Cannabis-based medicines and medical cannabis for adults with cancer pain
Background: Pain is a common symptom in people with cancer; 30% to 50% of people with cancer will experience moderateâtoâsevere pain. This can have a major negative impact on their quality of life. Opioid (morphineâlike) medications are commonly used to treat moderate or severe cancer pain, and are recommended for this purpose in the World Health Organization (WHO) pain treatment ladder. Pain is not sufficiently relieved by opioid medications in 10% to 15% of people with cancer. In people with insufficient relief of cancer pain, new analgesics are needed to effectively and safely supplement or replace opioids.Objectives: To evaluate the benefits and harms of cannabisâbased medicines, including medical cannabis, for treating pain and other symptoms in adults with cancer compared to placebo or any other established analgesic for cancer pain.Search methods: We used standard, extensive Cochrane search methods. The latest search date was 26 January 2023.Selection criteria: We selected doubleâblind randomised, controlled trials (RCT) of medical cannabis, plantâderived and synthetic cannabisâbased medicines against placebo or any other active treatment for cancer pain in adults, with any treatment duration and at least 10 participants per treatment arm.Data collection and analysis: We used standard Cochrane methods. The primary outcomes were 1. proportions of participants reporting no worse than mild pain; 2. Patient Global Impression of Change (PGIC) of much improved or very much improved and 3. withdrawals due to adverse events. Secondary outcomes were 4. number of participants who reported pain relief of 30% or greater and overall opioid use reduced or stable; 5. number of participants who reported pain relief of 30% or greater, or 50% or greater; 6. pain intensity; 7. sleep problems; 8. depression and anxiety; 9. daily maintenance and breakthrough opioid dosage; 10. dropouts due to lack of efficacy; 11. all central nervous system adverse events. We used GRADE to assess certainty of evidence for each outcome.Main results: We identified 14 studies involving 1823 participants. No study assessed the proportions of participants reporting no worse than mild pain on treatment by 14 days after start of treatment.We found five RCTs assessing oromucosal nabiximols (tetrahydrocannabinol (THC) and cannabidiol (CBD)) or THC alone involving 1539 participants with moderate or severe pain despite opioid therapy. The doubleâblind periods of the RCTs ranged between two and five weeks. Four studies with a parallel design and 1333 participants were available for metaâanalysis.There was moderateâcertainty evidence that there was no clinically relevant benefit for proportions of PGIC much or very much improved (risk difference (RD) 0.06, 95% confidence interval (CI) 0.01 to 0.12; number needed to treat for an additional beneficial outcome (NNTB) 16, 95% CI 8 to 100). There was moderateâcertainty evidence for no clinically relevant difference in the proportion of withdrawals due to adverse events (RD 0.04, 95% CI 0 to 0.08; number needed to treat for an additional harmful outcome (NNTH) 25, 95% CI 16 to endless). There was moderateâcertainty evidence for no difference between nabiximols or THC and placebo in the frequency of serious adverse events (RD 0.02, 95% CI â0.03 to 0.07). There was moderateâcertainty evidence that nabiximols and THC used as addâon treatment for opioidârefractory cancer pain did not differ from placebo in reducing mean pain intensity (standardised mean difference (SMD) â0.19, 95% CI â0.40 to 0.02).There was lowâcertainty evidence that a synthetic THC analogue (nabilone) delivered over eight weeks was not superior to placebo in reducing pain associated with chemotherapy or radiochemotherapy in people with head and neck cancer and nonâsmall cell lung cancer (2 studies, 89 participants, qualitative analysis). Analyses of tolerability and safety were not possible for these studies.There was lowâcertainty evidence that synthetic THC analogues were superior to placebo (SMD â0.98, 95% CI â1.36 to â0.60), but not superior to lowâdose codeine (SMD 0.03, 95% CI â0.25 to 0.32; 5 singleâdose trials; 126 participants) in reducing moderateâtoâsevere cancer pain after cessation of previous analgesic treatment for three to four and a half hours (2 singleâdose trials; 66 participants). Analyses of tolerability and safety were not possible for these studies.There was lowâcertainty evidence that CBD oil did not add value to specialist palliative care alone in the reduction of pain intensity in people with advanced cancer. There was no difference in the number of dropouts due to adverse events and serious adverse events (1 study, 144 participants, qualitative analysis).We found no studies using herbal cannabis.Authors' conclusions: There is moderateâcertainty evidence that oromucosal nabiximols and THC are ineffective in relieving moderateâtoâsevere opioidârefractory cancer pain. There is lowâcertainty evidence that nabilone is ineffective in reducing pain associated with (radioâ) chemotherapy in people with head and neck cancer and nonâsmall cell lung cancer. There is lowâcertainty evidence that a single dose of synthetic THC analogues is not superior to a single lowâdose morphine equivalent in reducing moderateâtoâsevere cancer pain. There is lowâcertainty evidence that CBD does not add value to specialist palliative care alone in the reduction of pain in people with advanced cancer

Bile acid sequestrants in poor healing after endoscopic therapy of Barrettâs esophagus
