    Household fish preparation hygiene and cholera transmission in Monrovia, Liberia.

    BACKGROUND: In the 1980s Vibrio cholerae was found to be an autochthonous resident of aquatic environments. As result, ingestion of undercooked, contaminated fish has been associated with cholera transmission. An alternative mechanism of transmission associated with fish was hypothesised by Schürmann et al. in 2002. He described a cholera case that was more likely to have been infected by contamination on the patient's hands rather than by ingestion of contaminated fish. METHODOLOGY: With fish being the main diet in Liberia, we decided to examine fish samples and preparation techniques in Monrovia. Excreta of 15 fish, caught in the estuarine waters of Monrovia, were analysed for V. cholerae. In addition, fish preparation methods were observed in 30 households. RESULTS: Two fish samples were found positive. Observations revealed that hygiene measures during the gutting process of fish were limited; although hands were usually rinsed, in all cases soap was not used. Furthermore, contaminated water was frequently reused during food preparation. CONCLUSIONS: Since the cooking process of fish (and thus elimination of bacteria) in Monrovia usually consists of both frying and boiling, it seems plausible that in this context, the hypothesis by Schürmann et al. could be applicable. Further research is necessary to confirm this association, which could be a starting point for more context-specific health education campaigns addressing food preparation hygiene as risk factor for cholera

    Emotional stability, conscientiousness, and self-reported hypertension in adulthood

    This study aimed to investigate social and psychological factors in childhood and adulthood associated with self-reported hypertension in adulthood. Using data from the National Child Development Study, a nationally representative sample of 17,415 babies born in Great Britain in 1958 and followed up at 11, 33, and 50 years of age. Self-reported diagnosed hypertension by 50 years was the outcome measure. In total, 5753 participants with complete data on parental social class at birth, childhood cognitive ability test scores at 11 years, educational qualifications at 33 years, personality traits, occupational levels, and self-reported hypertension (all measured at age 50 years) were included in the study. Using logistic regression analyses, results showed that sex (OR = 0.60: 0.49–0.73, p < .001), educational qualifications (OR = 0.59: 0.37–0.92, p < .05), and traits emotional stability (OR = 0.84: 0.77–0.91, p < .001) and conscientiousness (OR = 0.89: 0.82–0.98, p < .05) were all significantly associated with the occurrence of self-reported hypertension in adulthood. Both psychological factors and socio-demographic factors were significantly associated with self-reported hypertension in adulthood

    Preschool hyperactivity specifically elevates long-term mental health risks more strongly in males than females: a prospective longitudinal study through to young adulthood

    Evidence of continuities between preschool hyperactivity and adult mental health problems highlight the potential value of targeting early identification and intervention strategies. However, specific risk factors are currently unclear. This large-scale prospective longitudinal study aimed to identify which hyperactive preschoolers are at greatest long-term risk of poor mental health. One hundred and seventy children (89 females) rated as hyperactive by their parents and 88 non-hyperactive controls (48 females) were identified from a community sample of 4,215 3 year-olds. Baseline data relating to behavioral/emotional problems and background characteristics were collected. Follow-up mental health and functional impairment outcomes were collected between 14 and 25 years of age. At age 3 years, males and females in the hyperactive group had similarly raised levels of hyperactivity and other behavior problems. In adolescence/young adulthood, these individuals showed elevated symptoms of ADHD, conduct disorder, mood disorder, anxiety and autism, as well as functional impairment. Preschool hyperactivity was strongly predictive of poor adolescent/adult outcomes for males across domains with effects being specifically driven by hyperactivity. For females, the effects of preschool hyperactivity were smaller and dropped to non-significant levels when other preschool problems were taken into account. Environmental risk factors also differed between the sexes, although these may also have been mediated by genetic risk. In conclusion, these results demonstrate marked sex differences in preschool predictors of later adolescent/adult mental health problems. Future research should include a measure of preschool inattention as well hyperactivity. The findings highlight the potential value of tailored approaches to early identification strategies

    Water quality for young children in Cambodia : high contamination at collection and consumption level

