    Management of Isolated Gastric Varices by Devascularization and Proximal Gastrectomy in Cirrhotic Patients

    Twenty patients with liver cirrhosis were treated by surgery for bleeding from isolated gastric varices. The presence of tortuous and engorged gastric veins connecting with a large splenorenal shunt was demonstrated by transhepatic portography in all patients. The surgical procedures consisted of splenectomy, proximal gastrectomy, paragastric devascularization, and ligation of the splenorenal shunt. Sixteen patients survived the surgery. Four deaths were caused by emergency operation for uncontrollable hemorrhage in extremely poor risk patients. Of the 16 survivors, 15 had been followed wth endoscopy and portography for a mean period of 42 months. The other one died of hepatocellular carcinoma three years after surgery. There was no bleeding episode during the period of follow-up in these patients. Recurrent esophageal varices of mild degree were documented by endoscopy and portography in three patients. Portography demonstrated that several newly formed retroperitoneal veins arising from the junction of the portal and superior mesenteric veins joined to form recurrent varices in these three patients. There was no significant change of the mean portal venous pressure before and after surgery. Our data reveals that elective surgery may provide satisfactory results in patients with isolated gastric varices. Transhepatic portography is the method of choice in radiologic investigation for prominent gastric varices

    Chaos Synchronization Based Novel Real-Time Intelligent Fault Diagnosis for Photovoltaic Systems

    The traditional solar photovoltaic fault diagnosis system needs two to three sets of sensing elements to capture fault signals as fault features and many fault diagnosis methods cannot be applied with real time. The fault diagnosis method proposed in this study needs only one set of sensing elements to intercept the fault features of the system, which can be real-time-diagnosed by creating the fault data of only one set of sensors. The aforesaid two points reduce the cost and fault diagnosis time. It can improve the construction of the huge database. This study used Matlab to simulate the faults in the solar photovoltaic system. The maximum power point tracker (MPPT) is used to keep a stable power supply to the system when the system has faults. The characteristic signal of system fault voltage is captured and recorded, and the dynamic error of the fault voltage signal is extracted by chaos synchronization. Then, the extension engineering is used to implement the fault diagnosis. Finally, the overall fault diagnosis system only needs to capture the voltage signal of the solar photovoltaic system, and the fault type can be diagnosed instantly

    A pre-S gene chip to detect pre-S deletions in hepatitis B virus large surface antigen as a predictive marker for hepatoma risk in chronic hepatitis B virus carriers

    <p>Abstract</p> <p>Background</p> <p>Chronic hepatitis B virus (HBV) infection is an important cause of hepatocellular carcinoma (HCC) worldwide. The pre-S<sub>1 </sub>and -S<sub>2 </sub>mutant large HBV surface antigen (LHBS), in which the pre-S<sub>1 </sub>and -S<sub>2 </sub>regions of the LHBS gene are partially deleted, are highly associated with HBV-related HCC.</p> <p>Methods</p> <p>The pre-S region of the LHBS gene in two hundred and one HBV-positive serum samples was PCR-amplified and sequenced. A pre-S oligonucleotide gene chip was developed to efficiently detect pre-S deletions in chronic HBV carriers. Twenty serum samples from chronic HBV carriers were analyzed using the chip.</p> <p>Results</p> <p>The pre-S deletion rates were relatively low (7%) in the sera of patients with acute HBV infection. They gradually increased in periods of persistent HBV infection: pre-S mutation rates were 37% in chronic HBV carriers, and as high as 60% in HCC patients. The Pre-S Gene Chip offers a highly sensitive and specific method for pre-S deletion detection and is less expensive and more efficient (turnaround time 3 days) than DNA sequencing analysis.</p> <p>Conclusion</p> <p>The pre-S<sub>1/2 </sub>mutants may emerge during the long-term persistence of the HBV genome in carriers and facilitate HCC development. Combined detection of pre-S mutations, other markers of HBV replication, and viral titers, offers a reliable predictive method for HCC risks in chronic HBV carriers.</p

    Direct Radiofrequency Application Improves Pain and Gait in Collagenase-Induced Acute Achilles Tendon Injury

