    GridShield—Optimizing the Use of Grid Capacity during Increased EV Adoption

    With the increasing adoption rate of electric vehicles, power peaks caused by many cars simultaneously charging on the same low-voltage grid can cause local overloading and power outages. Smart charging solutions should spread this load, but there is a residual risk of incidental peaks. A decentralized and autonomous technology called GridShield is being developed to reduce the likelihood of a transformer’s fuse blowing when other congestion solutions have failed. It serves as a measure of last resort to protect the grid against local power failures from unpredicted congestion by temporarily limiting the virtual capacity of charging stations. This paper describes the technical development and demonstrates how GridShield can keep a transformer load below a critical limit using simulations and real-world tests. It optimizes grid capacity while ensuring grid reliability.</p

    Het programma Nu Niet Zwanger is gericht op het voorkomen van onbedoelde zwangerschappen bij mensen in kwetsbare omstandigheden. Hulpverleners in het sociaal en medisch domein gaan daartoe tijdens reguliere contactmomenten met hun eigen cliënten (m/v) in gesprek over hun kinderwens. Ze ondersteunen hen bij het maken van een geïnformeerde en vrijwillige keuze, en bieden desgewenst begeleiding bij het kiezen en het realiseren van anticonceptie. Onderzoek laat zien dat de ervaringen van hulpverleners en cliënten met Nu Niet Zwanger positief zijn en leverde ook verbeterpunten op. Begin 2023 wordt het programma in bijna twee derde van de gemeenten in Nederland uitgevoerd en ligt de focus op structurele, landelijke borging.The program Not Pregnant Now aims to prevent unintended pregnancies in vulnerable populations. To this end, care providers start an openconversation during regular social and healthcare visits with their clients about their desire to have children. They support their clients in making an informed and voluntary decision about this and, ifwanted, offer support in choosing and realizing contraception. Research showed that the experiences of the care providers and clients with the program arepositive and also resulted in recommendations for improvement. At the beginning of 2023, the program runs in three quarters of the municipalities in TheNetherlands and the focus is on structural, national maintenance

    Visualizing the 3D collagen structure of human atherosclerotic plaques using Diffusion Tensor Imaging

    Introduction Ischemic strokes and heart attacks are mainly caused by rupture of the fibrous cap of an atherosclerotic plaque. Reliable prediction of the fibrous cap rupture is, therefore, crucial to prevent these potentially lethal cardiovascular events. Since cap rupture occurs when the stresses in the cap exceed the strength of the cap, biomechanical modeling may help to improve cap rupture prediction. Biomechanical models depend strongly on the material parameters used as input. Previous studies focused on the anisotropic mechanical behaviour of atherosclerotic plaques and produced stiffness values for the collagen fibers in plaques [1]. However, for a more complete characterization knowledge of the global 3D collagen architecture in atherosclerotic plaques is required. Therefore, for the first time diffusion tensor imaging (DTI) was used to investigate the 3D collagen structure of human atherosclerotic plaques. Methods Until now five human carotid atherosclerotic plaques were obtained from endarterectomy patients and embedded in 4 % type VII agarose. The samples were placed in a 9.4 T horizontal-bore MRI scanner to conduct DTI. DTI enabled the tracking of the fiber directions and visualisation of the collagen fibers [2]. Results The consistent results of five different plaques suggest that collagen fibers are deposited in a new layer in a different direction during the development of atherosclerosis (see figure for one representative result). Two distinct layers of collagen fibers were found; an outer layer, where the collagen is aligned in the circumferential direction (14.5°±28.0°), similar to healthy arteries [2], and an inner layer where the collagen follows a longitudinal direction (77.4°±22.4°). Conclusions DTI allowed the visualization of the global 3D collagen architecture of atherosclerotic plaques. The inner collagen layer showed a surprising result and implies a change of strain distribution in the artery during the later stage of atherosclerosis, possibly due to the thickening and stiffening of the diseased intimal tissue. These data, combined with collagen stiffness data found in previous studies [1], will be used as input for biomechanical models including the anisotropic mechanical behaviour of plaque tissue. Models using general over-simplified assumptions like isotropic behaviour can be replaced by models including the anisotopic behavior and thereby improve the stress analysis of plaques. Improved models might help in the diagnosis and treatment of plaque rupture preventing heart attacks and ischemic strokes.     Endovascular repair versus open surgery in patients with ruptured abdominal aortic aneurysms: Clinical outcomes with 1-year follow-up

