Plasma free amino acid profile in quiescent Inflammatory Bowel Disease patients orally administered with Mastiha (Pistacia lentiscus); a randomised clinical trial
Background: Natural products have been studied regarding their effectiveness on Inflammatory Bowel Disease (IBD). Hypothesis/Purpose: To examine the effects of Mastiha (Pistacia lentiscus var. Chia) on clinical course and amino acid (AA) profile of patients in remission. Study design: This is a randomised, double-blind, placebo-controlled clinical trial. Methods: Patients (n = 68) were randomly allocated to Mastiha (2.8 g/day) or placebo adjunct to stable medication. Free AAs were identified applying Gas Chromatography-Mass Spectrometry in plasma. Medical-dietary history, Inflammatory Bowel Disease Questionnaire, Harvey-Bradshaw Index, Partial Mayo Score, biochemical, faecal and blood inflammatory markers were assessed. Primary endpoint was the clinical relapse rate at 6 months. Secondary endpoints included variations in free AAs, inflammatory biomarkers and quality of life. Statistical significance was set at 0.05. Results: Concerning AAs and biochemical data, alanine (p = 0.006), valine (p = 0.047), proline (p = 0.022), glutamine (p < 0.001) and tyrosine (p = 0.043) along with total cholesterol (p = 0.032) and LDL cholesterol (p = 0.045) increased only in placebo group compared with baseline and the change between the study groups was significantly different. Inflammatory markers had not a significantly different change between the two groups, even serum IL-6, faecal calprotectin and faecal lactoferrin increased only in the placebo group. Although Mastiha was not proven superior to placebo in remission rate (17.6% vs. 23.5%, p = 0.549), attenuation in increase of free AAs levels in verum group is reported. Conclusion: Mastiha inhibited an increase in plasma free AAs seen in patients with quiescent IBD. Since change of AAs is considered an early prognostic marker of disease activity, this indicates a potential role of Mastiha in remission maintenance
Exercise tolerance and quality of life in patients with known or suspected coronary artery disease
Background: Coronary artery disease (CAD) is known to impact on patients’ physical and mental health. The relationship between performance on treadmill exercise tolerance test (ETT) and health-related quality of life (HRQL)has never been specifically investigated in the setting of CAD. Methods: Consecutive patients undergoing an ETT with the Bruce protocol during a diagnostic workup for CAD (n = 1,631, age 55 ± 12 years) were evaluated. Exercise-related indices were recorded. Detailed information on cardiovascular risk factors and past medical history were obtained. HRQLwas assessed with the use of the validated 36-Item Short Form Survey (SF-36) questionnaire. Results: Increasing age and the presence of cardiovascular risk factors and comorbidities correlated with lower scores on the physical and mental health component of SF-36(all P < 0.05). Subjects with arrhythmias during exercise and slow recovery of systolic blood pressure had lower scores on the physical health indices or the Social Role Functioning component (P < 0.05). Achieved target heart rate and good exercise tolerance were independently associated with better scores of the physical and mental health domains of SF-36 and overall HRQLscores (β = 0.05 for target HR and PCS-36, β = 1.86 and β = 1.66 per increasing stage of exercise tolerance and PCS-36 and MCS-36, respectively, P < 0.001 for all associations). Ischemic ECG changes were associated with worse scores on Physical Functioning (β = − 3.2, P = 0.02) and Bodily Pain (β = − 4.55, P = 0.026). Conclusion: ETT parameters are associated with HRQL indices in patients evaluated for possible CAD. Physical conditioning may increase patient well-being and could serve as a complementary target in conjunction with cardiovascular drug therapy
Light Propagation and Large-Scale Inhomogeneities
We consider the effect on the propagation of light of inhomogeneities with
sizes of order 10 Mpc or larger. The Universe is approximated through a
variation of the Swiss-cheese model. The spherical inhomogeneities are
void-like, with central underdensities surrounded by compensating overdense
shells. We study the propagation of light in this background, assuming that the
source and the observer occupy random positions, so that each beam travels
through several inhomogeneities at random angles. The distribution of
luminosity distances for sources with the same redshift is asymmetric, with a
peak at a value larger than the average one. The width of the distribution and
the location of the maximum increase with increasing redshift and length scale
of the inhomogeneities. We compute the induced dispersion and bias on
cosmological parameters derived from the supernova data. They are too small to
explain the perceived acceleration without dark energy, even when the length
scale of the inhomogeneities is comparable to the horizon distance. Moreover,
the dispersion and bias induced by gravitational lensing at the scales of

"Swiss-Cheese" Inhomogeneous Cosmology & the Dark Energy Problem
We study an exact swiss-cheese model of the Universe, where inhomogeneous LTB
patches are embedded in a flat FLRW background, in order to see how
observations of distant sources are affected. We find negligible integrated
effect, suppressed by (L/R_{H})^3 (where L is the size of one patch, and R_{H}
is the Hubble radius), both perturbatively and non-perturbatively. We
disentangle this effect from the Doppler term (which is much larger and has
been used recently \cite{BMN} to try to fit the SN curve without dark energy)
by making contact with cosmological perturbation theory.Comment: 35 pages, 6 figure
Local non-Gaussianity from rapidly varying sound speeds
We study the effect of non-trivial sound speeds on local-type non-Gaussianity
during multiple-field inflation. To this end, we consider a model of
multiple-field DBI and use the deltaN formalism to track the super-horizon
evolution of perturbations. By adopting a sum separable Hubble parameter we
derive analytic expressions for the relevant quantities in the two-field case,
valid beyond slow variation. We find that non-trivial sound speeds can, in
principle, curve the trajectory in such a way that significant local-type
non-Gaussianity is produced. Deviations from slow variation, such as rapidly
varying sound speeds, enhance this effect. To illustrate our results we
consider two-field inflation in the tip regions of two warped throats and find
large local-type non-Gaussianity produced towards the end of the inflationary

