    A cross-linguistic fMRI study of perception of intonation and emotion in Chinese

    Conflicting data from neurobehavioral studies of the perception of intonation (linguistic) and emotion (affective) in spoken language highlight the need to further examine how functional attributes of prosodic stimuli are related to hemispheric differences in processing capacity. Because of similarities in their acoustic profiles, intonation and emotion permit us to assess to what extent hemispheric lateralization of speech prosody depends on functional instead of acoustical properties. To examine how the brain processes linguistic and affective prosody, an fMRI study was conducted using Chinese, a tone language in which both intonation and emotion may be signaled prosodically, in addition to lexical tones. Ten Chinese and 10 English subjects were asked to perform discrimination judgments of intonation (I: statement, question) and emotion (E: happy, angry, sad) presented in semantically neutral Chinese sentences. A baseline task required passive listening to the same speech stimuli (S). In direct between‐group comparisons, the Chinese group showed left‐sided frontoparietal activation for both intonation (I vs. S) and emotion (E vs. S) relative to baseline. When comparing intonation relative to emotion (I vs. E), the Chinese group demonstrated prefrontal activation bilaterally; parietal activation in the left hemisphere only. The reverse comparison (E vs. I), on the other hand, revealed that activation occurred in anterior and posterior prefrontal regions of the right hemisphere only. These findings show that some aspects of perceptual processing of emotion are dissociable from intonation, and, moreover, that they are mediated by the right hemisphere

    Characteristics of CD8+ T cell subsets in Chinese patients with chronic HIV infection during initial ART

    <p>Abstract</p> <p>Background</p> <p>CD8+ T cells may play an important role in protecting against HIV. However, the changes of CD8+ T cell subsets during early period of ART have not been fully studied.</p> <p>Methods</p> <p>Twenty-one asymptomatic treatment-naive HIV-infected patients with CD4 T+ cells less than 350 cells/Îźl were enrolled in the study. NaĂŻve, central memory(CM), effective memory(EM) and terminally differentiated effector (EMRA) CD8+ cell subsets and their activation and proliferation subsets were evaluated in blood samples collected at base line, and week 2, 4, 8 and 12 of ART.</p> <p>Results</p> <p>The total CD8+ T cells declined and the NaĂŻve and CM subsets had a tendency of increase. Activation levels of all CD8+ T cell subsets except EMRA subset decreased after ART. However, proliferation levels of total CD8+ T cells, EMRA, EM and CM subsets increased at the first 4 weeks of ART, then decreased. Proliferation level of the naĂŻve cells decreased after ART.</p> <p>Conclusion</p> <p>The changes of CD8+ T cell subsets during initial ART are complex. Our results display a complete phenotypical picture of CD8+ cell subsets during initial ART and provide insights for understanding of immune status during ART.</p

    Neural correlates of segmental and tonal information in speech perception

    The Chinese language provides an optimal window for investigating both segmental and suprasegmental units. The aim of this cross‐linguistic fMRI study is to elucidate neural mechanisms involved in extraction of Chinese consonants, rhymes, and tones from syllable pairs that are distinguished by only one phonetic feature (minimal) vs. those that are distinguished by two or more phonetic features (non‐minimal). Triplets of Chinese monosyllables were constructed for three tasks comparing consonants, rhymes, and tones. Each triplet consisted of two target syllables with an intervening distracter. Ten Chinese and English subjects were asked to selectively attend to targeted sub‐syllabic components and make same‐different judgments. Direct between‐group comparisons in both minimal and non‐minimal pairs reveal increased activation for the Chinese group in predominantly left‐sided frontal, parietal, and temporal regions. Within‐group comparisons of non‐minimal and minimal pairs show that frontal and parietal activity varies for each sub‐syllabic component. In the frontal lobe, the Chinese group shows bilateral activation of the anterior middle frontal gyrus (MFG) for rhymes and tones only. Within‐group comparisons of consonants, rhymes, and tones show that rhymes induce greater activation in the left posterior MFG for the Chinese group when compared to consonants and tones in non‐minimal pairs. These findings collectively support the notion of a widely distributed cortical network underlying different aspects of phonological processing. This neural network is sensitive to the phonological structure of a listener's native language

    Activation of the left planum temporale in pitch processing is shaped by language experience

