    Cardiovascular Risk Assessment in a Cohort of Newly Diagnosed Patients with Obstructive Sleep Apnea Syndrome

    Objectives. Obstructive sleep apnea syndrome (OSAS) is associated with increased cardiovascular morbidity and mortality. The aim of this study was to assess whether the 10-year risk for cardiovascular disease in newly diagnosed patients with OSAS is increased. Materials and Methods. Recently diagnosed, with polysomnography, consecutive OSAS patients were included. The Systematic Coronary Risk Evaluation (SCORE) and the Framingham Risk Score (FRS) were used to estimate the 10-year risk for cardiovascular disease. Results. Totally, 393 individuals (73.3% males), scheduled to undergo a polysomnographic study with symptoms indicative of OSAS, were enrolled. According to apnea-hypopnea index (AHI), subjects were divided in four groups: mild OSAS (AHI 5–14.9/h) was diagnosed in 91 patients (23.2%), moderate OSAS (AHI 15–29.9/h) in 58 patients (14.8%), severe OSAS (AHI > 30/h) in 167 patients (42.5%), while 77 individuals (19.6%) had an AHI < 5/h and served as controls. Increased severity of OSAS was associated with increased SCORE p<0.001 and FRS values p<0.001. More specifically, a significant correlation was observed both between AHI and SCORE r=0.251, p<0.001 and AHI and FRS values r=0.291, p<0.001. Furthermore, a negative correlation was observed between FRS values and sleep efficiency r=−0.224, p=0.006. Conclusions. The 10-year risk for cardiovascular morbidity and mortality seems to increase with severity of OSAS. Physicians should bear this finding in mind, in order to seek for and consecutively eliminate risk factors for cardiovascular disease and to prevent future cardiovascular events in OSAS patients

    The effect of inhibition of a1 adrenoreceptor at the signaling pathways of cell proliferation in rat prostate

