Predictores de deterioro neurologico precoz en pacientes con enfermedad vascular cerebral isquemica
Determinar prevalencia y construir modelo de predicci贸n de deterioro
neurol贸gico precoz en pacientes con Enfermedad cerebrovascular (ECV)
isqu茅mico.
Material y m茅todo: Estudio observacional, anal铆tico, transversal, se evalu贸 191
pacientes con ECV isqu茅mico distribuidos en 2 grupos: Grupo I (13 pacientes con
deterioro neurol贸gico precoz y Grupo II (178 pacientes sin Deterioro Neurol贸gico
precoz). Se investig贸 la influencia de: edad, sexo, Hipertensi贸n Arterial, diabetes
mellitus tipo 2, presi贸n Arterial Sist贸lica , score de la Escala de Glasgow
(ECG), nitr贸geno ureico en la sangre (BUN) , creatinina s茅rica, raz贸n BUN
/creatinina, hemoglobina, glucosa, lipoprote铆nas de alta densidad, triglic茅ridos,
sodio, potasio, prote铆na C reactiva (PCR), en el desarrollo de Deterioro Neurol贸gico
Precoz (DNP).
Se aplic贸 el Test de Chi cuadrado y se calcul贸 el Odds Ratio para asociaci贸n
significativa entre las variables, y an谩lisis multivariado de regresi贸n log铆stica binaria
para evaluar simult谩neamente los factores de riesgo ajustado.
Resultados: Las variables cl铆nicas con asociaci贸n significativa a DNP tras un ECV
isqu茅mico fueron: sexo OR = 0,20 (P=0,041), puntuaci贸n ECG al tercer d铆a OR= 0,70
(p=0,002) y nivel de potasio a la admisi贸n OR= 2,53 (P=0,030), controlando el
resto de variables. El modelo tuvo un rendimiento diagn贸stico de 80,53%.
Conclusiones: Prevalencia de DNP en pacientes con ECV isqu茅mica fue 6.8%. El
modelo de predicci贸n para DNP en pacientes con ECV isqu茅mica estuvo integrado
por las variables: sexo, score de ECG al tercer d铆a, y niveles de K a la admisi贸n con rendimiento diagn贸stico de 80,53%.Determine prevalence and make a predictive model of early neurological
deterioration in patients with ischemic cerebrovascular disease (CVD).
Method: It was conducted an observational, analytical, cross-sectional study,
evaluated 191 patients with ischemic CVD, which were divided into two groups.
Group I (13 patients with early neurological deterioration and Group II (178 patients
without early neurological deterioration); the influence of :sex, Arterial
hypertension, diabetes mellitus type 2, systolic blood pressure, score of the Glasgow
Coma Scale (GCS), blood urea nitrogen (BUN) , serum creatinine, BUN / creatinine
ratio, hemoglobin, glucose, High density lipoprotein (HDL), triglycerides, sodium,
potassium, C-reactive protein were investigated in the development of early
Neurological deterioration (END).
Chi square was applied and Odds Ratio鈥檚 result shows significant association
between the variables test, it is also performed the multivariate analysis of logistic
binary regression where evaluated simultaneously the risk factors in study.
Results: The clinical variables with significant association to early neurological
impairment after ischemic cerebrovascular disease were: sex OR = 0.20 (P = 0.041),
ECG score in the third day OR = 0.70 (p = 0.002) and the level of potassium
admission OR = 2.53 (P = 0.030), controlling the other variables. The model had a
80.53% diagnostic performance.
Conclusions: The prevalence of DNP in patients with ischemic CVD was 6.8%.
The prediction model for DNP for patients with ischemic CVD consisted of
variables: sex, ECG score on the third day, and potassium levels to admission
diagnostic yield of 80.53%