    Implicit, abstract knowledge acquired through language experience can alter cortical processing of complex auditory signals. To isolate prelexical processing of linguistic tones (i.e., pitch variations that convey part of word meaning), a novel design was used in which hybrid stimuli were created by superimposing Thai tones onto Chinese syllables (tonal chimeras) and Chinese tones onto the same syllables (Chinese words). Native speakers of tone languages (Chinese, Thai) underwent fMRI scans as they judged tones from both stimulus sets. In a comparison of native vs. non‐native tones, overlapping activity was identified in the left planum temporale (PT). In this area a double dissociation between language experience and neural representation of pitch occurred such that stronger activity was elicited in response to native as compared to non‐native tones. This finding suggests that cortical processing of pitch information can be shaped by language experience and, moreover, that lateralized PT activation can be driven by top‐down cognitive processing

    Neural basis of first and second language processing of sentence-level linguistic prosody

    A fundamental question in multilingualism is whether the neural substrates are shared or segregated for the two or more languages spoken by polyglots. This study employs functional MRI to investigate the neural substrates underlying the perception of two sentence‐level prosodic phenomena that occur in both Mandarin Chinese (L1) and English (L2): sentence focus (sentence‐initial vs. ‐final position of contrastive stress) and sentence type (declarative vs. interrogative modality). Late‐onset, medium proficiency Chinese‐English bilinguals were asked to selectively attend to either sentence focus or sentence type in paired three‐word sentences in both L1 and L2 and make speeded‐response discrimination judgments. L1 and L2 elicited highly overlapping activations in frontal, temporal, and parietal lobes. Furthermore, region of interest analyses revealed that for both languages the sentence focus task elicited a leftward asymmetry in the supramarginal gyrus; both tasks elicited a rightward asymmetry in the mid‐portion of the middle frontal gyrus. A direct comparison between L1 and L2 did not show any difference in brain activation in the sentence type task. In the sentence focus task, however, greater activation for L2 than L1 occurred in the bilateral anterior insula and superior frontal sulcus. The sentence focus task also elicited a leftward asymmetry in the posterior middle temporal gyrus for L1 only. Differential activation patterns are attributed primarily to disparities between L1 and L2 in the phonetic manifestation of sentence focus. Such phonetic divergences lead to increased computational demands for processing L2. These findings support the view that L1 and L2 are mediated by a unitary neural system despite late age of acquisition, although additional neural resources may be required in task‐specific circumstances for unequal bilinguals

    The association between procalcitonin and acute kidney injury in patients stung by wasps

    Introduction: The aim of this study was to investigate the status of serum procalcitonin (PCT) in patients stung by wasps and evaluate the association between PCT levels and acute kidney injury (AKI).Methods: Patients stung by wasps admitted to two tertiary hospitals between January 2017 and December 2020 were screened for enrollment. We evaluated serum PCT levels on admission in patients stung by wasps. The patients were divided into an AKI group and a non-AKI group. A logistic regression model was used to analyze the association between PCT status and AKI. The performance of PCT concentrations in predicting the occurrence of AKI was evaluated by the area under the receiver operating characteristic curve (AUROC).Results: A total of 138 patients were enrolled, and 66 patients suffered AKI. PCT levels were elevated in 78.99% of patients stung by wasps. Nearly half of the patients (47.83%) developed AKI. PCT levels were correlated with creatinine levels on admission (r = 0.787, 95% CI: 0.713–0.844). PCT levels in patients with AKI were higher than those in patients without AKI (p &lt; 0.001). After adjustment for covariates, PCT levels on admission were independently associated with AKI (OR: 1.575, 95% CI: 1.071–2.317, p = 0.021). The AUROC of PCT levels on admission was 0.837 (95% CI, 0.771–0.902, p &lt; 0.001). A PCT level of 0.57 μg/L was the cutoff for maximizing the Youden index; the specificity was 79.45%, and the sensitivity was 73.43%.Conclusion: Serum PCT levels may be a potential biomarker of AKI in patients stung by wasps

    Radiogenomic Analysis of Papillary Thyroid Carcinoma for Prediction of Cervical Lymph Node Metastasis: A Preliminary Study

