Nadroparin in the prophylaxis and treatment of thromboembolic complications in cancer patients
Thromboembolic events are the most frequent complications and second leading cause of death among cancer patients. The most common forms include deep venous thrombosis of lower extremities and pulmonary embolism. The risk of thrombosis is correlated with underlying, patient’s clinical characteristic, functions of coagulation system as well as anticancer treatment. There is a well-established evidence for using of antithrombotic prophylaxis in cancer patients undergoing surgery and hospitalised due to different causes. There are some scores, based on laboratory and clinical factors, that facilitate qualification to the prophylaxis (e.g. Caprini score, Padua Prediction Score). The role of low-molecular-weight heparins (LMWH) in prophylaxis of thrombosis in cancer patients was established based on the results of many randomised clinical trials. Currently, the use this group of drugs — both in the treatment and in prophylaxis of venous thromboembolism (VTE) — is a part of standard of care. Nadroparyna w profilaktyce i leczeniu powikłań zakrzepowo-zatorowych u chorych na nowotwory złośliwe
Zdarzenia zakrzepowo-zatorowe wśród chorych na nowotwory złośliwe są najczęstszym powikłaniem. Są one również drugą z kolei przyczyną zgonu tych chorych. Najczęstszymi postaciami klinicznymi są: zakrzepica żył głębokich kończyn dolnych oraz zatorowość płucna. Ryzyko rozwoju zakrzepicy związane jest zarówno z rozwijającym się nowotworem złośliwym, jak i indywidualnymi cechami chorego wpływającymi na funkcjonowanie układu krzepnięcia krwi, a także z leczeniem przeciwnowotworowym. Udokumentowano zasadność stosowania profilaktyki przeciwzakrzepowej u chorych na nowotwory poddawanych zabiegom chirurgicznym oraz hospitalizowanych z różnych przyczyn. Kwalifikację chorych do takiego postępowania ułatwia zastosowanie odpowiednich skal kliniczno-laboratoryjnych (m.in. skala Capriniego, skala Padewska). Miejsce heparyn drobnocząsteczkowych w profilaktyce zakrzepicy u chorych na nowotwory złośliwe zostało ustalone na podstawie wyników licznych randomizowanych badań klinicznych. Obecnie stosowanie leków z tej grupy zarówno w leczeniu, jak i w profilaktyce żylnej choroby zakrzepowo-zatorowej u chorych na nowotwory złośliwe jest elementem standardowego postępowania. Nadroparyna jest jedną z heparyn drobnocząsteczkowych o udokumentowanej skuteczności i bezpieczeństwie stosowania u pacjentów z chorobą nowotworową
Powikłania hematologiczne u chorych na raka płuca
Rak płuca jest najczęstszym nowotworem złośliwym pod względem zachorowalności i umieralności
w Polsce. Stanowi przyczynę zgonu większej liczby chorych na nowotwory złośliwe niż łącznie rak piersi, jelita
grubego i gruczołu krokowego. Mimo że w ostatnich latach w terapii raka płuca osiągnięto istotny postęp,
wyniki leczenia wciąż pozostają w dużym stopniu niezadowalające — wskaźnik przeżycia 5-letniego wynosi
bowiem jedynie 6–8%.
Leczenie raka płuca z zastosowaniem radioterapii, chemioterapii, a zwłaszcza chemioradioterapii wiąże się
z wysokim ryzykiem powikłań. Wśród nich szczególne miejsce zajmują powikłania hematologiczne, które
mogą dotyczyć układu czerwonokrwinkowego, białokrwinkowego lub układu hemostazy. W niektórych
przypadkach ich pojawienie się może o wiele miesięcy wyprzedzać rozpoznanie nowotworu. Wystąpienie
powikłań hematologicznych jest czynnikiem ograniczającym stosowanie chemioterapii i radioterapii w określonym
rytmie czasowym i odpowiedniej dawce, może także niekorzystnie wpływać na stan sprawności
i jakość życia chorych oraz na wyniki leczenia. Wydaje się, iż wnikliwe rozważenie możliwości wystąpienia
działań niepożądanych i potencjalnych korzyści, jakie chory może odnieść z leczenia, ma w terapii raka płuca
szczególne znaczenie. Dlatego leczenie wspomagające, choć jest przedmiotem wielu kontrowersji, stanowi
ważny element terapii raka płuca.Lung cancer remains the leading cause of cancer death in Poland, resulting in more deaths than the next
three most common cancers combined (colon cancer, breast cancer and prostate cancer). Unfortunately,
despite significant progress in cancer therapy in recent years, the expected 5-year survival rate for all
patients in whom lung cancer is diagnosed ranges from 6–8%.