Background/Aims Endoscopic therapy for neoplastic Barrettâs esophagus (BE) has become the standard of care over the past two decades. In clinical practice, we regularly encounter patients who fail to achieve complete squamous epithelialization of the esophagus. Although the therapeutic strategies in the individual stages of BE, dysplasia, and esophageal adenocarcinoma are well studied and largely standardized, the problem of inadequate healing after endoscopic therapy is only marginally considered. This study aimed to shed light on the variables influencing inadequate wound healing after endoscopic therapy and the effect of bile acid sequestrants (BAS) on healing. Methods Retrospective analysis of endoscopically treated neoplastic BE in a single referral center. Results In 12.1% out of 627 patients, insufficient healing was present 8 to 12 weeks after previous endoscopic therapy. The average follow-up duration was 38.8±18.4 months. Complete healing was achieved in 13 patients already after intensifying proton pump inhibitor therapy. Out of 48 patients under BAS, 29 patients (60.4%) showed complete healing. An additional eight patients (16.7%) improved, but only partial healing was achieved. Eleven (22.9%) patients showed no response to BAS augmented therapy. Conclusions In cases of insufficient healing even under exhaustion of proton pump inhibitors, treatment with BAS can be an option as an ultimate healing attempt

Genetic, biochemical, and transcriptional responses of Saccharomyces cerevisiae to the novel immunomodulator FTY720 largely mimic those of the natural sphingolipid phytosphingosine.
Sphingolipids are signaling molecules that influence diverse cellular functions from control of the cell cycle to degradation of plasma membrane proteins. The synthetic sphingolipid-like compound FTY720 is an immunomodulating agent in clinical trials for transplant graft maintenance. In this report, we compare the effects of the natural yeast sphingolipid phytosphingosine with FTY720 in Saccharomyces cerevisiae. We show that the multicopy suppressor genes that induce growth resistance to FTY720 also confer resistance to growth-inhibitory concentrations of phytosphingosine. In addition, mutants for ubiquitination pathway proteins are shown to be resistant to the growth-inhibiting effect of both FTY720 and phytosphingosine. We observe fewer similarities between sphingosine and FTY720 than between FTY720 and phytosphingosine as revealed by genetic studies. Yeast cells lacking the specific sphingosine kinase LCB4 are sensitive to phytosphingosine and FTY720 but resistant to sphingosine, suggesting that FTY720 and phytosphingosine have a more related mechanism of action. Gene expression profile comparisons of sensitive and resistant yeast cells exposed to FTY720 and phytosphingosine highlight a number of similarities. In response to treatment with these compounds, approximately 77% of the genes that are regulated >2-fold by FTY720 also respond to phytosphingosine in the same direction in the parent strain. In addition, a close inspection of TAT1 and TAT2 transporters following exposure to phytosphingosine indicates that TAT1 protein is degraded in a similar fashion upon treatment with FTY720 and phytosphingosine. There were differences, however, with respect to the TAT2 protein level and the expression profiles of a subset of genes. The genetic, transcriptional, and biochemical data together indicate that FTY720 and phytosphingosine influence similar pathways in yeast cells. These findings offer further insights into the physiological pathways influenced by these compounds in all eukaryotic cells and help us to understand the therapeutic consequences of FTY720 in humans
Finite element simulation of permeable fault influence on a medium deep borehole thermal energy storage system
Solutions for seasonal energy storage systems are essential for the reliable use of fluctuating renewable energy sources. As part of the research project SKEWS, a medium deep borehole thermal energy storage system with a depth of 750 m is under construction at Campus Lichtwiese in Darmstadt, Germany, to demonstrate this innovative technology. Prior to the design of SKEWS, the geological context in the surroundings of the project location was investigated using archive drilling data and groundwater measurements. The geologic survey suggests the assumption that the uppermost part of the intended storage domain is crosscut by a normal fault, which displaces the Permian rocks east of Darmstadt against granodioritic rocks of the Odenwald crystalline complex. A 3D finite-element numerical model was implemented to estimate the effect of the potentially higher hydraulic conductivity of the fault zone on the planned storage system. For this purpose, a storage operation over a time span of 30 years was simulated for different parametrizations of the fault zone. The simulations reveal a limited but visible heat removal from the storage region with increasing groundwater flow in the fault zone. However, the section of the borehole thermal energy storage system affected by the fault is minor compared to the total depth of the system. This only constitutes a minor impairment of the storage efficiency of approximately 3%. In total, the amount of heat extracted varies between 320.2 GWh and 326.2 GWh for the different models. These findings can be helpful for the planning and assessment of future medium deep borehole thermal energy storage systems in fractured and faulted crystalline settings by providing data about the potential impact of faults or large fractures crosscutting the storage system
