Zmiany w błonie śluzowej żołądka u dorosłych chorych na celiakię
Wstęp: Celiakia jest przewlekłą chorobą zapalną przewodu
pokarmowego o podłożu autoimmunologicznym, występującą
u osób predysponowanych genetycznie. Znaczenie zmian patologicznych w błonie śluzowej żołądka u chorych na celiakię
i ich wpływ na przebieg kliniczny tej choroby nie zostały
dotąd w pełni poznane.
Cel: Ocena częstości występowania zmian histopatologicznych w błonie śluzowej żołądka u dorosłych osób z celiakią
oraz wpływu tych zmian na obraz kliniczny choroby.
Materiał i metody: Do badań włączono 90 pacjentów, w tym
35 chorych na celiakię i 55 osób z objawami dyspeptycznymi
(grupa kontrolna). Obejmowały one wywiad chorobowy, badanie przedmiotowe, badania krwi oraz panendoskopię. Wycinki z błony śluzowej żołądka i części pozaopuszkowej
dwunastnicy barwiono hematoksyliną i eozyną. Oceniano
nasilenie zmian zapalnych oraz liczbę limfocytów śródnabłonkowych (intraepithelial lymphocytes - IEL). Za pomocą badania immunohistochemicznego z zastosowaniem przeciwciał
monoklonalnych anty-CD3+ identyfikowano limfocyty CD3+.
Ocenę bioptatów z dwunastnicy w kierunku celiakii wykonano zgodnie z klasyfikacją Marsha.
Wyniki: U chorych na celiakię, zarówno nieleczonych, jak
i leczonych dietą bezglutenową, obserwowano wzrost IEL,
jednak ich liczba była znamiennie większa u chorych niestosujących diety bezglutenowej. Limfocytarne zapalenie żołądka
rozpoznano u 8,6% chorych na celiakię. Osoby te podawały
nasilone dolegliwości dyspeptyczne. W badaniu immunohistochemicznym wykazano obecność nacieków limfocytów
T CD3+ zarówno w bioptatach błony śluzowej żołądka, jak
i części pozaopuszkowej dwunastnicy.Introduction: Coeliac disease is a chronic inflammatory disorder of the gastrointestinal tract, of autoimmune etiology, in
genetically predisposed persons. The role of the gastric
lesions and their influence on the clinical course of that disease have not been explained.
Aim: To evaluate the frequency of morphological lesions in
the gastric mucosa in adults with coeliac disease and their
effect on the clinical picture of gluten enteropathy.
Material and methods: Ninety patients, including 35 patients
with coeliac disease and 55 with dyspeptic symptoms (control
group) were examined. Clinical studies comprised history
of disease, physical examination, blood tests and gastroscopies. Biopsies from the gastric and distal part of the duodenal mucosa were stained with haematoxylin-eosin, for estimation of the intensity of mucosal changes and a count
of the intraepithelial lymphocytes (IEL). Immunohistochemical
studies with anti-CD3 antibodies were performed for the identification of CD3+ lymphocytes.
Results: An increased number of IEL was observed in coeliac
patients treated as well as not treated with gluten, compared
with the control group. Lymphocytic gastritis (LG) was
observed in 8.6% of coeliac patients. Patients with coeliac disease and LG presented intense dyspeptic symptoms. Presence of CD3+ T lymphocytes was revealed in the gastric and
duodenal mucosa with immunohistochemical staining.
Conclusions: Morphological lesions in the gastric mucosa
belong to gastroenteropathy in the course of coeliac disease
and may induce dyspeptic symptoms. Taking biopsies from
the distal part of the duodenum in patients with dyspeptic
symptoms should be considered
Ab initio quantum scattering calculations and a new potential energy surface for the HCl()-O() system: collision-induced line-shape parameters for O-perturbed R(0) 0-0 line in HCl
The remote sensing of abundance and properties of HCl -- the main atmospheric
reservoir of Cl atoms which directly participate in ozone depletion -- are
important for monitoring the partitioning of chlorine between "ozone-depleting"
and "reservoir" species. Such remote studies require knowledge of the shapes of
molecular resonances of HCl, which are perturbed by collisions with the
molecules of the surrounding air. In this work, we report the first fully
quantum calculations of collisional perturbations of the shape of a pure
rotational line in HCl perturbed by an air-relevant molecule (as the
first model system we choose the R(0) line in HCl perturbed by O). The
calculations are performed on our new highly-accurate
HCl()-O() potential energy surface. In addition
to pressure broadening and shift, we determine also their speed dependencies
and the complex Dicke parameter. This gives important input to the community
discussion on the physical meaning of the complex Dicke parameter and its
relevance for atmospheric spectra (previously, the complex Dicke parameter for
such systems was mainly determined from phenomenological fits to experimental
spectra and the physical meaning of its value in that context is questionable).
We also calculate the temperature dependence of the line-shape parameters and
obtain agreement with the available experimental data. We estimate the total
combined uncertainties of our calculations at 2% relative RMSE residuals in the
simulated line shape at 296~K. This result constitutes an important step
towards computational population of spectroscopic databases with accurate ab
initio line-shape parameters for molecular systems of terrestrial atmospheric

