Fixing Fiji's civil service overweight problem: a challenge

    Get PDF
Fiji's ‘overweight’ civil service was the centre of attention of previous regimes, yet not much action was taken to resolve the problem. The attempt by the interim regime to address this problem by implementing a 30 per cent reduction of the civil workforce by 2011 is a bold decision, considering the economic and political uncertainties facing the country. The retirement age and redundancy policies are central measures of the reforms. While the earlier retirement age policy might have an immediate impact, redundancy is expected to be instrumental in the later stage of the process. The interplay between Fiji's institutional setting and the country's civil service culture presents a major test for the policy

Analgesic, anti-inflammatory and antipyretic activities of ethanol extract of Annona senegalensis leaves in experimental animal models

Background: This study was carried out to establish the analgesic, anti-inflammatory and antipyretic effects of the ethanol extract of Anonna senegalensis leaves in experimental animals. Methods: The analgesic activity was measured using the abdominal constriction and tail flick tests. The anti-inflammatory activity was performed using xylene and egg-albumen paw induced tests, while the antipyretic activity was measured using brewer’s yeast and 2, 4 dinitrophenol induced pyrexia tests, respectively. Results: The leaf extract at all doses used exhibited significant (p<0.05) analgesic, anti-inflammatory and antipyretic activities. Conclusions: Results show that ethanol leaf extract of Anonna senegalensis possess therapeutic potential against pains and feverish conditions, supporting the claims of its this plant as remedy for similar ailments

Abstract

Background

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is increasingly recognized as an inherited arterial disease leading to a step-wise decline and eventually to dementia. CADASIL is caused by mutations in <it>NOTCH3 </it>epidermal growth factor-like repeat that maps to chromosome 19. CADASIL cases have been identified in most countries of Western and Central Europe, the Americas, Japan, Australia, the Caribbean, South America, Tanzania, Turkey, South Africa and Southeast Asia, but not in Arabs.

Methods

We studied three families from Saudi Arabia (Family A), Kuwait (Family B) and Yemen (Family C) with 19 individuals affected by CADASIL.

Results

The mean age of onset was 31 ± 6 and the clinical presentation included stroke in 68%, subcortical dementia in 17% and asymptomatic leukoariosis detected by MRI in 15%. Migraine and depression were frequently associated, 38% and 68% respectively. The mean age of death was 56 ± 11. All <it>NOTCH3 </it>exons were screened for mutations, which revealed the presence of previously reported mutations c.406C>T (p.Arg110>Cys) in two families (family A&B) and c.475C>T (p.Arg133>Cys) mutation in family C.

Conclusion

CADASIL occurs in Arabs, with clinical phenotype and genotype similar to that in other ethnic groups.</p

Constitutive basal and stimulated human small bowel contractility is enhanced in obesity

Small bowel contractility may be more prominent in obese subjects, such that there is enhanced nutrient absorption and hunger stimulation. However, there is little evidence to support this. This study examined in vitro small bowel contractility in obese patients versus non-obese patients

The Role of Language Severity and Education in Explaining Performance on Object and Action Naming in Primary Progressive Aphasia

    Get PDF
Despite the common assumption that atrophy in a certain brain area would compromise the function that it subserves, this is not always the case, especially in complex clinical syndromes such as primary progressive aphasia (PPA). Clinical and demographic information may contribute to PPA phenotypes and explain the manifested impairments better than atrophy. In the present study, we asked how much variance of the object and action naming impairments observed in PPA may be attributed to atrophy in the language network alone vs. additional clinical and demographic factors including language severity and education. Thirty-nine participants with PPA underwent magnetic resonance imaging (MRI) for volumetric analysis and a complete neuropsychological examination, including standardized tests of object and action naming. We used stepwise regression models to compare atrophy (volumetric model) to clinical/demographic variables (clinical-demographic model) for naming objects and actions. The clinical-demographic model was the best-fit model that explained the largest amount of variance in both object and action naming. Brain volume measurements alone explained little variance in both object and action naming. Clinical factors, particularly language severity, and demographic factors, particularly education, need to be considered in conjunction with brain volumes in PPA. The present study emphasizes the complexity of PPA as a syndrome and provides an example of how volumetric, clinical and demographic factors may interact in determining naming performance/deterioration
