بررسي مقايسهاي تأثیر تمرينات Frankel و Swiss Ball بر بهبود بالانس و افسردگي افراد مبتلا به مولتيپل اسكلروزيس
مقدمه: مولتيپل اسكلروزيس شايعترين بيماري نرولوژي پيشرونده در جوانان بالغ ميباشد. اين بيماري باعث تخريب ميلين در آكسونهاي عصبي شده، در نتيجه ضايعات عملكردي ايجاد ميكند. يكي از مناطق مهم درگير در MS مخچه ميباشد. مشكل در اين ساختار باعث ايجاد اختلال در تعادل و ... ميشود كه به تبع آن كيفيت زندگي كاهش مييابد. بنابراين استفاده از روشهاي كم هزينه و بدون عوارض جانبي براي برطرف كردن اين مشكل ضروري به نظر ميرسد.
مواد و روشها: اين مطالعه از نوع آيندهنگر، شبه تجربي و يك سويه كور بوده، كه در سال 88-87 در شهر اصفهان انجام گرفته است. در اين مطالعه طي 10 جلسه، به 23 بيمار مبتلا به MS كه در هر گروه درماني قرار ميگرفت، تمرينات Frankel يا Swiss Ball به مدت 45 دقيقه در هر جلسه تحت نظارت مستقيم تراپيست آموزش داده ميشد. قبل و بعد از كامل شدن جلسات درماني جهت ارزيابي بالانس و افسردگي به ترتيب از پرسشنامههاي معتبر Berg balance scale and beck استفاده شد. جهت تجزيه وتحليل دادهها با توجه به كمي بودن آنها در هر گروه از آزمون Dependent t-test و بين دو گروه از Independent t-test موجود در نرمافزار آماري 13SPSS استفاده شد.
یافتهها: بين وضعيت تعادل بيماران، قبل و بعد از درمان در گروه تمرينات Frankel و در گروه تمرينات Swiss Ball تفاوت معنيداري وجود داشت (01/0 > P). بين وضعيت افسردگي بيماران، قبل و بعد از درمان در گروه تمرينات Frankel و در گروه تمرينات Swiss Ball تفاوت معنيداري وجود داشت (01/0 > P). بين وضعيت تعادل بيماران، قبل از درمان بين دو گروه تفاوت معنيداري وجود نداشت (18/0 = P). اما بعد از درمان بين دو گروه تفاوت معنيداري وجود داشت (01/0 > P). بين وضعيت افسردگي بيماران، قبل از درمان بين دو گروه تفاوت معنيداري وجود نداشت (29/0 = P). همچنين بعد از درمان بين دو گروه تفاوت معنيداري وجود داشت (30/0 = P).
نتیجهگیری: انجام تمرينات Frankel و Swiss Ball به صورت مداوم و با فرض ثابت بودن شرايط بيمار باعث بهبودي در تعادل و افسردگي بيماران مبتلا به MS شد. همچنين تمرينات Frankel در بهبود تعادل مؤثرتر از تمرينات Swiss Ball است.

The role of stem cell therapy in multiple sclerosis: An overview of the current status of the clinical studies
The complexity of multiple sclerosis (MS) and the incompetence of a large number of promised treatments for MS urge us to plan new and more effective therapeutic approaches that aim to suppress ongoing autoimmune responses and induction of local endogenous regeneration. Emerging data propose that hematopoietic, mesenchymal, and neural stem cells have the potential to restore self-tolerance, provide in situ immunomodulation and neuroprotection, as well as promote regeneration. Thus, in this article, we will first provide an overview of the cell sources for proposed mechanisms that contribute to the beneficial effects of stem cell transplantation, the ideal route and/or timing of stem cell-based therapies for each main stem cell group, and finally, an overview of the current status of stem cell research in clinical trial stages in MS by comparable and healthy therapeutic effects of different stem cell therapies for MS patients
Effect of MS14® on physical activity of multiple sclerosis patients : a randomized triple-blind placebo-controlled clinical trial
Introduction: Multiple Sclerosis (MS) is a neurological disorder with an increasing global prevalence and severe complications. MS14® is a Persian-medicine-derived natural product with herbal and marine origin which has shown beneficial effects in the management of MS complications. In this study, its effect on physical activity of MS patients was investigated. Methods: A triple-blind placebo-controlled clinical trial was conducted. Participants used either MS14 capsule or placebo 3 times a day for 3 weeks. At baseline and end of the study, physical activity indices were assessed using international physical activity questionnaire (IPAQ). Secondary outcome measures were Fatigue Severity Scale (FSS), timed 10 m walk, Ashworth scale, and Timed Get up and Go. Results: A total number of 80 MS patients completed the study. At the end of study, improvement of general physical activity (p-value=0.047) and Timed 10 m walk index (p-value=0.003) in the MS14 group was significant when compared to placebo. No serious adverse effects were observed in this study. Conclusion: Considering the improvement of some physical activity indices, MS14® is seems to be a safe natural product which could be considered as a supplementary treatment in MS patients. Future larger trials are suggested to further evaluate its efficacy
