    Surgical site infection after wound closure with staples versus sutures in elective knee and hip arthroplasty:a systematic review and meta-analysis

    PURPOSE: This systematic review and meta-analysis aimed to study surgical site infection of wound closure using staples versus sutures in elective knee and hip arthroplasties. METHODS: A systematic literature review was performed to search for randomized controlled trials that compared surgical site infection after wound closure using staples versus sutures in elective knee and hip arthroplasties. The primary outcome was surgical site infection. The risk of bias was assessed with the Cochrane risk of bias assessment tool. The relative risk and 95% confidence interval with a random-effects model were assessed. RESULTS: Eight studies were included in this study, including 2 studies with a low risk of bias, 4 studies having ‘some concerns’, and 2 studies with high risk of bias. Significant difference was not found in the risk of SSI for patients with staples (n = 557) versus sutures (n = 573) (RR: 1.70, 95% CI: 0.94–3.08, I(2) = 16%). The results were similar after excluding the studies with a high risk of bias (RR: 1.67, 95% CI: 0.91–3.07, I(2) = 32%). Analysis of studies with low risk of bias revealed a significantly higher risk of surgical site infection in patients with staples (n = 331) compared to sutures (n = 331) (RR: 2.56, 95% CI: 1.20–5.44, I(2) = 0%). There was no difference between continuous and interrupted sutures (P > 0.05). In hip arthroplasty, stapling carried a significantly higher risk of surgical site infection than suturing (RR: 2.51, 95% CI: 1.15–5.50, I(2) = 0%), but there was no significant difference in knee arthroplasty (RR: 0.87, 95% CI: 0.33–2.25, I(2) = 22%; P > 0.05). CONCLUSIONS: Stapling might carry a higher risk of surgical site infection than suturing in elective knee and hip arthroplasties, especially in hip arthroplasty.     Hysterectomy Does Not Cause Constipation

    PURPOSE: This study was designed to evaluate the risk on development and persistence of constipation after hysterectomy. METHODS: We conducted a prospective, observational, multicenter study with three-year follow-up in 13 teaching and nonteaching hospitals in the Netherlands. A total of 413 females who underwent hysterectomy for benign disease other than symptomatic uterine prolapse were included. All patients underwent vaginal hysterectomy, subtotal abdominal hysterectomy, or total abdominal hysterectomy. A validated disease-specific quality-of-life questionnaire was completed before and three years after surgery to assess the presence of constipation. RESULTS: Of the 413 included patients, 344 (83 percent) responded at three-year follow-up. Constipation had developed in 7 of 309 patients (2 percent) without constipation before surgery and persisted in 16 of 35 patients (46 percent) with constipation before surgery. Preservation of the cervix seemed to be associated with an increased risk of the development of constipation (relative risk, 6.6; 95 percent confidence interval, 1.3-33.3; P=0.02). Statistically significant risk factors for the persistence of constipation could not be identified. CONCLUSIONS: Hysterectomy does not seem to cause constipation. In nearly half of the patients reporting constipation before hysterectomy, this symptom will disappear

    Uterine Artery Embolization in Patients with a Large Fibroid Burden: Long-Term Clinical and MR Follow-up

    Uterine artery embolization (UAE) in patients with a large fibroid burden is controversial. Anecdotal reports describe serious complications and limited clinical results. We report the long-term clinical and magnetic resonance (MR) results in a large series of women with a dominant fibroid of >10 cm and/or an uterine volume of >700 cm3. Seventy-one consecutive patients (mean age, 42.5 years; median, 40 years; range, 25–52 years) with a large fibroid burden were treated by UAE between August 2000 and April 2005. Volume reduction and infarction rate of dominant fibroid and uterus were assessed by comparing the baseline and latest follow-up MRIs. Patients were clinically followed at various time intervals after UAE with standardized questionnaires. There were no serious complications of UAE. During a mean follow-up of 48 months (median, 59 months; range, 6–106 months), 10 of 71 patients (14%) had a hysterectomy. Mean volume reduction of the fibroid and uterus was 44 and 43%. Mean infarction rate of the fibroid and overall fibroid infarction rate was 86 and 87%. In the vast majority of patients there was a substantial improvement of symptoms. Clinical results were similar in patients with a dominant fibroid >10 cm and in patients with large uterine volumes by diffuse fibroid disease. In conclusion, our results indicate that the risk of serious complications after UAE in patients with a large fibroid burden is not increased. Moreover, clinical long-term results are as good as in other patients who are treated with UAE. Therefore, a large fibroid burden should not be considered a contraindication for UAE

