    Long-term renal function and hypertension in adult survivors of childhood sarcoma

    Aim: Little data is available on long-term renal impairment in survivors from childhood sarcoma. We investigated the prevalence of renal impairment and hypertension after very long-term follow-up in survivors who reached adulthood after treatment for childhood sarcoma. Methods. A cross-sectional single center study was performed. Outcomes included estimating glomerular filtration rate (eGFR), albuminuria, glycosuria, serum phosphate and magnesium, tubular reabsorption phosphate (TRP), chronic kidney disease (CKD) according to the “Kidney Disease: Improving Global Outcomes” (KDIGO) guidelines and blood pressure (BP). Results: Out of 87>5-year sarcoma survivors, 30 adults (10F/20M, median age at diagnosis 9 years, median age at investigation 26 years, median follow-up 16 years, mean 19 years) were identified. Renal impairment was detected in 4 cases (13.3%); 3 of these fulfilled the criteria for CKD. Among the adult survivors, a subgroup of 15 cases (50%) had received ifosfamide without confounding factors such as a diagnosis of genito-urinary rhabdomyosarcoma or administration of other potentially nephrotoxic chemotherapy (platinum-based drugs or methotrexate); no renal dysfunction was detected in this subgroup. In the whole cohort of sarcoma survivors, hypertension was diagnosed in 4 cases (13.3%); BP was significantly correlated with body mass index [p 0.014] Conclusion: In our series of adult survivors treated for a diagnosis of sarcoma in their childhood, the prevalence of CKD was 10%. We found survivors treated with ifosfamide as the only nephrotoxic agent did not present glomerular or tubular toxicity at long term follow-up, but further studies including a larger number of cases are required to confirm it.

    Surgical treatment of retrosternal extraosseous Ewing Sarcoma in a 6-years old female: a clamshell approach with hemysternectomy and application of a non-crosslinked extracellular matrix

    Background Ewing Sarcoma (ES) and Neuroblastoma (NB) belong to a family of tumours of primitive neuroectodermal origin (PNET) that occurs in both bone and soft tissue. Notwithstanding ES and NB are two distinct malignant tumours, sometimes there could be a link between them. Case report We describe a case of an extraosseous ES localized in the retrosternal region and the upper lobe of the right lung, which had been previously treated for NB in a 6 years old female. We treated this case with a clamshell approach which allows, in a one-step surgery, a complete excision of the mass reconstructing the hemysternectomy with a non-crosslinked matrix. Conclusion the clamshell approach is therefore useful to achieve the retrosternal space and the lung with a single surgical access. According to our experience, we consider appropriate to use a non-crosslinked matrix for sternal reconstruction

    Representations and beliefs in haitian voodoo : filmic approach of a religious community from Port-au-Prince

    A travers une exploration filmique menĂ©e auprĂšs d'une communautĂ© religieuse de Port-au-Prince, l’analyse des images enregistrĂ©es avec les personnes filmĂ©es et l’expĂ©rience pratique de constructions audiovisuelles participatives, la recherche interroge le systĂšme de reprĂ©sentation des adeptes et le rapport qu'il entretient avec la notion de croyance. Elle Ă©tudie simultanĂ©ment par ce biais la rĂ©ception de formes audiovisuelles spĂ©cifiques et l’observation diffĂ©rĂ©e des enregistrements en tant qu’outil privilĂ©giĂ© d'une anthropologie audiovisuelle appliquĂ©e Ă  l'Ă©tude des systĂšmes symboliques de reprĂ©sentation et Ă  leur dynamique Ă©volutive. Cette recherche est marquĂ©e par trois Ă©tapes de rĂ©alisations concrĂštes selon des paradigmes audiovisuels dĂ©finis : un multimĂ©dia interactif, un film d’observation descriptif chronologique, et un film linĂ©aire poursuivant l’objectif d’une mise en scĂšne de l’autoreprĂ©sentation des agents suivant les prĂ©cĂ©dentes Ă©tapes collaboratives d’observations, d’interprĂ©tations et de constructions audiovisuelles participatives qui forment l’enquĂȘte. L’image, sous ses multiples acceptions, est envisagĂ©e ici en tant qu’objet transactionnel agissant Ă  diffĂ©rentes Ă©chelles sociales, depuis ses usages et fabrications Ă  un niveau individuel jusqu’à ceux-ci Ă  un niveau collectif et transnational. Cet aspect est analysĂ©, d’abord, au moyen de la confrontation successive entre imagerie religieuse produite au sein de la confrĂ©rie et reprĂ©sentations audiovisuelles produites par la recherche elle-mĂȘme et retournĂ©es aux agents, et ensuite, par confrontation avec l’iconographie religieuse et les reprĂ©sentations anciennes et contemporaines produites par des sources endogĂšnes et exogĂšnes Ă  l’aire culturelle haĂŻtienne. Le texte analyse ainsi la construction de reprĂ©sentations et leurs effets depuis les expĂ©riences pratiques menĂ©es avec les individus d’une communautĂ© jusqu’à celles qui intĂšgrent certains Ă©vĂšnements sociohistoriques, certains clichĂ©s et certaines figures mythologiques d’inspirations vodou ayant intĂ©grĂ©s la culture occidentale Ă  travers, notamment, des reprĂ©sentations romanesques et hollywoodiennes ayant eu un fort succĂšs et continuant Ă  alimenter l’imaginaire occidental et haĂŻtien aujourd’hui.The Haitian voodoo presents a profusion of practices and perceptible representations, symbolic ones, as material as corporeal. He influences the arts and the literature of a whole culture, well beyond the circle of his followers. If it is plural from origin, it was also collected and represented in a multiple way by his outer observers. Forged during a tempestuous history and present, its representation system seems to present a character at the same time persistent and permeable, connected to the faith itself. Through the analysis of worship images, a cinematic exploration conducted within a religious community of Port-au-Prince, and finally through a participatory experience of confrontation of the recorded images with the agents and film construction according to three different modalities, the study attempts to question the representation system of the followers and the relationship which it maintains with the faith. The research also tries to examine restitutions forms and specific visual anthropology tools applied to the study of symbolic representation systems and their dynamics
