    Older people's experiences of changed medication appearance : a survey

    This report details a survey of older people's experiences of changed medication appearance. The aims of the study were: • To develop a questionnaire in partnership with older people to survey older people’s views on fluctuating medication appearance • To elicit older people’s experiences of medication that changed appearance due to ‘generic prescribing’ and ‘parallel import’ practices and its impact on their medication taking practices. These common pharmacy practices mean that the same tablet medication can be issued to older people in different colours, sizes and shapes to their previous prescriptions. Older people from a local User/Carer Forum highlighted these problems to the research team and asked that we investigate to explore the extent of the problem. An eight-item questionnaire was developed and distributed to 2000 older people (50 years+) across participating PCTs in Greater Manchester in 2008. A 29% response rate was achieved. The data was analysed using the SPSS statistical package. Findings include: •63.3% experienced a change in the appearance of their tablet medications. •74.1% did not seek advice regarding the change in the appearance of their tablet medications. •Older people noted changes to the actual tablets, tablet packaging and written information that accompanies tablets. Changes are occurring to the colour, size and shape of tablets more than changes to packaging and written information. The majority of respondents had experienced changes in the appearance of their prescribed tablet medication in the previous two years which were not due to change in medication or dose etc. Worryingly, for some respondents, these changes prompted negative experiences such as anxiety, confusion and upset. Of particular concern was that a small number omitted the affected tablet medications and did not seek help or advice from GPs, pharmacists or relatives. Six older people have been study advisors from inception to dissemination of this study and significantly added to its quality. The study demonstrates substantive public engagement / user involvement in research. The findings suggest nurses and others have a role to play in promoting better medicines management and identifying those most at risk from changed appearance of medications

    Simulating air temperature in an urban street canyon in all weather conditions using measured data at a reference meteorological station

    The definitive version may be found at www.wiley.com Copyright © 2006 Royal Meteorological SocietyA model is proposed that adapts data from a standard meteorological station to provide realistic site-specific air temperature in a city street exposed to the same meso-scale environment. In addition to a rudimentary description of the two sites, the canyon air temperature (CAT) model requires only inputs measured at standard weather stations; yet it is capable of accurately predicting the evolution of air temperature in all weather conditions for extended periods. It simulates the effect of urban geometry on radiant exchange; the effect of moisture availability on latent heat flux; energy stored in the ground and in building surfaces; air flow in the street based on wind above roof height; and the sensible heat flux from individual surfaces and from the street canyon as a whole. The CAT model has been tested on field data measured in a monitoring program carried out in Adelaide, Australia, in 2000-2001. After calibrating the model, predicted air temperature correlated well with measured data in all weather conditions over extended periods. The experimental validation provides additional evidence in support of a number of parameterisation schemes incorporated in the model to account for sensible heat and storage flux.     Anomalous Radio-Wave Scattering from Interstellar Plasma Structures

    This paper considers scattering screens that have arbitrary spatial variations of scattering strength transverse to the line of sight, including screens that are spatially well confined, such as disks and filaments. We calculate the scattered image of a point source and the observed pulse shape of a scattered impulse. The consequences of screen confinement include: (1) Source image shapes that are determined by the physical extent of the screen rather than by the shapes of much-smaller diffracting microirregularities. These include image elongations and orientations that are frequency dependent. (2) Variation with frequency of angular broadening that is much weaker than the trademark \nu^{-2} scaling law (for a cold, unmagnetized plasma), including frequency-independent cases; and (3) Similar departure of the pulse broadening time from the usually expected \nu^{-4} scaling law. We briefly discuss applications that include scattering of pulses from the Crab pulsar by filaments in the Crab Nebula; image asymmetries from Galactic scattering of the sources Cyg X-3, Sgr A*, and NGC 6334B; and scattering of background active galactic nuclei by intervening galaxies. We also address the consequences for inferences about the shape of the wavenumber spectrum of electron density irregularities, which depend on scaling laws for the image size and the pulse broadening. Future low-frequency (< 100 MHz) array observations will also be strongly affected by the Galactic structure of scattering material. Our formalism is derived in the context of radio scattering by plasma density fluctuations.     A caenagnathid tibia (Theropoda: Oviraptorosauria) from the upper Campanian Kirtland Formation of New Mexico

    Caenagnathid oviraptorosaurian dinosaurs, long considered enigmas, have now become relatively easy to recognize in the fossil record of Asia and North America. This has revealed their presence in several formations spanning the Late Cretaceous, providing new insights including showing that they were more widespread in the southern parts of North America than previously recognized. Here we add to this record by describing a nearly complete right tibia from the upper Campanian De-na-zin Member of the Kirtland Formation of New Mexico, which represents the first caenagnathid material from these beds. The bone is nearly identical in morphology and size to its counterpart in the Mongolian Elmisaurus rarus, reinforcing a suite of previously-identified features that allow for straightforward recognition of caenagnathid tibiae. The growing body of caenagnathid material continues to blur boundaries between features once considered distinct, raising doubts that some taxa established from sparse material, including Ojoraptorsaurus boerei, are actually valid