    Purpose: In press-fit total hip arthroplasty (THA), primary stability is needed to avoid micromotion and hereby aseptic loosening, the main reason for early revision. High aseptic loosening revision rates of the seleXys TH+ cup (Mathys Medical) with Ceramys ceramic-on-ceramic (CoC) bearing are seen in literature. Since CoC is presumed to overcome long-term wear-related revisions, the reason for early failure of this cup is important to clarify. The aim is to investigate its ten year outcomes and differentiate between potential causes and identify risk factors for aseptic loosening. Methods: Retrospective screening of a prospectively documented series of 315 THAs was performed. Primary outcome was cumulative incidence of cup revision due to aseptic loosening. Secondary outcomes were component revision and reoperation. Additionally, potential predictive factors for aseptic loosening were evaluated. Results: At the median follow-up of 9.7 years [IQR 4.4; 10.3], 48 TH+ (15.2%) were revised due to aseptic loosening. Competing risk analysis showed a ten year cumulative incidence of cup revision due to aseptic loosening of 15.6% (95% CI 12.0–20.2). Stabilization of early revision rates was observed, following a high rate of respectively 81.3% (n = 39) and 95.8% (n = 46) within the first two and three years. No significant predictive factors for aseptic loosening were found. Conclusion: The ten year results of seleXys TH+ cup with Ceramys CoC bearing showed an unacceptable high aseptic loosening rate, which stabilized over time after a high early failure incidence. This could be attributed to a problem with osseointegration during the transition of primary to definitive stability

    Three-dimensional stress analysis of threaded cups – a finite element analysis

    A three-dimensional model of the left acetabulum with inserted threaded cup has been generated, based on the finite element method, to calculate stress patterns in the standing phase during walking. In this study, a hemispherical cup with sharp threads, a parabolic cup with flat threads and a conical cup with sharp threads were analysed and compared. Stress patterns in both implant components and adjacent bony structures were calculated in a directly postoperative situation. The different cups were found to induce different stress patterns, deformations and shifting tendencies. The inlays deform notably and show characteristic rotational movement patterns together with the shell. The inclination angle increases in the hemispherical cup and decreases in the parabolic cup. The conical cup levers outward almost parallel to the bone stock by approximately 0.05 mm. The pole surfaces of the various cups – especially the very convex area next to the threads – induce increased compressive stress in the superior section of the acetabular base. This is increased by a factor of three in the conical cup in comparison to the hemispherical cup and less so in comparison to the parabolic cup. This study illustrates that three-dimensional stress calculations are suitable for procuring additional biomechanical information to augment clinical studies, for evaluating implants and for establishing stability prognoses, especially for newly developed prototypes

    The rise of flex crops and commodities: implications for research

    As a concept and phenomenon, ‘flex crops and commodities’ feature ‘multiple-ness’ and ‘flexible-ness’ as two distinct but intertwined dimensions. These key crops and commodities are shaped by the changing global context that is itself remoulded by the convergence of multiple crises and various responses. The greater multiple-ness of crops and commodity uses has altered the patterns of their production, circulation and consumption, as novel dimensions of their political economy. These new patterns change the power relations between landholders, agricultural labourers, crop exporters, processors and traders; in particular, they intensify market competition among producers and incentivize changes in land-tenure arrangements. Crop and commodity flexing have three main types – namely, real flexing, anticipated/speculative flexing and imagined flexing; these have many intersections and interactions. Their political-economic dynamics involve numerous factors that variously incentivize, facilitate or hinder the ‘multiple-ness’ and/or ‘flexible-ness’ of particular crops and commodities. These dynamics include ‘flex narratives’ by corporate and state institutions to justify promotion of a flex agenda through support policies. In particular, a bioeconomy narrative envisages a future ‘value web’ developing more flexible value chains through more interdependent, interchangeable products and uses. A future research agenda should investigate questions about material bases, real-life changes, flex narratives and political mobilization
