play an important role in morphological changes of lung adenocarcinomas. We also investigated the relationship between allelic losses on 3p and histological subtypes of lung adenocarcinoma. The frequencies of allelic losses at 3p14.2 and telomeric region of 3p21.3 were higher in papillary type tumour (nine out of 14, 64% and 11 out of 15, 73%) than in bronchioloalveolar carcinoma-type tumour (one out of 8, 13%; P=0.031 and four out of 12, 33%; P = 0.057). These results indicated that allelic losses at 3p14.2 and telomeric region of 3p21.3 are related to pattern of the proliferation of lung adenocarcinoma

    Gene Expression of the Tumour Suppressor LKB1 Is Mediated by Sp1, NF-Y and FOXO Transcription Factors

    The serine/threonine kinase LKB1 is a tumour suppressor that regulates multiple biological pathways, including cell cycle control, cell polarity and energy metabolism by direct phosphorylation of 14 different AMP-activated protein kinase (AMPK) family members. Although many downstream targets have been described, the regulation of LKB1 gene expression is still poorly understood. In this study, we performed a functional analysis of the human LKB1 upstream regulatory region. We used 200 base pair deletion constructs of the 5β€²-flanking region fused to a luciferase reporter to identify the core promoter. It encompasses nucleotides βˆ’345 to +52 relative to the transcription start site and coincides with a DNase I hypersensitive site. Based on extensive deletion and substitution mutant analysis of the LKB1 promoter, we identified four cis-acting elements which are critical for transcriptional activation. Using electrophoretic mobility shift assays as well as chromatin immunoprecipitations, we demonstrate that the transcription factors Sp1, NF-Y and two forkhead box O (FOXO) family members FOXO3 and FOXO4 bind to these elements. Overexpression of these factors significantly increased the LKB1 promoter activity. Conversely, small interfering RNAs directed against NF-Y alpha and the two FOXO proteins greatly reduced endogenous LKB1 expression and phosphorylation of LKB1's main substrate AMPK in three different cell lines. Taken together, these results demonstrate that Sp1, NF-Y and FOXO transcription factors are involved in the regulation of LKB1 transcription

    Citizenship Education and Liberalism: A State of the Debate Analysis 1990–2010

    What kind of citizenship education, if any, should schools in liberal societies promote? And what ends is such education supposed to serve? Over the last decades a respectable body of literature has emerged to address these and related issues. In this state of the debate analysis we examine a sample of journal articles dealing with these very issues spanning a twenty-year period with the aim to analyse debate patterns and developments in the research field. We first carry out a qualitative analysis where we design a two-dimensional theoretical framework in order to systematise the various liberal debate positions, and make us able to study their justifications, internal tensions and engagements with other positions. In the ensuing quantitative leg of the study we carry out a quantitative bibliometric analysis where we weigh the importance of specific scholars. We finally discuss possible merits and flaws in the research field, as evidenced in and by the analysis
