    Giant Angiolymphoid Hyperplasia with Eosinophilia on the Chest

    Angiolymphoid hyperplasia with eosinophilia is a rare vascular proliferation characterized by single or multiple purplish, brownish papules and subcutaneous nodules, sometimes associated with pain or pruritus. This rare benign process occurs with a female predominance. Approximately 85% of the lesions occur in the skin of the head and neck; most of them are around the ear or on the forehead or scalp. Whether angiolymphoid hyperplasia with eosinophilia represents a benign neoplasm or an unusual reaction to varied stimuli, including trauma, the etiology remains unclear. Histopathologically, the lesions consist of a proliferation of blood vessels of variable size lined by large epithelioid endothelial cells and a variable inflammatory infiltrate of lymphocytes and eosinophils, sometimes with lymphoid follicle formation. The lesion is benign but may be persistent and is difficult to eradicate. We report on a case of a 58-year-old Caucasian man who presented a purplish pink dome-shaped tumor of size up to 8 cm in diameter located on the chest.     Unmixing of Magnetic Hysteresis Loops Through a Modified Gamma‐Cauchy Exponential Model

    Abstract Quantifying the contributions of distinct mineral populations in bulk magnetic experiments greatly enhances the analysis of environmental and rock magnetism studies. Here, we develop a new method of parametric unmixing of susceptibility components in hysteresis loops. Our approach is based on a modified Gamma‐Cauchy exponential model that accounts for variable skewness and kurtosis. The robustness of the model is tested with synthetic curves that examine the effects of noise, sampling, and proximity (similar coercivities) of susceptibility components. We provide a Python‐based script, the Hist‐unmix, which allows the user to adjust a forward model of up to three ferromagnetic components as well as a dia/paramagnetic contribution. Optimization of all the parameters is achieved through least squares fitting (Levenberg‐Marquardt method), with uncertainties of each inverted parameter calculated through a Monte Carlo error propagation approach. For each ferromagnetic component, it is possible to estimate the saturation magnetization (Ms), saturation remanent magnetization (Mrs) and the mean coercivity (Bc). Finally, Hist‐unmix was applied to a set of weakly magnetic carbonate rocks from Brazil, which typically show distorted hysteresis loops (wasp‐waisted and potbellied). For these samples, we resolved two components with distinct coercivities. These results are corroborated by previous experimental data, showing that the lower branch of magnetic hysteresis can be modeled by the presented approach and might offer important mineralogical information for rock magnetic and paleomagnetic studies

    Methaemoglobinemia Induced by Poppers and Bupropion Intoxication in the Emergency Department.

    A 40-year-old man presented to the emergency department with dyspnoea and fatigue after bupropion and popper consumption. Clinical examination was remarkable for central cyanosis not responding to supplementary oxygen. Arterial blood gas analysis showed a methaemoglobin value of 30.3%. Methaemoglobinemia was diagnosed and the patient was treated with methylene blue. However, during methylene blue administration, the patient developed a generalized tonic-clonic seizure that was successfully managed with diazepam. Combined intoxications can be a critical problem in the emergency department. Early recognition and treatment of poisoning are key for good patient outcome. LEARNING POINTS: Distinguishing toxidromes is critical for adequate treatment of patients with drug intoxication; the most common side effect of bupropion consumption is dose-dependent seizures.The diagnosis of methaemoglobinemia requires a high index of suspicion, particularly in a patient presenting with central cyanosis not responding to supplementary oxygen.Treatment with methylene blue is recommended when the percentage of methaemoglobin is above 30% or when the patient has symptoms related to methaemoglobinemia.info:eu-repo/semantics/publishedVersio
