    ADFICE_IT harmonized dataset

    In het kader van de ADFICE_IT studie zijn zes Europese cohort studies onder ouderen geharmoniseerd. Het resultaat is een enkele grote dataset met data over: vallen; risicofactoren en voorspellers van vallen, waaronder medicatiegebruik; (genetische) biomarkers; chronische aandoeningen; emotioneel, cognitief en fysiek functioneren; leefstijlfactoren; en socio-demografische kenmerken. De geharmoniseerde cohorten zijn de Longitudinal Aging Study Amsterdam (LASA), de B-vitamins for the PRevention Of Osteoporotic Fractures (B-PROOF), de Activity and Function in the Elderly in Ulm study (ActiFE Ulm), de Irish Longitudinal Study on Ageing (TILDA), en het Erasmus Rotterdam Gezondheidsonderzoek (of Rotterdam Study). De ADFICE_IT dataset bevat cross-sectionele data voor alle zes cohorten en prospectieve data over vallen voor drie van de zes cohorten (LASA, B-Proof, en ActiFE Ulm).     Delphi study to derive expert consensus on a set of criteria to evaluate discharge readiness for adult ICU patients to be discharged to a general ward—European perspective

    Abstract Background/purpose Discharge decisions in Intensive Care Unit (ICU) patients are frequently taken under pressure to free up ICU beds. In the absence of established guidelines, the evaluation of discharge readiness commonly underlies subjective judgements. The challenge is to come to the right decision at the right time for the right patient. A premature care transition puts patients at risk of readmission to the ICU. Delayed discharge is a waste of resources and may result in over-treatment and suboptimal patient flow. More objective decision support is required to assess the individual patient’s discharge readiness but also the current care capabilities of the receiving unit. Methods In a modified online Delphi process, an international panel of 27 intensive care experts reached consensus on a set of 28 intensive care discharge criteria. An initial evidence-based proposal was developed further through the panelists’ edits, adding, comments and voting over a course of 5 rounds. Consensus was defined as achieved when ≥ 90% of the experts voted for a given option on the Likert scale or in a multiple-choice survey. Round 1 to 3 focused on inclusion and exclusion of the criteria based on the consensus threshold, where round 3 was a reiteration to establish stability. Round 4 and 5 focused on the exact phrasing, values, decision makers and evaluation time frames per criterion. Results Consensus was reached on a standard set of 28 ICU discharge criteria for adult ICU patients, that reflect the patient’s organ systems ((respiratory (7), cardiovascular (9), central nervous (1), and urogenital system (2)), pain (1), fluid loss and drainages (1), medication and nutrition (1), patient diagnosis, prognosis and preferences (2) and institution-specific criteria (4). All criteria have been specified in a binary decision metric (fit for ICU discharge vs. needs further intensive therapy/monitoring), with consented value calculation methods where applicable and a criterion importance rank with “mandatory to be met” flags and applicable exceptions. Conclusion For a timely identification of stable intensive care patients and safe and efficient care transitions, a standardized discharge readiness evaluation should be based on patient factors as well as organizational boundary conditions and involve multiple stakeholders

    Comparative transcriptomics of the garden dormouse hypothalamus during hibernation

    Torpor or heterothermy is an energy-saving mechanism used by endotherms to overcome harsh environmental conditions. During winter, the garden dormouse (Eliomys quercinus) hibernates with multiday torpor bouts and body temperatures of a few degrees Celsius, interrupted by brief euthermic phases. This study investigates gene expression within the hypothalamus, the key brain area controlling energy balance, adding information on differential gene expression potentially relevant to orchestrate torpor. A de novo assembled transcriptome of the hypothalamus was generated from garden dormice hibernating under constant darkness without food and water at 5 °C. Samples were collected during early torpor, late torpor, and interbout arousal. During early torpor, 765 genes were differentially expressed as compared with interbout arousal. Twenty-seven pathways were over-represented, including pathways related to hemostasis, extracellular matrix organization, and signaling of small molecules. Only 82 genes were found to be differentially expressed between early and late torpor, and no pathways were over-represented. During late torpor, 924 genes were differentially expressed relative to interbout arousal. Despite the high number of differentially expressed genes, only 10 pathways were over-represented. Of these, eight were also observed to be over-represented when comparing early torpor and interbout arousal. Our results are largely consistent with previous findings in other heterotherms. The addition of a transcriptome of a novel species may help to identify species-specific and overarching torpor mechanisms through future species comparisons

    A Neural Signature of Anorexia Nervosa in the Ventral Striatal Reward System

    Objective: Animal studies assessing mechanisms of self-starvation under conditions of stress and diet suggest a pivotal role for the mesolimbic reward system in the maintenance of core symptoms in anorexia nervosa, which is corroborated by initial empirical evidence in human studies. The authors examined activity in the ventral striatal system in response to disease-specific stimuli in women with acute anorexia nervosa. Method: Participants were 14 women with acute anorexia nervosa and 14 matched healthy comparison women who underwent functional magnetic resonance imaging (fMRI) during evaluation of visual stimuli depicting a female body with underweight, normal weight, and overweight canonical whole-body features according to standardized body mass indices. Participants were required to process each stimulus in a self-referring way. Ratings for each weight category were used as the control task. Results: Behaviorally, women with anorexia nervosa provided significantly higher positive ratings in response to underweight stimuli than in response to normal-weight stimuli, while healthy comparison women showed greater preference for normal-weight stimuli relative to underweight stimuli. Functionally, ventral striatal activity demonstrated a highly significant group-by-stimulus interaction for underweight and normal-weight stimuli. In women with anorexia nervosa, activation was higher during processing of underweight stimuli compared with normal-weight stimuli. The reverse pattern was observed in healthy comparison women. Conclusions: These findings are consistent with predictions in animal studies of the pivotal role of the human reward system in anorexia nervosa and thus support theories of starvation dependence in maintenance of the disorder

    Examining the factor structure and validity of the multidimensional assessment of interoceptive awareness

    The goal of this study was to psychometrically evaluate the Multidimensional Assessment of Interoceptive Awareness (MAIA), a popular self-report questionnaire claimed to assess the most important subjective aspects of interoception. We collected data in two samples (N = 644 and N = 1,516) and focused on the factor structure and validity of MAIA, as well as its associations with personality traits. Confirmatory Factor Analysis suggested that six of the eight subscales measure a common general factor of self-reported interoception; two MAIA subscales, Not-Worrying and Not-Distracting were only weakly related to this factor. Whereas the general factor correlated strongly with a measure of perceived attentiveness to normal nonemotive body processes, and moderately with Extraversion, Openness and Conscientiousness, the Not-Worrying factor showed moderate to strong negative correlations with Emotionality, pain catastrophization, and anxiety-related aspects of body focus. Not-Distracting was only weakly associated with the validating scales. Overall, these findings do not support the claimed eight-factor structure of the MAIA but indicate the existence of an overarching general factor. Additionally, this study provides evidence that interoceptive awareness, as measured by the MAIA, is related to, but distinct from personality.
