Renaal zoutverlies bij chronische nierinsufficiëntie : een adaptieve reactie op renale waterretentie
Indien vee! vocht aan patienten met ernstige chronische nierinsufficientie
(CNI) wordt toegediend, ontstaat waterretentie met dilutie-hyponatriemie
(Bull, 1955; Schwartz & Polak, 1960; Armbruster e.a., 1974;
Curtis & Williams, 1975). Het is anderzijds bekend, dat waterretentie bij
personen met een normale nierfunctie aanleiding kan geven tot renaal
natriumverlies bij een dalend natriumgehalte van het plasma (Bartter &
Schwartz, 1967; Van 't Laar, 1967). Men kan zich dus afvragen ofeen
dergelijk renaal natriumverlies tijdens waterretentie ook bij patienten
met ernstige chronische nierinsufficii!ntie ontstaat. Als dit inderdaad
het geval is, zou de dating van het plasma-natrium niet slechts het gevolg
zijn van dilutie, maar ook van natriumdepletie. Er zijn spaarzame aanwijzingen
in de literatuur, dat toediening van een grote hoeveelheid water
aan deze patienten met CNI kan leiden tot een negatieve natriumbalans
(Coleman e.a., 1966).
Een andere vorm van zoutverlies bij patienten met CNI wordt waargenomen
bij de overgang van een zouthoudend naar een praktisch zoutloos
dieet (Nickel e.a., I953;Levin &Cade, 1965;Gonicke.a., 1964;
Kleeman e.a., 1966; Coleman e.a., 1966). Drie mechanismen zijn voorgesteld
om deze zogenaamde salt wastage of neiging tot zoutverlies bij
CNI te verklaren. Het eerste is de verhoogde GFR per nefron, waardoor
het aanbod van natrium aan de tubulus groter is dan kan worden geresorbeerd
(Bricker e.a., 1964). Het tweede is de structurele laesies van de
resterende nefronen, waardoor het natriumreabsorberend vermogen van
de tubuli is verminderd (Walker e.a., 1965). Het derde is de toeneming
van gefiltreerde minder permeabele anionen als fosfaten en sulfaten en/
of van ureum per nefron, waardoor de tubulaire natriumreabsorptie
wordt belemmerd (Maxwell e.a., 1964; Gonick e.a., 1964; Kleeman e.a.,
1966; Coleman e.a., 1966).Terwijl vee! werk is verricht over het natriumverlies tijdens een vrijwel
zoutloos dieet, is weinig onderzoek gedaan over het mechanisme
van renaal natriumverlies tijdens waterbelasting bij CNI. Volgens Coleman
e.a. (1966) en Seldin e.a. (1971) zou dit laatste berusten op het
onvermogen van de distale tubulus de natriumconcentratie in de urine
(UNa) onder een gefixeerde, relatief hoge, waarde te brengen. De toegenomen
natriumexcretie zou dan uitsluitend het gevolg zijn van de toegenomen
diurese. Deze verklaring komt volgens de auteurs overeen met de
bovengenoemde verhoogde osmotische belasting van de resterende nefronen,
welke de tubulaire terugresorptie van natrium belemmert. De
chronische waterbelasting werd door Coleman e. a. (1966) en Seldin e.a.
(1971) echter slechts gedurende 4 dagen volgehouden en wei uitsluitend
tijdens drastische zoutbeperking.
Wij hebben ons nude volgende vragen gesteld:
a) is het renaal natriumverlies tijdens waterretentie bij patienten met
chronische nierinsufficientie inderdaad het gevolg van een onvermogen
van de tubulus om UNa voldoende te reduceren?
b) berust dit renaal natriumverlies tijdens waterretentie bij chronische
n_ierinsufficientie op hetzelfde mechanisme als bij normale nierfunctie?
c) is er een verband tussen het zoutverlies tijdens waterretentie en de
zogenaamde salt wastage bij chronische nierinsufficientie
Economic impact of extended time on peritoneal dialysis as a result of using polyglucose: the application of a Markov chain model to forecast changes in the development of the ESRD programme over time
BACKGROUND: The use of polyglucose as a peritoneal dialysis (PD) fluid
extends time on PD treatment. It is anticipated, therefore, that the share
of patients treated with PD will be positively influenced. The
relationship between extension of PD treatment time and an increase of the
PD treatment share, however, is complex and needs further investigation.
In this paper, a Markov chain model was applied to investigate the impact
of extended time on PD treatment for the PD share in all dialysis patients
in The Netherlands. Furthermore, the economic impact of the extended time
on treatment (ETOT) was explored. METHODS: Scenarios were forecast over a
10 year period using aggregate data from the End-Stage Renal Registry in
The Netherlands (Renine). Three scenarios were simulated in which the
median PD technique survival was extended by 8, 10 and 12 months. Two
other scenarios explored the impact of the combined effect of ETOT of 10
months together with a 10% and 20% increase of PD inflow shares.
Reductions of costs to society due to ETOT were estimated using Dutch cost
data on renal replacement therapies. RESULTS: PD share increases from
30.0% in the null scenario to 34.5% in the scenario with an ETOT of 10
months and an increased PD inflow share of 20%. The reduction in total
costs to society of the renal replacement therapies is 0.96%. The average
societal costs per discounted patient year for haemodialysis (HD) are 84
100 euros. For PD, these costs are 60 300 euros. A shift from HD to PD
results in average cost savings of 28% per patient year. CONCLUSIONS: In
view of high dialysis costs to society, a reduction of 0.96% can be
considered to be relevant for healthcare policy makers
