    EPIDEMIOLOGIA DE LA OBESIDAD EN EL PARAGUAY

    Introducción: Paraguay no escapa a la epidemia global de obesidad. Este estudio describió la prevalencia y los determinantes sociodemográficos, clínicos y conductuales asociados en el país. Métodos: Estudio transversal tipo encuesta poblacional con representatividad nacional, incluyó personas entre edades de 15 y 74 años. El muestreo fue probabilístico, trietápico sin reemplazo. STEPSwise fue la metodología y encuesta aplicada durante junio-setiembre 2011. Se consideró obesidad un índice de masa (IMC) corporal >=30. Este valor se distribuyó según las variables sociodemográficas, clínicas y conductuales. Por regresión logística se estimó asociación entre las variables, en odd ratios (OR) con intervalos de confianza del 95%(IC95%). Resultados: Se incluyeron 2501 participantes. Fueron obesos 23,5% de la población, 20,2% y 26,0% hombres y mujeres, respectivamente. Además del sexo, mostraron diferencias significativas: tener >35 años 3,17(2,11-4,76) que los menores; hombres con residencia urbana 1,94(1,35-2,79) veces más que los del área rural. Los hombres en pareja 2,52(1,80-3,53) veces más obesos que los solteros; funcionario público 2,57(1,57-4,26) veces más que otros trabajos. Baja actividad física presentó obesidad 1,75(1,19-2,57) veces más que tener una actividad física mínimamente aceptable. En hombres el quintil de ingreso superior presentó 3,87(2,17-6,92) más obesidad que el quintil inferior. En mujeres el bajo nivel educativo es 2,01(1,43-2,83) veces más que un mayor nivel. Conclusiones: El patrón observado de la distribución de la obesidad describe entornos y conductas más obesogénicos que otros en Paraguay. Estos resultados sirven para tomar decisiones e intervenciones específicas en salud pública, más allá de las medidas poblacionales

    Effects of epibiosis on consumer-prey interactions

    In many benthic communities predators play a crucial role in the population dynamics of their prey. Surface characteristics of the prey are important for recognition and handling by the predator. Because the establishment of an epibiotic assemblage on the surface of a basibiont species creates a new interface between the epibiotized organism and its environment, we hypothesised that epibiosis should have an impact on consumer-prey interactions. In separate investigations, we assessed how epibionts on macroalgae affected the susceptibility of the latter to herbivory by the urchin Arbacia punctulata and how epibionts on the blue mussel Mytilus edulis affected its susceptibility to predation by the shore crab Carcinus maenas. Some epibionts strongly affected consumer feeding behavior. When epibionts were more attractive than their host, consumer pressure increased. When epibionts were less attractive than their host or when they were repellent, consumer pressure decreased. In systems that are controlled from the top-down, epibiosis can strongly influence community dynamics. For the Carcinus/Mytilus system that we studied, the insitu distribution of epibionts on mussels reflected the epibiosis-determined preferences of the predator. Both direct and indirect effects are involved in determining these epibiont-prey-consumer interactions

    Eliminación de la hepatitis C. Documento de posicionamiento de la Asociación Española para el Estudio del Hígado (AEEH)