    Introduction and objectives: The quinazoline based alpha-1-adrenoreceptor antagonists have been documented to induce apoptosis in prostate cells potentially via activation of the transforming growth factor TGF-β signal transduction pathway and via disruption of cell attachment to the extracellular matrix (anoikis). On the other hand some recent experimental evidence points to the aspect that a1 antagonists could potentially affect cell proliferation. Ιn the present study, we have investigated the effects of terazosin, an alpha-1-adrenoreceptor antagonist, on the expression of cell cycle regulators cyclin E, cyclin D and cell cycle inhibitors p21, p16 and p27, as well as on Ki 67 nuclear antigen which are considered to be cell proliferation markers, in order to determine whether the mechanism of action of this specific drug is implicated in cell proliferation.Materials and methods: Twelve 120 days old male Wistar rats were given terazosin orally every second day for 120 days, while 10 animals treated with normal saline served as controls. The expression of cell proliferation markers cyclin E, cyclin D1, p16, p27, p21 and Ki 67 was assessed immunohistochemically using corresponding antibodies.Results: The data indicate that terazosin did neither inhibit cell proliferation or induce cell cycle blockade, as the expression of all proliferation markers (cyclin E, cyclin D1, p16, p27, p21 and Ki 67) were equivalent in both groups. In addition terazosin treatment did not affect the weight of the ventral prostate gland.Conclusions: These findings suggest that terazosin has no anti-proliferative effects on rat prostate cells, supporting even more previous experimental evidence that quinazoline based alpha-1-adrenoreceptor antagonists may regulate prostate growth mainly by inducing apoptosis in both epithelial and stromal cells and not by affecting cell proliferation. Taken together with previous reports they provide an interesting insight into the effects of Quinazolines in prostate gland.Σκοπός: Οι κιναζολινικοί α1 αδρενεργικοί ανταγωνιστές θεωρείται ότι προκαλούν απόπτωση στα προστατικά κυτταρα πιθανώς μέσω της απορρύθμισης μηχανισμών μεταβίβασης του σήματος για τους παράγοντες ΤGF-β και EGF, καθώς επίσης και μέσω της αποδιοργάνωσης της προσκόλλησης των κυττάρων στο εξωκυττάριο στρώμα και την επακόλουθη επαγωγή του φαινομένου “anoikis”. Από την άλλη πλευρά προσφατα πειραματικά δεδομένα υποστηρίζουν ότι ο μηχανισμός δρασης των α1 αδρενεργικών αναστολέων δε στηρίζεται αποκλειστικά στην επαγωγή του φαινομένου της απόπτωσης αλλά και σε μεταβολές του κυτταρικού πολλαπλασιασμού. Στην παρούσα μελέτη μελετήσαμε την μεταβολή της έκφρασης των οδών μεταβίβασης σήματος για κυτταρικό πολλαπλασιασμό στον προστάτη αδένα επιμύων μετά από χορήγηση τεραζοσίνης, ενός αναστολέα των α1- αδρενεργικών υποδοχέων και πιο συγκεκριμένα την έκφραση των ρυθμιστών του κυτταρικού κύκλου κυκλίνης D1, κυκλίνης Ε, των αναστολέων του κυτταρικού κύκλου p21, p27 και p16 καθώς και του πυρηνικού αντιγόνου Κi 67 που χαρακτηρίζονται σα δείκτες κυτταρικού πολλαπλασιασμού, ώστε να διευκρινιστεί αν ο μηχανισμός δράσης του συγκεκριμένού φαρμάκου εμπλέκεται στο φαινόμενο της μεταβολής του κυτταρικού πολλαπλασιασμού.Υλικά και μέθοδοι: Σε δώδεκα αρσενικούς αρουραίους τύπου Wistar χορηγήθηκε από του στόματος τεραζοσίνη κάθε δεύτερη μέρα και για 120 μέρες ένω τα υπόλοιπα 10 ζώα στα οποία χορηγήθηκε φυσιολογικός ορός χρησιμοποιήθηκαν σαν ομάδα ελέγχου. Η έκφραση των δεικτών κυτταρικού πολλαπλασιασμού κυκλίνης D1 και Ε, των αναστολέων κυκλινών p21, p16 και p27 καθώς και του πυρηνικού αντιγόνου Κi 67 αξιολογήθηκαν ανοσοιστοχημικά χρησιμοποιώντας τα αντίστοιχα αντισώματα.Αποτελέσματα: Τα στοιχεία δείχνουν ότι η χορήγηση τεραζοσίνης δεν ανέστειλε τον κυτταρικό πολλαπλασιάσμο ούτε προκάλεσε την επάγωγή της αναστολής του κυτταρικού κύκλου καθώς η έκφραση όλων των δεικτών πολλαπλασιασμού (κυκλίνης Ε, D1, p21, p16, p27 και Κi 67) ήταν παρόμοια και στις 2 ομάδες. Επιπρόσθετα η χορήγηση τεραζοσίνης δεν επηρέασε το βάρος του κοιλιακού προστάτη.Συμπεράσματα: Αυτά τα ευρήματα υποδεικνύουν ότι η τεραζοσίνη δεν εχει αντιπολλαπλασιαστική επίδραση στα προστατικά κύτταρα του αρουραίου, υποστηρίζοντας ακόμα περισσότερο τα ήδη υπάρχοντα επιστημονικά δεδομένα, δηλαδή ότι οι κιναζολινικοί α1 αδρενεργικοί ανταγωνιστές ρυθμίζουν την προστατική ανάπτυξη κυρίως προκαλώντας απόπτωση τόσο στα επιθηλιακά όσο και στα στρωματικά κύτταρα χωρίς να επηρεάζουν τον κυτταρικό πολλαπλασιασμό. Μία συνεκτίμηση με προηγούμενες αναφορές δίνει μια πολύ ενδιαφέρουσα διεισδυτική ματιά στην επίδραση των κιναζολονών στον προστάτη αδένα

    Reduced Serum Vitamin D Levels Are Associated with Insulin Resistance in Patients with Obstructive Sleep Apnea Syndrome

