    Strings in gravity with torsion

    A theory of gravitation in 4D is presented with strings used in the material action in U4U_4 spacetime. It is shown that the string naturally gives rise to torsion. It is also shown that the equation of motion a string follows from the Bianchi identity, gives the identical result as the Noether conservation laws, and follows a geodesic only in the lowest order approximation. In addition, the conservation laws show that strings naturally have spin, which arises not from their motion but from their one dimensional structure.Comment: 16 page

    Thick domain walls around a black hole

    We discuss the gravitationally interacting system of a thick domain wall and a black hole. We numerically solve the scalar field equation in the Schwarzschild spacetime and obtain a sequence of static axi-symmetric solutions representing thick domain walls.     Factors influencing adherence to dietary guidelines: a qualitative study on the experiences of patients with type 2 diabetes attending a clinic in Cape Town

    Objective: The purpose of this study was to explore the experiences of patients with type 2 diabetes mellitus attending the Groote Schuur Hospital Diabetes Clinic in relation to contextual factors that promote or impede adherence to nutrition care guidelines. Subjects and setting: Eight diabetic patients (four males and four females) attending the Groote Schuur Hospital Diabetes Clinic who were between 40-70 years old. Outcome measures: Qualitative analysis of factors influencing adherence to dietary guidelines. Method: An explorative study, using a qualitative approach with eight semi-structured interviews, was used. Patients were interviewed at the diabetes outpatient clinic in Cape Town. Semi-structured interviews were recorded and transcribed, and thereafter analysed using ATLAS/ti®. Results: Various themes as to what influences adherence to dietary guidelines emerged. An ecological analysis is offered to understand the different levels of influence on participants’ dietary behaviour. The main identified factors at individual level were motivation, individual knowledge, perceptions of moderation, self- responsibility, taste concept or cravings, and temptations. At small group (family and friends) level, family relations with the patients were identified as the main support system used to manage the diabetes. At the organisational or health systems level, long waiting times and the theme of seeing different doctors emerged as problematic factors, but overall, patients were satisfied with the clinic service. At community and policy level, culture and the cost of food were identified as key influential factors with regard to adherence to nutrition care guidelines. Conclusion: This study shows the usefulness of adopting an ecological model in identifying factors that influenced adherence to dietary guidelines by patients with type 2 diabetes. Some factors acted as enablers and others as barriers. These had an impact on patient adherence to nutrition care guidelines. These factors should be considered by diabetes educators, including dietitians and nurses and doctors, when planning treatment modalities for patients with type 2 diabetes mellitus

    Effect of ospemifene on moderate or severe symptoms of vulvar and vaginal atrophy.

    Objectives To determine whether assessment of all moderate-to-severe symptoms at baseline gives a more accurate evaluation of the treatment effect of ospemifene in vulvovaginal atrophy (VVA) than the most bothersome symptom (MBS) approach. Methods Data were pooled from two pivotal phase-III clinical trials evaluating the efficacy and safety of oral ospemifene 60 mg/day for the treatment of symptoms of VVA (n = 1463 subjects). Symptoms of vaginal dryness, dyspareunia, and vaginal and/or vulvar irritation/itching reported as moderate or severe at baseline were evaluated. Clinically relevant differences between ospemifene and placebo were analyzed using a four-point severity scoring system and presented as improvement, substantial improvement, or relief. Results Subjects in these studies reported statistically significant improvement, substantial improvement, and relief for vaginal dryness (p \u3c 0.00001), dyspareunia (p \u3c 0.001) and statistically significant improvement and relief for vaginal and/or vulvar irritation/itching (p \u3c 0.01) from baseline to week 12 with ospemifene compared with placebo. A similar trend was observed for women who reported substantial improvement of vaginal and/or vulvar irritation/itching. Conclusions For drug registration purposes, the use of the MBS model is appealing because of its simplicity and ease of scientific validation. However, the MBS model may underestimate the total magnitude of the clinical benefit of ospemifene treatment for symptomatic women suffering from VVA

    Energy flux through the horizon in the black hole-domain wall systems

    We study various configurations in which a domain wall (or cosmic string), described by the Nambu-Goto action, is embedded in a background space-time of a black hole in (3+1)(3+1) and higher dimensional models. We calculate energy fluxes through the black hole horizon. In the simplest case, when a static domain wall enters the horizon of a static black hole perperdicularly, the energy flux is zero. In more complicated situations, where parameters which describe the domain wall surface are time and position dependent, the flux is non-vanishing is principle. These results are of importance in various conventional cosmological models which accommodate the existence of domain walls and strings and also in brane world scenarios.Comment: references added, accepted for publication in JHE

    Retrieval analysis of ceramic-coated metal-on-polyethylene total hip replacements

    PURPOSE: Ceramic coatings have been used in metal-on-polyethylene (MOP) hips to reduce the risk of wear and also infection; the clinical efficacy of this remains unclear. This retrieval study sought to better understand the performance of coated bearing surfaces. METHODS: Forty-three coated MOP components were analysed post-retrieval for evidence of coating loss and gross polyethylene wear. Coating loss was graded using a visual semi-quantitative protocol. Evidence of gross polyethylene wear was determined by radiographic analysis and visual inspection of the retrieved implants. RESULTS: All components with gross polyethylene wear (n = 10) were revised due to a malfunctioning acetabular component; 35 % (n = 15) of implants exhibited visible coating loss and the incidence of polyethylene wear in samples with coating loss was 54 %, significantly (p = 0.02) elevated compared to samples with intact coatings (14 %). CONCLUSIONS: In this study we found evidence of coating loss on metal femoral heads which was associated with increased wear of the corresponding polyethylene acetabular cups

    Dynamically enhancing qubit-oscillator interactions with anti-squeezing

    The interaction strength of an oscillator to a qubit grows with the oscillator's vacuum field fluctuations. The well known degenerate parametric oscillator has revived interest in the regime of strongly detuned squeezing, where its eigenstates are squeezed Fock states. Owing to these amplified field fluctuations, it was recently proposed that squeezing this oscillator would dynamically boost its coupling to a qubit. In a superconducting circuit experiment, we observe a two-fold increase in the dispersive interaction between a qubit and an oscillator at 5.5 dB of squeezing, demonstrating in-situ dynamical control of qubit-oscillator interactions. This work initiates the experimental coupling of oscillators of squeezed photons to qubits, and cautiously motivates their dissemination in experimental platforms seeking enhanced interactions.Comment: 21 pages, 15 figure
