Background The prognostic significance of obstructive sleep apnea ( OSA ) in patients with acute coronary syndrome ( ACS ) in the contemporary era is unclear. We performed a large, prospective cohort study and did a landmark analysis to delineate the association of OSA with subsequent cardiovascular events after ACS onset. Methods and Results Between June 2015 and May 2017, consecutive eligible patients admitted for ACS underwent cardiorespiratory polygraphy during hospitalization. OSA was defined as an apnea-hypopnea index ≥15 events·h-1. The primary end point was major adverse cardiovascular and cerebrovascular event ( MACCE ), including cardiovascular death, myocardial infarction, stroke, ischemia-driven revascularization, or hospitalization for unstable angina or heart failure. OSA was present in 403 of 804 (50.1%) patients. During median follow-up of 1 year, cumulative incidence of MACCE was significantly higher in the OSA group than in the non- OSA group (log-rank, P=0.041). Multivariate analysis showed that OSA was nominally associated with incidence of MACCE (adjusted hazard ratio, 1.55; 95% CI, 0.94-2.57; P=0.085). In the landmark analysis, patients with OSA had 3.9 times the risk of incurring a MACCE after 1 year (adjusted hazard ratio, 3.87; 95% CI, 1.20-12.46; P=0.023), but no increased risk was found within 1-year follow-up (adjusted hazard ratio, 1.18; 95% CI, 0.67-2.09; P=0.575). No significant differences were found in the incidence of cardiovascular death, myocardial infarction, and ischemia-driven revascularization, except for a higher rate of hospitalization for unstable angina in the OSA group than in the non- OSA group (adjusted hazard ratio, 2.10; 95% CI, 1.09-4.05; P=0.027). Conclusions There was no independent correlation between OSA and 1-year MACCE after ACS . The increased risk associated with OSA was only observed after 1-year follow-up. Efficacy of OSA treatment as secondary prevention after ACS requires further investigation

Objective: The prognostic significance of obstructive sleep apnea (OSA) in patients with acute coronary syndrome (ACS) according to diabetes mellitus (DM) status remains unclear. We aimed to elucidate the association of OSA with subsequent cardiovascular events in patients with ACS with or without DM. Research design and methods: In this prospective cohort study, consecutive eligible patients with ACS underwent cardiorespiratory polygraphy between June 2015 and May 2017. OSA was defined as an Apnea Hypopnea Index ≥15 events/hour. The primary end point was major adverse cardiovascular and cerebrovascular events (MACCEs), including cardiovascular death, myocardial infarction, stroke, ischemia-driven revascularization, or hospitalization for unstable angina or heart failure. Results: Among 804 patients, 248 (30.8%) had DM and 403 (50.1%) had OSA. OSA was associated with 2.5 times the risk of 1 year MACCE in patients with DM (22.3% vs 7.1% in the non-OSA group; adjusted HR (HR)=2.49, 95% CI 1.16 to 5.35, p=0.019), but not in patients without DM (8.5% vs 7.7% in the non-OSA group, adjusted HR=0.94, 95% CI 0.51 to 1.75, p=0.85). Patients with DM without OSA had a similar 1 year MACCE rate as patients without DM. The increased risk of events was predominately isolated to patients with OSA with baseline glucose or hemoglobin A1c levels above the median. Combined OSA and longer hypoxia duration (time with arterial oxygen saturation22 min) further increased the MACCE rate to 31.0% in patients with DM. Conclusions: OSA was associated with increased risk of 1 year MACCE following ACS in patients with DM, but not in non-DM patients. Further trials exploring the efficacy of OSA treatment in high-risk patients with ACS and DM are warranted

Bioactive SrO-SiO2 glass with well-ordered mesopores: Characterization, physiochemistry and biological properties

