Teoria de enfermagem de médio alcance para o cuidado transpessoal domiciliar

Defesa : Curitiba, 14/12/2021Inclui referências: p. 149-170Área de concentração: Prática Profissional de EnfermagemResumo: Pesquisa teórica com o objetivo de construir uma teoria de enfermagem de médio alcance para o cuidado transpessoal domiciliar, realizada no período de 2018 a 2021, na qual foram adotadas as estratégias para desenvolvimento de teorias propostas por Walker e Avant (2019), isto é, análise de conceito, síntese e derivação. Para auxílio da primeira etapa foi desenvolvida uma revisão integrativa da literatura. Após a identificação da pergunta de pesquisa, deu-se o estabelecimento das etapas seguindo os descritores "Home care services", "Home Nursing" e "Nursing Care". Foi realizada busca controlada nas bases: Literatura Latino-Americana e do Caribe em Ciências da Saúde, Pubmed-National Library, Science Direct, Oasis Br, Cummulative Index to Nursing and Allied Health Literature e Bases de Dados de Enfermagem. Dessa revisão integrativa da literatura, resultaram 59 estudos que subsidiaram a elaboração do conceito. O conceito e as afirmações ao serem analisados e sintetizados, foram construídos por meio da derivação do referencial teórico da Ciência do Cuidado Unitário de Jean Watson. Após a realização da análise do conceito do cuidado domiciliar, o seguinte conceito foi construído: cuidado desenvolvido no ambiente do domicílio pela enfermagem e equipe interprofissional, do nascimento à terminalidade, que inclui a interligação entre aquele que cuida e aquele que é cuidado, utilizando o processo de cuidar com vistas ao healing. Requer reconhecer as peculiaridades do contexto domiciliar, da família e rede social de apoio com planejamento, conhecimento, habilidades técnico-científicas, respeito às crenças, cultura e aos valores individuais. Após, identificaram-se outros conceitos relacionados ao cuidado domiciliar, ou seja: saúde, ser humano, ambiente, cuidador, família, rede social de apoio, segurança do ser cuidado no domicílio e formação profissional. Na sequência, deu-se a síntese das afirmações entre os conceitos com a construção dos pressupostos e proposições, e a modelagem final da teoria de médio alcance para o cuidado transpessoal domiciliar, com as manifestações de cuidado para este contexto. Acredita-se que a teoria de enfermagem de médio alcance para o cuidado transpessoal domiciliar irá contribuir para os cuidados realizados no domicílio ao ser cuidado, cuidador e família, respondendo às necessidades fisiológicas, psicológicas, socioculturais, desenvolvimentais e espirituais. Pode ser adotada em diferentes contextos de domicílio, grupos populacionais e em todo o ciclo de vida. No entanto, torna-se necessário, futuramente, uma validação teórica e empírica, para que seus conceitos, pressupostos, proposições e manifestações de cuidado sejam validados, refutados ou renovados.Abstract: Theoretical research, which aimed to build a middle range nursing theory for the transpersonal home care. It was conducted between 2018 and 2021. In order to achieve the proposed objective, the strategies for the development of theories by Walker and Avant (2019) were adopted, that is, concept analysis, synthesis and derivation. An integrative literature review was developed for helping the first step of the research. After the identification of the research question, the steps of the review were established by following the descriptors "Home care services", "Home Nursing" e "Nursing Care". Controlled searching was held in the databases, as follows: Latin American and Caribbean Health Sciences Literature (LILACS), Pubmed-National Library, Science Direct, Oasis Br, Cummulative Index to Nursing and Allied Health Literature, and Nursing Database. From that integrative literature review, 59 studies were retrieved, which underpinned the concept elaboration. The concept and statements while analyzed and synthesized, were built by means of the parent theory derivation, Jean Watson's Science of the Unitary Care. After conducting the concept analysis of the home care, the following concept was built: care delivered at the home context by nursing and professional teams, from birth to terminality, including the interrelationship between the caring and the cared beings, using the caring process towards healing. It requires to know the idiosyncrasies of the home context, family and support network by means of planning, knowledge, technical and scientific skills, respect to beliefs, culture and individual values, apart from the identification of other home care-related concepts, such as: health, human being, environment, caregiver, family, social support network, patient home safety and professional qualification. Subsequently, the synthesis of the statements between the concepts occurred, with the construction of the assumptions and propositions, and the final modelling of the middle range transpersonal home care theory, with the care manifestations for that context. The middle range nursing theory for the transpersonal home care is believed to contribute to care rendered at home settings to the cared being, caregiver and family, meeting their physiological, psychological, sociocultural, developmental and spiritual needs. It can be adopted in distinct home contexts, population groups, and throughout the life cycle. However, further theoretical and empirical validation is deemed necessary so that its concepts, assumptions, propositions and care manifestations can be validated, refuted or renewed