The chemotherapy, radiotherapy and, particularly, chemo-radiotherapy protocols applied to patients with
lung cancer are associated with severe toxicity, including haematological complications. These may affect
leukocytes, red blood cells and platelets as well as the haemostatic system. In some cases they may be the
first clinical manifestation of the disease, preceding the diagnosis of cancer by several months. The occurrence
of haematological complications poses restrictions on the administration of chemotherapy and/or
radiotherapy within a given time frame and at a specified dose. In addition this can adversely affect
performance status and quality of life as well as the survival of lung cancer patients. Careful assessment of
the risk of toxicity and of the potential benefits of therapy is vitally important in the treatment of lung

Therapy interfering with EGFR activation in patients with cancers of oesophagus, stomach, pancreas, liver and bile ducts
Prognosis of patients with advanced cancers of the upper digestive tract remains dismal, despite significant
improvements in diagnostic and surgical techniques in recent years. In part, it may be attributed
to the considerable resistance to the standard cytotoxic treatment. Thus, there is ongoing extensive
search for so called “targeted therapies” that are aimed at specific molecular abnormalities in cancer
cells. Epidermal growth factor receptor (EGFR) intracellular signaling transduction pathways may constitute
such targets. Importantly, molecular abnormalities related to above mentioned signaling pathways are
frequently encountered in upper digestive tract neoplasms. To date, based on positive phase III clinical
trials results, two novel drugs were introduced to clinical practice, e.g. trastuzumab (first line treatment
of advanced gastric cancer) and erlotinib (first line treatment of advanced pancreatic cancer). However,
there are significant differences in biology and pathogenesis of upper digestive tract cancers that make
formulation of general treatment recommendations in this patient population impossible. It seems that
in the future the indications for specific targeted therapy will be based on individual patients’ profile of
molecular abnormalities in cancer cells. Moreover, it will require identification of predictive and prognostic
factors enabling clinicians to select a group of patients who will derive most significant benefits from
specific therapies. It should be noticed that prospective assessment of various predictive and prognostic
factors was incorporated in protocols of many phase III clinical trials that were completed in recent
years. However, final results of majority of these analyses are not available yet. EGFR may be the target
for small molecule tyrosine kinase inhibitors or monoclonal antibodies. In this paper we present a review
of the results of clinical trials assessing targeted therapy interfering with EGFR in patients with upper
digestive tract cancers that were reported in recent years.

poważne, mimo istotnego postępu, jaki osiągnięto w ostatnich latach w zakresie diagnostyki i leczenia
chirurgicznego. Wynika to między innymi z faktu rozpoznawania choroby w zaawansowanych stadiach
klinicznych, dodatkowo zaś z jej małej wrażliwości lub niewrażliwości na klasyczne leczenie cytotoksyczne.
