    A Comparison of Pancreatectomy and Pancreatic Duct Drainage in Chronic Pancreatitis

    Surgical therapy was performed in 25 cases of chronic pancreatitis at the Second Department of Surgery, Hiroshima University School of Medicine from January 1973 to October 1984. Thirteen cases were considered related to the excessive intake of alcoholic drinks, 2 cases each to acute pancreatitis and gall stones, and 8 cases to unknown etiology. Complication of pancreatic stone and marked dilatation or partial constriction of the pancreatic duct were observed in 11 of the 25 cases. Pancreaticoduodenectomy was performed in 7 cases, distal pancreatectomy in 8 cases, longitudinal pancreaticojejunostomy in 6 cases, and pancreaticoplasty in 2 cases, and biliary surgery in 2 cases. Out of 21 cases with abdominal pains, pain disappeared in 12 cases, improved in 6 cases and remained uncharged in 3 cases, the effect from surgery being almost satisfactory. No difference was observed in the effect between the surgical procedure as described above. Endocrine function tests with 50 g OGTT revealed improvement in 2 cases and aggravation in another two cases. The function remained unchanged before and after operation in the remaining 21 cases. Pancreatic exocrine function tests with PFD revealed almost no changes before and after operation and no difference between the surgical procedures. Pancreatic exocrine function was found correlated with the advanced conditions of pancreatic fibrosis rather then with the surgical procedures. Cases with less advanced fibrosis maintained the function in a more satisfactory condition both before and after operation. There occurred no cases of death directory related to operation. Four cases of death, no relating to operation itself, were observed in the pancreatectomy group. Twenty cases are now under rehabilitated conditions. Results of our surgical treatment for chronic pancreatitis are almost satisfactory in respect to pain-relieving effect but unsatisfactory in respect to improvement of the endocrine and exocrine function. It may be necessary to consider surgical operation at an early stage before the aggravation of fibrosis, because various types of drainage procedures that aim at preserving the pancreatic tissue and reducing the pancreatic duct pressure are logically capable of improving pancreatic functions

    We examined the difference of the effects of different format video media to the human brain function by means of brain electric activity measuring method we have developed. As the visual materials for evaluation, three materials recorded by high definition (HD) video format, which has the highest visual quality now in practical use, were chosen and edited to the test pieces. Subsequently, they were down-converted to NTSC video format. On the presentation of visual images, we set three sound condition ; the music rich in natural high frequency components above the audible range (full-range sound), the same sound but from which high frequency components above 26kHz were eliminated (high-cut sound), and no sound. As a resullt, when the HD video were presented, the average of α-EEG (electroence-phalogram) potentials were significantly higher than NTSC video (p<0.05, post hoc test with Fisher\u27s PLSD). When we used the video of natural materials, the difference of a-EEG potential between HD presentation and NTSC presentation showed statistically significance. Although no significant difference in α-EEG was observed by using the more artificial visual material. When the video were presented with full-range sound, the average of α-EEG potential were significantly higher than the presentation with high-cut sound as well as no sound. These results showed that the quality of electric visual information could be evaluated by the α-EEG potential, which is the physiological and quantitative index of brain activity. It was also suggested that the improvement of the audio-visual format would be effective for the higher education

Plasma of an acute pancreatitis patient followed by disseminated intravascular coagulation possessed the same amount of urokinase activity as normal plasma, and was confirmed to be composed of two types of urokinase, a high molecular weight form and a low molecular weight form with a molecular weight of 33,000

    It was confirmed that the thiol protease inhibitory activities in human lung cancer extracts were significantly higher than normal lung tissue extracts. Furthermore the thiol protease inhibitor was extracted from human lung cancer tissue and purified by papain-Sepharose and Sephadex G-100 column chromatographies.     A case of Cronkhite-Canada Syndrome (CCS), a 58-year old male, is reported. The case, who had multiple polypoid lesions in the stomach and colon, was hospitalized in a cachexic condition due to malabsorption with the chief complaint of watery diarrhea. Distal gastrectomy and total colectomy were performed due to marked watery diarrhea and the diagnosis of gastric cancer upon biopsy of the gastric polyp. After hospitalization, the case presented the typical ectodermal syndrome of hair loss, skin pigmentation and nail atrophy, but at present, ten months after his operation, his symptoms have improved and his general condition is stable and he lives a normal social life