    Unsafe drinking water is a leading cause of child morbidity, especially among young children in low-income settings. Safe water consumption requires high-quality water available at its source and at point of use. We examined the quality of drinking water at point of collection and point of use in 796 households in three provinces, in Cambodia. Microbiological testing for coliform andEscherichia colicontamination was conducted for samples collected. Bivariable analysis and multivariable logistic regression were used to examine associations between various factors and the deterioration in water quality (increase in the risk according coliform orE. coliconcentration) between point of collection and point of use. Contamination with both coliforms andE. coliwas higher at point of use than at point of collection, with contamination at point of collection to account for 76.6% (coliforms) and 46.3% (E. coli). Results suggest that child drinking water represents a considerable pathway for the ingestion of pathogens, in Cambodia. Area of residence, seasonality, type of water source, and water chlorination were associated with coliform concentration between the point of collection and point of use, whereas only seasonality was associated withE. colicontamination (OR = 1.46; 95% CI [1.05, 2.02]). Children living in rural settings were two times more likely to drink water with a deteriorating coliform concentration between the two-time points than children living in urban settings (OR = 2.00; 95% CI [1.22, 3.30]). The increase in coliform andE. coliconcentrations between point of collection and point of use indicates that water contamination mostly occurs within the household. Strengthening national legislation on water quality standards and promoting safe water management at the household are needed

    Hunt-Vitell’s General Theory of Marketing Ethics Predicts ‘Attitude-Behaviour’ Gap in Pro-Environmental Domain

    The inconsistency between pro-environmental attitudes and behaviour, known as the ‘attitude-behaviour’ gap, is not uncommon to ethical decision-making, however it’s exceptionally pronounced in scenarios associated with ‘green’ choice. Despite existing research offering numerous attempts to investigate the causes of the ‘attitude-behaviour’ gap in the pro-environmental domain, it is surprising that the major factors driving the ‘attitude-behaviour’ gap are still unknown. Therefore, we have grounded this study in Hunt-Vitell’s moral philosophy-based framework of ethical decision-making, which assumes morality as the central force impacting one’s behaviour and tested its effectiveness in predicting pro-environmental intentions vs. behaviours. The results from an online study of 612 MTurk participants from the US revealed that participants’ decision-making indeed depended on deontological and teleological framing of pro-environmental scenarios, and this in turn predicted the declining relationship between intention vs. behaviour. These findings suggest that morality is central to pro-environmental decision-making, and the ‘attitude-behaviour’ gap is the result of the disintegrated effects of moral dimension. For this reason, strengthening the impact of morality could be sufficient for aligning intentions with behaviours and thus closing the ‘attitude-behaviour’ gap

    A VicRK Signal Transduction System in Streptococcus mutans Affects gtfBCD, gbpB, and ftf Expression, Biofilm Formation, and Genetic Competence Development

    Bacteria exposed to transient host environments can elicit adaptive responses by triggering the differential expression of genes via two-component signal transduction systems. This study describes the vicRK signal transduction system in Streptococcus mutans. A vicK (putative histidine kinase) deletion mutant (SmuvicK) was isolated. However, a vicR (putative response regulator) null mutation was apparently lethal, since the only transformants isolated after attempted mutagenesis overexpressed all three genes in the vicRKX operon (Smuvic(+)). Compared with the wild-type UA159 strain, both mutants formed aberrant biofilms. Moreover, the vicK mutant biofilm formed in sucrose-supplemented medium was easily detachable relative to that of the parent. The rate of total dextran formation by this mutant was remarkably reduced compared to the wild type, whereas it was increased in Smuvic(+). Based on real-time PCR, Smuvic(+) showed increased gtfBCD, gbpB, and ftf expression, while a recombinant VicR fusion protein was shown to bind the promoter regions of the gtfB, gtfC, and ftf genes. Also, transformation efficiency in the presence or absence of the S. mutans competence-stimulating peptide was altered for the vic mutants. In vivo studies conducted using SmuvicK in a specific-pathogen-free rat model resulted in significantly increased smooth-surface dental plaque (Pearson-Filon statistic [PF], <0.001). While the absence of vicK did not alter the incidence of caries, a significant reduction in SmuvicK CFU counts was observed in plaque samples relative to that of the parent (PF, <0.001). Taken together, these findings support involvement of the vicRK signal transduction system in regulating several important physiological processes in S. mutans