Z tego powodu trwają poszukiwania terapii celowanych ukierunkowanych na cele molekularne w komórce
nowotworowej. Celami takiej terapii mogą być szlaki przekaźnictwa sygnałów wewnątrzkomórkowych
generowanych przez rodzinę receptorów czynnika wzrostu naskórka (EGFR). Zaburzenia molekularne
związane z tymi szlakami przekaźnictwa sygnałów wewnątrzkomórkowych występują dość często u chorych
na nowotwory złośliwe przewodu pokarmowego. Dotychczas, na podstawie pozytywnych wyników badań
klinicznych III fazy, do codziennej praktyki klinicznej wprowadzono dwa preparaty: trastuzumab (leczenie
chorych na HER2-dodatniego uogólnionego raka żołądka) oraz erlotynib (leczenie chorych na raka
trzustki w zaawansowanych stadiach klinicznych choroby). Różnice w biologii i patogenezie nowotworów
złośliwych przewodu pokarmowego uniemożliwiają sformułowanie ogólnych zaleceń terapeutycznych
dotyczących leczenia interferującego z EGFR w tej populacji pacjentów. Wydaje się, iż kwalifikacja chorych
na nowotwory złośliwe przewodu pokarmowego do leczenia celowanego będzie w przyszłości zależna od
wyniku oceny indywidualnego profilu zaburzeń molekularnych u danego chorego. Będzie to wymagało
identyfikacji czynników predykcyjnych i prognostycznych, co z kolei mogłoby umożliwić wyodrębnienie
grupy chorych, którzy mogliby odnieść największe korzyści z określonej terapii. Należy podkreślić,
iż prospektywna ocena wartości predykcyjnej i prognostycznej różnych czynników była niezwykle istotnym
elementem zakończonych w ostatnich latach badań klinicznych III fazy oceniających stosowanie leków
z kategorii terapii celowanych u chorych na nowotwory złośliwe przewodu pokarmowego. Ostateczne
wyniki wielu z tych analiz dotychczas nie zostały przedstawione.
Obecność zaburzeń w szlakach przekaźnictwa sygnałów wewnątrzkomórkowych związanych z EGFR
można wykorzystać w celach terapeutycznych. Receptor EGFR może być celem terapii wykorzystujących
drobnocząsteczkowe inhibitory kinaz tyrozynowych EGFR lub przeciwciała monoklonalne przeciwko tym
receptorom. W niniejszej pracy dokonano przeglądu piśmiennictwa dotyczącego terapii ukierunkowanej
na rodzinę receptorów EGFR u chorych na nowotwory złośliwe górnego odcinka przewodu pokarmowego.

Leczenie interferujące z receptorem czynnika wzrostu naskórka (EGFR) u chorych na raka jelita grubego
Over the last decade systemic treatment of colorectal cancer patients has changed considerably. New
cytotoxics, such as oxaliplatin, irinotecan and capecitabine have been introduced to everyday clinical
practice. More importantly, introduction of targeted therapy aimed at molecular targets present in cancer
cells has substantially improved treatment outcomes. Currently, two major strategies of targeted therapy
are used in colorectal cancer in routine clinical practice: blocking angiogenesis by using bevacizumab,
a monoclonal antibody directed against VEGF and inhibition of the epidermal growth factor receptor
(EGFR) by monoclonal antibodies — cetuximab and panitumumab.
Results of the studies that evaluated association between histologic grade of malignancy and EGFR
expression on colorectal cancer cells have been controversial. Interestingly, clinical studies reported in
recent years have not showed unequivocally a significant association between EGFR expression and
survival of colorectal cancer patients. Nevertheless, it has been demonstrated in several phase II and III
clinical trials that the addition of monoclonal antibodies directed against EGFR to palliative chemotherapy in
advanced colorectal cancer patients without a mutation in KRAS gene (wild-type KRAS) leads to significant
improvements in response rates and significant prolongation of progression-free survival (and in some
trials improvement of overall survival was documented, too). Intriguingly, it has been showed in several
studies that using monoclonal antibodies directed against EGFR in combination with chemotherapy for
advanced colorectal cancer patients with mutation in KRAS gene (mutant KRAS) has been associated with
inferior treatment results. These observations have served as the scientific foundation for introduction of
KRAS gene mutation analysis to clinical practice as the first biomarker in targeted therapy of advanced
colorectal cancer patients.
Results of phase III clinical trials were the basis of current indications of cetuximab and panitumumab.
These antibodies are indicated for EGFR-expressing, KRAS wild-type metastatic colorectal cancer, in
combination with first-line or second-line chemotherapy, or as a single agents in patients who have failed
oxaliplatin- and irinotecan-based therapy and who are intolerant to chemotherapy. Additionally, the activity
of EGFR tyrosine kinase inhibitors has been evaluated in several phase I and II clinical trials. The aim of
this paper is to review data from prospective randomised clinical trials that assessed targeted therapy
directed against EGFR in the treatment of colorectal cancer patients.