    ObjectiveTo compare the clinical outcomes of treatment after endovascular repair and open surgery in patients with ruptured infrarenal abdominal aortic aneurysms (AAAs), including 1-year follow-up.MethodsAll consecutive conscious patients with ruptured infrarenal AAAs who presented to our tertiary care teaching hospital between January 1, 2001, and December 31, 2005, were included in this study (n = 55). Twenty-six patients underwent endovascular repair, and 29 patients underwent open surgery. Patients who were hemodynamically too unstable to undergo a computed tomography angiography scan were excluded. Outcomes evaluated were intraoperative mortality, 30-day mortality, systemic complications, complications necessitating surgical intervention, and mortality and complications during 1-year follow-up. The statistical tests we used were the Student t test, χ2 test, Fisher exact test, and Mann-Whitney U test (two sided; α = .05).ResultsThirty-day mortality was 8 (31%) of 26 patients who underwent endovascular repair and 9 (31%) of 29 patients who underwent open surgery (P = .98). Systemic complications and complications necessitating surgical intervention during the initial hospital stay were similar in both treatment groups (8/26 [31%] and 5/26 [19%] for endovascular repair, respectively, and 9/29 [31%] and 8/29 [28%] for open surgery, respectively; P > .40). During 1-year follow-up, two patients initially treated with endovascular repair died as a result of non–aneurysm-related causes; no death occurred in the open surgery group. Complications during 1-year follow-up were 1 (5%) of 20 for endovascular repair and 4 (16%) of 25 for open surgery (P = .36).ConclusionsOn the basis of our study with a highly selected population, the mortality and complication rates after endovascular repair may be similar compared with those after open surgery in patients treated for ruptured infrarenal AAAs

    Trace incorporation of heavy water reveals slow and heterogeneous pathogen growth rates in cystic fibrosis sputum

    Effective treatment for chronic infections is undermined by a significant gap in understanding of the physiological state of pathogens at the site of infection. Chronic pulmonary infections are responsible for the morbidity and mortality of millions of immunocompromised individuals worldwide, yet drugs that are successful in laboratory culture are far less effective against pathogen populations persisting in vivo. Laboratory models, upon which preclinical development of new drugs is based, can only replicate host conditions when we understand the metabolic state of the pathogens and the degree of heterogeneity within the population. In this study, we measured the anabolic activity of the pathogen Staphylococcus aureus directly in the sputum of pediatric patients with cystic fibrosis (CF), by combining the high sensitivity of isotope ratio mass spectrometry with a heavy water labeling approach to capture the full range of in situ growth rates. Our results reveal S. aureus generation times with a median of 2.1 d, with extensive growth rate heterogeneity at the single-cell level. These growth rates are far below the detection limit of previous estimates of CF pathogen growth rates, and the rates are slowest in acutely sick patients undergoing pulmonary exacerbations; nevertheless, they are accessible to experimental replication within laboratory models. Treatment regimens that include specific antibiotics (vancomycin, piperacillin/tazobactam, tobramycin) further appear to correlate with slow growth of S. aureus on average, but follow-up longitudinal studies must be performed to determine whether this effect holds for individual patients

    In 2014, the debate on the indication of revascularization in case of asymptomatic carotid disease continued, while another one regarding the use of surgery vs. stenting addressed some new issues regarding the long-term cardiac risk of these patients. Renal arteries interventions trials were disappointing, as neither renal denervation nor renal artery stenting was found associated with better blood pressure management or outcome. In contrast, in lower-extremities artery disease, the endovascular techniques represent in 2014 major alternatives to surgery, even in distal arteries, with new insights regarding the interest of drug-eluting balloons. Regarding the aorta, the ESC published its first guidelines document on the entire vessel, emphasizing on the role of every cardiologist for screening abdominal aorta aneurysm during echocardiography. Among vascular wall biomarkers, the aorta stiffness is of increasing interest with new data and meta-analysis confirming its ability to stratify risk, whereas carotid intima-media thickness showed poor performances in terms of reclassifying patients into risk categories beyond risk scores. Regarding the veins, new data suggest the interest of D-dimers and residual venous thrombosis to help the decision of anti-coagulation prolongation or discontinuation after the initial period of treatment for deep vein thrombosis

    Lessons to be learned from the coherent photoproduction of pseudoscalar mesons

    We study the coherent photoproduction of pseudoscalar mesons---particularly of neutral pions---placing special emphasis on the various sources that put into question earlier nonrelativistic-impulse-approximation calculations. These include: final-state interactions, relativistic effects, off-shell ambiguities, and violations to the impulse approximation. We establish that, while distortions play an essential role in the modification of the coherent cross section, the uncertainty in our results due to the various choices of optical-potential models is relatively small (of at most 30%). By far the largest uncertainty emerges from the ambiguity in extending the many on-shell-equivalent representations of the elementary amplitude off the mass shell. Indeed, relativistic impulse-approximation calculations that include the same pionic distortions, the same nuclear-structure model, and two sets of elementary amplitudes that are identical on-shell, lead to variations in the magnitude of the coherent cross section by up to factors of five. Finally, we address qualitatively the assumption of locality implicit in most impulse-approximation treatments, and suggest that the coherent reaction probes---in addition to the nuclear density---the polarization structure of the nucleus.Comment: Manuscript is 27 pages long and includes 11 eps figure
