    The psychosocial impact of assistive device scale: Italian validation in a cohort of nonambulant people with neuromotor disorders

    The importance of adaptive seating system on body structure and function is widely accepted, but its impact on psychosocial aspects needs more consideration by health professionals. This article describes the Italian validation of the Psychosocial Impact of Assistive Device Scale (IT-PIADS) for non-ambulant people with neuromotor disorders. Once agreement has been given by the original authors, the scale was translated and adapted to the Italian culture. The IT-PIADS was administered to different wheelchairs users with heterogeneous diagnosis. The internal consistency and test-retest reliability were examined. Its concurrent validity was evaluated with the Italian version of the WheelCon-M-SF. The IT-PIADS was administered to 87 subjects. Cronbach's α was 0.92 (p < 0.05), and the testretest reliability (ICC) for competence, adaptability and self-esteem subscales were 0.96, 0.90, 0.93 respectively. The Pearson correlation coefficient of the IT-PIADS with the WheelCon-M-I-SF scores showed significant data for competence and adaptability subscales. Psychosocial perception on assistive devices can be reliably measure. The IT-PIADS showed good psychometric properties and it is possible to confirm its validity for clinical and research purposes. Nevertheless, before using this measure with greater confidence, further psychometric properties tests of the IT-PIADS are recommended

    HIRESSS: a physically based slope stability simulator for HPC applications

    HIRESSS (<b>HI</b>gh <b>RE</b>solution <b>S</b>lope <b>S</b>tability <b>S</b>imulator) is a physically based distributed slope stability simulator for analyzing shallow landslide triggering conditions in real time and on large areas using parallel computational techniques. The physical model proposed is composed of two parts: hydrological and geotechnical. The hydrological model receives the rainfall data as dynamical input and provides the pressure head as perturbation to the geotechnical stability model that computes the factor of safety (FS) in probabilistic terms. The hydrological model is based on an analytical solution of an approximated form of the Richards equation under the wet condition hypothesis and it is introduced as a modeled form of hydraulic diffusivity to improve the hydrological response. The geotechnical stability model is based on an infinite slope model that takes into account the unsaturated soil condition. During the slope stability analysis the proposed model takes into account the increase in strength and cohesion due to matric suction in unsaturated soil, where the pressure head is negative. Moreover, the soil mass variation on partially saturated soil caused by water infiltration is modeled. <br><br> The model is then inserted into a Monte Carlo simulation, to manage the typical uncertainty in the values of the input geotechnical and hydrological parameters, which is a common weak point of deterministic models. The Monte Carlo simulation manages a probability distribution of input parameters providing results in terms of slope failure probability. The developed software uses the computational power offered by multicore and multiprocessor hardware, from modern workstations to supercomputing facilities (HPC), to achieve the simulation in reasonable runtimes, compatible with civil protection real time monitoring. <br><br> A first test of HIRESSS in three different areas is presented to evaluate the reliability of the results and the runtime performance on large areas

    Guselkumab for treatment of moderate-to-severe plaque psoriasis: real-life effectiveness and drug-survival for up to 148 weeks

    Background: Real-world data are useful to guide the management of psoriasis. Here, we present data on the effectiveness and survival of guselkumab in moderate-to-severe chronic plaque psoriasis for up to 148 weeks. Research design and methods: Cross-sectional study of 122 patients receiving guselkumab (100 mg at weeks 0 and 4, and then every 8 weeks thereafter) for>12 weeks, from November 2018 to April 2022. Main outcome measures: Clinical features and drug survival were analyzed up to 148 weeks. Results: Obese patients (32.8%) and those receiving prior biologics (64.8%) were included. Guselkumab treatment was associated with a rapid decrease in PASI, from 16.2 to 3.2 at week 12, and long-term improvements in all subgroups (97.6%, 82.9%, and 63.4% of patients, respectively, achieved PASI 75, 90, and 100 after 148 weeks). More non-obese than obese patients achieved PASI 100 at week 148 (86.4% vs 38.9%), as did bio-naïve vs bio-experienced patients (86.7% vs 50.0%). Previous biologic therapy was a negative prognostic factor for achieving PASI 100 over the long-term by multivariate analysis (p = 0.005). Overall, 96% of patients were on treatment after 2 years. Conclusions: Real-world data confirm the long-term effectiveness of guselkumab in patients with psoriasis

    Angular dependence of copper surface damage induced by an intense coherent thz radiation beam

    In this work, we show the damage induced by an intense coherent terahertz (THz) beam on copper surfaces. The metallic surface was irradiated by multiple picosecond THz pulses generated by the Free Electron Laser (FEL) at the ISIR facility of the Osaka University, reaching an electric field on the sample surface up to ~4 GV/m. No damage occurs at normal incidence, while images and spectroscopic analysis of the surface point out a clear dependence of the damage on the incidence angle, the electric field intensity, and polarization of the pulsed THz radiation. Ab initio analysis shows that the damage at high incidence angles could be related to the increase of the absorbance, i.e., to the increase of the temperature around or above 1000â—¦ C. The experimental approach we introduced with multiple fast irradiations represents a new powerful technique useful to test, in a reproducible way, the damage induced by an intense electric gradient on copper and other metallic surfaces in view of future THz-based compact particle accelerators

    Impact of Upadacitinib on Atopic Keratoconjunctivitis Exacerbated by Dupilumab Treatment in Atopic Dermatitis Patients: A Prospective Dermatological and Ophthalmological Clinical Evaluation in Common Clinical Practice

    introduction: atopic dermatitis (AD) is a prevalent chronic inflammatory skin condition with a substantial impact on patients, particularly due to ocular involvement known as atopic keratoconjunctivitis (AKC). current therapeutic approaches, such as dupilumab, often lead to conjunctivitis, prompting exploration of alternative treatments like upadacitinib. methods: we collected dermatological and ophthalmological prospective clinical evaluations of six adults with moderate-to-severe AD, undergoing treatment with upadacitinib after discontinuation of dupilumab due to the onset of AKC during therapy and the worsening of dermatitis in particular in the head and neck region. clinical evaluations, including EASI scores, itch and sleep NRS, DLQI, and ocular parameters, were performed at baseline (during screening assessment before switching to upadacitinib) and then at week 12 and week 24. clinical evaluation of AKC was performed by a team of ophthalmologists. results: upadacitinib not only improved atopic dermatitis in terms of EASI, itching, and sleep NRS, but also demonstrated a notable reduction in ocular signs and symptoms, as indicated by the visual analogue scale (VAS), the Efron scale, and the ocular surface disease index symptom severity (OSDISS) scores. discussion: our observation of common clinical practice underscores the substantial impact of biological and small-molecule therapies on AD, emphasizing the limitation posed by dupilumab-associated conjunctivitis. switching to upadacitinib significantly improved both clinical and functional ocular outcomes, suggesting its potential as an alternative therapeutic option for AD patients with ocular involvement. conclusion: the presented data provides insights into the complex interplay between systemic therapies and ocular manifestations in AD. upadacitinib emerges as a promising option to address dupilumab-associated conjunctivitis, offering improved quality of life for patients