    Radiofrequency (RF) is often used as a supplementary and alternative method to alleviate pain for chronic tendinopathy. Whether or how it would work for acute tendon injury is not addressed in the literatures. Through detailed pain and gait monitoring, we hypothesized that collagenase-induce acute tendinopathy model may be able to answer these questions. Gait parameters, including time, distance, and range of motion, were recorded and analyzed using a walking track equipped with a video-based system. Expression of substance P (SP), calcitonin gene related peptide (CGRP), and galanin were used as pain markers. Beta-III tubulin and Masson trichrome staining were used as to evaluate nerve sprouting, matrix tension, and degeneration in the tendon. Of fourteen analyzed parameters, RF significantly improved stance phase, step length, preswing, and intermediary toe-spread of gait. Improved gait related to the expression of substance P, CGRP, and reduced nerve fiber sprouting and matrix tension, but not galanin. The study indicates that direct RF application may be a valuable approach to improve gait and pain in acute tendon injury. Altered gait parameters may be used as references to evaluate therapeutic outcomes of RF or other treatment plan for tendinopathy

    MRE11 promotes oral cancer progression through RUNX2/CXCR4/AKT/FOXA2 signaling in a nuclease-independent manner

    MRE11, the nuclease component of RAD50/MRE11/NBS1 DNA repair complex which is essential for repair of DNA double-strand-breaks in normal cells, has recently garnered attention as a critical factor in solid tumor development. Herein we report the crucial role of MRE11 in oral cancer progression in a nuclease-independent manner and delineate its key downstream effectors including CXCR4. MRE11 expression in oral cancer samples was positively associated with tumor size, cancer stage and lymph node metastasis, and was predictive of poorer patient survival and radiotherapy resistance. MRE11 promoted cell proliferation/migration/invasion in a nuclease-independent manner but enhanced radioresistance via a nuclease-dependent pathway. The nuclease independent promotion of EMT and metastasis was mediated by RUNX2, CXCR4, AKT, and FOXA2, while CXCR4 neutralizing antibody mitigated these effects in vitro and in vivo. Collectively, MRE11 may serve as a crucial prognostic factor and therapeutic target in oral cancer, displaying dual nuclease dependent and independent roles that permit separate targeting of tumor vulnerabilities in oral cancer treatment

    Women with endometriosis have higher comorbidities: Analysis of domestic data in Taiwan

    AbstractEndometriosis, defined by the presence of viable extrauterine endometrial glands and stroma, can grow or bleed cyclically, and possesses characteristics including a destructive, invasive, and metastatic nature. Since endometriosis may result in pelvic inflammation, adhesion, chronic pain, and infertility, and can progress to biologically malignant tumors, it is a long-term major health issue in women of reproductive age. In this review, we analyze the Taiwan domestic research addressing associations between endometriosis and other diseases. Concerning malignant tumors, we identified four studies on the links between endometriosis and ovarian cancer, one on breast cancer, two on endometrial cancer, one on colorectal cancer, and one on other malignancies, as well as one on associations between endometriosis and irritable bowel syndrome, one on links with migraine headache, three on links with pelvic inflammatory diseases, four on links with infertility, four on links with obesity, four on links with chronic liver disease, four on links with rheumatoid arthritis, four on links with chronic renal disease, five on links with diabetes mellitus, and five on links with cardiovascular diseases (hypertension, hyperlipidemia, etc.). The data available to date support that women with endometriosis might be at risk of some chronic illnesses and certain malignancies, although we consider the evidence for some comorbidities to be of low quality, for example, the association between colon cancer and adenomyosis/endometriosis. We still believe that the risk of comorbidity might be higher in women with endometriosis than that we supposed before. More research is needed to determine whether women with endometriosis are really at risk of these comorbidities

    Study of Intelligent Photovoltaic System Fault Diagnostic Scheme Based on Chaotic Signal Synchronization

    As the photovoltaic system consists of many equipment components, manual inspection will be very costly. This study proposes the photovoltaic system fault diagnosis based on chaotic signal synchronization. First, MATLAB was used to simulate the fault conditions of solar system, and the maximum power point tracking (MPPT) was used to ensure the system's stable power and capture and record the system fault feature signals. The dynamic errors of various fault signals were extracted by chaotic signal synchronization, and the dynamic error data of various fault signals were recorded completely. In the photovoltaic system, the captured output voltage signal was used as the characteristic values for fault recognition, and the extension theory was used to create the joint domain and classical domain of various fault conditions according to the collected feature data. The matter-element model of extension engineering was constructed. Finally, the whole fault diagnosis system is only needed to capture the voltage signal of the solar photovoltaic system, so as to know the exact fault condition effectively and rapidly. The proposed fault diagnostor can be implemented by embedded system and be combined with ZigBee wireless network module in the future, thus reducing labor cost and building a complete portable renewable energy system fault diagnostor
