    Dietary Dracocephalum Kotschyi Essential Oil Improved Growth, Haematology, Immunity and Resistance to Aeromonas Hydrophila in Rainbow Trout (Oncorhynchus Mykiss)

    In the present study, the effect of Dracocephalum kotschyi essential oil (DKE) was investigated on growth, haematology, immune and antioxidant defence system and resistance of rainbow trout juveniles to bacterial infection (Aeromonas hydrophila). For this purpose, the fish were fed a diet containing different concentrations of DKE including 0 (control), 0.2, 0.25 and 0.3 mg/kg diet in three replicates for 60 days. After feeding trial, the fish were challenged with a pathogenic dose of A. hydrophila. Based on the results, immune components in plasma (alternative complement activity [ACH50], IgM content, lysozyme activity, total protein and total albumin) and mucus (protease activity, IgM content and lysozyme activity) significantly elevated in fish fed diet containing 0.2 and 0.25 mg DKE/kg diet compared with other groups (p < 0.01). The plasma bactericidal activity increased in all DKE-supplemented fish. Supplementation of fish with 0.3 mg DKE/kg diet depressed the mucosal lysozyme activity and IgM content. The liver antioxidant enzymes, glutathione peroxidase (GPx), catalase (CAT) and superoxide dismutase (SOD) showed increased activity in response to 0.25 and 0.3 mg DKE/kg diet compared with other experimental diets (p < 0.01). Dietary DKE changed the haematology of the fish. The haematocrit, red blood cell count (RBC), white blood cell (WBC), haemoglobin content and mean erythrocyte cell haemoglobin content (MCHC) increased in response to DKE. In contrast, the mean corpuscular haemoglobin (MCH) decreased in fish fed 0.2 and 0.25 mg DKE/kg diet compared with other diets (p < 0.01). The supplementation of fish with 0.2 and 0.25 mg DKE/kg diet decreased the expression of cytokine-related genes, TNF-α and IL-8. In contrast, the DKE up-regulated the expression of the immune-related genes, C3 and lysozyme. DKE at concentration of 0.3 mg/kg diet depressed the C3 and lysozyme gene expressions. The DKE supplementation decreased the mortality rate of the fish after bacterial challengecompared with non-supplemented ones (p < 0.01). In conclusion, the findings of this study revealed the immune-stimulating effects of DKE at optimized dietary concentrations. In addition, DKE decreased the mortality induced by A. hydrophila infection

    A New Model of Delirium Care in the Acute Geriatric Setting: Geriatric Monitoring Unit

    <p>Abstract</p> <p>Background</p> <p>Delirium is a common and serious condition, which affects many of our older hospitalised patients. It is an indicator of severe underlying illness and requires early diagnosis and prompt treatment, associated with poor survival, functional outcomes with increased risk of institutionalisation following the delirium episode in the acute care setting. We describe a new model of delirium care in the acute care setting, titled Geriatric Monitoring Unit (GMU) where the important concepts of delirium prevention and management are integrated. We hypothesize that patients with delirium admitted to the GMU would have better clinical outcomes with less need for physical and psychotropic restraints compared to usual care.</p> <p>Methods/Design</p> <p>GMU models after the Delirium Room with adoption of core interventions from Hospital Elder Life Program and use of evening bright light therapy to consolidate circadian rhythm and improve sleep in the elderly patients. The novelty of this approach lies in the amalgamation of these interventions in a multi-faceted approach in acute delirium management. GMU development thus consists of key considerations for room design and resource planning, program specific interventions and daily core interventions. Assessments undertaken include baseline demographics, comorbidity scoring, duration and severity of delirium, cognitive, functional measures at baseline, 6 months and 12 months later. Additionally we also analysed the pre and post-GMU implementation knowledge and attitude on delirium care among staff members in the geriatric wards (nurses, doctors) and undertook satisfaction surveys for caregivers of patients treated in GMU.</p> <p>Discussion</p> <p>This study protocol describes the conceptualization and implementation of a specialized unit for delirium management. We hypothesize that such a model of care will not only result in better clinical outcomes for the elderly patient with delirium compared to usual geriatric care, but also improved staff knowledge and satisfaction.     Design of water supply networks for water transfer to the urban area Case study: Balikpapan city

    The growing population and the development of industries in all countries of the world have created a very important and complex issue for water supply to cities. Today, many parts of the world are facing the problem of water shortage and this problem cannot be easily solved. In addition to the proper use of water resources and preventing the loss of natural water, the establishment of regional water supply networks is effective in meeting the future needs of the people. A water distribution network (water supply network) is a set of interconnected pipelines used to transport and distribute water in a complex. In designing the water distribution network, factors such as the type of water distribution network, water pressure, water velocity, design flow, minimum pipe diameter, pipe material and many other factors should be considered. In this study, we have tried to design the water supply network of a part of Balikpapan city in Indonesia. The design method led to the determination of pressure values in the connection nodes, pipe diameters, flow rate and velocity in the pipes. All the existing criteria are considered in the design of the water supply network. Although this study has been implemented for a specific study area, it can be of great help to designers in designing the water supply network
