    Outbreak of gastroenteritis caused by Yersinia pestis in Afghanistan.

    Plague, which is most often caused by the bite of Yersinia pestis-infected fleas, is a rapidly progressing, serious disease that can be fatal without prompt antibiotic treatment. In late December 2007, an outbreak of acute gastroenteritis occurred in Nimroz Province of southern Afghanistan. Of the 83 probable cases of illness, 17 died (case fatality 20·5%). Being a case was associated with consumption or handling of camel meat (adjusted odds ratio 4·4, 95% confidence interval 2·2-8·8, P<0·001). Molecular testing of patient clinical samples and of tissue from the camel using PCR/electrospray ionization-mass spectrometry revealed DNA signatures consistent with Yersinia pestis. Confirmatory testing using real-time PCR and immunological seroconversion of one of the patients confirmed that the outbreak was caused by plague, with a rare gastrointestinal presentation. The study highlights the challenges of identifying infectious agents in low-resource settings; it is the first reported occurrence of plague in Afghanistan

    Influence of blade aerodynamic model on prediction of helicopter rotor aeroacoustic signatures

    Brown’s vorticity transport model has been used to investigate how the local blade aerodynamic model influences the quality of the prediction of the high-frequency airloads associated with blade–vortex interactions, and thus the accuracy with which the acoustic signature of a helicopter rotor can be predicted. The vorticity transport model can accurately resolve the structure of the wake of the rotor and allows significant flexibility in the way that the blade loading can be represented. The Second Higher-Harmonic Control Aeroacoustics Rotor Test was initiated to provide experimental insight into the acoustic signature of a rotor in cases of strong blade–vortex interaction. Predictions of two models for the local blade aerodynamics are compared with the test data. A marked improvement in accuracy of the predicted high-frequency airloads and acoustic signature is obtained when a lifting-chord model for the blade aerodynamics is used instead of a lifting-line-type approach. Errors in the amplitude and phase of the acoustic peaks are reduced, and the quality of the prediction is affected to a lesser extent by the computational resolution of the wake, with the lifting-chord model producing the best representation of the distribution of sound pressure below the rotor

    Are autistic traits in the general population stable across development?

    There is accumulating evidence that autistic traits (AT) are on a continuum in the general population, with clinical autism representing the extreme end of a quantitative distribution. While the nature and severity of symptoms in clinical autism are known to persist over time, no study has examined the long-term stability of AT among typically developing toddlers. The current investigation measured AT in 360 males and 400 males from the general population close to two decades apart, using the Pervasive Developmental Disorder subscale of the Child Behavior Checklist in early childhood (M = 2.14 years; SD = 0.15), and the Autism-Spectrum Quotient in early adulthood (M = 19.50 years; SD = 0.70). Items from each scale were further divided into social (difficulties with social interaction and communication) and non-social (restricted and repetitive behaviours and interests) AT. The association between child and adult measurements of AT as well the influence of potentially confounding sociodemographic, antenatal and obstetric variables were assessed using Pearson's correlations and linear regression. For males, Total AT in early childhood were positively correlated with total AT (r = .16, p = .002) and social AT (r = .16, p = .002) in adulthood. There was also a positive correlation for males between social AT measured in early childhood and Total (r = .17, p = .001) and social AT (r = .16, p = .002) measured in adulthood. Correlations for non-social AT did not achieve significance in males. Furthermore, there was no significant longitudinal association in AT observed for males or females. Despite the constraints of using different measures and different raters at the two ages, this study found modest developmental stability of social AT from early childhood to adulthood in boys

    Influence of blade aerodynamic model on the prediction of helicopter high-frequency airloads

    Brown’s vorticity transport model has been used to investigate the influence of the blade aerodynamic model on the accuracy with which the high-frequency airloads associated with helicopter blade–vortex interactions can be predicted. The model yields an accurate representation of the wake structure yet allows significant flexibility in the way that the blade loading can be represented. A simple lifting-line model and a somewhat more sophisticated liftingchord model, based on unsteady thin aerofoil theory, are compared. A marked improvement in the accuracy of the predicted high-frequency airloads of the higher harmonic control aeroacoustic rotor is obtained when the liftingchord model is used instead of the lifting-line approach, and the quality of the prediction is affected less by the computational resolution of the wake. The lifting-line model overpredicts the amplitude of the lift response to blade–vortex interactions as the computational grid is refined, exposing the fundamental deficiencies in this approach when modeling the aerodynamic response of the blade to interactions with vortices that are much smaller than its chord. The airloads that are predicted using the lifting-chord model are relatively insensitive to the resolution of the computation, and there are fundamental reasons to believe that properly converged numerical solutions may be attainable using this approach