For a biomaterial to be considered suitable for bone repair it should ideally be both bioactive and have a capacity for controllable drug delivery; as such, mesoporous SiO2 glass has been proposed as a new class of bone regeneration material by virtue of its high drug-loading ability and generally good biocompatibility. It does, however, have less than optimum bioactivity and controllable drug delivery properties. In this study, we incorporated strontium (Sr) into mesoporous SiO2 in an effort to develop a bioactive mesoporous SrO–SiO2 (Sr–Si) glass with the capacity to deliver Sr2+ ions, as well as a drug, at a controlled rate, thereby producing a material better suited for bone repair. The effects of Sr2+ on the structure, physiochemistry, drug delivery and biological properties of mesoporous Sr–Si glass were investigated. The prepared mesoporous Sr–Si glass was found to have an excellent release profile of bioactive Sr2+ ions and dexamethasone, and the incorporation of Sr2+ improved structural properties, such as mesopore size, pore volume and specific surface area, as well as rate of dissolution and protein adsorption. The mesoporous Sr–Si glass had no cytotoxic effects and its release of Sr2+ and SiO44− ions enhanced alkaline phosphatase activity – a marker of osteogenic cell differentiation – in human bone mesenchymal stem cells. Mesoporous Sr–Si glasses can be prepared to porous scaffolds which show a more sustained drug release. This study suggests that incorporating Sr2+ into mesoporous SiO2 glass produces a material with a more optimal drug delivery profile coupled with improved bioactivity, making it an excellent material for bone repair applications. Discovering and Explaining the Non-Causality of Deep Learning in SAR ATR

In recent years, deep learning has been widely used in SAR ATR and achieved excellent performance on the MSTAR dataset. However, due to constrained imaging conditions, MSTAR has data biases such as background correlation, i.e., background clutter properties have a spurious correlation with target classes. Deep learning can overfit clutter to reduce training errors. Therefore, the degree of overfitting for clutter reflects the non-causality of deep learning in SAR ATR. Existing methods only qualitatively analyze this phenomenon. In this paper, we quantify the contributions of different regions to target recognition based on the Shapley value. The Shapley value of clutter measures the degree of overfitting. Moreover, we explain how data bias and model bias contribute to non-causality. Concisely, data bias leads to comparable signal-to-clutter ratios and clutter textures in training and test sets. And various model structures have different degrees of overfitting for these biases. The experimental results of various models under standard operating conditions on the MSTAR dataset support our conclusions. Evidence on the existence and impact of corruption in state asset sales in China 1

    Get PDF
We document evidence of corruption in Chinese state asset sales. These sales involved stakes in partially privatized firms, providing a benchmark – the price of publicly traded shares – to measure under-pricing. We document under-pricing of more than 70 percent, which is correlated with deal attributes associated with misgovernance and corruption. Sales by “disguised ” owners that misrepresenting their state ownership to elude regulatory scrutiny are discounted 5-10 percentage points more than sales by other owners; related party transactions are similarly discounted. Post-transfer profitability is higher, though uncorrelated with under-pricing, suggesting that ownership transfer improved efficiency, even when the transfers themselves were corrupted

Hierarchical Disentanglement-Alignment Network for Robust SAR Vehicle Recognition

Vehicle recognition is a fundamental problem in SAR image interpretation. However, robustly recognizing vehicle targets is a challenging task in SAR due to the large intraclass variations and small interclass variations. Additionally, the lack of large datasets further complicates the task. Inspired by the analysis of target signature variations and deep learning explainability, this paper proposes a novel domain alignment framework named the Hierarchical Disentanglement-Alignment Network (HDANet) to achieve robustness under various operating conditions. Concisely, HDANet integrates feature disentanglement and alignment into a unified framework with three modules: domain data generation, multitask-assisted mask disentanglement, and domain alignment of target features. The first module generates diverse data for alignment, and three simple but effective data augmentation methods are designed to simulate target signature variations. The second module disentangles the target features from background clutter using the multitask-assisted mask to prevent clutter from interfering with subsequent alignment. The third module employs a contrastive loss for domain alignment to extract robust target features from generated diverse data and disentangled features. Lastly, the proposed method demonstrates impressive robustness across nine operating conditions in the MSTAR dataset, and extensive qualitative and quantitative analyses validate the effectiveness of our framework