Onkol. Prak. Klin. 2010; 6, 5: 241–254W ostatniej dekadzie leczenie systemowe zaawansowanego raka jelita grubego uległo znaczącym zmianom. Do praktyki klinicznej wprowadzono nowe leki cytotoksyczne, takie jak oksaliplatyna, irynotekan
i kapecytabina, a także, co istotne, leki z kategorii terapii ukierunkowanych na cele molekularne zlokalizowane w komórce nowotworowej. Przykładem terapii celowanej wprowadzonej do leczenia chorych na raka jelita grubego jest stosowanie bewacyzumabu, przeciwciała monoklonalnego przeciwko czynnikowi wzrostu śródbłonka naczyń (VEGF), w skojarzeniu z chemioterapią paliatywną. Innym celem molekularnym wykorzystywanym w terapii celowanej chorych na raka jelita grubego w ostatnich latach stał się receptor czynnika wzrostu naskórka (EGFR).
Wyniki badań oceniających związek pomiędzy stopniem złośliwości histologicznej i ekspresją EGFR na komórkach raka jelita grubego są kontrowersyjne. Interesujące jest, iż w przeprowadzonych dotychczas
badaniach nie wykazano jednoznacznie istnienia korelacji pomiędzy ekspresją EGFR a czasem przeżycia chorych na raka jelita grubego. Niemniej jednak w badaniach klinicznych II i III fazy wykazano,
że stosowanie przeciwciał monoklonalnych przeciwko EGFR w skojarzeniu z chemioterapią paliatywną prowadzoną u chorych na zaawansowanego raka jelita grubego powoduje istotne zwiększenie odsetka odpowiedzi na leczenie oraz znamienne wydłużenie czasu przeżycia wolnego od progresji choroby(a w niektórych badaniach także wydłużenie czasu całkowitego przeżycia) w grupie chorych bez obecności mutacji w genie KRAS (wild-type KRAS) w porównaniu ze stosowaniem wyłącznie chemioterapii. Co ciekawe, wyniki niektórych badań sugerują, iż stosowanie przeciwciał monoklonalnych przeciwko EGFR w skojarzeniu z chemioterapią w grupie chorych z mutacją w genie KRAS (mutant KRAS) prowadzi do pogorszenia wyników leczenia. Powyższe obserwacje stały się podstawą do wprowadzenia do praktyki klinicznej oceny obecności mutacji w genie KRAS jako pierwszego biomarkera w terapii celowanej chorych na zaawansowanego raka jelita grubego.
Wyniki badań klinicznych III fazy stały się podstawą dokonania aktualnych zapisów rejestracyjnych cetuksymabu i panitumumabu. Przeciwciała te zaleca się w leczeniu chorych z zaawansowanym
rakiem jelita grubego z przerzutami wykazującym ekspresję EGFR z prawidłowo funkcjonującym genem KRAS (wild-type KRAS) w monoterapii po niepowodzeniu leczenia oksaliplatyną i irynotekanem lub przy nietolerancji irynotekanu, a także w przypadku cetuksymabu, w skojarzeniu z chemioterapią paliatywną pierwszej lub drugiej linii opartą na irynotekanie bądź oksaliplatynie. Ponadto liczne badania kliniczne I i II fazy poświęcono ocenie aktywności drobnocząsteczkowych inhibitorów kinazy tyrozynowej EGFR. W niniejszej pracy przedstawiono przegląd aktualnych danych pochodzących
z badań klinicznych, w których poddano ocenie stosowanie leczenia interferującego z EGFR w terapii chorych na raka jelita grubego.

Direct Oral Anticoagulants in Cancer Patients. Time for a Change in Paradigm.