    Superscaling of non-quasielastic electron-nucleus scattering

    The present study is focused on the superscaling behavior of electron-nucleus cross sections in the region lying above the quasielastic peak, especially the region dominated by electroexcitation of the Delta. Non-quasielastic cross sections are obtained from all available high-quality data for Carbon 12 by subtracting effective quasielastic cross sections based on the superscaling hypothesis. These residuals are then compared with results obtained within a scaling-based extension of the relativistic Fermi gas model, including an investigation of violations of scaling of the first kind in the region above the quasielastic peak. A way potentially to isolate effects related to meson-exchange currents by subtracting both impulsive quasielastic and impulsive inelastic contributions from the experimental cross sections is also presented.Comment: RevTeX, 34 pages including 11 figure

    The complex network of global cargo ship movements

    Transportation networks play a crucial role in human mobility, the exchange of goods, and the spread of invasive species. With 90% of world trade carried by sea, the global network of merchant ships provides one of the most important modes of transportation. Here we use information about the itineraries of 16,363 cargo ships during the year 2007 to construct a network of links between ports. We show that the network has several features which set it apart from other transportation networks. In particular, most ships can be classified in three categories: bulk dry carriers, container ships and oil tankers. These three categories do not only differ in the ships' physical characteristics, but also in their mobility patterns and networks. Container ships follow regularly repeating paths whereas bulk dry carriers and oil tankers move less predictably between ports. The network of all ship movements possesses a heavy-tailed distribution for the connectivity of ports and for the loads transported on the links with systematic differences between ship types.     Non-Gaussian Radio-Wave Scattering in the Interstellar Medium

    It was recently suggested by Boldyrev & Gwinn that the characteristics of radio scintillations from distant pulsars are best understood if the interstellar electron-density fluctuations that cause the time broadening of the radio pulses obey non-Gaussian statistics. In this picture the density fluctuations are inferred to be strong on very small scales (1081010cm\sim 10^8-10^{10} {cm}). We argue that such density structures could correspond to the ionized boundaries of molecular regions (clouds) and demonstrate that the power-law distribution of scattering angles that is required to match the observations arises naturally from the expected intersections of our line of sight with randomly distributed, thin, approximately spherical ionized shells of this type. We show that the observed change in the time-broadening behavior for pulsar dispersion measures 30pccm3\lesssim 30 {\rm pc} {\rm cm}^{-3} is consistent with the expected effect of the general ISM turbulence, which should dominate the scattering for nearby pulsars. We also point out that if the clouds are ionized by nearby stars, then their boundaries may become turbulent on account of an ionization front instability. This turbulence could be an alternative cause of the inferred density structures. An additional effect that might contribute to the strength of the small-scale fluctuations in this case is the expected flattening of the turbulent density spectrum when the eddy sizes approach the proton gyroscale.Comment: 15 pages, 3 figures, accepted to Ap

    New disease outbreak affects two dominant sea urchin species associated with Australian temperate reefs

    Diseases of sea urchins have been implicated in dramatic transitions of marine ecosystems. Although no definitive causal agent has been found for many of these outbreaks, mostare hypothesised to be waterborne and bacterial. Here we show the first report of a novel diseaseaffecting at least 2 species of urchins off the south-eastern coast of Australia. The aetiologicalagent, identified via a range of molecular techniques, immuno-histology and inoculation experi-ments, was found to be the opportunistic pathogen Vibrio anguillarum . The disease appears to betemperature-dependent, with a faster transmission rate and increase in prevalence during ex -perimental trials conducted at higher temperatures. Furthermore, analysis of long-term field datasuggests that it may have already reached epidemic proportions. With the increases in ocean temperatures brought about by climate change, this novel urchin disease may pose a severe problem for the organisms associated with the temperate reefs off Australia and/or the ecosystemas a whole

    Finite Density QCD in the Chiral Limit

    We present the first results of an exact simulation of full QCD at finite density in the chiral limit. We have used a MFA (Microcanonical Fermionic Average) inspired approach for the reconstruction of the Grand Canonical Partition Function of the theory; using the fugacity expansion of the fermionic determinant we are able to move continuously in the (βμ\beta -\mu) plane with m=0m=0.Comment: 3 pages, LaTeX, 3 figures, uses espcrc2.sty, psfig.