    Background and objectives: Obstructive sleep apnea syndrome (OSAS) is associated with cardiovascular and metabolic risk factors, such as insulin resistance. Furthermore, OSAS has been associated with decreased levels of vitamin D (Vit D). The aim of the study was to assess the association between Vit D levels (expressed as 25(OH)D serum levels) and insulin resistance in patients with OSAS. Materials and Methods: Serum 25(OH)D levels were measured in consecutive subjects who had undergone polysomnography and pulmonary function testing. OSAS patients were divided into those with (homeostatic model assessment [HOMA-IR] &#8805; 2) and without insulin resistance (HOMA-IR &lt; 2). Results: Overall, 92 patients (81 males) were included in the study. OSAS patients with insulin resistance significantly differed from those without insulin resistance in terms of the body-mass index (BMI) (36.3 &#177; 5.8 compared to 32 &#177; 5.6 kg/m2, respectively, p = 0.001), apnoea-hypopnoea index (AHI) (57.4 &#177; 28.9 compared to 40.9 &#177; 27.9 events/h, respectively, p = 0.009) and indices of hypoxia during sleep. Patients with OSAS and insulin resistance had lower levels of serum 25 (OH) D compared with OSAS but without insulin resistance (19.3 &#177; 11.5 vs 26.7 &#177; 12.2 ng/mL, respectively, p = 0.005). Regression analysis demonstrated a negative association of 25(OH)D levels (&#946; = &#8722;0.048, odds ratio [OR]: 0.953, 95% confidence interval [CI]: 0.913&#8722;0.995, p = 0.030) and a positive association of BMI (&#946; = 0.110, OR: 1.116, 95% CI: 1.007&#8722;1.237, p = 0.036) with insulin resistance. Conclusions: Vit D insufficiency was significantly more frequent among OSAS patients with insulin resistance. Both low 25(OH)D levels and high BMI were associated with the risk of insulin resistance in this population

    Developing a Diagnostic Multivariable Prediction Model for Urinary Tract Cancer in Patients Referred with Haematuria: Results from the IDENTIFY Collaborative Study

    377siBackground: Patient factors associated with urinary tract cancer can be used to risk stratify patients referred with haematuria, prioritising those with a higher risk of cancer for prompt investigation. Objective: To develop a prediction model for urinary tract cancer in patients referred with haematuria. Design, setting, and participants: A prospective observational study was conducted in 10 282 patients from 110 hospitals across 26 countries, aged ≥16 yr and referred to secondary care with haematuria. Patients with a known or previous urological malignancy were excluded. Outcome measurements and statistical analysis: The primary outcomes were the presence or absence of urinary tract cancer (bladder cancer, upper tract urothelial cancer [UTUC], and renal cancer). Mixed-effect multivariable logistic regression was performed with site and country as random effects and clinically important patient-level candidate predictors, chosen a priori, as fixed effects. Predictors were selected primarily using clinical reasoning, in addition to backward stepwise selection. Calibration and discrimination were calculated, and bootstrap validation was performed to calculate optimism. Results and limitations: The unadjusted prevalence was 17.2% (n = 1763) for bladder cancer, 1.20% (n = 123) for UTUC, and 1.00% (n = 103) for renal cancer. The final model included predictors of increased risk (visible haematuria, age, smoking history, male sex, and family history) and reduced risk (previous haematuria investigations, urinary tract infection, dysuria/suprapubic pain, anticoagulation, catheter use, and previous pelvic radiotherapy). The area under the receiver operating characteristic curve of the final model was 0.86 (95% confidence interval 0.85-0.87). The model is limited to patients without previous urological malignancy. Conclusions: This cancer prediction model is the first to consider established and novel urinary tract cancer diagnostic markers. It can be used in secondary care for risk stratifying patients and aid the clinician's decision-making process in prioritising patients for investigation. Patient summary: We have developed a tool that uses a person's characteristics to determine the risk of cancer if that person develops blood in the urine (haematuria).