Thrombosis is a more common occurrence in cancer patients compared to the general population and is one of the main causes of death in these patients. Low molecular weight heparin (LMWH) has been the recognized standard treatment for more than a decade, both in cancer-related thrombosis and in its prevention. Direct oral anticoagulants (DOACs) are a new option for anticoagulation therapy. Recently published results of large randomized clinical trials have confirmed that DOAC may be a reasonable alternative to LMWH in cancer patients. The following review summarizes the current evidence on the safety and efficacy of DOAC in the treatment and prevention of cancer-related thrombosis. It also draws attention to the limitations of this group of drugs, knowledge of which will facilitate the selection of optimal therapy
The therapy interfering EGFR activation in breast cancer patients
Nadmierna ekspresja receptora HER2, stwierdzana u około 20% chorych na raka piersi, wiąże się
z agresywnym przebiegiem choroby, skróceniem czasu przeżycia do nawrotu choroby po radykalnym
leczeniu chirurgicznym oraz czasu całkowitego przeżycia chorych. Jednocześnie nadmierna ekspresja
tego receptora lub amplifikacja genu HER2 jest czynnikiem predykcyjnym, związanym z możliwością
uzyskania odpowiedzi na leczenie z zastosowaniem trastuzumabu. Trastuzumab jest przeciwciałem monoklonalnym,
którego celem molekularnym jest zewnątrzkomórkowa domena receptora HER2. Połączenie
się trastuzumabu z tym receptorem na powierzchni komórki nowotworowej powoduje zahamowanie
szlaku przekaźnictwa wewnątrzkomórkowego związanego z funkcją receptorów HER2. Stosowanie
trastuzumabu w skojarzeniu z chemioterapią u chorych na HER2-dodatniego raka piersi w zaawansowanych
stadiach klinicznych choroby wiąże się z istotną poprawą odsetka odpowiedzi na leczenie
oraz znamiennym wydłużeniem czasu całkowitego przeżycia chorych. Dołączenie trastuzumabu do
chemioterapii neoadiuwantowej doprowadza u znacznego odsetka chorych na HER2-dodatniego miejscowo
zaawansowanego raka piersi do uzyskania całkowitej remisji patologicznej. Ponadto stosowanie
trastuzumabu w skojarzeniu z chemioterapią uzupełniającą u chorych na wczesnego HER2-dodatniego
raka piersi wiąże się ze zmniejszeniem ryzyka nawrotu choroby o około 50% oraz zmniejszeniem ryzyka
zgonu o około 30%. Istotne jest, iż wprowadzenie trastuzumabu do praktyki klinicznej otworzyło nowe
kierunki badań nad terapią ukierunkowaną na cele molekularne zlokalizowane w komórce nowotworowej.
Szlak przekazywania sygnałów do wnętrza komórki nowotworowej związany z receptorami HER2 stał się
przedmiotem tysięcy badań, zarówno eksperymentalnych, jak i klinicznych, których ostatecznym celem
jest poprawa wyników leczenia chorych na tę agresywną, obciążoną niekorzystnym rokowaniem, postać
raka piersi. W efekcie do praktyki klinicznej wprowadzane są nowe leki wykazujące aktywność w leczeniu
chorych, u których stwierdzono niepowodzenie uprzedniej terapii z zastosowaniem trastuzumabu. Wiele
nowych cząsteczek znajduje się aktualnie w końcowych fazach badań klinicznych. W niniejszej pracy
dokonano przeglądu danych uzyskanych w ostatnich latach w ramach kontrolowanych badań klinicznych,
dotyczących najważniejszych aspektów terapii interferujących z receptorem HER2 u chorych na
HER2-dodatniego raka piersi.

aggressive course of the disease and shortening of disease-free and overall survival. Simultaneously,
overexpression of this receptor or HER2 gene amplification is a predictive factor for treatment with trastuzumab.
Trastuzumab is a monoclonal antibody directed against extracellular domain of HER2 receptor.
