    A Rare Case of Lung Hypoplasia, Cardiac Anomalies and Ovarian Tumour in a Patient with MRKH Syndrome

    Hypoplasia of the lung is an uncommon congenital abnormality of the respiratory system in contrast to Pulmonary agenesis. Mayer-Rokitansky-KĂĽster-Hauser (MRKH) syndrome is the congenital absence of the upper two-thirds of the vagina and uterus with normal secondary sexual characteristics, ovary, and normal karyotype. Here we describe a case of left lung hypoplasia and congenital cardiac malformations with MRKH syndrome and Leiomyoma of the ovary. A 31-year-old female presented with cough with expectoration, left side chest pain and breathlessness for four years to Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER). She was evaluated for amenorrhea and diagnosed as MRKH syndrome and the patient underwent right side oophorectomy for right ovarian torsion with a tumour. Computed Tomography Pulmonary Angiogram (CTPA) and fiberoptic endoscopy were suggestive of left lung hypoplasia, and the patient was advised symptomatic treatment for lung hypoplasia and planned for vaginoplasty.     The incidence of acute myeloid leukemia (AML) is age dependent and increasing among the patient groups aged above 60 years. The overall presentations in a case of AML is due to marrow infiltration of leukemic cells which results in fever and chills due to neutropenia, bleeding due to thrombocytopenia, fatigue and weakness due to anemia. Incidence of venous thromboembolism is very rare in hematological malignancies. We describe a case of adolescent male presented with features of lung consolidation later found to be a pulmonary infarct and finally diagnosed to have acute leukemia (AL)

    A sudden drop of air-fluid level in the pneumonectomy space in the absence of a bronchopleural fistula and pleural infection is termed benign emptying of the post pneumonectomy space (BEPS). We herein report a case of benign emptying of pneumonectomy space, briefly review the literature and enumerate possible mechanisms for the same. It is an extremely rare entity and follows a benign course. Patients with BEPS are clinically stable, afebrile with no fluid expectoration, and have a normal white blood cell count. Bronchoscopy reveals an intact bronchial stump and pleural fluid cultures are often sterile. Close monitoring and early detection of a bronchopleural fistula is the management. Clinicians should keep BEPS among the differential diagnosis in case of a drop in the air-fluid level of the post pneumonectomy space. Awareness of this entity is crucial as it helps prevent unnecessary and morbid surgical interventions. Keywords: Pneumonectomy; BEPS; Bronchopleural Fistula; India

    An overview of treatment options for COVID-19

    Severe acute respiratory syndrome- coronavirus-2 (SARS-CoV-2) which emerged in Wuhan initially as pneumonia of unknown origin in December 2019, later spread to whole world and became pandemic on 11th March, 2020. Many drugs have been proposed but are backed without clinical evidence. Scientific bodies are in the row to discover a reliable vaccine and effective drugs against the novel coronavirus. Many antiviral and anti-parasitic drugs which were thought to have some effect on Coronavirus disease 2019 (COVID-19) have been tried during the crisis but none have shown concrete evidence of action. Randomized clinical trials on the repurposed drugs are now registered under clinical trial registry to look at the safety profile and efficacy of the drugs to be used against SARS-CoV-2. Many meta-analyses are being conducted worldwide to frame evidence for the fight against this novel coronavirus.     Iidiopathic pulmonary fibrosis (IPF) diagnosis and treatment during this COVID-19 pandemic have been affected.
