    La libertad de expresión en el contexto de la seguridad nacional

    This article considers the versions of the constitutional consolidation of freedom of speech in the post-Soviet space in conjunction with the restrictive reasons for national security. Given that the latter is inherent in all the states, the authors, based on an analysis of their constitutional texts and doctrinal discussions about the divide between rights and freedoms, identified two of their principal groups. The first includes constitutions in which the freedom of speech is considered through the law and the second - where freedom of speech is considered through its guaranteeing. The article concludes that freedom is an independent, recognized as the legal ability of a person to choose the measure of his behavior, which in this case is outlined by the requirements of the norms of morality, religion, law, national security.Este artículo considera las versiones de la consolidación constitucional de la libertad de expresión en el espacio postsoviético junto con las razones restrictivas de la seguridad nacional. Dado que este último es inherente a todos los estados, los autores, basándose en un análisis de sus textos constitucionales y discusiones doctrinales sobre la división entre derechos y libertades, identificaron dos de sus grupos principales. El primero incluye las constituciones en las que la libertad de expresión se considera a través de la ley y el segundo, donde la libertad de expresión se considera a través de su garantía. El artículo concluye que la libertad es una capacidad independiente, reconocida como la capacidad legal de una persona para elegir la medida de su comportamiento, que en este caso está delineado por los requisitos de las normas de moralidad, religión, ley y seguridad nacional

    Risk Assessment at the Cosmetic Product Manufacturer by Expert Judgment Method

    A case study was performed in a cosmetic product manufacturer. We have identified the main risk factors of occupational accidents and their causes. Risk of accidents is assessed by the expert judgment method. Event tree for the most probable accident is built and recommendations on improvement of occupational health and safety protection system at the cosmetic product manufacturer are developed. The results of this paper can be used to develop actions to improve the occupational safety and health system in the chemical industry

    С момента первой вспышки SARS-Cov-2 в Китае большое внимание медицинского сообщества было уделено людям с сахарным диабетом, так как хорошо известно, что сахарный диабет повышает риск развития ряда инфекционных заболеваний. Причина этого многофакторная: возраст, пол, этническая принадлежность, сопутствующие заболевания, такие как гипертония, сердечно-сосудистые заболевания, ожирение, а также провоспалительное и прокоагулянтное состояние – все это способствует более тяжелому течению SARS-Cov-2 у пациентов с сахарным диабетом. Более того, тяжелая инфекция SARS-Cov-2 сама по себе может представлять собой ухудшающий фактор для людей с сахарным диабетом, поскольку она может вызвать острые метаболические осложнения через прямое негативное воздействие на функцию β-клеток. Данный обзор призван обеспечить системную оценку потенциальных прогностических факторов и взаимных эффектов у пациентов с сахарным диабетом, ожирением и SARS-Cov-2

    Autoimmune gastritis: comorbid pathology in type 1 diabetes

    In recent years noted an increased number of patients with autoimmune diseases (AID), and the special attention deserves combination of several autoimmune pathologies collected in one patient, because such patients requires special tactics of management. Autoimmune diseases of the gastrointestinal tract are less researched and, nevertheless, prognostically unfavorable. Autoimmune gastritis (AIG) has a special place in other stomach diseases, which develops in the aggregate with type 1 diabetes. AIG is a chronic inflammation of the mucous membrane of the body of the stomach, leading to the appearance of atrophy and hypoxecretion. AIG is found in 510% of individuals in general population, with a higher incidence of AIG in patients with diabetes and other established autoimmune pathology. AIG is asymptomatic at the early stage of the disease and clinical manifestations appear after the atrophic changes in the mucous membrane of the stomach develop. Since autoimmune gastritis does not have pathognomonic signs, it can manifest itself in hematological and oncological complications at later stages. This is why screening, early diagnosis, prevention and treatment are very important. Early diagnosis and prevention of AIG and its complications play the main role. This article provides an overview of the worldwide data dedicated to this issue

    A clinical case of exocrine pancreatic insufficiency in a patient with type 1 diabetes mellitus