Trastuzumab binding with this receptor on the cancer cell’s surface results in an inhibition of intracellular
signaling pathway associated with HER2 receptor. It has been shown that trastuzumab in combination with chemotherapy significantly improved response rate and prolonged overall survival of advanced
HER2-positive breast cancer patients, compared to patients treated exclusively with chemotherapy. Moreover,
addition of trastuzumab to neoadjuvant chemotherapy leads to pathological complete remission
in a significant proportion of HER2-positive locally advanced breast cancer patients. Furthermore, use of
trastuzumab in combination with adjuvant chemotherapy in HER2-positive early breast cancer patients
was associated with about 50% reduction of risk of disease recurrence and about 30% decrease in
the risk of death. Most importantly, introduction of trastuzumab into clinical practice has opened a new
avenue of research focused on targeted therapy directed against molecular targets located in cancer
cell. Since than, intracellular signaling pathway associated with HER2 receptor has been the subject of
thousands of experimental and clinical studies. The ultimate aim of these studies is a steady improvement
in treatment results and prolongation of survival of patients suffering from this aggressive form of breast
cancer. As a result, introduction into clinical practice of new drugs that are active in the treatment of trastuzumab-
refractory patients is anticipated in the near future. A plethora of new drugs are at various stages
of clinical development at that moment. In this paper we present a review of data that were obtained in
controlled clinical trials in recent years, regarding the most important aspects of therapies interfering with
HER2 receptor in HER2-positive breast cancer patients.

Clinical assessment of the efficacy of photodynamic therapy in the treatment of oral lichen planus
The study objective was clinical assessment of the efficacy of photodynamic therapy (PDT) in the treatment of oral lichen planus (OLP). There were 23 patients aged 31–82 included in the study with oral lichen planus diagnosed clinically and histopathologically. In all patients photodynamic therapy was performed with the use of chlorin e6 (Photolon(®)), containing 20 % chlorin e6 and 10 % dimethyl sulfoxide as a photosensitizer. PDT was performed using a semiconductor laser, with power up to 300 mW and a wavelength of 660 nm. A series of illumination sessions was conducted with the use of superficial light energy density of 90 J/cm(2). Changes of lesion size were monitored at one, two, five, and ten PDT appointments from the series of ten according to the authors' own method. The sizes of clinical OLP lesions exposed to PDT were reduced significantly (on average by 55 %). The best effects were observed for the lesions on the lining mucosa (57.6 %). The therapy was statistically significantly less effective when masticatory mucosa was affected (reduction, 30.0 %). Due to substantial efficacy and noninvasiveness, PDT can be useful in the treatment of OLP lesions

The incidence of malignant gastrointestinal cancers in Poland has been constantly growing, which hasled to an intensification of the search for new markers of the early clinical stage of this disease. The oral cavity,as the first part of the gastrointestinal tract, has a very important role. The oral cavity presents symptoms of bothtypically stomatological and systemic diseases. Oral cancers, benign or malignant, may originate and grow in anyof the tissues of the mouth, and within this small area they may be of varied clinical, histological and biologicalfeatures. These can be lesions typically observed in the oral cavity, but also characteristic of cases where thesymptoms occur both in the mouth and in other body parts. The aim of this study was to present a cytologicalpicture of the oral mucosa in patients with gastric and colon cancer and to compare the cytological picture withthat obtained from a group of patients with no cancer, using the Papanicolaou classification and the Bethesdasystem. The study was conducted in 126 patients treated surgically in the II General and GastroenterologicalSurgery Clinic between 2006 and 2008. All patients were divided into two groups based on the type of lesions. Inboth of the studied groups, more than half of the patients did not present any abnormalities in the mucosa of themouth, lips and cheeks in the physical examination. None of the patients had erosion, ulceration or lesionstypical of leukoplakia or lichen planus. No malignant cells were detected in either of the studied groups, andthere were no well-defined lesions found in the oral cavity that would distinguish the patients with gastrointestinalcancer. Inboth of the studied groups, more than half of the patients did not present any abnormalities in the mucosa of themouth, lips and cheeks in the physical examination. None of the patients had erosion, ulceration or lesionstypical of leukoplakia or lichen planus. No malignant cells were detected in either of the studied groups, andthere were no well-defined lesions found in the oral cavity that would distinguish the patients with gastrointestinalcancer.