    Background Papillary thyroid carcinoma (PTC) is characterized by frequent metastases to cervical lymph nodes (CLNs), and the presence of lymph node metastasis at diagnosis has a significant impact on the surgical approach. Therefore, we established a radiomic signature to predict the CLN status of PTC patients using preoperative thyroid ultrasound, and investigated the association between the radiomic features and underlying molecular characteristics of PTC tumors. Methods In total, 270 patients were enrolled in this prospective study, and radiomic features were extracted according to multiple guidelines. A radiomic signature was built with selected features in the training cohort and validated in the validation cohort. The total protein extracted from tumor samples was analyzed with LC/MS and iTRAQ technology. Gene modules acquired by clustering were chosen for their diagnostic significance. A radiogenomic map linking radiomic features to gene modules was constructed with the Spearman correlation matrix. Genes in modules related to metastasis were extracted for Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses, and a protein-protein interaction (PPI) network was built to identify the hub genes in the modules. Finally, the screened hub genes were validated by immunohistochemistry analysis. Results The radiomic signature showed good performance for predicting CLN status in training and validation cohorts, with area under curve of 0.873 and 0.831 respectively. A radiogenomic map was created with nine significant correlations between radiomic features and gene modules, and two of them had higher correlation coefficient. Among these, MEmeganta representing the upregulation of telomere maintenance via telomerase and cell-cell adhesion was correlated with 'Rectlike' and 'deviation ratio of tumor tissue and normal thyroid gland' which reflect the margin and the internal echogenicity of the tumor, respectively. MEblue capturing cell-cell adhesion and glycolysis was associated with feature 'minimum calcification area' which measures the punctate calcification. The hub genes of the two modules were identified by protein-protein interaction network. Immunohistochemistry validated that LAMC1 and THBS1 were differently expressed in metastatic and non-metastatic tissues (p=0.003; p=0.002). And LAMC1 was associated with feature 'Rectlike' and 'deviation ratio of tumor and normal thyroid gland' (p<0.001; p<0.001); THBS1 was correlated with 'minimum calcification area' (p<0.001). Conclusions The radiomic signature proposed here has the potential to noninvasively predict the CLN status in PTC patients. Merging imaging phenotypes with genomic data could allow noninvasive identification of the molecular properties of PTC tumors, which might support clinical decision making and personalized management

    Rapid Turnover of 2-LTR HIV-1 DNA during Early Stage of Highly Active Antiretroviral Therapy

    BACKGROUND: Despite prolonged treatment with highly active antiretroviral therapy (HAART), the infectious HIV-1 continues to replicate and resides latently in the resting memory CD4+ T lymphocytes, which blocks the eradication of HIV-1. The viral persistence of HIV-1 is mainly caused by its proviral DNA being either linear nonintegrated, circular nonintegrated, or integrated. Previous reports have largely focused on the dynamics of HIV-1 DNA from the samples collected with relatively long time intervals during the process of disease and HAART treatment, which may have missed the intricate changes during the intervals in early treatment. METHODOLOGY/PRINCIPAL FINDINGS: In this study, we investigated the dynamics of HIV-1 DNA in patients during the early phase of HARRT treatment. Using optimized real time PCR, we observed significant changes in 2-LTR during the first 12-week of treatment, while total and integrated HIV-1 DNA remained stable. The doubling time and half-life of 2-LTR were not correlated with the baseline and the rate of changes in plasma viral load and various CD4+ T-cell populations. Longitudinal analyses on 2-LTR sequences and plasma lipopolysaccharide (LPS) levels did not reveal any significant changes in the same treatment period. CONCLUSIONS/SIGNIFICANCE: Our study revealed the rapid changes in 2-LTR concentration in a relatively large number of patients during the early HAART treatment. The rapid changes indicate the rapid infusion and clearance of cells bearing 2-LTR in the peripheral blood. Those changes are not expected to be caused by the blocking of viral integration, as our study did not include the integrase inhibitor raltegravir. Our study helps better understand the dynamics of HIV-DNA and its potential role as a biomarker for the diseases and for the treatment efficacy of HAART

    The electrophysiological and behavioral effects of distractor discriminability on auditory selective attention

    Three experiments were conducted to examine the electrophysiological and behavioral effects of distractor discriminability on auditory selective attention to targets. The experiments differed in the range of overlap between the relevant and irrelevant channels and in the number of targets and distractors presented in each channel. In each experiment, participants attended and discriminated targets in one channel (ear) while ignoring interspersed distractors in another channel (ear). Target discrimination was performed in a baseline condition and in a set of filtering conditions, a modified version of the paradigm used by Melara and Mounts (1994). Behavioral and event-related potential (ERP) measurements were made during task performance. Increased distractor variability caused poorer accuracy and slower reaction times relative to baseline, a result known as Garner interference (1974), which reflects the failure of selective attention to the relevant channel. Physiologically, distraction discriminability reduced the P3 response to targets, and increased the P2 response to distractors. Channel separation (Nd) did not change across conditions. These results suggested that Garner interference is caused by the diminished distinctiveness of target representations in working memory, as distractors activate a greater range of the task relevant continuum. A pattern of early long-lasting negativity to targets and later positivity to distractors suggested that auditory selective attention operates through two distinct processing mechanisms: an excitatory mechanism to relevant signals and an inhibitory mechanism to irrelevant signals