Surowiczy piaszczakorak otrzewnej o agresywnym przebiegu: opis przypadku i przeglad literatury
Abstract Background. Primary serous peritoneal psammocarcinoma (PSPP) is a rare variant of serous carcinoma characterized by massive psammoma body formation and low-grade cytological features. Patients with serous psammocarcinoma have a protracted clinical course and relatively favourable prognosis, although a more aggressive course of PSPP may occur. Case presentation: A 52-year-old woman suffering from abdominal pain with ascites and serum CA-125 level substantially elevated underwent an exploratory laparoscopy which revealed bulk disease. The pathology report detected PPSP at the FIGO stage IIIC. The patient received neoadjuvant chemotherapy (3 courses of paclitaxel / pegylated liposomal doxorubicin / carboplatin). Optimal interval debulking surgery was performed as the next step, followed by three courses of adjuvant chemotherapy (paclitaxel / carboplatin). Due to the fact that the patient had residual disease, at the second-look surgery she received consolidation therapy with intraperitoneal and intravenous chemotherapy carboplatin. Eight months after the completion of treatment the patient developed disease recurrence in the peritoneum. Palliative surgery (enterostomy) was performed. Furthermore, the patient received two lines of chemotherapy consisting of cyclophosphamide / cisplatin and then gemcitabine. After twenty five months she developed brain metastases, treated with palliative radiotherapy. The patient died twenty eight months since her primary presentation of PSPP. Conclusion: PSPP is an infrequent variant of epithelial cancer with favourable prognosis. The disease may, however, take a more aggressive course. Thus, an aggressive therapy is required to postpone the progression.Streszczenie Pierwotny surowiczy piaszczakorak otrzewnej (PSPSP) jest rzadką odmianą raka surowiczego, charakteryzujący się obecnością licznych ciałek piaszczakowatych oraz niską złośliwością histologiczną. Chore mają względnie korzystne rokowanie, chociaż zdarza się przebieg bardziej agresywny tak jak w prezentowanej pracy. Opis przypadku: Chora lat 52 uskarżająca się na bole brzucha, z cechami wodobrzusza w badaniach obrazowych i znacznie podwyższonym markerem CA 125 została poddana zwiadowczej laparoskopii. Podczas operacji stwierdzono zaawansowaną chorobę nowotworową w jamie brzusznej przy niezmienionych przydatkach. W badaniu histopatologicznym rozpoznano PPSP. W zestawieniu z danymi klinicznymi zaawansowanie określono na IIIC wg klasyfikacji FIGO. Chorą zakwalifikowano do neoadjuwantowej chemioterapii (3 kursy Paklitaksel/ Pegylowana Liposomalna Doxorubicyna/Karboplatyna) a następnie poddano odroczonej optymalnej operacji cytoredukcyjnej, po której zastosowano 3 cykle adjuwantowej chemioterapii (Paklitaksel/ Karboplatyna). W związku z obecnością minimalnej choroby resztkowej stwierdzonej w trakcie operacji drugiego wglądu, zakwalifikowano pacjentkę do monoterapii karboplatyną (3 cykle) podawaną dootrzewnowo i dożylnie. Po ośmiu miesiącach od zakończenia chemioterapii stwierdzono nawrot choroby nowotworowej w otrzewnej. Z powodu niedrożności przewodu pokarmowego przeprowadzono paliatywną operację z wyłonieniem stomii na jelicie grubym. Po zabiegu pacjentka otrzymała dwie linie chemioterapii według schematu Cyklofosfamid/ Cisplatyna, następnie monoterapię Gemcytabiną. Po 25 miesiącach od rozpoznania PSPP chorą poddano paliatywnej radioterapii z powodu zmian przerzutowych do mózgu. Pacjentka zmarła 28 miesięcy po zdiagnozowaniu PSPP. PSPP jest rzadkim podtypem nowotworów nabłonkowych o względnie korzystnym rokowaniu. Choroba ta może mieć jednak agresywny przebieg tak, jak opisano w tej pracy i chorzy ci mogą wymagać intensyfikacji leczenia

Ab initio line-shape parameters for speed-dependent hard-collision profiles: applications to rovibrational lines of H 2
Nonlinear magneto-optical rotation with parametric resonance
We report on investigations of nonlinear magneto-optical rotation (NMOR) in
rubidium vapor subjected to a modulated magnetic field and continuous-wave (CW)
laser-light illumination. By superimposing modulation and a static (DC)
magnetic field, we demonstrate the appearance of resonances at both small and
large (compared to the ground-state relaxation rate) values of the static
field. Since in conventional NMOR, there is no rotation at high fields, this
suggests an existence of a novel mechanism generating anisotropy in the
considered case, which we identify as parametric resonance. The experiments are
performed using light of small ellipticity and rotation signals are
significantly enhanced by combining atom-induced polarization rotation with a
passive rotation induced with a wave plate. All the observations are supported
with theoretical simulations. The density-matrix formalism and angular-momentum
probability surfaces are used to provide intuitive explanation of the observed

A test of H
The close-coupling method is used to calculate purely rotational relaxation rates and
pressure broadening and shifting coefficients for H2-He collisions, in order to test
various potential energy surfaces. Downward rate coefficients k3→1(T) and k2→0(T) are compared with experimental data, but
the subtle differences in the potential energy surfaces are hardly reflected in these
rates. Helium pressure broadening and shifting generalized cross sections for the
isotropic Raman Q(1) lines of the fundamental bands of D2 and H2 as well as the purely
rotational Stokes S0(1) line of H2 are therefore also considered. While these
spectroscopic characteristics are much more sensitive to the precise form of the
interaction potential, a proper validation cannot be performed without taking into account
the influence of the translational motion on the molecular line shapes. After including
this, it is found that the potential energy surface of Bakr, Smith and Patkowski [B.W.
Bakr, D.G.A. Smith, K. Patkowski, J. Chem. Phys. 139, 144305 (2013)] allows
the best reproduction of the experimental data