    Benchmarking total knee replacement constructs using noninferiority analysis:the New Zealand Joint Registry study

    Abstract Background The aim of this study was to compare the relative performance of total knee replacement constructs and discern if there is variability in performance in currently commonly used prostheses in the New Zealand Joint Registry (NZJR) using a noninferiority analysis. Methods All patients who underwent a primary total knee replacement (TKR) registered in the NZJR between 1st January 1999 to June 2020 were identified. Using a noninferiority analysis, the performance of total knee replacement prostheses were compared with the best performing contemporary construct. Construct all-cause revision rate was estimated using the 1-Kaplan Meier survival function method to estimate net failure. The difference in all-cause revision rates between the contemporary benchmark and all other constructs was tested. Results In total 110 183 TKR were recorded and 25 constructs (102 717 procedures) had > 500 procedures at risk at 3 years post-primary of which 5 were inferior by at least 20 % relative risk of which, one was inferior by at least 100 % relative risk. 14 constructs were identified with > 500 procedures at risk at 10 years with 5 inferior by at least 20 %, of which 2 were inferior by > 100 % relative risk. Conclusions We discerned that there is great variability in construct performance and at all time points, greater than 25 % of constructs are inferior to the best performing construct by at least 20 %. These results can help inform patients, clinicians and health care funders when considering TKR surgery

    Benchmarking total hip replacement constructs using noninferiority analysis:the New Zealand joint registry study

    Abstract Background The aim of this study was to compare the relative performance of total hip replacement constructs and discern if there is substantial variability in performance in currently commonly used prostheses in the New Zealand Joint Registry (NZJR) using a noninferiority analysis. Methods All patients who underwent a primary total hip replacement (THR) registered in the NZJR between 1st January 1999 to June 2020 were identified. Using a noninferiority analysis, the performance of hip prostheses were compared with the best performing contemporary construct. Construct failure was estimated using the 1-Kaplan Meier survival function method to estimate net failure. The difference in failure between the contemporary benchmark and other constructs was examined. Results In total 135,432 THR were recorded comprising 1035 different THR constructs. Notably 328 constructs were used just once. Forty-eight constructs (62,251 THR) had > 500 procedures at risk at 3 years post-primary of which 28 were inferior by at least 20% relative risk of which, 10 were inferior by at least 100% relative risk. Sixteen constructs were identified with > 500 procedures at risk at 10 years with 9 inferior by at least 20%, of which one was inferior by > 100% relative risk. There were fewer constructs noninferior to the best practice benchmark when we performed analysis by gender. In females at 10 years, from 5 constructs with > 500 constructs at risk, 2 were inferior at the 20% margin. In males at 10 years, there were only 2 eligible constructs of which one was inferior at the 20% margin. Conclusions We discerned that there is substantial variability in construct performance and at most time points, just over half of constructs are inferior to the best performing construct by at least 20%. These results can facilitate informed decision-making when considering THR surgery

    Hare: a file system for non-cache-coherent multicores

    Hare is a new file system that provides a POSIX-like interface on multicore processors without cache coherence. Hare allows applications on different cores to share files, directories, and file descriptors. The challenge in designing Hare is to support the shared abstractions faithfully enough to run applications that run on traditional shared-memory operating systems, with few modifications, and to do so while scaling with an increasing number of cores. To achieve this goal, Hare must support features (such as shared file descriptors) that traditional network file systems don't support, as well as implement them in a way that scales (e.g., shard a directory across servers to allow concurrent operations in that directory). Hare achieves this goal through a combination of new protocols (including a 3-phase commit protocol to implement directory operations correctly and scalably) and leveraging properties of non-cache-coherent multiprocessors (e.g., atomic low-latency message delivery and shared DRAM). An evaluation on a 40-core machine demonstrates that Hare can run many challenging Linux applications (including a mail server and a Linux kernel build) with minimal or no modifications. The results also show these applications achieve good scalability on Hare, and that Hare's techniques are important to achieving scalability.Quanta Computer (Firm
