    Diet, physical exercise and cognitive behavioral training as a combined workplace based intervention to reduce body weight and increase physical capacity in health care workers - a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Health care workers comprise a high-risk workgroup with respect to deterioration and early retirement. There is high prevalence of obesity and many of the workers are overweight. Together, these factors play a significant role in the health-related problems within this sector. The present study evaluates the effects of the first 3-months of a cluster randomized controlled lifestyle intervention among health care workers. The intervention addresses body weight, general health variables, physical capacity and musculoskeletal pain.</p> <p>Methods</p> <p>98 female, overweight health care workers were cluster-randomized to an intervention group or a reference group. The intervention consisted of an individually dietary plan with an energy deficit of 1200 kcal/day (15 min/hour), strengthening exercises (15 min/hour) and cognitive behavioral training (30 min/hour) during working hours 1 hour/week. Leisure time aerobic fitness was planned for 2 hour/week. The reference group was offered monthly oral presentations. Body weight, BMI, body fat percentage (bioimpedance), waist circumference, blood pressure, musculoskeletal pain, maximal oxygen uptake (maximal bicycle test), and isometric maximal muscle strength of 3 body regions were measured before and after the intervention period.</p> <p>Results</p> <p>In an intention-to-treat analysis from pre to post tests, the intervention group significantly reduced body weight with 3.6 kg (p < 0.001), BMI from 30.5 to 29.2 (p < 0.001), body fat percentage from 40.9 to 39.3 (p < 0.001), waist circumference from 99.7 to 95.5 cm (p < 0.001) and blood pressure from 134/85 to 127/80 mmHg (p < 0.001), with significant difference between the intervention and control group (p < 0.001) on all measures. No effect of intervention was found in musculoskeletal pain, maximal oxygen uptake and muscle strength, but on aerobic fitness.</p> <p>Conclusion</p> <p>The significantly reduced body weight, body fat, waist circumference and blood pressure as well as increased aerobic fitness in the intervention group show the great potential of workplace health promotion among this high-risk workgroup.     The organic food philosophy. A qualitative exploration of the practices, values, and beliefs of Dutch organic consumers within a cultural-historical frame

    Food consumption has been identified as a realm of key importance for progressing the world towards more sustainable consumption overall. Consumers have the option to choose organic food as a visible product of more ecologically integrated farming methods and, in general, more carefully produced food. This study aims to investigate the choice for organic from a cultural-historical perspective and aims to reveal the food philosophy of current organic consumers in The Netherlands. A concise history of the organic food movement is provided going back to the German Lebensreform and the American Natural Foods Movement. We discuss themes such as the wish to return to a more natural lifestyle, distancing from materialistic lifestyles, and reverting to a more meaningful moral life. Based on a number of in-depth interviews, the study illustrates that these themes are still of influence among current organic consumers who additionally raised the importance of connectedness to nature, awareness, and purity. We argue that their values are shared by a much larger part of Dutch society than those currently shopping for organic food. Strengthening these cultural values in the context of more sustainable food choices may help to expand the amount of organic consumers and hereby aid a transition towards more sustainable consumption. © 2012 The Author(s)

    O objetivo deste estudo foi realizar a equivalência transcultural de escalas empregadas para a avaliação do nível de atividade física e que são utilizadas como estimativas da aptidão cardiorrespiratória, para posterior utilização em indivíduos idosos. Três escalas foram determinadas após revisão sistemática: Veterans Physical Activity Questionnaire (VSAQ), Rating of Perceived Capacity (RPC) e Physical Activity Rating (PA-R). Para a análise da equivalência, utilizou-se o modelo proposto por Herdman et al. Como parte do processo, a confiabilidade teste-reteste foi avaliada em uma amostra composta por 12 idosos com idade de 74,5 ± 3,5 anos, pelo cálculo dos coeficientes de concordância de Lin (CCL) e de correlação intraclasse (CCI). Detectou-se uma boa reprodutibilidade das escalas com exceção do RPC. Apesar de a quantidade de indivíduos não permitir conclusões mais aprofundadas, os resultados podem indicar necessidade de mudanças na estrutura de algumas escalas originais. Por fim, acredita-se que os resultados obtidos no presente estudo sugerem a adequação das versões das escalas para a língua portuguesa, havendo, todavia, a necessidade de um estudo de validade de critério das escalas.<br>This study aimed at establishing the cross-cultural equivalence of scales used to evaluate physical activity level and measure cardiorespiratory fitness, for further application in elderly subjects. Three scales were identified after systematic review: Veterans Physical Activity Questionnaire (VSAQ), Rating of Perceived Capacity (RPC), and Physical Activity Rating (PA-R). The model proposed by Herdman et al. was applied to analyze equivalence. Test-retest reliability was calculated in a sample of 12 elderly subjects (74.5 ± 3.5 years) using Lin's concordance coefficient and intraclass correlation coefficient. Good reproducibility was detected in all scales except RPC. Due to the small sample size, hardly any conclusions can de drawn, but the results point to the need for changes in the original scale's structures. The findings also suggest the adequacy of the scale's Portuguese-language version, although further validity studies appear to be necessary
