    Eosinophilic cystitis associated with urethral stricture disease from pelvic trauma. Case report and literature review

    We report a case of eosinophilic cystitis (EC) in a 65-year-old man with urethral stricture disease from blunt pelvic traumatic event. EC is a rare condition characterized by eosinophilic infiltration of the bladder wall, that usually presents with irritative voiding symptoms, suprapubic pain and hematuria. Etiology is still not clear although a review of the literature suggests that pathogenetic mechanisms probably engage an altered immune response in the bladder, with the inflammatory reaction caused by factors such as exogenous allergens and previous bladder injury or surgery to the bladder or the prostate. The diagnosis of EC has to be confirmed by biopsy, since in some cases it may manifest as other inflammatory and malignant bladder disorders. A conservative medical management is indicated initially, since this disease may be self-limited, with a benign course especially in children and young patients. In adults EC is more often a chronic recurrent condition that requires close follow-up since it may lead to serious progressive bladder and/or upper urinary tract disease. More invasive therapies (including transurethral resection, partial or total cystectomy) may also be required when conservative therapy fails

    <p>Abstract</p> <p>Background</p> <p>Cavernous lymphangioma is a rare lesion in the breast of adults. Only a few cases have been documented in literature.</p> <p>Case presentation</p> <p>We describe a 38-year-old woman who presented with a palpable breast lump, which measured 5 × 4 cm. A local excision of the lump was performed and a diagnosis of cavernous lymphangioma was made. The patient is alive and well, after five years of follow-up, with no complaints or recurrence.</p> <p>Conclusion</p> <p>To the best of our knowledge, this is the first case to be documented in a black African woman. Complete surgical excision seems to be the best modality of treatment of this lesion.</p

    The impact of global navigation satellite system (GNSS) zenith total delay data assimilation on the short-term precipitable water vapor and precipitation forecast over Italy using the Weather Research and Forecasting (WRF) model

    The impact of assimilating GNSS-ZTD (global navigation satellite system–zenith total delay) on the precipitable water vapor and precipitation forecast over Italy is studied for the month of October 2019, which was characterized by several moderate to intense precipitation events, especially over northwestern Italy. The WRF (Weather Research and Forecasting) model, version 4.1.3, is used with its 3D-Var data assimilation system to assimilate ZTD observations from 388 GNSS receivers distributed over the country. The dataset was built collecting data from all the major national and regional GNSS permanent networks, achieving dense coverage over the whole area. The water vapor forecast is verified for the forecast hours of 1–6 h after the last data assimilation time. Results show that WRF underestimates the atmospheric water vapor content for the period, and GNSS-ZTD data assimilation improves this underestimation. The precipitation forecast is verified in the phases of 0–3 and 3–6 h after the last data assimilation time using more than 3000 rain gauges spread over Italy. The application of GNSS-ZTD data assimilation to a case study improved the precipitation forecast by increasing the rainfall maximum and by better focusing the precipitation pattern over northeastern Italy, with the main drawback being the prediction of false alarms. Considering the study over the whole period, GNSS-ZTD data assimilation had a positive impact on rainfall forecast, with an improvement in the performance up to 6 h and with statistically significant results for moderate to intense rainfall thresholds (25–30 mm (3 h)−1).</p

    The importance of loading frequency, rate and vibration for enhancing bone adaptation and implant osseointegration

    Mechanical loading is one of the key factors that influence bone mass and the osseointegration of bone-anchored implants. From a clinical point of view, mechanical stimulation may be used to enhance bone strength and implant osseointegration. Among the many loading parameters that influence the response to mechanical loading, the effects of loading frequency and rate have been investigated in many studies. In this paper the most relevant animal studies that have addressed the effect of loading frequency, rate, and vibration on either bone adaptation or implant osseointegration are systematically reviewed. Apparently contradictory results are discussed and interpreted within the context of mechanotransduction and mechanoregulation of bone. A combined experimental and computational approach is suggested to address some of the remaining research questions

    A decidedly rare case of thrombophlebitis of left pampiniform plexus, causing acute development of a painful scroto-inguinal mass in a 28-year-old man with varicocele, is presented. The etiology of the disease remains obscure. A relationship to some unsuspected or indirect trauma or increased pressure in the pampiniform plexus has been postulated. Clinical features and difficulties in the differential diagnosis of other conditions producing acute cord enlargements are discussed with review of the literature. The diagnosis can be made preoperatively and condition can well be managed conservatively by antiphlogistic therapy. Oxerutina [O-(P-idrossietil)-rutosidea] has been used effectively and without adverse effects to control clinical signs and symptoms associated with thrombophlebitis of spermatic plexus. Surgical treatment of the varicocele, which may be a predisposing disease, is suggested after remission of inflammatory state to avoid relapses

    I fibroadenomi multipli e recidivanti della mammella rappresentano un evento patologico assai poco frequente. L’eziologia rimane tuttora sconosciuta, mentre la diagnosi strumentale non desta particolari problemi. Gli Autori presentano il caso di una giovane donna di 22 anni operata più volte per la presenza di multipli fibroadenomi mammari bilaterali plurirecidivanti. L’attegiamento terapeutico proposto per questa patologia rimane il controllo strumentale periodico, limitando l’asportazione chirurgica solo a quelle lesioni che raggiungano in breve tempo un significativo incremento volumetrico

    Enhancement of implant osseointegration by high-frequency low-magnitude loading

    Abstract Background Mechanical loading is known to play an important role in bone remodelling. This study aimed to evaluate the effect of high- and low-frequency axial loading, applied directly to the implant, on peri-implant bone healing and implant osseointegration. Methodology Titanium implants were bilaterally installed in rat tibiae. For every animal, one implant was loaded (test) while the other one was not (control). The test implants were randomly divided into 8 groups according to 4 loading regimes and 2 experimental periods (1 and 4 weeks). The loaded implants were subject to an axial displacement. Within the high- (HF, 40 Hz) or low-frequency (LF, 8 Hz) loading category, the displacements varied 2-fold and were ranked as low- or high-magnitude (LM, HM), respectively. The strain rate amplitudes were kept constant between the two frequency groups. This resulted in the following 4 loading regimes: 1) HF-LM, 40 Hz-8 µm; 2) HF-HM, 40 Hz-16 µm; 3) LF-LM, 8 Hz-41 µm; 4) LF-HM, 8 Hz-82 µm. The tissue samples were processed for resin embedding and subjected to histological and histomorphometrical analyses. Data were analyzed statistically with the significance set at p<0.05. Principal Findings After loading for 4 weeks, HF-LM loading (40 Hz-8 µm) induced more bone-to-implant contact (BIC) at the level of the cortex compared to its unloaded control. No significant effect of the four loading regimes on the peri-implant bone fraction (BF) was found in the 2 experimental periods. Conclusions The stimulatory effect of immediate implant loading on bone-to-implant contact was only observed in case of high-frequency (40 Hz) low-magnitude (8 µm) loading.