    The pancreas belongs to the glands of mixed secretion and simultaneously performs both endo- and exocrine functions. Exocrine pancreatic insufficiency (EPI) is the general name for the malabsorption process caused by inadequate production and decreased activation of the enzymes of the pancreas acinar cells, such as amylase, lipase and protease, which are necessary for digestion. The prevalence of EPI in patients with type 1 diabetes, according to many authors, varies from 25 to 59%, which is determine by the data of pancreatic elastase-1. In this work, we present a clinical case of confirmed exocrine pancreatic insufficiency in a patient with a 6-year history of type 1 diabetes, which became the main cause of the development of episodes of hypoglycemia after meals. In the course of further studies, antibodies to lactoferrin and a reduced prostate volume, determined by MRI data, high levels of antibodies to glutamate decarboxylase and zinc co-transporter 8, as well as residual insulin secretion based on the level of C-peptide on an empty stomach detected

    Environmental Behavior of Youth and Sustainable Development

    The relationship between people and nature is one of the most important current issues of human survival. This circumstance makes it necessary to educate young people who are receptive to global challenges and ready to solve the urgent problems of our time. The purpose of the article is to analyze the experience of the environmental behavior of young people in the metropolis. The authors studied articles and monographs that contain Russian and international experience in the environmental behavior of citizens. The following factors determine people’s behavior: the cognitive capabilities of people who determine the understanding and perception of nature and the value-affective component that determines the attitude towards nature. The next task of the study is surveying young people through an online survey and its analysis. The research was realized in Ekaterinburg, the administrative center of the Sverdlovsk region (Russia). The study of the current ecological situation in Ekaterinburg made it possible to conclude that the environmental problem arises not only and not simply as a problem of environmental pollution and other negative influences of human economic activity. This problem grows into transforming the spontaneous impact of society on nature into a consciously, purposefully, systematically developing harmonious interaction with it. The study results showed that, from the point of view of the youth of Ekaterinburg, the city’s ecological situation is one of the most pressing problems. Despite minor improvements over the past 3–5 years, this problem has not lost relevance, and regional authorities and city residents should be responsible for its solution. Young people know environmental practices, but they often do not apply them systematically. Ecological behavior is encouraged and discussed among friends/acquaintances. The key factors influencing the formation of environmental behavior practices are the mass media and social networks. The most popular social network for obtaining information on ecological practices among young people is Instagram, and the key persons are bloggers. This study did not reveal the influence of the socio-demographic characteristics of young people on the application of eco-behavior practices, which may indicate the need for a survey of a larger sample. © 2021 by the authors.     The role of the gut microbiota in the development of type 1 diabetes mellitus

    At the beginning of the XXI century, with the advent of technical capabilities and new methods of genes sequencing, the attention of researchers to the study of the human metagenome has significantly increased. The interaction between changes in the qualitative and quantitative composition of the gut microbiota (GM) and various diseases is being actively studied, a search for specific metabolites and genes of microorganisms that may be associated with the development, in particular, of immune-mediated diseases is underway. In recent years, a lot of new data have been published on the possible contribution of gut flora dysbiosis to the development of Type 1 Diabetes Mellitus (T1DM), while the first assumptions were put forward as far back as 1970s. The search for pathogenetic mechanisms of GM influence on the development and progression of T1DM is becoming an increasingly relevant objective, since in recent years the incidence of T1DM is rapidly increasing, which is a serious health problem throughout the world.This review discusses the current ideas about the role of GM in the immunopathogenesis of T1DM, new data on the near-term prospects in the study of the human macrogenome, current ideas about the role of GM in the immunopathogenesis of T1DM, and the possibility of applying this knowledge by the practitioner

    Slowly evolving, immune-mediated diabetes in 14-year-old patient: a case report

    Slowly developing immune-mediated diabetes, often called latent autoimmune diabetes in adults, is characterized by the presence of autoantibodies (ATs) to glutamic acid decarboxylase (GADA), the patient's age at the onset over 35 years, and the absence of the need for insulin therapy for 6-12 months to 6 years from the moment of diagnosis, according to the WHO classification of 2019, refers to hybrid forms of diabetes mellitus (DM). In this article, we present a case history of slowly developing immune-mediated diabetes in a 14-year-old boy who was transferred from metformin monotherapy and a diet with restriction of digestible carbohydrates to the intensified insulin therapy only 4 years after the onset of diabetes mellitus with a glycated hemoglobin (HbA1c) level of less than 6.5% throughout the disease. As a result of the studies, the patient was found to have a homozygous genotype highly predisposing to the development of Type 1 Diabetes Mellitus (T1DM), as well as increased levels of ATs to GADA and tyrosine phosphatase (IA-2A). The initially preserved level of basal C-peptide and the clinical course of the disease in this patient do not allow us to classify this case as a classic variant of the course of Type 1 Diabetes Mellitus