Moderate performance of serum S100A12, in distinguishing inflammatory bowel disease from irritable bowel syndrome
<p>Abstract</p> <p>Background</p> <p>S100A12, a calcium-binding proinflammatory protein secreted by granulocytes, has been associated with different diseases of inflammatory origin, including inflammatory bowel disease (IBD). In this study, the utility of serum S100A12, in discriminating IBD from irritable bowel syndrome (IBS), was tested.</p> <p>Methods</p> <p>S100A12 serum levels were determined in 64 patients with ulcerative colitis (UC), 64 with Crohn's disease (CD) and 73 with IBS, by means of an enzyme-linked immunosorbent assay. S100A12 serum levels were evaluated with respect to the levels of known inflammatory markers and patients' characteristics.</p> <p>Results</p> <p>The median values of serum S100A12 levels were 68.2 ng/mL (range: 43.4-147.4) in UC, 70 ng/mL (41.4-169.8) in CD and 43.4 ng/mL (34.4-74.4) in IBS patients. UC and CD patients had significantly higher serum S100A12 levels compared to IBS patients (<it>P </it>= 0.001 for both comparisons). Moreover, a cut-off for serum S100A12 levels of 54.4 ng/mL could predict both UC and CD with a 66.7% sensitivity and a 64.4% specificity. The area under curve was estimated at 0.67 with a 95% confidence interval of 0.60-0.75 (<it>P </it>< 0.001). Considering standard activity indices, higher serum S100A12 levels in active compared to inactive IBD were observed, although the recorded difference did not reach statistical significance. C-reactive protein (CRP) and serum amyloid A (SAA) levels, showed a statistically significant positive correlation with S100A12 (r = 0.39, <it>P </it>= 0.001 and r = 0.23, <it>P </it>= 0.02 respectively).</p> <p>Conclusions</p> <p>Increased levels of circulating S100A12 are found in IBD, compared to IBS. When used to distinguish IBD from IBS adult patients, serum S100A12 levels exhibit moderate performance. On the other hand, serum S100A12 may serve as an inflammatory marker in IBD, since it is well correlated with CRP and SAA.</p
Gauss-Bonnet Cosmology with Induced Gravity and Non-Minimally Coupled Scalar Field on the Brane
We construct a cosmological model with non-minimally coupled scalar field on
the brane, where Gauss-Bonnet and Induced Gravity effects are taken into
account. This model has 5D character at both high and low energy limits but
reduces to 4D gravity in intermediate scales. While induced gravity is a
manifestation of the IR limit of the model, Gauss-Bonnet term and non-minimal
coupling of scalar field and induced gravity are essentially related to UV
limit of the scenario. We study cosmological implications of this scenario
focusing on the late-time behavior of the solutions. In this setup, non-minimal
coupling plays the role of an additional fine-tuning parameter that controls
the initial density of predicted finite density big bang. Also, non-minimal

Emotional, behavioural problems and cigarette smoking in adolescence: findings of a Greek cross-sectional study
<p>Abstract</p> <p>Background</p> <p>Although several studies have reported findings concerning the association between smoking and emotional/behavioural problems, little research has investigated this association after controlling for confounding factors which have been found to be significantly correlated with both cigarette smoking and emotional/behavioural problems and may have a strong effect on the relationship between adolescents' mental health and smoking. The present study attempted to assess the association between adolescents' smoking status and their emotional/behavioural problems after controlling for a number of possible confounders (i.e. age, gender, parental smoking status, exposure to family smoking, family socioeconomic status, adolescents' leisure time) in a Greek nation-wide school-based sample.</p> <p>Methods</p> <p>Participants completed a questionnaire which retrieved information about age, gender, family socioeconomic status, smoking status, parental smoking, adolescents' leisure time and emotional/behavioural problems. Data were modelled using multiple logistic regression analysis with adolescents' smoking status as the dependent variable.</p> <p>Results</p> <p>A total of 1194 (i.e. 63% response rate) of self-reported questionnaires (40.1% boys, 59.9% girls; 12-18 years old) were returned. Data from 1030 participants with full data were analyzed. Cigarette smoking was strongly associated with higher levels of emotional/behavioural problems (p < 0.001) and the association was not moderated (OR = 1.13, 95% CI: 1.08-1.18) after controlling for the effects of other covariates. Emotional symptoms, conduct problems and hyperactivity/inattention were all significantly associated with adolescents' current smoking.</p> <p>Conclusions</p> <p>This study supports the association between smoking and emotional/behavioural problems among adolescents. Addressing adolescents' needs regarding their emotional/behavioural health could be helpful in the development of effective anti-smoking strategies in school environment and elsewhere.</p
The effect of inhomogeneous expansion on the supernova observations
We consider an inhomogeneous but spherically symmetric Lemaitre-Tolman-Bondi
model to demonstrate that spatial variations of the expansion rate can have a
significant effect on the cosmological supernova observations. A model with no
dark energy but a local Hubble parameter about 15% larger than its global value
fits the supernova data better than the homogeneous model with the cosmological
constant. The goodness of the fit is not sensitive to inhomogeneities in the
present-day matter density, and our best fit model has Omega_M ~ 0.3, in
agreement with galaxy surveys. We also compute the averaged expansion rate,
defined by the Buchert equations, of the best fit model and show explicitly