    La Asociación Española para el Estudio del Hígado (AEEH) está convencida de que la eliminación de la hepatitis C en España es posible siempre y cuando seamos capaces de emplear los recursos y las herramientas necesarias para la misma. Este documento refleja la posición de la AEEH respecto a la eliminación del virus de la hepatitis C (VHC), estableciendo una amplia serie de recomendaciones que se pueden agrupar en cinco categorías: 1) cribado del VHC en función de la edad, de la existencia de factores de riesgo clásicos de adquisición de la infección, búsqueda activa de pacientes diagnosticados con anterioridad y desarrollo de estrategias de microeliminación en poblaciones vulnerables; 2) simplificación del diagnóstico del VHC (diagnóstico en un solo paso y diagnóstico en el punto de atención del paciente); 3) simplificación del tratamiento de los pacientes y mejora de los circuitos asistenciales; 4) medidas de política sanitaria, y, finalmente, 5) establecimiento de indicadores de eliminación del VHC. The Spanish Association for the Study of the Liver (AEEH) is convinced that the elimination of hepatitis C virus (HCV) in Spain is possible as long as we are able to use the resources and tools necessary for it. This document reflects the position of the AEEH regarding the elimination of HCV, establishing a wide range of recommendations that can be grouped into five categories: 1) Screening of HCV according to age, of the existence of classic acquisition risk factors of infection, active search of previously diagnosed patients and development of microelimination strategies in vulnerable populations; 2) Simplification of HCV diagnosis (one-step diagnosis and diagnosis at the point of patient care); 3) Simplification of patient treatment and improvement of care circuits; 4) Health policy measures, and, finally, 5) Establishment of HCV elimination indicators

    Deep-sequencing reveals broad subtype-specific HCV resistance mutations associated with treatment failure

    A percentage of hepatitis C virus (HCV)-infected patients fail direct acting antiviral (DAA)-based treatment regimens, often because of drug resistance-associated substitutions (RAS). The aim of this study was to characterize the resistance profile of a large cohort of patients failing DAA-based treatments, and investigate the relationship between HCV subtype and failure, as an aid to optimizing management of these patients. A new, standardized HCV-RAS testing protocol based on deep sequencing was designed and applied to 220 previously subtyped samples from patients failing DAA treatment, collected in 39 Spanish hospitals. The majority had received DAA-based interferon (IFN) a-free regimens; 79% had failed sofosbuvir-containing therapy. Genomic regions encoding the nonstructural protein (NS) 3, NS5A, and NS5B (DAA target regions) were analyzed using subtype-specific primers. Viral subtype distribution was as follows: genotype (G) 1, 62.7%; G3a, 21.4%; G4d, 12.3%; G2, 1.8%; and mixed infections 1.8%. Overall, 88.6% of patients carried at least 1 RAS, and 19% carried RAS at frequencies below 20% in the mutant spectrum. There were no differences in RAS selection between treatments with and without ribavirin. Regardless of the treatment received, each HCV subtype showed specific types of RAS. Of note, no RAS were detected in the target proteins of 18.6% of patients failing treatment, and 30.4% of patients had RAS in proteins that were not targets of the inhibitors they received. HCV patients failing DAA therapy showed a high diversity of RAS. Ribavirin use did not influence the type or number of RAS at failure. The subtype-specific pattern of RAS emergence underscores the importance of accurate HCV subtyping. The frequency of “extra-target” RAS suggests the need for RAS screening in all three DAA target regions

    Preparation and use of maize tassels’ activated carbon for the adsorption of phenolic compounds in environmental waste water samples

    The determination and remediation of three phenolic compounds bisphenol A (BPA), ortho-nitrophenol (o-NTP), parachlorophenol (PCP) in wastewater is reported. The analysis of these molecules in wastewater was done using gas chromatography (GC) × GC time-of-flight mass spectrometry while activated carbon derived from maize tassel was used as an adsorbent. During the experimental procedures, the effect of various parameters such as initial concentration, pH of sample solution, eluent volume, and sample volume on the removal efficiency with respect to the three phenolic compounds was studied. The results showed that maize tassel produced activated carbon (MTAC) cartridge packed solid-phase extraction (SPE) system was able to remove the phenolic compounds effectively (90.84–98.49 %, 80.75–97.11 %, and 78.27–97.08 % for BPA, o-NTP, and PCP, respectively) . The MTAC cartridge packed SPE sorbent performance was compared to commercially produced C18 SPE cartridges and found to be comparable. All the parameters investigated were found to have a notable influence on the adsorption efficiency of the phenolic compounds from wastewaters at different magnitudes
